We understand that the breast screening programme was paused out of necessity due to the global COVID-19 pandemic, but we must now press play to ensure that all women can access breast screening, and we cannot afford for the programme to be paused again. Anyone know what this is? No, it is NOT a central line kit for a neonate! (Even if that’s what I thought it was at first). This is called a rapid infuser catheter , more commonly known as an RIC line. So what makes it special and how does it work? ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Many times, it can be difficult to get large bore IVs like 16s and 14s into certain patients, especially those hemorrhaging or at a massive volume loss. The RIC line is an 8.5 French catheter that sits over a dilator, that once inserted, allows you to deliver large amounts of volume to patients very quickly. Through one of these bad boys, you can infuse upwards of 1000mL of fluid in under a minute! ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Placing them is simple and easy using the Seldinger technique! 1️⃣ Insert a 20g IV. 2️⃣ Use the guide wire from the RIC set and thread it into the vessel via the IV. 3️⃣ Remove the IV over the wire and place the dilator and RIC together over the wire to the skin. 4️⃣ Using the knife 🔪, make a small nick in the skin at the site of wire entrance. 5️⃣ Advance both the dilator and catheter in until you have inserted about half the catheter. 6️⃣ Advance ONLY the catheter in the rest of the way, remove the dilator and wire, and hook up your fluids 💦 to the Luer lock hub. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Be sure to always consult the user manual whenever you use medical equipment to ensure proper use! ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Share your experience with RIC lines below! 👇🏻 ⠀⠀⠀⠀⠀⠀⠀⠀⠀ *For educational purposes only. Same Same . We primarily do PGS in our clinic and its safe to say that most US IVF clinics do a large proportion of them too. Bc of this we hatch embryos on Day 3 so that by the morning of Day 5, they are alreading extruding out the zona. This makes the biopsy faster and more efficient for the lab bc when we do not hatch we get embryos like pic 1. In those cases we would need to lazer the hole in the morning and then wait for the extrusion. Or, go in and pull out the cells, which we find more harmful. I recently conducted a survey that i sent to labs around the US and other countries and the vast majority hatch prior on Day 3, not the morning of biopsy, so we are in line with the majority. The 2 embryos at 4Aa i saw this week were actually a nice treat bc we did not need to biopsy them as it was not requested and we froze them instead. I just wanted to show you what 2 very similar aged embryos look like as the same grade but one hatched, one not. This way if you have embryos in either category you can relax and note that they are still the same. Embryos will hatch by themselves when its time! So let them! We just need to hatch Day 3 to make our work more efficient, so if you do not have hatching embryos on Day 5 and even Day 6......Dont Panic! . Enjoy the day! . In heterosexual, dual-earning families with kids under 13, mothers have reduced their work hours four to five times more than fathers. There are a few reasons why that might be happening. Kids go to mommy first; women are doing more emotional labor than ever. I have seen a lot of folks suggesting this is what families sign up for when they have children. Or, suggesting that its just about money. Many families need two incomes to stay afloat, for one. And it should go without saying that preventing women from contributing to society outside the home for a year if not more is an incalculable loss. But apparently I have to say it. Hundreds of thousands of people have been given two Chinese COVID-19 vaccine candidates as part of an emergency program, without a single case of infection or adverse effects, a senior official of a state-owned vaccine developer has said. The two vaccines, developed by China National Biotec Group (CNBG), are expected to enter the market as soon as this December, and two shots will cost less than 1,000 yuan (about 146 U.S. dollars), said Zhou Song, the companys general legal counsel. They are still undergoing phase three trials for safety and efficacy - usually the last step before regulatory approval - in more than 10 other countries, including the United Arab Emirates, Bahrain, Peru, Morocco, Argentina, and Jordan, since the epidemic has been basically put under control in China. The two vaccine candidates, together with another one developed by Beijing-based Sinovac, have been approved for civilian emergency use after reliable results in phase one and phase two trials. Tens of thousands of people vaccinated have traveled to countries and regions with high risks of COVID-19. No one has been infected so far, and this proved the effectiveness of the vaccines, Zhou said. How long does it take to generate antibodies after one gets vaccinated? Clinical trial results showed that individuals who received two doses of CNBGs inactivated vaccine at an interval of two to four weeks, usually started to generate antibodies seven days after the first shot, and all have neutralizing antibodies 28 days after the second shot, Zhou said. ***How long will the immunity last?