Prophylactic Antibiotics for Flare-ups High level evidence indicate no need for an antibiotic unless there is a clear presence of swelling. But what the studies do not generally review is the pre-operative condition of the tooth which is to say; was it a vital, non-vital, symptomatic or asymptomatic. Literature also fails to objectively define “flare-ups”. Let’s assume as Endodontists, we can differentiate between post-op pain and “flare-ups” (Swelling, Fever or acute exacerbation of pain levels in comparison to pre-op). In the past during my residency and academic career, I followed the textbook rules of antibiotic prescription. Until this asymptomatic case (2 years ago) ended up with post tx Buccal space infection 🤢🥴🤒. Most of my previous flare-ups have happened in similar cases (didn’t matter 1 or 2-visits) IMHO in long standing asymptomatic non-vital or retreatment cases, the host-immune/bacterial harmony is abruptly disrupted, and hence after this case I started recommending shortened post-op antibiotic course (3 days only) for asymptomatic PARL cases. Never had a true flare-up since... (I desperately want someone to disprove my anecdotal science) What’s is your post-Endo Antibiotic Prescription philosophy in general ? Especially for long standing necrotic/previously treated cases with Asymptomatic lesions. How much pressure does post-op patient experience, feedback to RD and practice reputation play a role in your decision ? Using gene therapy to restore vision to people with an inherited retinal disorder is one of the most exciting achievements in eye research. The one-time treatment for Leber congenital amaurosis uses a harmless virus to deliver healthy copies of a missing gene to the retina. Some young patients have regained vision within weeks after treatment. Despite the miraculous outcomes, ophthalmologist continue to refine their technique for delivering gene therapy to children’s eyes. In the June issue of Ophthalmology Retina, Audina Berrocal, MD, and colleagues show a new technique for gene delivery in children. Here’s a partial description of their technique: After loading the AAV vector syringe onto a 38G/25G Subretinal PolyTip Cannula and purging all air bubbles, the injector was delivered into the subretinal space with a beveled tip. Beveling of the cannula tip was performed with scissors at a 45-degree angle, and the cannula was entered into the subretinal space in a bevel-up fashion. The EVA Phaco-Vitrectomy System with the viscous fluid control system was used for a single-surgeon, foot pedal-controlled linear infusion of the drug. When bleb tension and risks of macular hole formation are deemed too high, more than 1 bleb is recommended. With more than 1 extrafoveal bleb, subretinal delivery began nearer to the arcades but included the macula. In all cases, a total of 300 mL of vector was released (image shows multiple elevated subretinal blebs with a foveal detachment after injection). Is immune testing necessary before #IVF? I frequently get this question. Sometimes more is not better and tests that are not proven to be useful may do more harm than good. Today is the #transferversary of this embryo that now is a healthy baby. After struggling at another fertility center, this couple had all kinds of expensive tests that are not necessary before IVF. My new patient was terrified that she had an immune problem that would prevent her from carrying a term pregnancy and that would affect her health. All kinds of interventions were recommended including blood thinners, steroids, intralipids. Take a step back. We knew why this couple had infertility and it had nothing to do with her immune system. She was otherwise normal and healthy. I suggested she see a medical immunologist who agreed these tests were not necessary or predictive of any underlying immune problem. I literally wrote in her chart that we are ignoring all the prior results. It would have been easy to accept them and let another doctor prescribe unnecessary medications. She might have even gotten pregnant in spite of them and thought they helped and were necessary for her future pregnancies too. Any way, on her first transfer at #columbiafertility she conceived with a natural cycle frozen embryo transfer and delivered a healthy baby. The ABDOMEN X-RAY: I alway found this exam to be tougher than the chest X-ray. So what do I look for? 1. Bowel - small bowel is central and large bowel is around the periphery. Look for dilated loops of bowel filled with gas and or air/fluid levels to suggest obstruction. 