Cancer sucks. Every single form of it. It destroys our loved ones. But one cancer that my family has become to hate the most is Uterine Cancer. My mom is heading into her 4th or 5th (there have been so many I can’t keep up!) round of Chemo. Her team of doctors keep switching up the type of chemo because unfortunately there is NO CURE for her type of cancer. This fucking sucks. My mom is a fighter and she has beat cancer before and will fight this no matter what it takes. But! Wouldn’t it be nice if we could raise funds to help doctors find a cure? Being a flight nurse is caring for all ages, races, religions, and creeds— a particularly challenging demographic has always been children. Children don’t possess the ability to think abstractly, often relying on signaling from the adults in the room. For children, illness and injury are terrifying because of the unknown. When children are hurt and require air medical evacuation, the industry attempts to keep a parent at their side for comfort and reassurance, however sometimes it’s not feasible due to safety. This leaves a very scared child. Our unit had the great pleasure of celebrating a milestone with a very special little girl today. Aly (@alysmonkeys ) is an 11 year old girl who approximately 2 years ago made it her mission to bring comfort to children. After being given her own stuffed monkey, Aly began her own ministry of ensuring other sick and injured kids had a monkey at their side during their journey. Today, Aly celebrates 100 little stuffed friends making journeys with the help of their airborne friends in blue in addition to thousands she’s helped circulate into the hospitals. I was beyond touched. As an adult, I recall my own flight in 2013 and the fear I felt. To know the empathy this little girl and her siblings have for other kids takes my breath away... So... for years I’ve never asked for anything or used my Instagram followership for anything. Today changes that. I maintain I’m no influencer, still but I’m asking you all— in my bio is a link for this awesome family’s grassroots organization. Please help them raise more money to bring more comfort to children suffering from cancer, traumatic injuries, respiratory and heart related illnesses and a variety of other things a child should never have to deal with. because as a Nurse Practitioner student I once had a preceptor tell me during an evaluation that “you bring a negative aura into the clinic” and that “No one likes working with you because you are difficult and not a team player”. 😳 In over 14 years of medical experience, I had never received any feedback that was even slightly close to this. It was a blow. A personal jab. It was an opinion shared in an attempt to make me feel inferior... and it worked. I talked today in my #instastories about some of the down sides of working in the nursing field when it comes to our “nursing culture”. It’s always been hard for me to understand and one that I’m committed to changing. I remember leaving the clinic after my preceptor gave me the above feedback and feeling like a failure...like I didn’t fit in...that I couldn’t do anything right...and that I was not going to make it thru my schooling. I literally laid in my bed that night and thought, “Heck! Getting along with people and being positive is probably my greatest strength...I must really not even know myself.” Lucky for me... I had an amazing professor who basically held my hand over the next four weeks to get me back on my feet and build my confidence. Other nurses / students who had been thru similar situations (unfortunately actually!) offered support. #DrCam who listened, understood, and debunked the opinion shared with me. Family & friends who cheered me on thruout my journey...and continue to! I’m committed to making a change for future nurses...because they are just that - THE FUTURE. Despite an apparent stability, bones are quite dynamic, being capable of regenerating upon injury with a pretty impressive velocity. On the other hand, and in order to repair damage and maintain quality, aged bones are resorbed, allowing the release of minerals stored in their structure, such as calcium, which is needed for many cellular functions. When this formation/resorption equilibrium is broken, bone structure is disturbed giving rise to disorders such as osteoporosis or osteopetrosis (bones harden becoming denser).  Bone formation is performed by Osteoblasts. Isolated, osteoblasts cannot make bones. They need to act in groups, as they depend on the formation of cell-cell junctions. These cellular groups are localized at the surface of forming bones, where they can be distinguished by their cuboidal shape. On the other hand, bone resorption is performed by Osteoclasts. These cells are specialized in eating and degrading bones, which requires many degradative enzymes.   