Splenic laceration It may not look like much, but this can be a deadly injury if not dealt with quickly and definitively. This injury is also one of the most frequent reasons for requiring emergency surgery after blunt force trauma. The spleen is part of your immune system that besides being a filter for old cells circulating in your blood, it also is the main source of the body’s Complement system, which enhances your body’s ability to fight off a specific type of bacteria. Therefore, while you can live without your spleen, one of the trade offs is that you will require ongoing vaccinations against the most common of these organisms. With a spike of 82,170 coronavirus cases and 1,039 deaths in 24 hours, Indias tally on Monday breached the 60 lakh-mark with a total of 60,74,702 cases. Out of these, 9,62,640 are currently active whereas the number of discharged cases stands at 50,16,520. While 95,542 lost the battle against the viral disease. On July 17, India had logged 10 lakh cases, which then doubled to 20 lakh on August 7. The country added another 10 lakh on August 23 and went past 40 lakh on September 5. In 11 days, it added another 10 lakh cases, amounting to 50 lakh cases. Twelve days later the tally has mounted to 60 lakh. Treating that solid why like a mirage, I often wonder if they were fearless or just too strong. I leave them, for them. Gifting them abandonment as if its a consolation prize, a participation medal in some game of hearts I dont remember participating. I dont know why I stick around watching as they swallow a piece of my void that I unintentionally left behind. The paint me as heartless, retelling about the pieces of love they worshipped me with whose absence they now carry around as callous souvenirs of resentment. I would have nodded in a sad understanding, had I not been so empty and they, not so fulfilled with their hate. One too many souls walks away, heartbroken, sorrowful and messed up, and I dont erase their mumbles or outright shouts claiming how big of a monster I am. Some promise to help me, to fix me, some tell me that I am afraid of what I feel and I want to give in on mornings my pain walks over my emptiness, I want to coddle myself in their ignorance but my head spins a memory of the times I ruined people who I failed to love. If only I registered as pain and not bliss. If only I had been the monster in the shadows, fated to remain unloved. Publishing in the ISME Journal, Ivan Mateus and colleauges show what happens at the molecular level when different fungi co-exist within the same plant. . Ivan says: “These results are very exciting as they show that multiple components of the molecular machinery controlling mating are activated when two genetically different [fungi] isolates meet inside the plant root”. . The activation signal for the mating-related genes was identified inside the host plant roots, suggesting that the host plant could play a role in the process. DEEP FRONT LINE - THE INNER ARCH - The deep front Line takes its path from the lower extremity to the torso and all the way up to the cervical region - Posterior tibialis, psoas,diaphragm and scalenes are few of the major muscles in this line - Based on ontogenesis (development of organism) the line starts from front of the spine where the DIAPHRAGM is present - Proper breathing mechanics leads to proper movement mechanics. Dysfunctional breathing pattern can cause both physiological problems (anxiety breathing, headache and general fatigue) and biomechanical problems - Few biomechanical implications are 1. Forward neck posture - leading to neck pain and headaches 2. Poor thoracic spine and scapular mechanics- shoulder and mid back pain 3. Increase muscle tone in erectors and hip flexors and inhibited psoas and hip extensors - Lower cross syndrome, low back pain, hip pain. 4. #RetinaRocks Optic disc drusen (ODD) are refractile calcific concretions that form anterior to the lamina cribrosa. They are felt to be extruded extracellular calcified mitochondria, and may be inherited as an autosomal dominant trait. It is often difficult to differentiate ODD from true optic nerve edema. This is often the dilemma when seeing a child with a possible swollen nerve, although there are several tests that can help. ODD will often hyperfluoresce on fundus autofluorescence. We have found ultrasonography to be unhelpful, since ODD don’t calcify until later in life, by which time they usually become much more visible on examination. B-scan OCT imaging is often not helpful, although newer swept-source machines can directly visual the drusen. The retinal nerve fiber layer will be variably thinned with ODD and thickened with true