It is shaped like a figure 8 and inflates proximal to distal one order to seat itself well within the mitral annulus. The primary goal is to “crack” the fused commissures of the rheumatic mitral valve. Access is obtained via the femoral vein and a transseptal puncture is used to gain access to the left atrium. . . . Severe MS is defined as: . ⭐️MVA 1.5 mm2 . ⭐️Mitral mean gradient of 5-10 mmHg . ⭐️keep in mind Afib and PH . . 🧮Mitral valve area calculation from P1/2t (220/PHT) is the most useful way to assess severity as MS is very load dependent and can change with varying heart rates. . . . 📚Other things to remember when assessing a patient for this procedure is the Wikin-Abascal score which looks at valvular calcification, leaflet thickness, leaflet mobility, and subvalvular thickening. A score of 8 is indicative a success for PBMC. To understand the protocol developed by the AHA one has to understand first the 𝘮𝘪𝘤𝘳𝘰𝘣𝘪𝘰𝘭𝘰𝘨𝘺 of the area in question; the common pathogen for instance for skin or GI procedures is different than the mouth. ❌ T𝐫𝐚𝐧𝐬𝐢𝐞𝐧𝐭 𝐯𝐢𝐫𝐢𝐝𝐚𝐧𝐬 𝐠𝐫𝐨𝐮𝐩 𝐬𝐭𝐫𝐞𝐩𝐭𝐨𝐜𝐨𝐜𝐜𝐚𝐥 𝐛𝐚𝐜𝐭𝐞𝐫𝐞𝐦𝐢𝐚 may result from any dental procedure that involves manipulation of the gingival or periapical region of teeth or perforation of the oral mucosa. Generally speaking in 𝐡𝐢𝐠𝐡 𝐫𝐢𝐬𝐤 𝐩𝐚𝐭𝐢𝐞𝐧𝐭𝐬 𝐎𝐍𝐋𝐘 An antibiotic for prophylaxis should be administered in a single dose before the procedure. ✳️ A𝐦𝐨𝐱𝐢𝐜𝐢𝐥𝐥𝐢𝐧 is the preferred choice for oral therapy because it is well absorbed in the GI tract and provides high and sustained serum concentrations. ▶️2 grams of amoxicillin given orally as a single dose 30-60 minutes before the procedure as the drug of choice. ✳️ For individuals who are allergic to penicillins or amoxicillin, the use of 𝐜𝐞𝐩𝐡𝐚𝐥𝐞𝐱𝐢𝐧 or another first-generation oral cephalosporin, 𝐜𝐥𝐢𝐧𝐝𝐚𝐦𝐲𝐜𝐢𝐧, 𝐚𝐳𝐢𝐭𝐡𝐫𝐨𝐦𝐲𝐜𝐢𝐧, 𝐨𝐫 𝐜𝐥𝐚𝐫𝐢𝐭𝐡𝐫𝐨𝐦𝐲𝐜𝐢𝐧 is recommended. ✳️Patients who are unable to tolerate an oral antibiotic may be treated with 𝐚𝐦𝐩𝐢𝐜𝐢𝐥𝐥𝐢𝐧, 𝐜𝐞𝐟𝐭𝐫𝐢𝐚𝐱𝐨𝐧𝐞, 𝐨𝐫 𝐜𝐞𝐟𝐚𝐳𝐨𝐥𝐢𝐧 administered intramuscularly or intravenously. ✳️ For Pen-allergic patients who are unable to tolerate an oral agent, therapy is recommended with parenteral c𝐞𝐟𝐚𝐳𝐨𝐥𝐢𝐧, 𝐜𝐞𝐟𝐭𝐫𝐢𝐚𝐱𝐨𝐧𝐞, 𝐨𝐫 𝐜𝐥𝐢𝐧𝐝𝐚𝐦𝐲𝐜𝐢𝐧. We came upon a parking attendant who told us that we could park over there and run over to the other side of the building. We both cant run, so I drove him over there and he hopped out of the car to do his errand. After that I went to the doctor for my 6 week checkup and he said that I could now sleep without my back brace on We both immediately started reminiscing about the time we got to ski the Grand Envers with Cédric Aubert. Intrauterine Insemination (IUI) is a fertility treatment that involves placing sperm inside a woman’s uterus to facilitate fertilization. The goal of IUI is to increase the number of sperm that reach the fallopian tubes and subsequently increase the chance of fertilization. IUI provides the sperm an advantage by giving it a head start but still requires a sperm to reach and fertilize the egg on its own. It is a less invasive and less expensive option compared to In Vitro Fertilization. When is IUI used? The most common reasons for IUI are low sperm count or decreased sperm mobility. However, IUI may be selected as a fertility treatment for any of the following conditions as well: • Unexplained infertility • A hostile cervical condition, including cervical mucus problems • Cervical scar tissue from past procedures which may hinder the sperms’ ability to enter the uterus • Ejaculation dysfunction How does IUI work? Before intrauterine insemination, ovulation-stimulating medications may be used, in which case careful monitoring will be necessary to determine when the eggs are mature. The IUI procedure will then be performed around the time of ovulation, typically about 24-36 hours after the surge in LH hormone that indicates ovulation will occur soon. A semen sample will be washed by the lab to separate the semen from the seminal fluid. A catheter will then be used to insert the sperm directly into the uterus. This process maximizes the number of sperm cells that are placed in the uterus, thus increasing the possibility of conception. The IUI procedure takes only a few minutes and involves minimal discomfort. The next step is to watch for signs and symptoms of pregnancy. What are the risks? The chance of becoming pregnant with multiples is increased if you take fertility medication when having IUI. E a intubação orotraqueal é sempre fácil de ser realizada? Não! . Alguns animais apresentam pouca abertura da cavidade oral, focinho comprido e com isso a visualização da glote torna-se mais desafiadora. . O que não pode acontecer é deixarmos de tentar e fazer o nosso melhor para que a intubação seja efetivada! . A intubação orotraqueal durante uma anestesia não serve apenas para que ocorra a anestesia inalatória. Ela é um modo seguro de mantermos patentes as vias aéreas dos nossos pacientes, evitando colapsos, aspirações (cuff inflado!), além de facilitar a oxigenação e a ventilação assistida caso seja necessário. . A posição esternal acaba sendo a mais comum nas rotinas, mas não é a única. Podemos intubar com o animal em decúbito