This was rather unusual as but that time in 1917 almost all troops would’ve just had the newer Small Box Respirator, meaning one of those soldiers still was using the PH Hood. —— The PH Hood/Helmet (PH meaning Phenate Hexamine) was an early British Gas mask of WW1, introduced in July 1915 after the ghastly Second Battle of Ypres which brought in chemical warfare. A funny part about this gas mask is when you use the exhale valve it makes a funny sound. During a Prolonged Field Care situation, you just took an airway on a multi-system trauma patient. You have a BVM with a PEEP valve but no ventilator. 1.) What would you consider for initial bag rate, “volume”, and PEEP setting on an average male using BVM? 2.) If your EMMA capnography says ETCO2 is 65, what can you change or consider? 3.) If your EMMA capnography says ETCO2 is 22, what can you change or consider? 4.) Your SpO2 monitor says 82% on your Multi-system trauma patient. What could be changed/considered on the BVM to help improve oxygen saturation? What else should you consider besides ventilation to improve the patient if they had poor values on these devices? It’s the details of their work that impact me the most. The intimacies of shaving and washing Covid positive patients, sitting holding their hands as they die, buying I-pads to connect isolated patients with family; these moments go beyond their work. Whilst the last thing I’d ever want to do is contradict the staff I’ve met, and I do agree that just doing your job does not make you a hero, the manner in which you do it can. Make no mistake, many of the staff were terrified while working during the first days of the crisis, some cried when they heard their wards were being switched to Red Zones ; yet despite this they still treated the sick with dignity and compassion. As one nurse told me, despite the risks of working with Covid positive patients “I did the job exactly the way I always have, I will care for my patients the way Ive always cared for them”. - extract from my article in this month’s @britishgq During May and June I documented the response to Covid-19 by Imperial College Healthcare NHS Trust in three hospitals: Charing Cross, St Mary’s and Hammersmith. De los fallecidos confirmados hoy, 3.199 son los que la provincia de Buenos Aires dio a conocer cuando modificó la metodología de carga El Ministerio de Salud de la Nación informó este jueves que, durante las últimas 24 horas, se registraron 3.352 muertes y 14.001 nuevos casos positivos por coronavirus. Con estos datos, el total de infectados en todo el país asciende a 765.002 y las víctimas fatales suman 20.288. Sin embargo, de los fallecimientos informados en el último parte, solo 29 corresponden al día de hoy. Sucede que 3.199 corresponden a los que la provincia de Buenos Aires dio a conocer cuando modificaron la metodología de carga. Sobre las 3.199 muertes reportadas por PBA, lideran en cantidad La Matanza (310), Merlo (197) y Quilmes (195). Hay 2 de abril, 12 de mayo, 73 de junio, 388 de julio, 974 de agosto y 1731 de septiembre. De los 3.352 fallecimientos comunicados, 1789 son hombres (1701 residentes en la provincia de Buenos Aires, uno en la ciudad de Buenos Aires, 25 en Chubut, cuatro en Chaco, dos en Córdoba, cinco en Jujuy, uno en Mendoza, tres en Neuquén, tres en Río Negro, 13 en Salta, 12 en San Juan, 10 en San Luis, cuatro en Santa Cruz, cuatro en Santa Fe y uno en La Rioja), y 1542 mujeres (1477 en la provincia de Buenos Aires, cinco en Chaco, 28 en Chubut, una en Córdoba, seis en Jujuy, cuatro en Salta, nueve en San Juan, una en Santa Fe, dos en La Rioja, tres en Neuquén y seis en San Luis). En las últimas 24 horas fueron realizados 26.662 testeos y desde el inicio del brote se realizaron 2.002.975 pruebas diagnósticas para esta enfermedad, lo que equivale a 44.141 muestras por millón de habitantes. El Ministerio de Salud bonaerense realizó esta semana testeos serológicos en estaciones de la línea sur del tren Roca para conocer la circulación del coronavirus en la región del Área Metropolitana de Buenos Aires (AMBA). Today marked the discharge of two COVID-19 patients with the longest stays at St. Marys General. Our patient, Graciela who resides in Clifton, was admitted on April 4th and spent a whopping 61 days as an inpatient, and treated between ICU and 6 East. Discharged on the same day by great coincidence, our patient Saina, a resident of Passaic, was admitted on April 13th and spent 52 days as an