Our Amrita Hospital performed a surgery on a rare case saving a mother and her newborn, as the first in India. We are extremely happy to share the birth of female baby born to a mother with juvenile pompe disease on #enzyme #replacement #therapy since 6 years at our #Amrita Institute of Medical Sciences, #Kochi campus. This is the first case of a woman with pompe disease giving birth to a baby from india. #Pompe #disease is a rare, multisystemic, #hereditary disease, which is caused by pathogenic abnormalities / mutations in the ‘GAA gene’. Patients with this condition have muscle weakness to spectrum of severe complications. As this was a very high risk pregnancy with the patient having severe proximal muscle weakness she was planned for elective Lower Segment Caesarian Surgery under regional aesthesia. The surgical team consisted of Dr. Radhamoni, Dr. Vinitha, Dr. Girija, Dr. Jean and Dr. Neethu as anaesthetist. Prenatal work up was done to check whether her husband was a carrier and as he was not a carrier for pompe disease invasive test was not offered as the baby will not be affected as the condition is an autosomal recessive disorder. Mother was continued on #enzyme #replacement #therapy throughout pregnancy and baby can be breast fed too. The genetic team includes Dr Sheela Namboothiri, HOD, Department of Pediatric Genetics, along with Dr Dhanya and Dr Shwetha. For years I have taken care of and advocated for infectious, sick, and dying people (and animals!). As bedside nurses we don’t feel like hero’s. We do it because we love helping and caring for others and wouldn’t have it any other way. Guarulhos continua a realizar teste rápido para Covid-19 A iniciativa tem por objetivo testar as pessoas que já tiveram contato com pacientes infectados pelo novo coronavírus. Aqueles que apresentam resultado positivo para Covid-19 são encaminhados para acompanhamento na Unidade Básica de Saúde onde são cadastrados. Até o final do mês mais quatro dias de mutirão serão realizados na cidade, todos eles das 9h às 15h. Nos dias 20 e 21, a testagem será realizada no CEU Pimentas, localizado na estrada do Caminho Velho, 351. E nos dias 27 e 28 será no CEU Presidente Dutra, situado no final da rua Maria de Paula Motta. Para fazer o teste no mutirão não é necessário agendamento prévio, basta levar documento de identidade e cartão do SUS. A única restrição é não estar com síndrome gripal no dia do exame. No entanto, as pessoas que já tiveram contato com pacientes infectados pelo novo coronavírus também têm a opção de agendar o teste rápido nas Unidades Básicas de Saúde. O agendamento deve ser feito de forma presencial. *Warning* not snowboard or bike content. This month I am volunteering to help staff a COVID19 and respiratory illness diagnosing clinic as a Medical Assistant. More people are doing so much more than me! Something that stuck with me from the experience was the power and toxicity of fear. Every patient, regardless of diagnosis was afraid. Understandably so. Having said that, a focus on fear alone is not helpful, nor does it solve problems. It’s difficult to give feelings of comfort through scrubs, a gown, face shield, mask, gloves... I do my best. I leave the clinic each day, thankful. I am proud of the wonderful people I work with, for the chance to help, for the wild, for my family, friends, home and life. My message for friends and fam is Positivity. I hope each of you get a chance to appreciate time with and talk to your loved ones, to enjoy the clean air, to thoroughly wash your hands, to distance appropriately and to reflect upon your own life and appreciate its value. Let’s keep our heads up and focus on what good can be done. Thank you everyone so much, from the bottom of my heart, for being the most amazing clinical group Ive ever had! This clinical rotation was so stressful, difficult, and mentally/emotionally taxing. We were placed on one of the busiest surgical units in RAH, had a Miranda Bailey minus the fun parts instructor, and were drowning from all the extra work given even after our shifts for the weekends. But through all our hardships, I felt like we came together more so than just colleagues, but as close friends who were all experiencing this hell together. With the amazing unit culture and nurses, and our friendship and support for each other, I can at least confidently say that the experience was all worth it because of you guys. Heading in to work today feels heavy. Isolation with my kids has been a beautiful reminder of family connection and my littles that I love to watch grow, play, and even fight 🤣. But for me, and for Derrick, there’s another side of this season. The side where we leave isolation and the safety it creates, and choose to go where we will be in harms way in order to take care of those who need it. Covid positives are now in my hospital. Supplies are dangerously low. Derrick is working extra shifts to cover those fireman out now with symptoms and in quarantine. This dance between peace and anxiety has been surreal. It’s a period of anxiety where we wait to see what this