There are many clinical reasons to perform a root canal treatment, but there are also practical reasons why saving the natural tooth is a wise choice. The patient decided to do a full mouth rehabilitation and the left lower canine was 90 degrees rotated. For prosthetic reasons, the RCT was done, the access cavity was made laterally on the buccal surface of the tooth and the canal was prepared only with one file (R25 blue @vdwdental ). A non-clinical view it’s rare to see in our treatments. One possibility would be to take an x-ray with a large angle. But in this clinical case, we can easily observe the internal anatomy due to the natural rotation of the tooth. Apparently he swallowed a sofa staple (how, we don’t know because none are missing) and is now waiting at the emergency clinic to likely go to surgery. Please pray he will be fine. My dear aunt who is 93 and in assisted living (unable to see family since March) was diagnosed with aggressive breast cancer after discovering a lump months ago. They wouldn’t let her see a doctor until yesterday. She’s now in isolation in the assisted living since she left to go to the doctor yesterday - she’s super unhappy about that. I pray whatever she chooses to do or not do that she will not be without seeing her family!! It’s the 28th anniversary of the disappearance of my childhood best friend Dail Dinwiddie. I’ve spoken with her parents today and the police called me with some questions between calls with the vet and my aunt. Please pray for her parents who have had no answers, and lots and lots of things that have not panned out over the last 28 years. Please pray for answers and peace for them. If you know anything about Dail Dinwiddie’s disappearance on September. 24, 1992 please call the Columbia, SC police. I know on some level everyone is hurting these days between politics, BLM, fires, hurricanes and of course COVID. Please know you’re in my thoughts and prayers. The ABDOMEN X-RAY: I alway found this exam to be tougher than the chest X-ray. So what do I look for? 1. Bowel - small bowel is central and large bowel is around the periphery. Look for dilated loops of bowel filled with gas and or air/fluid levels to suggest obstruction. 2. Check for free air under the diaphragm. That’s a sign of bowel perforation. Your next move should be to pick up the phone and call the referring physician. Do not miss this! 3. Look at the organs, particularly the liver and spleen and determine if normal in size or enlarged. 4. Scan the kidneys and ureters and check them for calcifications indicating stones. 5. Look for calcifications in the right upper quadrant which indicates gallstones. 6. Check the bones of the spine and pelvis. Look for fractures or destructive lesions. Honestly, there is a lot more information that can be gathered from an X-ray of the abdomen, but those are some of the basics and I hope this helps you guys refine your search pattern. #anatomyoverlays - @theradiologyguy on Instagram

Prophylactic Antibiotics for Flare-ups High level evidence indicate no need for an antibiotic unless there is a clear presence of swelling. But what the studies do not generally review is the pre-operative condition of the tooth which is to say; was it a vital, non-vital, symptomatic or asymptomatic. Literature also fails to objectively define “flare-ups”. Let’s assume as Endodontists, we can differentiate between post-op pain and “flare-ups” (Swelling, Fever or acute exacerbation of pain levels in comparison to pre-op). In the past during my residency and academic career, I followed the textbook rules of antibiotic prescription. Until this asymptomatic case (2 years ago) ended up with post tx Buccal space infection 🤢🥴🤒. Most of my previous flare-ups have happened in similar cases (didn’t matter 1 or 2-visits) IMHO in long standing asymptomatic non-vital or retreatment cases, the host-immune/bacterial harmony is abruptly disrupted, and hence after this case I started recommending shortened post-op antibiotic course (3 days only) for asymptomatic PARL cases. Never had a true flare-up since... (I desperately want someone to disprove my anecdotal science) What’s is your post-Endo Antibiotic Prescription philosophy in general ? Especially for long standing necrotic/previously treated cases with Asymptomatic lesions. How much pressure does post-op patient experience, feedback to RD and practice reputation play a role in your decision ? 