01/07/2017 Há 2 anos atrás, eu estava indo pra um hospital bem longe, onde eu não tinha ideia do que aconteceria quando chegasse lá, a única coisa que sabia era que ia ser difícil, eu teria que ser o mais forte possível e que era realmente uma cirurgia complicada com alguns riscos. A cirurgia acabou, os dias de internação foram passando, a dor era horrível, a sensação de que eu não iria mais ser a mesma era grande, e a vontade de desistir era constante, mas Deus esteve cada segundo ao meu lado, me deu forças, e finalmente tudo passou. Eu realmente mudei, tenho cuidados que não tinha, dou bem mais valor à vida, e sei que se não fosse essa cirurgia, hoje provavelmente estaria em uma cama, ou pior, e com problemas bem maiores. Sou grata a cada dia por isso, e por mais que ainda exista dificuldades, não teria uma alternativa melhor! Sou grata também a toda a equipe médica que esteve comigo me dando todos os cuidados possíveis, a minha família e amigos que me deram forças, e principalmente a Deus, pois sem Ele nada seria possível, e eu nada seria! Nós achamos que não vamos superar e que nada vai passar, mas isso é só mais uma etapa desse processo chamado vida. Hoje tenho meus 22 parafusos e 2 placas, sou feliz com eles, e levo como prova de que nenhuma dor é eterna, o sofrimento é necessário e somos capazes de superar tudo, só basta ter fé e confiar em nós mesmos! 2 anos de vida!🌟💛 - @ana_santiago.o on Instagram

I was feeling something wrong with my hand and legs from last year. Then.,,Paralysis was advanced rapidly from this year. As a result of a variety of inspection at the hospital, spinal cord damage due to cervical spondylosis is found to be a cause. and, I did the surgery to protect the nerves that existing one week before. Had perform surgery called “ACDF “ which takes the iliac bone from the pelvis and transplant it to the cervical spine and fix it with a metal plate. And, my cervical spine became a hybrid cervical spine. My surgery was a success. I became able to walk without assistance, My postoperative process is going well. Bueno seguimos con vida que es lo importante. Thank you to everyone for the kind messages, I was able to get the Neurostimulator yesterday after 2 tries (cause nothing is ever easy with me 🥴). The goal is for the two leads connected in my spinal cord to intercept the pain signals and minimize the amount and intensity of my lower back & leg pain. I’ll have to wear this for a full week and then my team and I will decide if it would be best to install a permanent device. A fratura se deslocou tanto que fez múltiplos ferimentos corto-contusos em face, além de expor segmento ósseo ao meio externo. O segmento mandibular fraturado ficou preso a somente 1cm de tecido mole. Quais as preocupações funcionais devem ser levadas em consideração para o tratamento? In life, most of us are beneficiaries of the outcomes we have contributed to - yesterday was a stark reminder of that for me. Our position in life cannot, or at least should not, be blamed on our parents, our teachers, the government, or our society – it’s largely based on the choices we make, and the attitudes we adopt. People have overcome poverty, drug addiction, prison, abuse, divorce, mental illness, victimisation, and virtually every challenge known to man. Life is full of examples of the uneducated, the mentally and physically challenged, people born into war-torn impoverished backgrounds, who could have complained about life being unfair, but who instead chose a different path – they chose to overcome the odds and to leave the world better than they found it. Regardless of the challenges they faced, they had the character to choose contribution over complaint. It doesn’t matter whether you are born with a silver spoon, plastic spoon, or no spoon at all. It’s not the circumstances by which you come into this world, but what you make of them once you arrive that matter. We all know the risks when we go out, you just never imagine it will be you. Last Thursday started like most peahi days grabbing a couple rides - it was looking to be an epic low wind day. Then on a not extraordinary large one, I tried to tuck into the barrel and got clipped by the lip, something I’ve done before. But this time as my board came back over the falls, it struck my leg rail first. I instantly knew something was wrong as my leg was squirreling around by itself in the white water. Luckily my cousin @trevor.808 who runs rescue was right there with the sled and was able to drag me into safety of the channel. From there the quick actions of water safety @kolomona1_ and @dklegend jumped into action. I am extremely blessed these guys were there and quickly assessed and got me to Maliko gulch to an awaiting Ambulance. Later that night surgery and a new 40cm rod in my leg. Truly blessed with how everything happened. I don’t think it could’ve gone smoother. Bummed this season is over. I now start the road back to again surf the wave I truly love. Again my Thank you to everyone who got me home safe and to the surgeon who put me back together. Ended up snapping my femur, clean break. Blessed 🙏🏼 - @ryan.funk on Instagram