This function is necessary because it allows bone remodelling and calcium recovery, which is then distributed through the blood. Osteoclast precursor cells accumulate in places where they perform absorption, but to do their job, they must fuse together, generating large multinucleated cells –the osteoclasts. Bats are Back! Since April, the Maine Department of Inland Fisheries & Wildlife (IF&W) in collaboration with US Fish and Wildlife and the International Union with the Conservation of Nature has continued to track emerging research regarding human-wildlife interactions and the novel coronavirus (SARS-CoV-2), particularly with regard to the care and release of bats. As research was conducted, wildlife rehabilitators in the state were prohibited from admitting bats in the case that they became exposed to COVID which could potentially decimate a critically low population. Why such a focus on bats? Unfortunately several species of bats in our region are imperiled, and as bats often come in contact with human dwellings, the novel virus poses a great risk to bats as well as humans. IF&W visited several facilities, including the Center for Wildlife, that care for distressed bats discussing disease transmission prevention protocols, new testing options, and some potential research projects specific to bats, which are underway and available for participation by Maine rehabilitators. Injured or orphaned bats are now authorized to go to one of four approved bat rehabilitation centers in Maine including the Center for Wildlife. An added bonus is that approved bat rehabilitation centers will participate in a Tufts University study that monitors and better understands potential human-to-animal transmission of SARS-CoV-2/COVID-19. We are so pleased to be working with IF&W to continue safely treating and admitting our wild bat patients! There are more than 1,100 species of bats throughout the world, with 40 species in the United States alone and additional species being discovered almost monthly. Though small in physical size, bats have a large footprint, and make-up one-quarter of the worlds mammals. The FDA estimates that bats provide over $3.5 billion of pest control to farmers each year - for FREE! Just one big brown bat is capable of eating over 1,000 insects an hour. UPDATE    I have been staying ready all year, constantly progressing and growing since moving teams. So when the call came for Milan, Italy I was ready. Whilst in camp, preparing for my fight I sustained a bad knee injury.    After scans and seeing the specialist it turns out I tore my MCL and ACL completely. I have just come out of surgery from the best doctor / knee specialist in the area Dr Tim Spalding. Today is day one on the road to recovery!    This injury, a slight stumble on my journey will give me a lot of time to work on other areas I previously wouldnt of had time to. I will come back from this a better athlete with an even stronger mindset and focus.    Without the lows of MMA, the highs wouldnt feel so great. Tricks of the Trade #16 Hemochromatosis 🩸 Excessive iron absorption through GI tract. Primary Hemochromatosis- generically transmitted. Defect in HFE gene causing a C282Y mutation. Juvenile and neonatal hemochromatosis- occurs in younger population much less common. Secondary hemochromatosis- comes from other conditions like liver disease or too many blood transfusions. More common in men between ages of 30-50. Women often get it after menopause because menstruel cycle is a natural blood letting mechanism. Symptoms include joint pain 🦴, Fatigue 😴, heart flutter ❤️, confusion. Complications - liver cirrhosis leading to liver cancer, pancreatic damage causing diabetes and skin color changes leaving a bronze hue. #celltember2020 Macrophage Macrophages are cells specialized in destruction of foreign elements, which relies on their capacity to eat particles, known as phagocytosis. Initially, this function was associated with the ingestion of pathogens, to avoid tissue damage. However, seminal observations by Ellie Metchnikoff changed this notion. Experimenting with live animals, he realized that macrophages were also capable to eat live (and dead) microorganisms as well as damaged cells, assisting tissue remodelling. All these observations lead him to propose a key role of phagocytes in maintaining the equilibrium of the organism, or homeostasis. Extremely visionary, Metchnikoff also proposed that in vertebrates, phagocytosis evolved from a primitive function, originally aimed for nutrition. Notably, most of his observations were confirmed over the years, leading to him being considered as the father of cellular immunology. However, life was not simple for Metchnikoff. His character and challenging ideas