disc edema. Fluorescein angiography is probably the best way to differentiate the two, since a swollen nerve will show late leakage, whereas ODD will not leak, often showing staining of the drusen. Our patient is a 15YO boy with quite pronounced bilateral optic nerve drusen visible clinically and on OCT. Although central vision was good, he was complaining of increasing peripheral vision loss due to the drusen compressing the surrounding nerve fibers. Unfortunately, there is no proven treatment although some have tried pressure lowering medications for neuroprotection. Whats the diagnosis? A 58-year old African-American woman presented with a 3-year history of scaly plaques on her legs. Prior treatment consisted of triamcinolone cream and compression stockings. Examination found thin, red-brown rhomboidal plaques with adherent scale on anterior legs bilaterally and no other skin involvement. Review of systems was positive for chronic dyspnea on exertion. Tuberculin test was negative. A punch biopsy found numerous deep dermal noncaseating granulomas composed of epithelioid cells and giant cells surrounded by a pauci-cellular cuff of lymphocytes. Bats are Back! Since April, the Maine Department of Inland Fisheries & Wildlife (IF&W) in collaboration with US Fish and Wildlife and the International Union with the Conservation of Nature has continued to track emerging research regarding human-wildlife interactions and the novel coronavirus (SARS-CoV-2), particularly with regard to the care and release of bats. As research was conducted, wildlife rehabilitators in the state were prohibited from admitting bats in the case that they became exposed to COVID which could potentially decimate a critically low population. Why such a focus on bats? Unfortunately several species of bats in our region are imperiled, and as bats often come in contact with human dwellings, the novel virus poses a great risk to bats as well as humans. IF&W visited several facilities, including the Center for Wildlife, that care for distressed bats discussing disease transmission prevention protocols, new testing options, and some potential research projects specific to bats, which are underway and available for participation by Maine rehabilitators. Injured or orphaned bats are now authorized to go to one of four approved bat rehabilitation centers in Maine including the Center for Wildlife. An added bonus is that approved bat rehabilitation centers will participate in a Tufts University study that monitors and better understands potential human-to-animal transmission of SARS-CoV-2/COVID-19. We are so pleased to be working with IF&W to continue safely treating and admitting our wild bat patients! There are more than 1,100 species of bats throughout the world, with 40 species in the United States alone and additional species being discovered almost monthly. Though small in physical size, bats have a large footprint, and make-up one-quarter of the worlds mammals. The FDA estimates that bats provide over $3.5 billion of pest control to farmers each year - for FREE! Just one big brown bat is capable of eating over 1,000 insects an hour. At Cobble Hill Cinemas the safety of our guests and employees is our top priority every day. While health and safety standards and procedures are followed on a daily basis at Cobble Hill Cinemas, we wanted to make you aware of the extra precautions we’re taking to ensure everyone stays healthy during this time of heightened concern about Coronavirus (COVID-19). What we are doing: We are closely monitoring daily updates and recommendations from the CDC. We are sanitizing seat armrests, and cup holders during theater cleanings before every show. Gloves are being worn by all staff. Every night our end-of-day cleaning team is disinfecting all hand-contact surfaces in guest-accessible areas (armrests, door handles, counters, railings, faucets). In addition to nightly deep cleaning, our theaters are increasing the frequency of cleaning and disinfecting all high-traffic areas throughout the day. Door handles and box office counters are being disinfected every 30 minutes during business hours. Er produziert Progesteron, das wiederum die Körpertemperatur ansteigen lässt und dafür sorgt, dass sie für die nächsten ca. 10-16 Tage erhöht bleibt. Deshalb ist der Temperaturanstieg eine sehr sichere Möglichkeit, um festzustellen, dass ein Eisprung stattgefunden hat. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ 💡Nach einer Befruchtung benötigt die Eizelle knapp eine Woche, um in die Gebärmutter zu wandern und weitere 1-2 Tage, um sich in der hoch aufgebauten Schleimhaut einzunisten. Bildet sich der Gelbkörper vorzeitig zurück, sinkt die Hormonkonzentration ab und die Periode tritt schon ein, bevor sich die Eizelle einnisten konnte. Zyklen mit zu kurzen Gelbkörperphasen sind deshalb unfruchtbar. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ 👉Wichtig: Eine Gelbkörperschwäche lässt sich nicht am Temperaturniveau der Hochlage erkennen und auch nicht daran, wie deutlich oder schnell der Anstieg ausfällt. Das einzige verlässliche Zeichen ist die Länge der Hochlage, die mindestens 10 Tage dauern sollte - gezählt wird von der ersten höheren Messung bis zum Tag vor der nächsten Periode. Wische einmal nach rechts, um in der #kurvedesmonats zu sehen, wie sich eine Gelbkörperschwäche in der Zykluskurve darstellt. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ 👉Nochmal wichtig: Auch wenn die Gelbkörperphase grundsätzlich stabiler ist als die Eireifungsphase, können auch besondere Lebensumstände wie zum Beispiel großer Stress oder das Absetzen der Pille kurzzeitig zu verkürzten Gelbkörperphasen führen. Das ist im Einzelfall unproblematisch und bedarf keiner weiteren Abklärung. Treten häufiger verkürzte Hochlagen auf, würde ich empfehlen, den Frauenarzt darauf anzusprechen. ⠀⠀⠀⠀⠀⠀⠀⠀ Insgesamt weisen knapp 10% aller Zyklen eine verkürzte Gelbkörperphase auf. This is probably many dermatopathologist’s favorite lesion. ⠀ ⠀ When you first examine skin biopsies under the microscope, identifying a Porokeratosis is all about the column of retained nuclei within the stratum corneum…(buzzword to remember: cornoid lamella).⠀ ⠀ Except its not.⠀ ⠀ Its actually about the changes seen here at the bottom, within the epidermis. Its a clonal disorder of abnormal keratinization, where keratinocytes don’t mature properly. There’s some reaction to these keratinocytes, with vacuolated cells and inflammation. ⠀ ⠀ But the bigger picture is even bigger than that! Porokeratosis is one of those perfect clinical-pathologic cases, where the changes under the microscope produce a specific change on the skin. For porokeratosis, the lesion is a ring, with an atrophic center lined by a ridge of these abnormal keratinocytes. This picture is just a cross section of that ring.⠀ ⠀ ⠀ I’m drawn to pathology that highlights the act of looking itself. Porokeratosis does just that. Been told your egg has Vacuoles? . The egg at the bottom has Vacs. They are incredibly easy to distinguish and look like circles or craters inside the cytoplasm. If you compare the egg to the other 3 you can see the vast difference . Its not known what causes them, but vacs are almost always from the advanced aged patients and therefore would probably link to egg quality. Occasionally i will see them in younger patients but their frequency is much lower . So what do we do?! Well if you have enough eggs its not something to be concerned about bc you can exclude them. But if you only have 3-4 eggs like this case, we would inject and keep them separate. I tend to try and avoid them at injection and half of the time they do fert and divide, but i know they produce lower quality embryos, if a blast at all . These morphologies in older patients are common so i thought it would be nice to show you just what it is we mean when we tell you “your eggs have vacs” . Enjoy the week . Low back pain is among the leading cause of disability globally, and nearly everyone will experience this at some point in their lifetime. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. This situation is called cauda equina, and is a neurosurgical emergency. Cyanobacteria (blue-green algae) are found in a broad range of environments, from hot springs to marine and fresh waters, and soil crusts to polar icecaps. By understanding exactly how cyanobacteria harvest light and convert it to energy, scientists may be able to engineer highly efficient photosynthetic units to help increase crop yields. I should be in San Francisco right now. I was at the airport, I had everything I needed, but no amount of preparation will ever be able to control the actions of others. Before I could even finish a sentence, the TSA officer dumped out my medical bag onto a dirty table and went through it, without as much as changing her gloves. My TPN bags were thrown around, while my requests to keep them in the bag were ignored and explanations met with back talk and rolled eyes. After talking to my TPN team and the infusion pharmacist, I decided that the safest option would be to cancel my trip and go back home where they could get me new TPN bags. For me, it was not worth the risk, especially when traveling to the other side of the country. I’ve been sick for nearly ten years, but today was the first time I actually put my health first. In the past