2D vs 3D Tooth #3: Cold +/- (skewed cos of the crown); Percussion/Palpation -ve. No significant finding on 2D image. No symptoms at the time or leading upto the patient’s visit. She just had a specific incident of soreness in UR over a month ago, but could not get in then due to lockdown scheduling. I have encountered clinicians across the spectrum, who are “yay” sayers and “nay” sayers for using CBCT on every case. My philosophy on 3D scan has varied from ordering too easily during residency to restricting it for just complex clinical scenarios in professional life. During the lockdown, I may have attended over 100 webinars (heck I presented few of em too) and most of them were spectacular but rarely changed my clinical philosophy. Until I encountered Dr John Khademi’s webinar on CBCT 🤯 It truly changed my perspective on the “timing” of ordering and analyzing the CBCTs. This was completely counterintuitive to my previous approach on trying to get all the information and blend it to solve the diagnostic puzzle. Unlike the medical field, Endodontists have to encompass the role of a diagnostician, radiologist and the surgeon. Excessive hoarding of information all at once without objective filtering can lead to higher cognitive bias in our decision making. I would highly recommend to check out his Webinar or textbook on CBCT. The Palatal cervical resorption was an incidental finding, something I could have missed in the ruckus of a busy schedule, if my focus on the CBCT was merely to evaluate the PA status. I would have definitely missed it, if I didn’t order a CBCT at all and potentially risking a hypochlorite accident 😰 this may be a weak example to articulate my thoughts, but hope you get the point! ‘Inner War’ is a recent piece created after a friend committed suicide completely out of the blue. I was having my brain scanned for a voice study and I asked to use the MRI to capture a portrait instead of payment. The portrait shows me attempting to hide, and stop the view of my expressions. Of course the radio waves pass right through me revealing not just what’s beneath my hands but my skull and brain. I am transparent and seen with intense truth. Mental illness is on the rise, and more than ever we need to talk and support each other. We are so much more complex than we can give light to, many quietly at war with themselves. It’s been quite around here because I recently endured a miscarriage. I have thought long and hard about if I wanted to share this part of my story. I needed a few weeks to attempt to grieve. October is pregnancy and infant loss awareness month. We are still nagivagating through. However, I wanted to honor our baby and bring awareness for all the women and families who suffer from this. After treatmeant for AML-APL leukemia I was told I most likely would not have more children. We have pursued some other options and every other option has been a, “no.” I don’t share every detail but I have shared a lot because AWARENESS is SO important! I now know the agony of losing a baby. I’m so so sorry for anyone who has endured the phyiscal and emotional pain and trauma caused by this. Or any type of infertility! Or any type of longing for something or missing someone. It has affected me in ways I could have never imagined and as you know we have been through a lot. This is a different pain. Leukemia is traumatizing, any life altering illness is. This is a different traumatizing! It’s not one and done after a few days or weeks. Cancer isn’t ether but it’s another hard I’m trying to learn to live with. I’ve needed time to physically go through it because the phyiscal effects are painful. However the emotional part is worse. I have shared my thyroid has had issues since I finished chemo. I didn’t realize how important normal range thyroid levels are crucial to how our the human bodies function. Especially how even more critical/life threatening it is for a healthy pregnancy. So once I learned I was pregnant my hematologist also ordered remission labs and thyroid labs and learned my thyroid had plummeted. I had emergency thyroid care and medications, adjustments to medications dosages, plus the fluctuation of hormonal levels due to being pregnant and then then going through a miscarriage. We were so HAPPY the past few months. We almost made it to 11 weeks. Been told your egg has Vacuoles? . The egg at the bottom has Vacs. They are incredibly easy to distinguish and look like circles or craters inside the cytoplasm. If you compare the egg to the other 3 you can see the vast difference . Its not known what causes them, but vacs are almost always from the advanced aged patients and therefore would probably link to egg quality. Occasionally i will see them in younger patients but their frequency is much lower . So what do we do?! Well if you have enough eggs its not something to be concerned about bc you can exclude them. But if you only have 3-4 eggs like this case, we would inject and keep them separate. I tend to try and avoid them at injection and half of the time they do fert and divide, but i know they produce lower quality embryos, if a blast at all . These morphologies in older patients are common so i thought it would be nice to show you just what it is we mean when we tell you “your eggs have vacs” . Enjoy the week . 🚨🤸🏽ALERT: endodontic failure of a “simple” mandibular canine. 