First I want to say, Im doing good and healing well. I struggled on whether I wanted to do a post. If I can help show my friends, colleagues, acquaintances the damage UV Rays can do then its worth it. I surely over the years spent plenty of time in the sun from just being out in the sun day to day, to going to the beach for over 30 years, and using tanning beds. Sometimes using sunscreen sometimes not. The first picture will show a small spot of skin damage that would not go away, so it was time to go to a dermatologist, it gets examined and Im given Cryosurgery, the use of liquid nitrogen extreme cold to destroy abnormal tissues. Actually did it twice & it did not go away. Next they slice it off, send it to a lab and it comes back as an Invasive Squamous Cell Carcinoma, the second most common form of skin cancer. Its usually found areas of the body damaged by UV rays from the sun or tanning beds. On Thursday I had Mohs Surgery which is considered the most effective technique, over 90+ percent for treating basal cell carcinomas (BCCs) and squamous cell carcinomas (SCCs), the two most common types of skin cancer. http://skincancer.org/skin-cancer-information/squamous-cell-carcinoma/ As I mentioned previously I am doing well, Thursday was tough after surgery when the anesthesia wore off with the throbbing of the area and entire head throbbing, now just some tenderness & can feel the tightness of the stitches that I will have in for two weeks along with a bandage I will have to be wearing on my head. I will be back to work tomorrow or Tuesday which is approved by the surgeon. You can see by the pictures that the swelling around the stitches has gone down as I heal. Quite often we receive phone calls from potential new patients who are confused because they have been referred to the DHC for headache/migraine from their current physician, but are not experiencing a “normal headache”. These patients describe their condition as “head pain” rather than a “headache”. Don’t fear, you’re in the right place! • Back of the head pain is a symptom of migraine, but it could also be a symptom common to other headache diagnoses. When these symptoms are not paired with the other frequent indications of migraine like nausea/vomiting or sensitivity to sound, light and odors it could alternatively be Occipital Neuralgia. • Occipital Neuralgia is a distinct type of headache characterized by piercing, throbbing, or electric-shock-like pain in the upper neck, back of the head, and behind the ears, usually on one side of the head. Typically, the pain of occipital neuralgia begins in the neck and then spreads upwards. • This diagnosis can be treated with other common migraine treatments such as a change in lifestyle: a healthier diet and exercise regimen. Neck pain can be relieved by heat, massage and physical therapy. If this treatment plan is unsuccessful, occipital nerve blocks or radiofrequency ablations may be an alternative action if physician deems appropriate. We are kicking off a new series about digital dentistry with the leadership of our very own @celin.arce  Digital dentistry is a term that is used a lot and encompasses many aspects of dentistry (optical impressioning, digital radiography, intra/extra-oral photography, design/planning software, CAD/CAM fabrication, digital dentures, 3D printing, etc).  For this first foray into digital dentistry, we are going to talk about applications of digital dentistry for which you combine a 3D intraoral digital model with a 3D CBCT radiograph. CBCT stands for Cone Beam Computer Tomography.  These images are used in surgery (pathology, implant placement), endodontics (location of canals), periodontics (bone visualization), TMJ (visualization of condyle in fossa) to see the 3rd dimension (buccal-lingual). CBCT differs from a spiral CT (ie those used in hospitals) due to the fact the machine operates by rotating the x-ray source/detector around the patients head rather than moving the patient through a tube.  CBCTs are used in dentistry because they produce less radiation exposure (about 10x less for maxillofacial region) and the machines are lower in cost (around $75,000-100,000) than conventional CTs. See story for comparison of radiation doses. CBCTs are saved as DICOM files (these are a standard file format for medical imaging data) and can be viewed on DICOM viewers (some free ones are Blue Sky Bio, Planmecca Romexis Viewer). Digital models can be obtained by using an intraoral optical scanner (ie 3shape, CEREC) or by digitizing a conventional impression/model (laboratory scanner).  Most intraoral scanners are now “open”, meaning that you can directly export the scan file (which is saved as an .stl file).  And there are many different software programs that can open .stl files.  If you want to edit an .stl file or merge an .stl file and DICOM file, you will need some additional software.  We will be getting more into these topics over the next couple weeks.  Stay tuned…. #dental #digitaldentistry #CADCAM #3Dprinting #dentistry #dentalstudent #odontology - @dentinaltube on Instagram