inpatient, was vented for several weeks, and received care from the ICU and later 6 East. Both patients, aged 59, tested negative for COVID-19 upon discharge and were brought to tears when expressing their gratitude for the excellent work of our ICU and 6 East who saw them off today. Dear friends! As some of you might have already heard, a massive police raid took place at RAF25 last Saturday. Several law enforcement agencies combined efforts to organise one of the largest illegal operations the local scene have seen in recent years. There were threats, insults, destroyed and damaged property, i.e. all sorts of human rights violations. After they left, the show went on and most artists from the original Deprave x Monasterio lineup got to perform, although to a somewhat smaller crowd. We would like to thank everyone who happened to be inside the club at that moment for staying brave and compassionate – you all did great, and they clearly did not expect that. Bulan Agustus adalah bulan yang spesial. Selain memperingati HUT Republik Indonesia, bulan ini adalah bulan dimana anak pertama kami Agus Harimurti Yudhoyono (AHY) dan cucu pertama Almira Tunggadewi Yudhoyono berulangtahun. AHY lahir pada 10 Agustus 1978 di sebuah klinik di Dayeuhkolot, Bandung. Sedangkan Almira lahir pada 17 Agustus 2008, di Jakarta. Saya ingat, dulu waktu istri tercinta Ani mau melahirkan AHY, air ketubannya pecah di kendaraan jip militer yang saya kemudikan. Sehingga begitu sampai di pelataran poliklinik persalinan, saya langsung menggendongnya menuju ke kamar persalinan. Bahkan, saya menunggui dan memberikan semangat saat bidan membantu proses kelahiran AHY di ruang persalinan. Sedangkan di tahun 2008, saya dan Almh Ibu Ani sungguh berbahagia karena dikaruniai cucu perempuan yang lahirnya tepat di Hari Ulang Tahun Kemerdekaan RI ke-63. Saya ingat hari itu adalah hari yang sangat panjang. Tengah malam memimpin Renungan Suci di TMP Kalibata; pagi hari memimpin Upacara Detik-Detik Proklamasi; sore hari Penurunan Bendera serta malam harinya Resepsi Kenegaraan. Namun, sebelum upacara bendera pagi hari saya dan almarhumah menyempatkan waktu untuk menunggui proses persalinan Annisa di RSPI. Bahkan almarhumah menunggui dan memberikan semangat di ruang persalinan kepada Annisa. Selamat ulang tahun yang ke-42 untuk anakku AHY dan ke-12 untuk cucuku Almira. Jangan lupa mari kita sama-sama kirimkan Al Fatihah untuk almarhumah Memo tercinta. Semoga segala cita-citamu dapat tercapai dan semoga pula kalian selalu dilindungi Allah SWT. August 26th 2012 I get tasked to have my platoon go into a village that hadn’t seen NATO forces for a very long time. The intel that I gathered led me to discover that the village was being used as an enemy stronghold and they roamed freely during the day. I’m not going to get into the politics of war in this post, but I’ll just say that my recommendation was disregarded... Fast forward we’re on the ground and things immediately kick off on a bad note; aerial surveillance identified a spotter in the village...We continue to push forward and reach our dismount point. Switching to travel on foot, we push into the village where I encounter the next unusual occurrence, a group of children who would not come to me when I offered them some of the snacks that I always carried on patrol. Instead, the group of children waived for me to come to them, I ignore their gestures. We continue moving forward and then, as if things couldn’t get worse, we get a call that our surveillance is going down due to bad weather. A very eery feeling comes over everyone as we knew this significantly increased the chance of us being ambushed. I reach the entrance of the village with the lead element and we come across a Y-Junction where an odd situation begins to rapidly unfold. At the left fork we noticed a large group of people being ushered away from us and at the right fork was a small gathering of people waving for me to come over to them. I direct a group of my men to set up a machine gun team to cover down the right fork while I take a team to investigate the fleeing mass of people. I took off with my team fully expecting to get into a firefight...I was wrong, instead an enemy Soldier was hiding behind a wall amongst the people at the right fork; amongst the same people who had just waved me over to them several minutes before. This enemy Soldier popped out from behind the wall and let off a long spray of bullets from his AK-47.