*** The vaccines could protect an individual against the novel coronavirus for one to three years, based on the results of animal experiments, phase one and phase two trials, and similar vaccines using similar technology, according to Zhou We have kept monitoring the first 180 volunteers, who received the vaccines more than five months ago, and found the levels of the antibodies for the coronavirus are still stabilizing at the peak, and there is no sign of them dropping, he said. Today marked the discharge of two COVID-19 patients with the longest stays at St. Marys General. Our patient, Graciela who resides in Clifton, was admitted on April 4th and spent a whopping 61 days as an inpatient, and treated between ICU and 6 East. Discharged on the same day by great coincidence, our patient Saina, a resident of Passaic, was admitted on April 13th and spent 52 days as an inpatient, was vented for several weeks, and received care from the ICU and later 6 East. Both patients, aged 59, tested negative for COVID-19 upon discharge and were brought to tears when expressing their gratitude for the excellent work of our ICU and 6 East who saw them off today. #RetinaRocks Optic disc drusen (ODD) are refractile calcific concretions that form anterior to the lamina cribrosa. They are felt to be extruded extracellular calcified mitochondria, and may be inherited as an autosomal dominant trait. It is often difficult to differentiate ODD from true optic nerve edema. This is often the dilemma when seeing a child with a possible swollen nerve, although there are several tests that can help. ODD will often hyperfluoresce on fundus autofluorescence. We have found ultrasonography to be unhelpful, since ODD don’t calcify until later in life, by which time they usually become much more visible on examination. B-scan OCT imaging is often not helpful, although newer swept-source machines can directly visual the drusen. The retinal nerve fiber layer will be variably thinned with ODD and thickened with true disc edema. Fluorescein angiography is probably the best way to differentiate the two, since a swollen nerve will show late leakage, whereas ODD will not leak, often showing staining of the drusen. Our patient is a 15YO boy with quite pronounced bilateral optic nerve drusen visible clinically and on OCT. Although central vision was good, he was complaining of increasing peripheral vision loss due to the drusen compressing the surrounding nerve fibers. Unfortunately, there is no proven treatment although some have tried pressure lowering medications for neuroprotection. There is outrage, there are protests and public hanging of the culprits is vehemently demanded. While the outcry continues, the Uttar Pradesh police have been accused of sabotaging the case. Allegations have been raised by the family of the victim that police had initially tried to brush aside the case as fake news. The woman, a day after she was assaulted, gave a police statement and initially the case was investigated as an attempt to murder over old enmity. Rape charges added only in FIR on September 22, eight days after crime. On September 22, after the girls statement, rape charges were added to the FIR and three more people were booked, while one accused, Sandeep was arrested. The woman told the police in a statement on September 22 that the four men raped her on September 14 when she had gone to collect firewood. Hathras Superintendent of Police Vikrant Vir said on Thursday that after the woman’s statement, gang rape charges were added to the first information report and three persons were booked. The Uttar Pradesh Police also denied that the deceased was not a victim of sexual assault. The police, however, claims that the woman was attacked and allegedly strangled by four accused which caused damage to her nervous system and left her paralysed. Speaking to media, Hathras SP Vikrant Vir said that no signs of sexual assault were found on the victims body by doctors and reports of her tongue being cut were also false. No signs of sexual assault was confirmed by doctors in either Hathras or Aligarh. The matter will be probed by doctors through forensic help. I first went to my GP in 2012 and I was diagnosed in 2020. Private is obviously quicker but effing expensive. I wouldn’t take my ADHD away though for all the money in the world because coupled with my personality - it’s traits has enabled me to have certain ‘superpowers’ - playing piano for 6 hours straight and obtaining grade 8 piano, grade 7 flute; 12 GCSEs; 4 A Levels; a degree in Psycholinguistics at Kings and extremely high coping mechanisms; but it is a disability and trust me - it comes with a lot of problems that is hard to understand what is happening to you - or why you feel different from everyone else (I have since I was in nursery - yes I can remember that far back - I remember being in a pram, my first dream, my 3rd birthday, seeing my first reflection - all these are examples of superpowers!) But I have been abused because of peoples ignorance to ADHD. That’s a strong word to hear and quite a difficult thing to accept when a medical professional explains to me that this is what has happened to me. Im still very angry at a lot of people out there who have been these abusers; and I have a right to be; but instead of the old Mickey’s reaction which would blow up - instead I have a new quiet power where I will dump you out with the rest of my rubbish and it feels great. Advanced respiratory monitoring in COVID-19 patients: use less PEEP! . . . Discussion: We demonstrate that mechanically ventilated patientswith COVID-19 have a relatively high lung compliance, high dead space ventilation, and gas exchange impairment. In almost all patients, lung compliance decreased and dead space ventilation increased with increasing PEEP levels. The decrease in lung compliance and increase in dead space ventilation in response to higher PEEP levels indicate that COVID-19 lesions were not recruited