were controversial. In addition, he had a hard life and tried to commit suicide a couple of times. He also had vision troubles, which affected his research. His cellular theory of immunity (1883), in which immune cells would act as an army to defend against infectious agents, was highly criticized by the scientific community. Few years after his essay, scientists led by Paul Ehrlich (Celltember Eosinophil) identified soluble factors present in blood (antibodies) as responsible for the immune response. These two theories were opposed in a fervent war. In 1903, the war ended when scientists showed that soluble factors were capable of increasing phagocytosis, demonstrating the complementarity between these soluble and cellular theories. Peace was finally acted in 1908, when Ehrlich and Metchnikoff received the nobel prize of medicine for establishing the basis for the functioning of the immune system. Today is Physician Suicide Awareness Day, and I am joining the @drstephanydo #ibattlewithyou campaign. A few years ago, I started working with @colibriri @eleanorcastr0 @thehomerekha because we were worried about mental health of ourselves and classmates. The statistics are terrifying. 21.7% of healthcare workers have seriously considered taking their life in the last 30 days (23% of residents). 1 in 4 med students are depressed or anxious. 1 in 9 consider suicide. 74% of physicians experience burn out. 1 takes their life every day. This needs to change, and honestly, its not much better in graduate school. Theres so much pressure to be perfect and to not talk about mental illness. We cant keep moving forward on this path. Lyndsey was diagnosed with a medulloblastoma brain tumor just a week after her 12th birthday in 1997. She underwent brain surgery, but only part of the tumor was removed because it was located on her brainstem. At that time, treatment for medulloblastoma had a low survival rate and those kids who did survive had a low quality of life. Lyndsey was told she would have a hard time riding a bike or a horse after treatment, and that she would be unlikely to have children. Lyndsey was the first patient to participate in an experimental treatment. She endured six weeks of radiation followed by 8 rounds of chemotherapy over the following year. The treatment she received on the study run by Dr. Nick Foreman is still used to treat a majority of kids and teens diagnosed with medulloblastoma. Lyndsey says, “Over time, I learned how to ride both a bike and a horse again. It is officially 3 years ago today that I had a kidney and pancreas transplant at Addenbrookes Hospital in Cambridge. 47 Staples in my stomach from just below my chest down to the pubic bone and around the belly button as the kidney is inserted into my left side and the pancreas into the right side near the pelvic area of my stomach. This has lead to other complications with my spinal cord which caused me to have a spinal infarction that the T4 area of my spinal cord when I awoke I was paralyzed from the chest down still recovering and able to walk again and move but just cant walk very far and also lost some feeling. Tess recently shared some highlights of her nursing career along with some insights into working through this global pandemic. ⠀ “I started my nursing career as a 19 year old in January 1985 at St V’s East Melbourne. I just turned 55. That’s 35 years of doing something I love. Back then, in the 80’s, the feel of the hospital was welcoming and friendly - much like it is today; and I formed lifelong friendships with fellow nursing students who I still see and keep in touch with today. ⠀ I am now the Nurse Unit Manager on the second floor where we care for medical and orthopaedic surgical patients. Most recently, I have had the privilege to be part of the amazing team caring for aged care residents. Durante il mio periodo a Villa Turina era ricoverata una ragazza,Giulia, con cui avevo molte passioni in comune(atletica, stessa musica ecc) e con cui strinsi un forte legame. Mi sogno ancora ogni tanto, quando dalla mia finestra, mentre studiavo matematica (😩), vidi che la portavano via in ambulanza e la sua mano bianca cadaverica che penzolava fuori dalla barella... Fu la prima volta in cui, vedendo altri, mi resi conto della gravità della situazione e mi aggrappai alla vita con le unghie e i denti perché ne io ne le altre ragazze meritavamo tutta quella merda. Accettare di avere per mesi un sondino che dal naso scende fino allo stomaco e rende difficile la più banale cosa, come sedersi sul wc, non è stato facile. Ma è stato il più consapevole atto di amore che abbia fatto per me stessa. Volevo mettere fine al mio stillicidio. In questa foto non sono debole, sono forte davanti alle mie fragilità. La forza autodistruttiva che hai dentro non la puoi