The patient accidentally got his index finger caught between a trailer and a hitch, splitting his finger down the midline of the distal phalanx. Fortunately, the surgeons were able to save his finger, but he will be left with limited movement and feeling in the distal portion of the finger. Recovery from a fingertip injury may take several months. After your injury heals, mild to severe pain and sensitivity to cold may continue for up to a year or may even be permanent. Your doctor or a hand therapist may recommend specific exercises to improve range of motion and strengthen your hand and fingers. They may also suggest additional therapies to help promote healing and function. These therapies may include: heat and massage, fingertip desensitization, electrical stimulation of the nerves in the hand, activities to improve fine motor skills such as pinching and grasping. Maxillofavial Trauma Management - Airway ———————————————————————— After we handle the immediate life threats (massive hemorrhage), we have to consider the airway. Look, listen, and feel: take a look at the injuries and look inside the mouth for blood, vomit, broken teeth, dentures, chewing tobacco, etc. If any of these are present, grab your suction and clear it out. If dismounted or without mechanical suction, grab a 60cc syringe with a cathedral tip and a put an NPA on it to create a field expedient manual suction device. Give the lungs a listen and see if you’re getting adequate air movement. If the environment is keeping you from listening, place you’re hands on the chest and feel for bilaterally equal rise and fall of the chest. ———————————————————————— I would personally prefer to skip the intubation attempt and go for a surgical cricothyroidotomy. Orotracheal and nasotracheal intubation may be attempted but be aware that musculoskeletal tissue disruption will likely increase the difficulty of intubation. LMAs and I-Gels can be used to buy you some time to prep for a cric, but they don’t prevent regurgitation and aspiration of vomitus and may increase vomiting unless a paralytic has been administered (which is unlikely due to the fact that we likely haven’t gained vascular access at this point in our assessment). Ancef 2g) quantify energy pattern--wound size, fracture pattern, soft tissue injury size, contamination, neurovasc injury present. GUSTILO & ANDERSON CLASSIFICATION TYPE 1 - wound1cm, minimal damage TYPE 2 - wound 1cm, moderate soft tissue damage, minimal periosteal stripping TYPE 3a - severe soft tissue injury, contamination with adequate coverage TYPE 3b - severe soft tissue injury, contamination, inadequate coverage TYPE 3c - arterial injury needing repair 3. NEURO COMPROMISE 4. COMPARTMENT SYNDROME: can lead to vascular compromise, neuro injury or necrotic tissue. Risk of cellular destruction and hyperkalemia. 5. HIP DISLOCATIONS (not if prostetic) 6. OSTEO/SEPTIC JOINTS 7. PELVIC UNSTABLE FRACTURE: risk of serious vascular/neuro injury. May be life threatening bleeding. Place a pelvic binder. Will need examination of vaginal vault, male genitals, and/or rectal exam. May need massive transfusions and interventional radiology intervention. A 58-year-old male person was admitted to the Emergency Clinical Hospital Bucharest, approximately 7 hours after a traumatic injury of the left leg amputated by the blade of an electric saw. Replantation surgery of the left leg was performed under general anesthesia with ischemic time lasting 9 hours up to the arterial anastomosis. Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued.