WOW! LOOK AT BELLA’S NEW LEG! Bella’s surgery was almost 7 hours long, went perfectly and without any complications! Just one of these orthopedic surgeries is a big surgery. But, Bella had all 4 done in one major surgery. Bone and Joint specialist Dr. Guillou rebuilt her S shaped tibia, and she had a complete femur repair. She also had her torn ACL fixed and her grade 4 luxating patella corrected. Simply unbelievable and completely life changing. What a miracle that we were able to give Bella two brand new legs and now a new life without pain and suffering! A huge, and I mean huge THANK YOU to Eldad & @xoxoloreta from @hopeforpawsrescue for always being so generous and paying off the outrageous $10,000 bill for Bella! We are so honored to be rescue partners of Hope for Paws, and their love and support means everything. I have learned so much from Eldad & Loreta and continue to every single day. They are my friends, my mentors and are like family to me. Foxy and the Hounds is no doubt a better rescue because of them and I am forever grateful! Bella will be in the hospital for several days and then will return to her incredible foster Shawn Baxendale who will give her lots of love and the crate rest she needs. This has been an unbelievable journey and we thank everybody who has donated, prayed and helped change Bella’s life forever. We will post next week once she is released from the hospital and will continue to give updates. Bella will be available for adoption in a couple of months and we can’t wait to see her run for the first time ever! Some times it just isn’t your day, I fell topping out a climb on day one of our Teton climbing trip. About a 30 foot whip where I hit a ledge about 15 feet down breaking my leg and ankle, then continued to fall for another 15 feet upside down. Luckily the .1 Z4 held and I didn’t fall any further. I broke my tibia and fibula, as well dislocated and subluxed the bones that these bones rest on. This 26 year old male had a motor vehicle accident and unfortunately battered his frontal, orbital and nasal bones. The frontal bone had both anterior and posterior table fractures with persistent cerebrospinal fluid discharge. The whole frontal bone had dished in and also was pushing the right eye out of globe. Fortunately his vision was intact and there was minimal frontal lobe injury. We had to intervene early so took him to the OR. Scoliosis is no joke and I’m thankful my parents pushed me because Lord knows at the time I didn’t want to go forward with it. TUBERCULOSIS 👨🏽💻This was a tricky case - there was a clear abnormality at the right hilum with consolidation in the right mid zone but the more subtle findings were: ▫️Consolidation extending behind the right heart shadow - remember to look behind the heart, the density should be roughly equal on both sides ▫️Well defined apical opacity - compare both apices and look between the ribs, here there was an opacity on the right ▫️Left lower rib erosion - follow all of the ribs! Sometimes a subtle rib erosion can be the only abnormality on the film 👨🏽💻These findings could represent either a primary lung cancer or TB - given the age of the patient TB is the most likely diagnosis here. Low back pain is among the leading cause of disability globally, and nearly everyone will experience this at some point in their lifetime. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. This situation is called cauda equina, and is a neurosurgical emergency. Read on to find out more about my review areas on a hand X-Ray... 👨🏽💻Hand x-rays are performed in cases of patients with suspected arthritis to try and evaluate the cause but more commonly are performed in cases of trauma 👨🏽💻When it comes to trauma there are a few areas I look at in particular every time - these review areas have helped me become more confident when looking at these X-Rays and hopefully they can do the same for you: ▫️Always start any trauma related X-Ray by looking at the soft tissues - the site of swelling can lead you to the fracture ▫️Check the metacarpal heads - an indented 4th or 5th metacarpal is common after a punching injury - termed a Boxer’s fracture. Don’t stop there in these patients - draw a line across the base of each metacarpal and this should join up. Sometimes the 4th/5th metacarpals can dislocate and travel downwards so they overlap the carpal bones - here your line won’t join