pandemic will look like- especially as we watch those countries ahead of us fight this. At work, I’m surrounded by people who have also chosen to serve others medically through this time. I’ve been more connected with my colleagues in other areas of the country, and I have felt a deep sense of community and camaraderie that I haven’t felt ever since becoming a PA. I’m humbled and deeply grateful for the hours of preparation and behind the scenes work that all healthcare workers are putting in, that no one can see. We prepare and guess at what’s coming, and we wait. It’s also been an incredible time watching people and communities come together to support each other, small businesses, the marginalized, and humanity as a whole. I’ve seen more smiles from neighbors out on walks this week thank ever before! We knew what we were signing up for going in to these careers, but the reality of it has been intense. This is our part of fighting this battle, showing up to work for those who are sick and vulnerable. Let’s all do our part in this battle; even if staying home feels small- it’s significant and will protect so many. Help lower the impending burden on the healthcare system, and help protect healthcare workers as we choose to fight this pandemic on the front lines. ‘As experts in the use of medicines, pharmacists are essential members of the critical care team. They prescribe, tailor regimens to conditions, organ function and routes of administration — these are vital skills that optimise outcomes for critically ill patients. I have been working in critical care for over 20 years. It was only meant to be a two year job, but I became completely hooked! The pandemic was tough to deal with, especially at the beginning when there were so many unknowns. I am extremely fortunate to be part of many larger teams of pharmacists, within critical care, within the hospital, and also virtual teams right across the UK and beyond. During the first coronavirus surge, the critical care pharmacy team was expanded. Pharmacy added extra services to support our nursing and medical colleagues, such as provision of ready-to-use-medicines, and making individual doses of injectables that would have been very difficult for nurses in full PPE. This was achieved whilst keeping the supply chain flowing and all while whilst still providing direct patient care. End of day 70. Every shift is something new in Covidlandia. Ive said numerous times to be prepared for anything when you walk in. This stick of epi was handed to me in report by the off going nurse as she said I pass you the baton.⁣ ⁣ The back story is that our patient has coded twice during the night, as we thought might happen. She anticipated it would happen again; as it did during my shift (another 4 times). Routines arent something that you can stick with as a healthcare provider!The orange tackle boxes are frequently used ACLS meds meant to be used during cardiopulmonary arrest so that the code carts arent cracked open unless absolutely necessary. ⁣ ⁣The sad reality behind this photo is that the young individual was in the cusp of their 20s.⁣ Difficult conversations had to take place with loved ones regarding continued care, after life care, and the like. Despite the amount of available IBD therapies, many patients are unresponsive to these treatments or fail treatment. More options are needed. Fecal microbiota transplantation (FMT) is an alternative therapeutic strategy for the management of several gut disorders, including IBD, since the gut inflammatory process is frequently associated with dysbiosis. C était il y a a peine encore quelques tous petits jours et ce n est pas vraiment simple d’ en parler encore moins de trouver les mots justes(ne pas heurter) parce qu en vrai il n y en a pas ... C’était le second chapitre, celui du livre de ma vie,celui de notre histoire. cette épreuve de plus qui j’en suis sure aujourd’hui me permettra de vivre encore plus fort, de vieillir encore plus vieille (amèrement parfois) mais je l’espère de toutes mes forces. 2 ✌🏻comme 2 ovaires /2 trompes comme 2 seins que j’ai sacrifié dans cette bataille mais surtout 2 comme seconde chance : d’avoir eu cette double chance de donner la vie à mes 2 bébés mes 2 flammes de vie... 2 comme 2 fois plus de raison d’y aller, deux fois plus convaincue « qu on ne m enlèverai rien, qui m’empêcherait de me tenir debout... solide... attachée à ce combat que je livrerai sans merci et qui n’entachera jamais ma soif de VIVRE, ma volonté féroce de voir grandir mes filles, et d aimer plus ardemment celui qui en redemande quand je lui dis « j vais prendre 1000 ans j srai flétris et fanée🤣 » vendeur comme programme 🤪! Y a des photos moins glam que d autre... la vraie vie c’est celle ci... c est celle qu on choisit de montrer... sans pudeur parce que ce genre de décision, ce choix médical n est pas anodin mais qu il est une solution à envisager quand c est encore possible alors nous devons être fièr(e)s d en parler même si ce n est pas simple à légender... à chaque jour suffit sa peine. Let’s talk about vaginismus! I’ve gotten so