guarire. Io imparato negli anni a trasformarla in determinazione e consapevolezza per raggiungere i miei obiettivi. Many things in life are out of our control. There are so many times that I feel helpless in a situation. With my son’s illness I have felt helpless more days than not. I committed to this cancer screening months ago. I could have cancelled. They would have understood. But I was helpful today and it felt good. Being a dermatologist is not “work” for me. When my mom passed away in January, I was lucky to find Imagine as an outlet for support during my grief. It provided me with a community I needed during such a difficult time; people who could understand exactly what I was going through. Then COVID happened. I am a step-down nurse in a hospital, so I became a nurse during this pandemic. I was quickly overwhelmed with the weight of so many sick and unstable patients, not knowing if and when things would ever get back to normal. The added anxiety and fear while driving to work was taking such a mental toll on my fellow coworkers and me. Grateful post! So 2 yrs ago doctors incidentally found a 4mm mass in my brain in an mri (it explains a lot about my personality 😆) I had a 1 in 3 chance that it could be a malignant tumour...33.333% recurring to be exact. Anyway it turned out that I had won the brain mass lottery because it was benign. Point is this 1) Community: my tribe banded together in my hour of need. The school made my kids lunches for 2 weeks straight while I recovered; an initiative led by them. Meals were shipped to my home from far and wide from friends and friends of friends. 2) Resilience: Reminiscing about this the other day with my new pal 😘 I remembered how much nothing else mattered except the welfare of my children. Death didnt scare me (which is surprising because I used to fear it a lot). Dying wasnt the issue, leaving my kids without a mother was. The journey taught me Im stronger than I thought I was. 3) Gratefulness: Heres a dose of reality. It does not take long to become ungrateful. The only way to remember for me was... 4) Meditate: Something I need to get back in to because I got really good at it. Basically it just allowed me time away from reality to reflect, be grateful and/or present. It grounded me and I need to do it more. Be f-ing grateful you have a body that allows you the gift of movement. Movement produces oxygen for our brain, promotes growth of new cells and helps create new synapses which naturally assists our neurons connect with one another. So... Lungs and RA can be mild or complicated... I guess I sit in the complicated department. I have learned to just go with the flow! As most of you see, I appear to be THRIVING (especially in my @thrive_construction shirts) on most days!😉) Like a lot of us, there are so many aspects to this disease that people outside of our homes would never know. I don’t show you guys every minute of my day, but I do talk a lot about my daily routine and what can be expected! People who develop RA-related scarring and interstitial lung disease often have a poor prognosis. That sounds terrible right “poor prognosis!”🙄 Thanks for that... If that didn’t sound awesome to you, then this next line might shock you! According to research that looked at a number of studies, the median survival rate for people with interstitial lung disease due to RA was 3.2 years to 8.1 years from the time of diagnosis.🤔This use to freak me out, but honestly I stopped worrying about the years, what doctors say, and just concentrated on ME and what I want from this one chance we get! I try my imperfect best every day! For those of you who might think I am crazy for my push up the peak or for attempting to compete wearing high heels, this might give you a bit more insight to my why! My lungs have presented more problems in the last couple of years, but I am committed to being a warrior, so let’s just end this post with that! Does this look silly? I wish this were a joke!! Unfortunately due to a massive shortage of personal protection equipment, this is what doctors on the front line are wearing. #cdcguidelines recommend covering your hair, wearing protective glasses, and wearing a mask. However since we dont have enough equipment, this shower cap, swim goggles, and bandana is CDC approved and what many health care workers will have to wear for protection. Again, this is what we health care providers have to wear NOT because it is the best way to prevent #covid_19 spread or because its the best way to protect ourselves as health professionals, but because there is NOT enough medical equipment for nurses, surgical techs, EMTs, PAs, MAs, medical trainees, doctors, etc. . I am scared to practice medicine. My husband and I are both doctors. We are doing everything we can to protect