2. Check for free air under the diaphragm. That’s a sign of bowel perforation. Your next move should be to pick up the phone and call the referring physician. Do not miss this! 3. Look at the organs, particularly the liver and spleen and determine if normal in size or enlarged. 4. Scan the kidneys and ureters and check them for calcifications indicating stones. 5. Look for calcifications in the right upper quadrant which indicates gallstones. 6. Check the bones of the spine and pelvis. Look for fractures or destructive lesions. Honestly, there is a lot more information that can be gathered from an X-ray of the abdomen, but those are some of the basics and I hope this helps you guys refine your search pattern. #anatomyoverlays - @theradiologyguy on Instagram

TUBERCULOSIS 👨🏽💻This was a tricky case - there was a clear abnormality at the right hilum with consolidation in the right mid zone but the more subtle findings were: ▫️Consolidation extending behind the right heart shadow - remember to look behind the heart, the density should be roughly equal on both sides ▫️Well defined apical opacity - compare both apices and look between the ribs, here there was an opacity on the right ▫️Left lower rib erosion - follow all of the ribs! Sometimes a subtle rib erosion can be the only abnormality on the film 👨🏽💻These findings could represent either a primary lung cancer or TB - given the age of the patient TB is the most likely diagnosis here. Been told your egg has Vacuoles? . The egg at the bottom has Vacs. They are incredibly easy to distinguish and look like circles or craters inside the cytoplasm. If you compare the egg to the other 3 you can see the vast difference . Its not known what causes them, but vacs are almost always from the advanced aged patients and therefore would probably link to egg quality. Occasionally i will see them in younger patients but their frequency is much lower . So what do we do?! Well if you have enough eggs its not something to be concerned about bc you can exclude them. But if you only have 3-4 eggs like this case, we would inject and keep them separate. I tend to try and avoid them at injection and half of the time they do fert and divide, but i know they produce lower quality embryos, if a blast at all . These morphologies in older patients are common so i thought it would be nice to show you just what it is we mean when we tell you “your eggs have vacs” . Enjoy the week . Cyanobacteria (blue-green algae) are found in a broad range of environments, from hot springs to marine and fresh waters, and soil crusts to polar icecaps. By understanding exactly how cyanobacteria harvest light and convert it to energy, scientists may be able to engineer highly efficient photosynthetic units to help increase crop yields. Same Same . We primarily do PGS in our clinic and its safe to say that most US IVF clinics do a large proportion of them too. Bc of this we hatch embryos on Day 3 so that by the morning of Day 5, they are alreading extruding out the zona. This makes the biopsy faster and more efficient for the lab bc when we do not hatch we get embryos like pic 1. In those cases we would need to lazer the hole in the morning and then wait for the extrusion. Or, go in and pull out the cells, which we find more harmful. I recently conducted a survey that i sent to labs around the US and other countries and the vast majority hatch prior on Day 3, not the morning of biopsy, so we are in line with the majority. The 2 embryos at 4Aa i saw this week were actually a nice treat bc we did not need to biopsy them as it was not requested and we froze them instead. I just wanted to show you what 2 very similar aged embryos look like as the same grade but one hatched, one not. This way if you have embryos in either category you can relax and note that they are still the same. Embryos will hatch by themselves when its time! So let them! We just need to hatch Day 3 to make our work more efficient, so if you do not have hatching embryos on Day 5 and even Day 6......Dont Panic! . Enjoy the day! . Keratoprosthesis is a surgical procedure where a diseased cornea is replaced with an artificial cornea. Traditionally, keratoprosthesis is recommended after a person has had a failure of one or more donor corneal transplants. More recently, a less invasive, non-penetrating artificial cornea has been developed which can be used in more routine cases of corneal blindness. While conventional cornea transplant uses donor tissue for transplant, an artificial