many messages recently on this. Unfortunately I cannot give advice on social media. However I would like to share some education! What is primary vaginismus? Primary vaginismus is when vaginal penetration has never been achieved ever. So this means you have never been able to put in a tampon, have penetrative intercourse or digital penetration. What is secondary vaginismus? Secondary vaginismus is when vaginal penetration has happened before, but is no longer possible, potentially due to factors such as gynecologic surgery, abuse, trauma, menopause or radiation. When people have pain with intercourse this is called Dyspareunia- just the medical term for it. It is often confused with vaginismus. People can have painful intercourse for many reasons that are not vaginismus (like endometriosis). Vaginismus is an involuntary tightening of the vaginal muscles which makes penetration difficult or impossible. Many people who have vaginismus often have a fear of vaginal penetration and decreased sexual desire related to penetration. Which is not surprising! My vaginismus patients are a motivated bunch seeking help! I am incredibly proud of my patients when they make progress with their treatment! Its daunting to work with a therapist, use dilators and suppositories and have injections to an area that is sensitive. To all the women struggling with vaginismus do not give up!! I see so much success so often! It’s truly amazing to see when women are able to have a sexually satisfying experience with or without penetration! (Which is key to remember that penetrative intercourse is not the only way to have sex.) You are not broken! Been late to update (been waiting till the right time to protect our brave allies). This is from 4th of July (but in the works since even earlier—back in March)! a couple months ago, our networks of nurses and doctors were being walked out in handcuffs for passing out PPE so we had to start an underground bottom-top network (the real roots of PPEforHCP) of free donations that even included the US Veterans Affairs. A few of us on OSCMS spun off a Signals group to design (medical) protective equipment for peaceful protesters (inspired by Hong Kong) as soon as June 1; this was to keep our community healthy/reduce hospital resource impacting (on the bigger scope/support freedom, liberty, and justice). We couldn’t do it without your support, the support of a few brave individuals going against the grain (sacrificing the REAL threat of getting fired for not “staying in their lane” or often worse) in US Customs, UPS, SJPD, hospitals, FEMA, fire department, Hong Kong/mainlanders, etc. Advocates for health and good doctors/nurses and frontlines come from all sides. I remember it being such an exciting time in my life everything felt surreal, I was so glad to finally be a dental student after the arduous application process 😅⁣ ⁣ I really feel for you guys that have to go through this ‘COVID era’ of dental school. I’m not going to lie having online lectures during your first few weeks isn’t ideal. It can be difficult to make friends and actually feel immersed into the course.⁣ ⁣ However, do not fret. The the future may feel uncertain at the moment and many new changes and modifications have been introduced into dental practice, but you guys are going to be the most equipped to face these situations if anything like this happens again. It’s one of those things that you just have to embrace. I hope things start to feel more normal over the next few months and you guys get to enjoy the rest of your time a dental school! Our REPARE team is always hard at work making sure that the biomedical equipment at SBH is in good working order. Recently, the autoclave in our surgery department stopped functioning. Our Chief Biomedical Equipment Engineer, Alix Ilophene, told us: “We know that to do surgery, sterilizing equipment is necessary. Without that, there is no surgery. It was not so difficult to diagnose the problem. The [autoclave’s] boiler displayed a low water alarm—we found the machine’s water piping was blocked. We cleaned it, and then cleaned the whole machine. We took about two hours to repair it. I can say without a doubt I have never experienced anything like it in my life. I have never experienced mental, emotional, and physical pain at the same time. When the results for my COVID test came two days later, doctors were doubtful about my reaction. I told them not to worry since Im a doctor myself. I knew that I could handle whatever the situation might be. Thats when they revealed that I was COVID-19 positive. As soon as the news spread, many of our neighbours panicked. The city administration council put up posters outside our door. People clicked photos and shot videos of my house. They shared it with my name, address, and personal details on different WhatsApp groups. Since people love to gossip on WhatsApp, ugly stories about me started circulating in social media. Even the news media put out the wrong kinds of stories. Despite going straight to the hospital from the airport, I had to be the victim of social