up. Overlapping bones on a hand X-Ray should always make you think! ▫️Outline the base of the thumb metacarpal - forced abduction can cause an intra-articular fracture known as Bennett’s fracture (or Rolando fracture if comminuted) ▫️Do all of the fingers look well aligned? Check for subluxation and dislocation - you may need further views to be sure ▫️When it comes to carpal bones, scaphoid injuries are most common but you will need a whole scaphoid series. A normal X-Ray can’t exclude a scaphoid fracture. Practice varies with what to do next, some bring back people with snuffbox tenderness in 7-10 days to do an X-Ray, some to do a repeat clinical assessment whilst in some institutions they come back for an MRI ▫️The spaces between the carpal bones should be equal, if you see a widened distance between the scaphoid and lunate consider scapholunate dissociation (Terry Thomas sign) 🔻As always take your time, zoom up and outline the cortex of each bone, use a system and check two views! @theradiologistpage - @theradiologistpage on Instagram

Thought I’d illustrate another cool #regenerative case. Patient had caries in the tooth before it even erupted. Not sure if @rickschwartz78 or @endogenie or anyone knows the term for that? I’ve always been curious. So tooth tested vital. But you can see I pulped out a little when I excavated. As expected. I placed MTA and later composite. Final PA is from her GP 3 years later. Tooth is still vital and fine. Been 8 years now. Will hopefully get a new Pa soon. But I wanted to show this because the pics are good and I placed way less MTA than the case I depicted before this one. The main takeaway is that a big apex leads to great blood supply and likely success. Dr Hargreaves says he and the literature recommends bleach and #chlorhexidine to rinse but I’ve always just used bleach. Was at the doctor yesterday and the fractures themselves are all pretty well healed. But when they measured the bone density, it appeared that I had osteoporosis. That means that my bones are not as stable as they should be. Another big stepback for me. The doctors said that it can be healed with the right nutrition but it‘s not sure. If not, I‘m not gonna be able to ride downhill ever again. Got my final exams this and next week and also lots of medical appointments coming up in the next few weeks. Luego nos lo trajeron a nosotros, estaba en muy malas condiciones, al otro día amaneció muerto. No mostraba signos notirios de lesiones o un estado particularmente deteriorado. Luego de su muerte constatamos en radiografías que tenia impactos de escopeta disparados desde bastante distancia porque tiene perdigones en todo el cuerpo. Soo back in May I was trying to get a clip on this rail and ended up dislocating and tearing my plantar plate in my left pinky toe. Who knew a pinky toe could be so serious but I got surgery in July and I had to get a pin that stuck out of my pinky toe to stabilize it. The doctor told me after surgery that this injury is one of the rarest type of injuries and that my case was the 2nd recorded case in the country. Now the doctor cleared me to skate but to take it slow and thankfully I was able to move out to California with some homies💯 Thank you God for all my blessings🙏🏾 - @keezo_ on Instagram

[ PODEMOS TRATAR CONSERVADORAMENTE FRATURAS DOS OSSOS DA PERNA?] As fraturas da diáfise da tíbia estão entre as fraturas mais frequentes dos ossos longos. Uma porção considerável delas são expostas e geralmente consequente a trauma de alta energia acometendo um grupo de pessoas jovens e ativas. Para essas situações, o tratamento cirúrgico está bem estabelecido através da osteossíntese interna, quer seja com placa quer seja com haste intramedular. Quando a cobertura cutânea apresenta lesão mais complexa, a fixação definitiva pode ser feita com fixador externo (linear ou circular). Entretanto, em situações especiais, podemos lançar mão do tratamento incruento preconizado por Sarmiento, chamado de tratamento funcional (imobilização gessada permitindo carga e mobilidade do joelho precoce). Atualmente, para o tratamento incruento, a fratura deve ter pouco desvio, não ter lesões cutâneas e reservada a pacientes com pouco demanda funcional. Sarmiento preconiza inicialmente gessada cruropodálica com possibilidade de carga de acordo com a tolerância do paciente, por um período inicial variando entre 3 a 6 semanas, dependendo da estabilidade da fratura. Após esse período, troca-se essa imobilização gessada por um gesso PTB (Patellar tendon bearing). Esse gesso circular, usa uma pequena quantidade de algodão ortopédico nas proeminências ósseas, moldagem no tendão patelar de tal forma que permita extensão e flexão total do joelho, tornozelo mantido plantígrado e carga estimulada desde o início.
