- Re-post.... PSA for my healthcare buddies and friends that use this equipment. A lady in one of my Ag groups works as a CNA at night. The strap broke on the sling while lifting a patient in a hoyer lift. Patient is OK. CNA got stiches and 3 days off work. WOW-shes a tough one.

Ive styled my hair the same exact way for around 13 years which means this is roughly my 1582nd time doing it. For me it feels no different than brushing my food annihilators, or slicing my face fluff. It does have a little more flavor than most mundane toiletry tasks, but Im mostly lost in thought for the majority of the process. The last time I made a video like this was in 2012 (scroll to the end) and its made me laugh seeing my hands in autopilot, making the same strange shapes. I took a few stills from the video on certain phases Ive never taken the proper time to enjoy. Also, it gave me a reason to time it from start to finish (18 min) not too shabby! Me vejo ofertando esses peixes. Amarro-os em minha pele, sentindo as escamas a cortarem e a pressão dos corpos dificultarem a minha respiração. Permaneço imóvel até esquecer dos sentidos que residem em mim. Meu pescoço treme com o que ofereço, percebo que a carne quando fora do seu lugar, pesa e com tempo se decompõe. O cheiro se intensifica a cada pássaro que se aproxima, em outras condições, eles já teriam os devorado, mas a quentura do meu corpo os afasta. Por mim eu nadaria em águas, não acredito na natureza dessa oferta, mas quando a maré tá baixa é difícil de se caminhar pela terra debaixo do rio, o chão gruda nos pés enquanto sinto minha boca seca pedindo pela cheia. Me acostumo em não ver pelos olhos, em sentir meus lábios racharem. O sol que senti em minha pele do lado direito, já sinto estar em cima de mim. Questiono o porquê da ação. Penso em como a cabeça, peixe e rio se relacionam de diferentes formas para cada corpo que os encontra. Sinto perder minha cabeça no rio e encontrar outras. Me vejo ofertando esses peixes. Tento achar formas de não precisar ofertar. Busco compreender a malícia em negociar. O cheiro se intensifica. Escuto cada vez mais presenças próximas a mim, elas questionam se minha natureza está viva ou morta. Me finjo de morta. Meu corpo continua quente. Myxedema Coma Rare form of hypothyroidism. Need to have I high index of suspicion when you see the constellation of altered mental status, hypothermia and bradycardia. Often goes misdiagnosed and could be deadly. Unlike its nomenclature, most patients will not present in coma unless severe disease. Most patients are altered and hypothermic. Generally comes from patients with long-standing hypothyroidism who either have gone misdiagnosed or non- compliant with their meds. The pathophysiology of MC is basically disruption of the body’s homeostasis and thermoregulation. When sever, patients will also be hemodynamically compromised which is why all patients with MC need to be admitted to the ICU. Patients typically present with hypotension, hypothermia, AMS, dry skin, delayed DTRs and some times respiratory distress. Blood work will confirm severe hypothyroidism, anemia, at times, hypoglycemia and an elevated CK. MC is a medical emergency and should be in your differential of all patients with these sxs. Treatment consists of monitoring ABCs (admit to ICU), at times, treatment for sepsis and IV replacement of thyroid hormone. Replacing thyroid hormone is tricky because high doses can lead to tachydysrrhythmias and low dose won’t fix the problem. Careful monitoring in ICU setting is paramount. Blood flow is decreased and when rewarding patients, central measures should be considered as warming blankets may continue to decrease blood flow. Fluid restriction or isotonic saline should be used because of possible hyponatremic encephalopathy. If patients are symptomatic from their hyponatremia, consider 3% saline. Continue to check glucose as dérangements may occur with adrenal insufficiency.