cornea is used in the Keratoprosthesis procedure. The surgery is performed to restore vision in patients suffering from severely damaged cornea due to congenital birth defects, infections, injuries and burns. Keratoprotheses are made of clear plastic with excellent tissue tolerance and optical properties. They vary in design, size and even the implantation techniques may differ across different treatment centers. The procedure is done by ophthalmologists, often on an outpatient basis. Although many keratoprostheses have been developed, only four models are currently in commercial use: the Boston keratoprosthesis, Osteo-Odonto-Keratoprosthesis (OOKP), AlphaCor and the KeraKlear Artificial Cornea. Celebrando 10 anos! 🎂 Fazer tratamentos em amigos é (muitos concordarão comigo 😆) um factor que pode tender a que todas as complicações possíveis apareçam..... Em 2010, este 37 de um grande amigo apresentava uma fissura com sintomas típicos de um “síndrome do dente fissurado”. 🤔 Foi reabilitado com Recobrimento cúspideo com onlay, o que resolveu os sintomas por completo. 👌🏼 Passado pouco tempo, desenvolveu pulpite irreversível... 🤦🏻♂️ Foi feito tratamento endodôntico, num dente que aparentava ser fácil, mas que tinha afinal sistema canalar em C. 😬 Foi feita obturação com Onda Contínua de Condensação e reabilitação com coroa fixa total. Thought I’d illustrate another cool #regenerative case. Patient had caries in the tooth before it even erupted. Not sure if @rickschwartz78 or @endogenie or anyone knows the term for that? I’ve always been curious. So tooth tested vital. But you can see I pulped out a little when I excavated. As expected. I placed MTA and later composite. Final PA is from her GP 3 years later. Tooth is still vital and fine. Been 8 years now. Will hopefully get a new Pa soon. But I wanted to show this because the pics are good and I placed way less MTA than the case I depicted before this one. The main takeaway is that a big apex leads to great blood supply and likely success. Dr Hargreaves says he and the literature recommends bleach and #chlorhexidine to rinse but I’ve always just used bleach. Today is exactly one year since my surgery! It has been the hardest year of my life but also the best year of my life. I am in pain everyday and mentally that can wear on a person but I stay positive and think about all the room my heart has to beat and all the air my lungs get now. I can walk up the stairs and not be short of breath. I can go to the grocery store and not have to sit down because I feel like I’m going to pass out. 🚨🤸🏽ALERT: endodontic failure of a “simple” mandibular canine. 1 treated, 1 missed canal. Never underestimate any case. Dx: Previously treated tooth and Symptomatic apical periodontitis. High speed profile por desobturation. R25 for instrumentation of both canals. Patency was not achieved in lingual canal, in fact obturation was shorter than 2 mm from the radiographic apex. Nevertheless, 5 year follow-up shows radiographic healing. Hace días encontaron una zorrita en Catemu, fue llevada a VetCare quienes le dieron la atención de emergencia. Luego nos lo trajeron a nosotros, estaba en muy malas condiciones, al otro día amaneció muerto. No mostraba signos notirios de lesiones o un estado particularmente deteriorado. Luego de su muerte constatamos en radiografías que tenia impactos de escopeta disparados desde bastante distancia porque tiene perdigones en todo el cuerpo. El Zorro Culpeo es una de las especies protegidas por el Estado, esto quiere decir que no está permitida su caza, ya que se trata de un animal beneficioso cumple su rol de predador tope, junto con el Puma, manteniendo el equilibrio de los ecosistemas. What a day yesterday. I got to spend some time with @teagueholmes and were both injured. We came upon a parking attendant who told us that we could park over there and run over to the other side of the building. We both cant run, so I drove him over there and he hopped out of the car to do his errand. After that I went to the doctor for my 6 week checkup and he said that I could now sleep without my back brace on We both immediately started reminiscing about the time we got to ski the Grand Envers with Cédric Aubert. Sadly, the overall prognosis is worse for men than for women which is likely due to presenting at later stages. Since men aren’t routinely screened, they don’t show up to be evaluated until the mass has become palpable. Be mindful of this disease when men present to your clinic for breast pain, lump, or skin changes.