media hatred. I didnt even go home to collect my personal belongings to make sure that I didnt transmit the virus to anyone. Thankfully, I started recovering and, after a few days, tested negative. But the hounding didnt stop after I was back home. I knew that going back home will be an unpleasant experience. Therefore, to avoid attention from neighbours and media, I insisted on going home late at night. Not just me but my entire family had to experience the harassment. My younger brother didnt step out of the house for two months. Neither did I. I would only step out during the night to avoid people. When I step out, I can still sense the eerie stares and gossip. It is like a bad dream - Im all alone. People refuse to hear when I plead to them that I didnt do anything wrong. The only way we can fight this pandemic is with the right balance of precaution and empathy. Yes, we need to be careful. But we also shouldnt let our fears overpower humanity within us”. With a spike of 82,170 coronavirus cases and 1,039 deaths in 24 hours, Indias tally on Monday breached the 60 lakh-mark with a total of 60,74,702 cases. Out of these, 9,62,640 are currently active whereas the number of discharged cases stands at 50,16,520. While 95,542 lost the battle against the viral disease. On July 17, India had logged 10 lakh cases, which then doubled to 20 lakh on August 7. The country added another 10 lakh on August 23 and went past 40 lakh on September 5. In 11 days, it added another 10 lakh cases, amounting to 50 lakh cases. Twelve days later the tally has mounted to 60 lakh. Ordinary glass panels can act as solar panels when a layer of nanoparticles is sandwiched between two panes. This could help us take advantage of solar energy in cities where space for solar panels may be limited.⁠ ⁠. “If we’ve done our job, no one will even know that they’re there,” says Hunter McDaniel at UbiQD, a materials manufacturing firm in the US. He and his colleagues have developed transparent solar panels that are indistinguishable from regular glass.⁠ ⁠. Test installations, involving panels that are 1 square metre in size, are under way in buildings in the US and the Netherlands. The panels have a power conversion efficiency of 3.6 per cent, a measure of how much sunlight is converted into electricity. Opaque solar panels, in comparison, have efficiencies between 15 and 20 per cent.⁠ ⁠. These transparent panels are made from two layers of glass glued together with a polymer that contains nanoparticles known as quantum dots. With a core of copper indium sulphide and a shell of zinc sulphide, these quantum dots are tiny semiconductors that can manipulate light.⁠ ⁠. When the nanoparticles are excited by exposure to UV light, they release photons that travel along the transparent panel towards its edge. The perimeter is fitted with solar cells, which convert the photons into electrical current. Ontem, antes de dormir, dei uma última olhada nas notícias (que por sinal é algo que nunca devemos fazer) e me deparei com essa notícia: a AstraZeneca, que está nas fases finais de desenvolvimento da mais promissora vacina contra a covid-19, precisou suspender os testes clínicos por causa de um efeito colateral sério, e inesperado, em um dos pacientes. A vacina já estava em testes no Reino Unido, na Índia, na África do Sul, nos Estados Unidos e inclusive no Brasil. E fiquei pensando em quantas pessoas estão fazendo planos e contando com a vacina em janeiro. Desde Trump, que “prometeu” a vacina para antes da eleição americana, até amigos e conhecidos que já estão marcando viagens e ensaiando a retomada de projetos que ficaram suspensos. Quando insisto que não devemos contar com isso, as pessoas me chamam de pessimista. Eu tento explicar que não é pessimismo: é informação. Acompanhar as notícias de boas fontes, analisar gráficos e ler estudos além das manchetes me fez entender há bastante tempo que a jornada muito provavelmente será longa. Nessa história toda tem algo que até agora muita gente ainda não assimilou: nós não conseguimos controlar tudo. A humanidade está tentando se adaptar a uma situação inédita sem ter sido treinada para isso, e acho até que estamos nos virando. Mas nosso cansaço e angústia não podem simplesmente decretar o fim da pandemia. Quem está esperando a chegada da vacina para retomar algum plano, aceite que ela pode demorar um pouco mais para chegar. Já está sendo feita em tempo recorde. Falo isso desde março: adaptabilidade é o nome do jogo. Sem visibilidade, é preciso navegar com a intuição: manter-se em movimento para não cair; tomar decisões de curto prazo; e não se culpar pelo que não estiver dando certo. Estamos tentando fazer o melhor possível com o que temos nas mãos hoje. Não podemos confundir o que queremos que aconteça com o que vai de fato acontecer. Expectativa e esperança são coisas diferentes. Hoje foi um dia sensacional! Realizei pela manhã uma Cirurgia de Prótese de Quadril no H. Regional de São José pela via Anterior, utilizando a técnica AMIS ®️ . A paciente apresentava uma fratura de Colo Femoral e a cirurgia foi um sucesso.
