This is probably many dermatopathologist’s favorite lesion. ⠀ ⠀ When you first examine skin biopsies under the microscope, identifying a Porokeratosis is all about the column of retained nuclei within the stratum corneum…(buzzword to remember: cornoid lamella).⠀ ⠀ Except its not.⠀ ⠀ Its actually about the changes seen here at the bottom, within the epidermis. Its a clonal disorder of abnormal keratinization, where keratinocytes don’t mature properly. There’s some reaction to these keratinocytes, with vacuolated cells and inflammation. ⠀ ⠀ But the bigger picture is even bigger than that! Porokeratosis is one of those perfect clinical-pathologic cases, where the changes under the microscope produce a specific change on the skin. For porokeratosis, the lesion is a ring, with an atrophic center lined by a ridge of these abnormal keratinocytes. This picture is just a cross section of that ring.⠀ ⠀ ⠀ I’m drawn to pathology that highlights the act of looking itself. Porokeratosis does just that. At Cobble Hill Cinemas the safety of our guests and employees is our top priority every day. While health and safety standards and procedures are followed on a daily basis at Cobble Hill Cinemas, we wanted to make you aware of the extra precautions we’re taking to ensure everyone stays healthy during this time of heightened concern about Coronavirus (COVID-19). What we are doing: We are closely monitoring daily updates and recommendations from the CDC. We are sanitizing seat armrests, and cup holders during theater cleanings before every show. Gloves are being worn by all staff. Every night our end-of-day cleaning team is disinfecting all hand-contact surfaces in guest-accessible areas (armrests, door handles, counters, railings, faucets). In addition to nightly deep cleaning, our theaters are increasing the frequency of cleaning and disinfecting all high-traffic areas throughout the day. Door handles and box office counters are being disinfected every 30 minutes during business hours. Cyanobacteria (blue-green algae) are found in a broad range of environments, from hot springs to marine and fresh waters, and soil crusts to polar icecaps. By understanding exactly how cyanobacteria harvest light and convert it to energy, scientists may be able to engineer highly efficient photosynthetic units to help increase crop yields. In a paper published recently in Nature Plants (doi.org/10.1038/s41477-020-0718-z) Vincenzo Mascoli (PhD student at Vrije Universiteit @vuamsterdam) and colleagues investigated cyanobacteria from shaded environments to learn more about the trade-offs between covering the largest portion of the solar spectrum for photosynthesis and achieving the highest yields of energy conversion. A beautiful picture of a cell we injected today in my lab with a fluorescent dye. This is a Broad thorny retinal ganglion cell (species: common marmoset/callithrix jacchus), a type of neuron located in the retina. These neurons help provide us with our sense of sight and are involved in a number of complex processes mediated through our visual neural circuitry. Cells like this one are involved in processes such as “visual pursuit” which guides our eyes as they focus on different or moving targets. For reference, this cell has a diameter of approximately 300 um, or about 3/10 of a mm. DEEP FRONT LINE - THE INNER ARCH - The deep front Line takes its path from the lower extremity to the torso and all the way up to the cervical region - Posterior tibialis, psoas,diaphragm and scalenes are few of the major muscles in this line - Based on ontogenesis (development of organism) the line starts from front of the spine where the DIAPHRAGM is present - Proper breathing mechanics leads to proper movement mechanics. Dysfunctional breathing pattern can cause both physiological problems (anxiety breathing, headache and general fatigue) and biomechanical problems - Few biomechanical implications are 1. Forward neck posture - leading to neck pain and headaches 2. Poor thoracic spine and scapular mechanics- shoulder and mid back pain 3. Increase muscle tone in erectors and hip flexors and inhibited psoas and hip extensors - Lower cross syndrome, low back pain, hip pain. 4. Whats the diagnosis? A 58-year old African-American woman presented with a 3-year history of scaly plaques on her legs. Prior treatment consisted of triamcinolone cream and compression stockings. Examination found thin, red-brown rhomboidal plaques with adherent scale on anterior legs bilaterally and no other skin involvement. Review of systems was positive for chronic dyspnea on exertion. Tuberculin test was negative. A punch biopsy found numerous deep dermal noncaseating granulomas composed of epithelioid cells and giant cells surrounded by a pauci-cellular cuff of lymphocytes. Fite and periodic acid–Schiff stains were negative, and foreign body was not seen on birefringence. . A. Granulomatous cutaneous T cell lymphoma B. Ichthyosis vulgaris C. Ichthyosiform sarcoidosis D. A range of symptoms that accompany migraine are reversible. They all relate to the 12 cranial nerves that come in and out of the brainstem. As the brainstem becomes hypersensitive it amplifies the response from these nerves. For example, our senses (smell, sight & hearing) become exaggerated & amplified, to the point where they are uncomfortable. Hence the desire to be in a dark, quiet room. The Vagus nerve, which goes to the heart, lung and stomach, creates issues such as nausea and vomiting, as it stops the stomach from absorbing food and drink as it normally would. The first boy who shackled me with sweet words and thoughtful but measured gestures knew exactly why not to. Treating that solid why like a mirage, I often wonder if they were fearless or just too strong. I leave them, for them. Gifting them abandonment as if its a consolation prize, a participation medal in some game of hearts I dont remember participating. I dont know why I stick around watching as they swallow a piece of my void that I unintentionally left behind. The paint me as heartless, retelling about the pieces of love they worshipped me with whose absence they now carry around as callous souvenirs of resentment. I would have nodded in a sad understanding, had I not been so empty and they, not so fulfilled with their hate. One too many souls walks away, heartbroken, sorrowful and messed up, and I dont erase their mumbles or outright shouts claiming how big of a monster I am. Some promise to help me, to fix me, some tell me that I am afraid of what I feel and I want to give in on mornings my pain walks over my emptiness, I want to coddle myself in their ignorance but my head spins a memory of the times I ruined people who I failed to love. If only I registered as pain and not bliss. If only I had been the monster in the shadows, fated to remain unloved.