⁣Maybe its time to get that splint?⠀ ⠀ ⠀ ⠀ Sixty year old female patient referred as an endodontic emergency. She has two acutely uncomfortable teeth in the lower left quadrant and cannot isolate a culprit tooth. Small FOV CBCT notes there is a PARL associated with the root tips of 37, there is deep isolated probing defect on the distal of this tooth (the bone defect is also visible on the CBCT). Pulp extirpation performed as a temporary salve, until the patient can consider her extraction and rehabilitation options. The tooth 36 was noted to be vital and within normal limits to other clinical tests, despite the hypercementosis and osteitis condensitis. I advised against endodontic treatment for the time being, opting to keep this tooth under review and suggested she speak to her general dentist about an occlusal splint.⠀ ⠀ ⠀ ⠀ She returns two months later with pain from tooth 36, she cannot tolerate pressure and thermal fluids. She was desperate to retain this tooth, already extracting the 37 to a deep distal crack. Endodontic treatment performed over two visits. Four canals shaped and irrigated with NaOCl 5%. The canals were dressed with Odontopaste and the tooth temporarily restored. Obturation with gutta percha and AH plus sealer and access closure with dual cure Luxacore composite resin. She returned to her general dentist in two weeksto begin her rehabilitation, and following this will be getting an occlusal splint.⠀ ⠀ ⠀ ⠀ Teeth with hypercementosis and osteitis condensistis can be challenging to manage, in particular because it can be difficult to radiographically attain an accurate working length. Typically because the radiopacity of the calcifications can make it challenging to discern where the roots end. Your meniscus helps to absorb these forces so that the bone surfaces are not damaged🦵 . 👶Typically, in the younger population, your knee meniscus is usually torn traumatically, by twisting on a slightly flexed knee. 🧓In the older adult, the tear may be due to a natural age-related degeneration of the meniscus. . Meniscus tear healing is dependent upon your meniscal blood supply, which is limited. Your meniscus receives its nutrition from blood and synovial fluid within the joint capsule. Your meniscus has two distinct regions that affect their ability to heal. . 🧠The red zone (outer edges) has blood supply, which means they can heal naturally. These meniscal tears may heal without the need for surgery. The white zone (inner edge) doesn’t have a blood supply and won’t heal naturally. These injuries often require surgery to remove the torn component. . In this post we have some early stage rehab exercises that you can do following a meniscus injury to the red zone which will give it the best chance of recovery. Today we bring you a case of a 65-year-old patient with a dysphonia history of approximately 1 year evolving with a growing cervical mass. Cervical dissection was then performed using Crile (sacrifice of the accessory nerve, jugular vein and sternocleidomastoid muscle). A flap of the pectoralis major muscle was planned to cover the defect and protect the carotid artery. Note that the design and planning of the flap that involves the entire vascularization considering the thoracoacromial artery and its pedicles that irrigate this entire region. What a day yesterday. I got to spend some time with @teagueholmes and were both injured. We came upon a parking attendant who told us that we could park over there and run over to the other side of the building. We both cant run, so I drove him over there and he hopped out of the car to do his errand. After that I went to the doctor for my 6 week checkup and he said that I could now sleep without my back brace on We both immediately started reminiscing about the time we got to ski the Grand Envers with Cédric Aubert. Alguns animais apresentam pouca abertura da cavidade oral, focinho comprido e com isso a visualização da glote torna-se mais desafiadora. . O que não pode acontecer é deixarmos de tentar e fazer o nosso melhor para que a intubação seja efetivada! . A intubação orotraqueal durante uma anestesia não serve apenas para que ocorra a anestesia inalatória. Ela é um modo seguro de mantermos patentes as vias aéreas dos nossos pacientes, evitando colapsos, aspirações (cuff inflado!), além de facilitar a oxigenação e a ventilação assistida caso seja necessário. . A posição esternal acaba sendo a mais comum nas rotinas, mas não é a única. Podemos intubar com o animal em decúbito lateral, e o modo mais fácil de intubar sem auíxilio de outra pessoa, é no decúbito dorsal (como na primeira foto!). . Então a dica de hoje é: treinem a intubação em outras posições, saiam da zona de conforto e treinem também os colegas que trabalham com você! Uma intubação realizada de modo correto e no tempo certo, pode salvar vida! . O treinamento permite que a gente vá aperfeiçoando a técnica e nos traz mais segurança em situações desafiadoras. . . Let’s talk Cluster Headache! • The fall season is the most common trigger for the onset of cluster cycles. The term “Cluster” refers to the fact that these attacks happen in groups • Cluster headache is the least common but probably the most painful headache condition. It occurs mostly in men starting between age 20-40 years. During a cluster cycle, though brief, excruciating pain recurs between 1 every other day to 8 times per day lasting 15 minutes to 3 hours each attack. Cluster cycles can last from a few week to a few months and are usually separated by remission periods, which (🤞🏼) can last years! 