1 treated, 1 missed canal. Never underestimate any case. Dx: Previously treated tooth and Symptomatic apical periodontitis. High speed profile por desobturation. R25 for instrumentation of both canals. Patency was not achieved in lingual canal, in fact obturation was shorter than 2 mm from the radiographic apex. Nevertheless, 5 year follow-up shows radiographic healing. After a massive research I decided to go with a Panthera from @moticeurope because the price-performance ratio turned out to be amazing. My Panthera is now equipped with a 20 megapixel camera from a small German company. The software was developed by the local microscope seller himself and is just amazing! I used a lot of scopes and software but I just love this one because it has a lot of easy to use applications. 🌱 Today Id like to present you one of my first images showing the extremophile cyanobacterium Chroococcidiopsis in 10x DarkField. This genus holds a lot of surprises but is very difficult to identify if it comes down to species level even if youve got DNA sequences. The agglomerating character of the uni cellular and tiny Chroococcidiopsis makes it even harder because morphological features are hard to visualize and describe. 🌱 Im going to update you about my impressions with this microscope and also the story around Chroococcidiopsis. Thought I’d illustrate another cool #regenerative case. Patient had caries in the tooth before it even erupted. Not sure if @rickschwartz78 or @endogenie or anyone knows the term for that? I’ve always been curious. So tooth tested vital. But you can see I pulped out a little when I excavated. As expected. I placed MTA and later composite. Final PA is from her GP 3 years later. Tooth is still vital and fine. Been 8 years now. Will hopefully get a new Pa soon. But I wanted to show this because the pics are good and I placed way less MTA than the case I depicted before this one. The main takeaway is that a big apex leads to great blood supply and likely success. Dr Hargreaves says he and the literature recommends bleach and #chlorhexidine to rinse but I’ve always just used bleach. Caso raro de fratura isolada do maléolo posterior (tornozelo) com grave instabilidade da sindesmose (articulação entre a tíbia e a fibula - ossos da perna que formam o tornozelo) mesmo após a fixação do maléolo posterior! Muito comum esse tipo de lesão ser encarado como uma lesão simples no pronto socorro, mas pode levar a artrose do tornozelo quando não é tratado de forma adequada. Day 14 in the Hospital after my injury at #SteamersLane in Santa Cruz. I’m in the immobilization & recovery period. I’m incredibly lucky to have had such a skilled surgical team and thankful for my Dr’s, Nurse’s, CNA’s and social workers here at #skagitvalleyhosptial. So far 3 surgeries and they were a success. ⠀ ⠀ This injury has been a really big challenge being paraplegic with with 1 limb to use. I have a new appreciation for being a para and having 1 arm. ⠀ ⠀ 14 days down with probably another week to go before I can go home. We’ve had some pretty major problems though with my health insurance. @kpthrive won’t pay for a certain lift I need called a “take along lift with a “seated sling” for my home. It’s the only lift PT & OT could find that will work with my injury based on the pectoral tear and the surgical area. Kaiser has declined it now 3x and was supposed to have an independent review and that party was still a part-time Kaiser employee which seems really wrong. I’m really bummed this has already happened and can’t surf for quite a while. To also be having to fight for my insurance to do the right thing is really tough. I’ve had good and bad past experiences with Kaiser but generally they always do the right thing. This time, it makes no sense... because of covid-19 and the extra challenges that adds. I can’t have friends come and help lift me safely at home, nor can I afford the lift on my own. They are not giving me any options but possibly having to stay in a skilled nursing facility which I’m scared to death of having to go into with Covid-19. Does anyone or anyone who follows @wagsofsci have any ideas or recommendations? That’s unfortunately the only seated lift that will work, as No Hoyer lift and or other sling combos will not cause damage to my repaired injury. I don’t understand how they will pay a crazy amount of money for 6 weeks at a skilled facility that really isn’t needed or necessary but won’t cover a $2600 manual lift. Soo back in May I was trying to get a clip on this rail and ended up dislocating and tearing my plantar plate in my left pinky toe. Who knew a pinky toe could be so serious but I got surgery in July and I had to get a pin that stuck out of my pinky toe to stabilize it. The doctor told me after surgery that this injury is one of the rarest type of injuries and that my case was the 2nd recorded case in the country.