and that higher PEEP levels cause hyperinflation of the more compliant parts of the lung [1]. These results are in accordance with recent findings in COVID-19 patients [2]. When lung compliance increases in response to higher PEEP levels (patient 1), recruitment is likely and PEEP should be set accordingly [1, 2]. All patients responded extremely well to prone positioning, although the exact mechanism is unclear. Redistribution of blood flow seems to be an important mechanism. In conclusion, we show that higher PEEP levels de-crease lung compliance and in most cases increase dead space ventilation, indicating that high PEEP levels probably cause hyperinflation in patients with COVID-19. Assistant Professor of Pediatrics Dwi Kemaladewi, PhD, only joined UPMC Children’s in 2019. Yet, already she’s making a name for herself. With a big smile and contagious enthusiasm about her work, it’s easy to understand why. Appointed a Mellon Scholar by the hospital’s Richard King Mellon Institute for Pediatric Research and a recipient of a 2019 Children’s Trust grant, Dr. Kemaladewi has built a career around investigating pediatric rare diseases. She studies a debilitating, deadly form of muscular dystrophy called muscular dystrophy type 1A (MDC1A). Kids with MDC1A have mutations in the LAMA2 gene that cause their muscles and other bodily functions to shut down. Children diagnosed with MDC1A lose the ability to walk, and sadly, many do not make it to their teens. Dr. Kemaladewi and her colleagues wondered whether they could improve therapies for patients with MDC1A using genetic technologies. So, they set to work studying a gene called LAMA1. LAMA1 is closely related to LAMA2, but only active in utero. Using cutting-edge CRISPR gene-editing, Dr. Kemaladewi and her collaborators “switched on” LAMA1 in mouse models with MDC1A. The results? Amazingly, the mice given the therapy demonstrated markedly improved motor function and decreased paralysis! While more work needs to be done, Dr. Kemaladewi is characteristically optimistic. “Generating new scientific knowledge is endlessly rewarding,” she grins. “But ultimately, I just want to be able to help these kids and families.” - @wishnubagoesoka on Instagram

Momok atau stigma yang dialami para penderita PCOS ternyata cukup besar. Seringkali mereka dianggap mandul. Weits...memang kita harus hati2 dalam menggunakan kata mandul karena implikasinya sangat besar. Sekali lagi, PCOS adalah gangguan endokrinologi atau hormonal yang dapat dialami SIAPAPUN. Bahkan yang tadinya tidak pcos sekalipun. Dalam hal ini faktor gaya hidup berperan penting. Sedentary life style atau gaya hidup perkotaan yang tinggi stres, sering kumpul makan2, dan jarang olahraga menyebabkan perubahan metabolisme menjadi kurang baik. Jadi, perbaikan POLA HIDUP sebenarnya adalah HARGA MATI untuk semua kasus PCOS. Konsisten dan Disiplin adalah kunci keberhasilannya. Cukup dengan ini saja umumnya kesuburan akan kembali dalam 6-12 bulan. Lainnya mungkin butuh bantuan obat dan cara lain. Tapi bukan berarti tidak bisa hamil atau mandul. Estou começando hoje uma sessão nova aqui neste perfil. Toda semana (espero! 😅) vou comentar sobre algum estudo científico. Não será necessariamente o estudo mais recente, mas será certamente algum trabalho relevante. . E para começar, vou explicar um trabalho do brilhante Kevin Hall, colocando à prova o modelo carboidrato-insulina da obesidade. . Este foi um estudo cruzado contrabalanceado, com 19 pessoas com obesidade. Elas ficaram internadas em uma unidade metabólica por 2 semanas, onde fizeram uma dieta Low-Carb por 6 dias e, após 2-4 semanas de intervalo, fizeram uma dieta Low-Fat por 6 dias. . Além do controle preciso do consumo e do gasto energético, os pesquisadores avaliaram diversos parâmetros metabólicos. O propósito era testar duas hipóteses: 1) a de que a insulina reduziria o gasto energético; 2) a de que não seria possível emagrecer sem reduzir a secreção/circulação de insulina. . Os resultados principais estão nas imagens do post. Em resumo: a dieta Low-Carb resultou em maior perda de peso, provavelmente resultante da perda hídrica. A dieta Low-Carb também resultou em maior oxidação de gorduras. Apesar disso, a maior perda de gordura corporal ocorreu com a dieta Low-Fat, provavelmente por causa do maior déficit energético.  . Vale dizer que esse estudo não avaliou qual dieta é clinicamente mais eficiente para pessoas com obesidade. Ao contrário, o forte controle das variáveis e a curta duração das dietas (6 dias) permitiram testar apenas os pressupostos que sustentam a hipótese de que dietas Low-Carb teriam vantagem metabólica sobre dietas Low-Fat. Social Media Friends. This is the time we need you and your support the most. @bp.surfs suffered a blast injury this morning when she was pumping up a board with an industrial compressor before our surf camp. She was thrown backwards by the blast and hit her head on the ground. She has 7 stitches in the back of her head, a concussion, and a carpel fracture from where the compressor hose hit her hand. She was driven to Salida Hospital from BV in an ambulance where the accident happened. After a CT scan and X-rays in Salida they determined she needed to be flown to St. Anthony’s in Denver so she could be assessed by a neurologist. She has air in her skull and brain hemorrhaging from the blast injury. As you all know medical transport and treatments are extremely expensive. The best thing for Brittany from here on out is to ONLY have to worry about HEALING. We can all make sure of this by donating! We have been creating fake Olive Ridley turtle eggs (a vulnerable turtle found in Costa Rica and Nicaragua).