10-15% of cluster headache sufferers experience chronic symptoms without remissions • Cluster headache manifests as unilateral (one sided) pain that occurs near or above the bone framing the eye and/or at he temple. Associated symptoms of cluster headache occur on the same side of the head that the pain is taking place: red or teary eyelid swelling, forehead and facial sweating, tearing, eyelid drooping, abnormally small pupil, nasal congestion and runny nose • A cluster headache can be successfully treated in most cases by individualizing acute(emergent) and preventative treatment. C était il y a a peine encore quelques tous petits jours et ce n est pas vraiment simple d’ en parler encore moins de trouver les mots justes(ne pas heurter) parce qu en vrai il n y en a pas ... C’était le second chapitre, celui du livre de ma vie,celui de notre histoire. cette épreuve de plus qui j’en suis sure aujourd’hui me permettra de vivre encore plus fort, de vieillir encore plus vieille (amèrement parfois) mais je l’espère de toutes mes forces. 2 ✌🏻comme 2 ovaires /2 trompes comme 2 seins que j’ai sacrifié dans cette bataille mais surtout 2 comme seconde chance : d’avoir eu cette double chance de donner la vie à mes 2 bébés mes 2 flammes de vie... 2 comme 2 fois plus de raison d’y aller, deux fois plus convaincue « qu on ne m enlèverai rien, qui m’empêcherait de me tenir debout... solide... attachée à ce combat que je livrerai sans merci et qui n’entachera jamais ma soif de VIVRE, ma volonté féroce de voir grandir mes filles, et d aimer plus ardemment celui qui en redemande quand je lui dis « j vais prendre 1000 ans j srai flétris et fanée🤣 » vendeur comme programme 🤪! Y a des photos moins glam que d autre... la vraie vie c’est celle ci... c est celle qu on choisit de montrer... sans pudeur parce que ce genre de décision, ce choix médical n est pas anodin mais qu il est une solution à envisager quand c est encore possible alors nous devons être fièr(e)s d en parler même si ce n est pas simple à légender... à chaque jour suffit sa peine. C’était il y a 5 petits jours ... tout est allé si vite... c est fait. Le temps fera le reste ... je me prépare aux changements à venir avec le plus de sérénité possible l espoir au creux de mes entrailles de ce ventre étonnamment moins vide depuis. Todas essas abordagens acima variam apenas na via de acesso, trauma cirúrgico e cicatriz no pós-operatório, pois a cirurgia sempre será a mesma. Quando a cirurgia ocorre sem intercorrências, poderá ser realizada pelo cirurgião geral ou pelo cirurgião do aparelho digestivo. Contudo, caso haja passado de icterícia (pele e olhos amarelos), suspeita de tumoração ou mesmo nas reoperações é aconselhável que essa cirurgia seja realizada por um cirurgião do aparelho digestivo, pois se forem necessárias correções de complicações ali existentes através da exploração cirúrgica das vias biliares, retirada dos gânglios linfáticos na presença de tumor ou mesmo correção de lesão biliar em cirurgia previa, requer experiência extra para evitar complicações graves. First of all, these are photos of 2 dog’s airways. Just thought I’d make that clear as it’s not the first thing I saw when I looked at them myself 😂 The left photo is the airway of a non-brachycephalic dog (such as a Labrador) and the right photo is an airway of a brachycephalic dog (such as a pug). This is why a lot of Brachy dogs cannot even breathe properly, let alone exercise. However, a short nose does NOT directly correlate to restricted airways. This is why it’s important to research REPUTABLE breeders who HEALTH TEST their dogs and PROVE their ability to perform in dog sports rather than just looking for a breeder that breeds “long nosed pugs”. Outcrossing is also something that would be worth looking into for anyone interested. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. Intrauterine Insemination (IUI) is a fertility treatment that involves placing sperm inside a woman’s uterus to facilitate fertilization. The goal of IUI is to increase the number of sperm that reach the fallopian tubes and subsequently increase the chance of fertilization. IUI provides the sperm an advantage by giving it a head start but still requires a sperm to reach and fertilize the egg on its own. It is a less invasive and less expensive option compared to In Vitro Fertilization. When is IUI used? The most common reasons for IUI are low sperm count or decreased sperm mobility. However, IUI may be selected as a fertility treatment for any of the following conditions as well: • Unexplained infertility • A hostile cervical condition, including cervical mucus problems • Cervical scar tissue from past procedures which may hinder the sperms’ ability to enter the uterus • Ejaculation dysfunction How does IUI work? Before intrauterine insemination, ovulation-stimulating medications may be used, in which case careful monitoring will be necessary to determine when the eggs are mature. The IUI procedure will then be performed around the time of ovulation, typically about 24-36 hours after the surge in LH hormone that indicates ovulation will occur soon. VITAL PULP THERAPY because It’s not all about anteriors in real life dentistry !!! Big decay on tooth 17, no pain, sensitivity test is normal. Partial removal failed. I went for full pulpotomy and direct pulp capping with bioceramics (total fill cement and total fill bc putty fast set) and direct composite as long term restorative treatment. To me, premixed putty bioceramics take the huge advantage that it can be drilled with no setting time. So much faster than MTA or Biodentine. More expensive also !!!
