- Update to my cousin’s foot injury. He has it on a vac that gets changed every six hours. I’m blown away by the progress it has made so far.
UPDATE on Israel 💚 • Israel is slowly but surely making progress. He’s sitting up in a wheelchair everyday. He’s been able to go outside for the first time this week. He also is able to push the wheelchair with his one good hand! • He has a long journey ahead of himself and a lot of healing to do. The doctors and nurses have his pain regulated. He’s being moved to Health Bridge Children’s Hospital for physical therapy and rehabilitation. • Thank you for all your prayers and support. It’s been difficult but the baby helps bring so much joy to our family. Israel should be able to see his sisters and niece very soon! Please pray that he would be filled with joy. Dont worry, alles is weer onder controle! Je schrikt toch even als de baby dagenlang non-stop de samba aan het dansen is en de volgende dag opeens niet meer beweegt.. 2 stemmen in mn hoofd: 1)stel je niet aan en 2) neem je verantwoordelijkheid!! Toch maar even mn verloskundige gebeld. Zij kwam direct langs en stuurde me door naar het ziekenhuis waar ik 45 min aan de monitoren werd gelegd. Uiteindelijk bleek alles goed te zijn, maar Ali had gewoon even een rustdagje genomen en was gedraaid. Iedereen in het ziekenhuis was zo lief en zei dat je bij twijfel altijd langs moet komen, ookal is het 20x. Dus mensen bij twijfel over gezondheid: altijd aan de bel trekken!! - A rare look into a pediatric heart transplant procedure. The recipient in this case is a 13 month-old patient with congenital heart disease.
Today’s case we have this 20 year old male who was involved in a motor cycle accident! The poor guy was just riding along when he was tboned by a car that was not paying attention. The patient was NOT wearing any safety riding equipment (including helmet) at the time of the accident. As a result, his foot was crushed between the car and the cycle. To add salt to the wound (no pun intended), the car that struck him fled the scene! . Upon arrival to the ED, trauma protocol was followed as this mangled foot could just be a distracting injury. Luckily, no other injuries were found, including on the helmet-less head and neck! They found his foot to have a double lisfranc fracture (displacement of the metatarsophalangeal joints due to fracture or injury), dislocated navicular bone and dislocated and fracture talus. In this case, they were able to save his foot! Due to the extensive damage to both bone and muscle, however, it will take many monthly to years to be able to walk normally again! . I propose a question to everyone...would you rather having this outcome whereas your foot is saved, but, even after possibly years of rehab, you may not be able to reestablish full motor control and sensory back to the foot OR have the foot amputated and just learn how to walk with a prosthetic? Stupid hurts. Sunday, I banged my shin on a bike rack in the garage. Started bleeding like crazy. I didn’t want to pay for co-pay at urgent care so I came up with my own wrap. Was still bleeding a day later so then I had to go to urgent care anyway...and after paying the co-pay they sent me to the darned ER! And then the ER doc said too much time had passed so they couldn’t do stitches. So now I paid for urgent care and ER visit and yet got nothing but some over priced band aids. 4 years ago I jumped out of my bedroom window to escape a home invasion. I shattered my heels and fractured my back and broke both hands and ended up in the ICU. I experienced pain and suffering I didn’t even know existed. After two weeks in the hospital and several surgeries I was bound, isolated in my home for months watching everything I held dear and had worked for, dying and disappearing. While my body was fighting for it’s life, the trauma separated and shot my higher self out into the universe and I went on a separate journey where I discovered a kind of spiritual awakening, a pilgrimage that I am thankful for today. Sometimes I lay awake at night and ask myself “What should I do with this knowledge God has given me?” I have had an intense need to figure it all out and have a concrete answer and to be fully healed. The truth is, healing is a life long journey. And it’s the little steps along the way, the little changes and the little signs that are transformative. Healing does not have a finish line. It’s the planting of a seed, the struggle through the soil, the birth of the bud and the first breath of the morning dew, the warmth of the sun rays, the fight through the rain and the pain of bursting into bloom and rejoicing into beauty, just to whiter, and then…grow again. What I’ve learned so far, is to practice healthy boundaries. I have learned self compassion and to have patience with myself. There are days when I am a fully blossomed sun flower and days when I recluse into a nocturnal cactus. And there are days I am hidden in the dirt. Today it’s 4 years since my life changed forever and today I am thankful for it all even though I am still dealing with PTSD. People often say:”I don’t wish my pain on my worse enemy” I say: “I do”, Because the worst experience in my life turned out to be the most enlightened and important lesson God could ever give me”. I am grateful. I am blessed. I am God’s child and I was given a journey which gave me an experience that transformed me and made me a better person and in the end, made me shed my false self and discover my true self. A 22-year-old woman was out tubing with her friends in Fort Lauderdale when a shark sunk its teeth into her right leg. The fish – thought to be a bull shark – also whacked her on the head with its fin before she had any idea what was even happening to her. She was rushed to shore where she was taken to the hospital and had a two-hour operation to repair the damage to her leg. Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued. 📚🔬 Taping for Patellofemoral Pain Syndrome ℹ️ℹ️ BACKGROUND: Patellofemoral pain syndrome (PFPS) is characterized by anterior knee pain and/or pain in the retro-patellar and/or peri-patellar region. ℹ️ Pain typically increases with activities such as: Squatting. Kneeling. Stair climbing. Prolonged sitting. ℹ️ The underlying causes of PFPS are multifactorial and may be associated with biomechanical and or neurophysiological changes at the pelvis, hip, knee or ankle regions. ℹ️ Taping is frequently used as part of the multi-modal management for PFPS, including: McConnell Patellofemoral Joint Taping (PFJT). Tibial Internal Rotation Limitation Taping (TIRLT). ℹ️ Clifford et al. (2020), investigated the effects of TIRLT, PFJT and no taping on perceived pain and lower limb kinematics during a lunge and single leg squat (SLS) in people with PFPS. 🔬🔬 METHODS: This cross-sectional study compared the effects of TIRLT, PFJT and no taping, on knee pain and lower limb kinematics during two pain-provoking movements in people with PFPS. 🔬 Participants with PFPS (n = 23) performed a lunge and SLS under three randomised conditions: TIRLT, PFJT and no taping. 🔬 The Codamotion system captured and analysed lower limb kinematic data in the sagittal, transverse and coronal planes. 🔬 Peak knee pain intensity during the movement was assessed using the Numerical Rating Scale (NRS). 📊📊 RESULTS: Participants reported significantly less pain with the TIRLT and PFJT techniques compared with no tape during the lunge and SLS. 📊 There was no evidence of altered lower limb kinematics accompanying pain reductions with either taping technique. ✅✅ CLINICAL SIGNIFICANCE: Both forms of taping may be useful adjuncts as the short-term benefit of pain relief may enable participation in more active forms of rehabilitation. 🧠🧠 Thoughts? Traumatic injury of the left foot! A 58-year-old male person was admitted to the Emergency Clinical Hospital Bucharest, approximately 7 hours after a traumatic injury of the left leg amputated by the blade of an electric saw. Replantation surgery of the left leg was performed under general anesthesia with ischemic time lasting 9 hours up to the arterial anastomosis. Metallic osteosynthesis and suture of the blood vessels, nerves and tendons were performed. On the initial physical examination this person presented no inflammatory signs of the skin cicatrisation process after replantation surgery, no edema and no pain syndrome.