lateral, e o modo mais fácil de intubar sem auíxilio de outra pessoa, é no decúbito dorsal (como na primeira foto!). . Então a dica de hoje é: treinem a intubação em outras posições, saiam da zona de conforto e treinem também os colegas que trabalham com você! Uma intubação realizada de modo correto e no tempo certo, pode salvar vida! . O treinamento permite que a gente vá aperfeiçoando a técnica e nos traz mais segurança em situações desafiadoras. . . VITAL PULP THERAPY because It’s not all about anteriors in real life dentistry !!! Big decay on tooth 17, no pain, sensitivity test is normal. Partial removal failed. I went for full pulpotomy and direct pulp capping with bioceramics (total fill cement and total fill bc putty fast set) and direct composite as long term restorative treatment. To me, premixed putty bioceramics take the huge advantage that it can be drilled with no setting time. So much faster than MTA or Biodentine. More expensive also !!! But time is even more...sorry for the pixels, screenshot from my screen. Too excited to wait and postprod the video ! Very happy with my new video set ! Splenic laceration It may not look like much, but this can be a deadly injury if not dealt with quickly and definitively. This injury is also one of the most frequent reasons for requiring emergency surgery after blunt force trauma. The spleen is part of your immune system that besides being a filter for old cells circulating in your blood, it also is the main source of the body’s Complement system, which enhances your body’s ability to fight off a specific type of bacteria. Therefore, while you can live without your spleen, one of the trade offs is that you will require ongoing vaccinations against the most common of these organisms. Keratoprosthesis is a surgical procedure where a diseased cornea is replaced with an artificial cornea. Traditionally, keratoprosthesis is recommended after a person has had a failure of one or more donor corneal transplants. More recently, a less invasive, non-penetrating artificial cornea has been developed which can be used in more routine cases of corneal blindness. While conventional cornea transplant uses donor tissue for transplant, an artificial cornea is used in the Keratoprosthesis procedure. The surgery is performed to restore vision in patients suffering from severely damaged cornea due to congenital birth defects, infections, injuries and burns. Keratoprotheses are made of clear plastic with excellent tissue tolerance and optical properties. They vary in design, size and even the implantation techniques may differ across different treatment centers. The procedure is done by ophthalmologists, often on an outpatient basis. Although many keratoprostheses have been developed, only four models are currently in commercial use: the Boston keratoprosthesis, Osteo-Odonto-Keratoprosthesis (OOKP), AlphaCor and the KeraKlear Artificial Cornea. Low back pain is among the leading cause of disability globally, and nearly everyone will experience this at some point in their lifetime. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. This situation is called cauda equina, and is a neurosurgical emergency. Isso começou 1 mês após a instalação de um piercing metálico em sua língua. Um episódio típico começou com dor paroxística do lado direito na região maxilar (V2) e mandibular (V3), seguida por hipoestesia 30 segundos depois. Os episódios foram descritos como “choques elétricos”, com duração de 10 a 30 segundos e recorrência de 20 a 30 vezes ao dia, aumentando em frequência e gravidade nas últimas semanas. Raramente ocorriam episódios quando ela estava mastigando ou falando e não a despertavam à noite. . 👉🏻 Foi considerado o diagnóstico de neuralgia do trigêmeo, e ela iniciou carbamazepina 200 mg duas vezes ao dia. A intensidade da dor não mudou, mas ocorreu com menos frequência. Após 1 semana de carbamazepina, a neuralgia do trigêmeo retornou com características anteriores; a carbamazepina foi descontinuada após 2 semanas. . 👉🏻 O artefato foi removido naquele dia e 48 horas depois seus sintomas desapareceram completamente. Um ano depois, ela continua sem todos os sintomas, com resultados normais ao exame físico. . 📍A síndrome foi provavelmente secundária a um implante metálico lingual e, embora os achados indiquem envolvimento do trigêmeo, a localização do piercing não deve ter resultado em lesão trigeminal. . 📍O mecanismo causal pode envolver irritação nervosa mecânica ou química com sensibilização secundária do tipo central e periférico. Outra possibilidade são os fenômenos galvânicos, nos quais se acredita que o galvanismo intraoral gere correntes elétricas que podem provocar nevralgia. . 🤭Marca seu amigo que usa piercing pra ele saber dos riscos!! . The MCA starts off the ICA on its first segment M1(sphenoparietal segment). Then it meets the ínsula and becomes M2 (insular segment) then goes on to the opercula (M3 segment) and turns into the cortical surface(M4 segment). At the surface the M4 MCA branches have actual first and last names... would you join me to put names to these numbers? I personally love how the brain looks in this picture...