ourselves, each other, our children, our parents - but it may not be enough. If your doctors are afraid, worried, or scared then you should be too. If you dont know of anyone who has been affected by Covid-19, you will. If you dont know of anyone who will die from Covid-19, you will. This is not some sort of scare tactic- this is our new reality. This is meant to warn you to take this seriously, like really seriously! . We are working hard to protect you and your family. Reposted from @drlorashahine “I Just Want My Body to Work” -anyone with PCOS One of my first patients with Polycystic Ovarian Syndrome taught me so much about the emotional impact of this common endocrine disorder. Early in my training I was doing what I was taught - PCOS patients either want to get pregnant or not and since this patient wanted to start her family I was focusing on the medications we could use to get her ovaries to recruit and release an egg to help her conceive. I was drawing a picture of an ovary and pituitary gland, showing with arrows how the meds would impact the hormones in her body to allow for ovulation - I was so proud of my drawing and teaching that I missed her tears for a few minutes. 👩🏻‍⚕️“I just want my body to work - I don’t want to take medication’.. I reassured her and comforted her. As a fertility doctor I viewed PCOS as a wonderful ‘problem’ to have because there are eggs to work with and medications to help recruit - I was so happy to have solutions to her problem and ways to help her start her family BUT that’s not the whole point. She helped me realize how important it is to explain the signs and symptoms she was having in relationship to hormones. She taught me about the emotional impact of being labeled with a diagnosis. She taught me how scary it can be to not understand why periods do not come regularly. ❤️I now tell patients “PCOS is not your fault, this is whats going on in your body, and here is what we can do to help. PCOS patients have a higher rate of anxiety and depression and it’s important to address while caring for them. It’s more than a medical diagnosis with a quick solution. Today at 4:30 a.m. - i held my moms hand as she took her last breaths just as i promised i would and she got her angel wings. ❤️ She left us peacefully while listening to Elvis - Crying in the Chapel & the Commodores Easy Like Sunday Morning. ❤️ Although my sister and I are heartbroken - we know we made the right decision in letting our Mom go because she was too much of a strong woman, a rockstar and a no Bullshit kind of a broad (yes that’s where i get it from - if you were wondering) to be living her life on life support and feeding tubes. ❤️ And we find such comfort and peace knowing she is NOW reunited with my dad - the love of her life for 56 years before his passing 8 years ago. ❤️ We are so overwhelmed by the outpouring of love, support and prayers we’ve received during this time. Clinic update! Fall quarter started 2 weeks ago and its been go go go ever since. We didnt have a break between summer and fall so I ended up having 8 clinic shifts in 9 days which was exhausting. And my shifts were in-person this past week and whew, they kicked my butt! I was so sweaty my face shield kept slipping down over my eyes. 🙈 BUT I learned so much this week. Some of the highlights were creating a natural treatment plan for anxiety, recommending antibiotic and natural therapies for a UTI, identifying the source of someones hip pain, and adjusting an upper back. I have three clinic shifts this term and a preceptorship with a naturopathic doctor in the community, so there will be lots more stories to come but for now, Im going to enjoy my day off from clinic and make some pancakes 🥞 Do you have any questions about naturopathic clinical education? Sous cet équipement qui cache mes émotions, il peut parfois se retrouver toute la detresse, le malaise dun monde qui est le mien, dont je fais partis, celui de lhôpital public. Mais rassurer vous, aujourdhui, sur cette photo, sous ce masque se trouve de la joie, un bonheur, la réjouissance de travailler chaque jour pour vous, à votre service, au côté de plusieurs corps de soignants, toujours aussi solidaire, ayant une force de croire à la vie, et ce à chaque instant. Purtroppo non ho molte foto che mi ritraggono dietro le quinte, ma non credo ve ne siano altre più esplicative di queste: il prima e il post covid. Libertà e contatto sono le cose che più mi mancano e vi posso assicurare che lavorare (soprattutto in piena estate) con questi dispositivi non è affatto semplice. Lutilizzo di tutti i DPI ha imposto delle barriere tra mamme e painter, ma se questo vuol dire aver cura del cliente e lavorare in sicurezza, ben venga. NB. Se la seconda tenuta vi sembra troppo esagerata, fidatevi, non è così!
