Traumatic injury of the left foot! A 58-year-old male person was admitted to the Emergency Clinical Hospital Bucharest, approximately 7 hours after a traumatic injury of the left leg amputated by the blade of an electric saw. Replantation surgery of the left leg was performed under general anesthesia with ischemic time lasting 9 hours up to the arterial anastomosis. Metallic osteosynthesis and suture of the blood vessels, nerves and tendons were performed. Lawnmower injury 🔪 shown here is a partial amputation of the 1st, 2 and 3rd phalanges (toes) of the right foot. These types of injuries are quite common in trauma and typically result in amputation due to the extensive damage caused by the lawnmower blades. Initial treatment includes Betadine rinse, Tdap vaccine if 10 years, and pain management. The patient was then treated with IV antibiotics and taken to the OR where an attempt to salvage was performed with pinning each toe. Ultimately, amputation of the 1st toe was preformed. Maxillofavial Trauma Management - Airway ———————————————————————— After we handle the immediate life threats (massive hemorrhage), we have to consider the airway. Look, listen, and feel: take a look at the injuries and look inside the mouth for blood, vomit, broken teeth, dentures, chewing tobacco, etc. If any of these are present, grab your suction and clear it out. If dismounted or without mechanical suction, grab a 60cc syringe with a cathedral tip and a put an NPA on it to create a field expedient manual suction device. Give the lungs a listen and see if you’re getting adequate air movement. If the environment is keeping you from listening, place you’re hands on the chest and feel for bilaterally equal rise and fall of the chest. ———————————————————————— I would personally prefer to skip the intubation attempt and go for a surgical cricothyroidotomy. Orotracheal and nasotracheal intubation may be attempted but be aware that musculoskeletal tissue disruption will likely increase the difficulty of intubation. LMAs and I-Gels can be used to buy you some time to prep for a cric, but they don’t prevent regurgitation and aspiration of vomitus and may increase vomiting unless a paralytic has been administered (which is unlikely due to the fact that we likely haven’t gained vascular access at this point in our assessment). Some of you may or may not know In 2012 I was involved in a major motocycle accident that almost ended my life a red P plater drunk and high on drugs ran a red light coming off a freeway and T boned me my left leg and other bones were completely crushed and broken Some of the injuries were toes broken shin bone broken in 3 places dislocated hip pelvis out of alignment femoral shaft (thai bone) broken in 3 places broken wrist and fingers radius (forarm bone) broken elbow dislocated and many others this is just of the top of my head 5 operations later I now have many screws, and pins and rods holding my leg and bones together For almost 2yrs I was unable to walk properly I moved around my house in a office chair and did the baby bum shuffle on the floor lost over 40+ kg went down to 60/70kg looking like skin and bone I got even more depressed looking at myself because of how small I became so I started eating pizza daily KFC and what ever shit food I can find to get my weight back up. Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. This is his story: I went to the emergency room because I was extremely dehydrated. When I explained to the E.R. doctor whats going on, he wanted to make sure that lupus wasnt attacking any of my organs... which is totally understandable. ▪ I did blood work, EKG, X-rays and two sonograms. When the results came back from the EKG it showed that something weird is happening to my heart. The doctors dont know if its caused by lupus or what... so now, I have to do a cardiac catheterization (as soon as possible) to find out whats going on with my heart. However, seeing how this world has become, and seeing more and more people live their lives in fear in each moment has made me realize maybe sharing this experience could help others see life in a new perspective. Just a little while ago, I was hit by an 18 wheeler going way above the speed limit. It was only by the shear grace of God that I was able to walk away from this accident with hardly anything but a scratch. STOP the bleeding, and then take a breath and figure out the next steps... Repost from @precisiontacmed • Another TQ save. This case was sent to us from a colleague. The patient punched a glass window and sustained a large laceration. Initially he presented to the ER and the physician tried to control bleeding with hemostats. This proved in effective. But luckily our colleague was present to quickly apply a tourniquet and then transport/transfer him to a local trauma center for surgical intervention. Tips: Hemostats work well for clamping vessels but it can be extremely difficult or impossible if you cannot adequately visualize the vessel. This requires you to have PROXIMAL control for arterial hemorrhage. You can have a tourniquet to control the hemorrhage or you can surgically expose the vessel more Proximal away from the area of trauma to have a clean field to work. Hi everybody…. I know there is so much going on in the world right now. This is a crazy, broken, yet beautiful time. And in the midst of all of that - in the midst of all of our awareness and becoming and learning to love and see each other - I’ve had this really unique experience of having an unanticipated open heart surgery. The only way I can explain my experience would be to ask you to imagine a non-runner who was signed up for a marathon. I didn’t want it, but I had to have it anyway and it was a week ago Wednesday. And as people heard about the surgery I started getting messages: “I’m praying for you” …“I’m praying for you”. People I worked with decades ago, people who have come to my concerts or listen to my music, my work family, people on social media, and my own friends and family all offered their prayers. And now, ten days later, I just want to say, from the moment I went to the hospital, if it really were a marathon race, I felt like I got into that runners block and as soon as it was time for the race to start there was this massive West Texas wind at my back.. just pushing me through. Even stuff I was really scared about felt like nothing more than just a deep breath and something supernatural pushed me through it. My recovery has honestly felt miraculous. And so I want to say thank you to each person who said a prayer for me. Prayer changes everything. Prone ECGs in COVID19 ICU Patients Many hospitals are obtaining 12-lead ECGs in prone-ventilated patients by placing leads on the patient’s back, posterior to lead placement in the traditional supine position. The hearts of prone patients fall forward (ventrally), abut the anterior chest wall, and move caudally, away from posteriorly placed leads. The more distant position of the heart from the leads, coupled with greater impedance from soft tissue (including disease lung in COVID19 patients) and bone interposed between myocardium and electrodes causes diminished QRS amplitude in leads V1-V5 and prominent Q waves in leads V1-V3, which may be misinterpreted as an anteroseptal infarct, unless the positioning is properly annotated. A 22-year-old woman was out tubing with her friends in Fort Lauderdale when a shark sunk its teeth into her right leg. The fish – thought to be a bull shark – also whacked her on the head with its fin before she had any idea what was even happening to her. She was rushed to shore where she was taken to the hospital and had a two-hour operation to repair the damage to her leg.