I would’ve gotten on that plane anyway and tried to survive four days without nutrition... I would’ve completely ignored the rational voice in my head for the sake of “living my life.” Instead, I did the thing I would’ve told someone else in my position to do: be safe and go home. In light of all this, I’m choosing to be thankful—thankful for my TPN NP who tried everything she could to salvage my trip, before she even started her day. For the kindness of the TSA supervisor who saw me crying on my way out and stopped to see if he could help. And, for my neighbor who drove all the way down to the airport to pick my sobbing self up, then talked to the airline to get my checked bag back and got me coffee before taking me home. The holidays are stressful for everyone, and I’m sure that woman didn’t mean to hurt me, so I’m choosing not to be angry. I’m upset I’m not in San Francisco with the rest of my family spending time with my grandparents, but I’m also proud of myself for finally putting my health first. Sometimes, my friends, it’s in the dark moments that we can see just how much we’ve grown💕 NOTE: I addressed the situation with the TSA supervisor and he handled it appropriately, therefore I do not feel a formal complaint is necessary at this time. - @fromthepatientperspective on Instagram
When I was a chief resident, a 19 year old kid was brought into STC having been shot multiple times in his chest. He rapidly deteriorated and cardiac arrested right in front of us. My team immediately responded with our resuscitative efforts. I then opened his left chest, quickly set up the Finochietto retractor, placed a clamp on his aorta and opened his pericardium to find a large amount of blood and multiple bullet fragments. I quickly whip stitched the holes in his atria and ventricle closed and began manual cardiac massage as we poured blood back into him. We worked on him for over 40 minutes. But we couldn’t get him back. I stood there holding this kids lifeless flaccid heart in my hands surrounded by a huge highly specialized team that could often do the impossible, but we failed that night. I failed that night. These moments make you feel like you aren’t good enough. They makes you question every single moment and decision. And then you begin to rationalize it some how and tell yourself “well this was some gang kid that probably got shot selling drugs etc.” And while that many times WAS the case in Baltimore, I never let those thoughts hang around in my mind too long. I didn’t want to use that as my crutch. I never wanted to dehumanize people, regardless of the circumstances that brought them in. The reality is that moments like this one, where I was unable to save a life, haunt me long after the time has past. I wake up at night in a cold sweat and routinely have nightmares about many of these dramatic moments where my efforts weren’t good enough. A lot of surgeons don’t feel comfortable openly talking about this because we are supposed to have extremely thick skin and be bulletproof ourselves... They are afraid to be vulnerable, but the reality is it’s a burden we all feel in one way or another. It’s also the reason why I ride my residents asses so hard and expect so much out of them because I truly want them to become physically and emotionally tough and better; better than I am and to never ever become complacent and always keep getting after it! - @adamthespecimen on Instagram

VITAL PULP THERAPY because It’s not all about anteriors in real life dentistry !!! Big decay on tooth 17, no pain, sensitivity test is normal. Partial removal failed. I went for full pulpotomy and direct pulp capping with bioceramics (total fill cement and total fill bc putty fast set) and direct composite as long term restorative treatment. To me, premixed putty bioceramics take the huge advantage that it can be drilled with no setting time. So much faster than MTA or Biodentine. More expensive also !!! Olha ela ai! A fomasa glândula tireoide. Variados são os motivos que podem resultar na necessidade de se realizar uma cirurgia na tireoide. Atualmente o motivo mais comum é para tratamento de neoplasia ou de nódulo suspeito de ser neoplásico. Nódulos suspeitos de serem malignos da são aqueles que no ultrassom são escuros (no laudo estará escrito hipoecogênico), sólidos, com margens irregulares ou mal definidas, com padrão de crescimento vertical e com calcificacões. No exame físico os nódulos endurecidos e presos aos músculos do pescoço também sugerem malignidade. Assim como nódulos que aumentam de tamanho muito rapidamente. Nódulos com essas características devem ser biopsiados. E casos suspeitos ou confirnados necessitam de tratamento.
