Bring the crash cart!! ‼️😨⁠ ⁠ Roles nurses can play during a code to make sure the event runs smoothly:⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁠ 🚨Recorder: The recorder should be writing the time the code began (or if you’re in ED, ask the EMT’s bringing the patient in what time compressions began) then record any medication dose given and the time. Be aware of the time because you will advise the physician how long it’s been since the last dose and when it’s time to check for a pulse ⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Airway: typically once a person is coding the physician will intubate. Once the patient has been intubated, the Respiratory Therapist present at the code will be maintaining the airway and using the AmbuBag to ventilate. The physician then focuses solely on running the code⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Compressions: there should be at least 2-3 people cycling through compressions. This is a great way for the new nurses to be involved in the code. Remember 2.5in deep on the compressions at a rate of 100-120bpm ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨IV’s/Medications: In ED, these full arrests may not have IV access. This becomes top priority since without it you cannot give medications. Two nurses should be obtaining access and if all else fails: IO access. Once access is available, one nurse who put in the IV should stay in place and administer the medications, being sure to call out loud and clear the dosages and when the medication goes in ⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Runner: For the code/full arrest to be effective there should always be 1-2 people at the ready to run and grab more supplies as needed. Today is World Suicide Prevention Day. Did you know over 300 physicians die by suicide annually in the US (JAMA, 2019)? Suicide rates for male doctors is 1.41 times higher than the general male population, and rates for female doctors is 2.27 times greater than the general female population (American Journal of Psychiatry). Many precipitating factors of burnout include work-load, lack of autonomy & meaning at work, and work-home conflict. It takes more than an occasional wellness activity to fix the issues at hand, but checking in with yourself and loved ones is the first step. ❤️ To be honest, I felt a little silly doing this pose. Seems people like to hold things out in front of them on instagram like...here is a stethoscope! 😆 . But, on a more serious note, let’s talk about matters of the heart. . 💔 Heart disease is the leading cause of death in the United States. It is the number one killer in females as well, which is often overlooked. Men present with the more classic, grip-your-chest type of symptoms. Females notoriously present with more subtle symptoms, which may even feel like indigestion. This is not meant to scare you, just to keep you aware. . 😯 I had an unusual symptom when I started running again after having my daughter. I went and got checked out, and turns out I was having benign symptomatic PVC’s (extra heart beats) but it was so reassuring to know that all was well. It was an expensive work-up to figure out that I was just out of shape! But, I am so glad I did it. . ✨ A few of my favorite heart health recommendations: . ❤️ Take CoQ10 100 mg twice per day. This is great for the heart. If you happen to be on a statin, you MUST be taking CoQ10 with it. (As a side note, this supplement is also great for fertility as well. Improves egg quality.) . 🥗 Aim for 7-9 servings of fruits and vegetables daily. More veggies than fruits. A serving size is a medium size piece of fruit, 1/2 cup cooked veggies or 1 cup raw veggies. . 🍎 Get at least 25 grams of fiber per day. With a mix of soluble and insoluble fiber. Fruits, veggies, beans/lentils, nuts, gluten-free grains. . 🥘 If you consume animal protein, try to think of it as a side, and not the main part of the meal. Your plate should be loaded with produce and healthy fats, and the meat as the “side.” . 🏃🏻‍♀️Walking just 30 minutes 5 days per week can lower your risk. . 🍫 Dark chocolate has heart healthy flavanols, that can improve blood flow, reduce blood pressure, and can boost mood. The Cleveland Clinic says 1 ounce a few times per week for heart health. Yes please! . 🧘🏻‍♀️ Manage stress! So what do I do on the nephrology aka renal aka kidney service? 💊 I get to the hospital around 8:30-9 AM depending on who is my preceptor that day. Some start their day earlier some much later. 💊9-10AM: I check all my patients overnight events, labs, new imaging, etc. and write everything down in an organized manner. 💊 10-11AM: Pre-round on all my patients. Do a targeted H&P. Answer the pts questions about dialysis or their kidney function. Ask nurses about their I&Os and any new or pending events. 💊11-12PM write progress notes and see new consults 💊12-2PM Round with my attending and go over renal topics such as hyponatremia, hyperkalemia, RTA, AKI, and CKD 💊2-3PM: update notes with any new plans then go home! . . Specialty services are different than regular inpatient medicine and I really enjoy just focusing on one thing! “Let’s talk about ‘Periods’. Yes, you read it right! If a female cannot talk comfortably about her periods, how can we expect the society to? Fortunately, I am blessed with a family where I can freely talk about it, and dont have to keep my pain wrapped in a black polythene bag. This happened on a hot, humid afternoon in May. It was the first day of my 3rd COVID duty rotation. I was posted in a ward having 30 patients, some of them in critical condition. I donned my PPE, entered the ward, and started my routine work of checking patients. I was feeling a little bloated, and had a slight pain in my lower abdomen. Yes, I was nearing the end of my menstrual cycle and was ‘prepared’ for it; but, I was praying that my periods should not start during my 6 hour duty, since the PPE alone was enough to make one uncomfortable and dehydrated. Half an hour later, one of the patients started feeling breathless. As I got up from my chair and approached the patient, I realized that my periods said ‘hello!’ to me. I had sudden cramps in my abdomen- a situation all girls can relate to! I checked the vitals of the patient and tried to stabilize him. The patient was worsening… so were my cramps. Finding another resident doctor to replace me would have taken almost an hour, and wasted one more PPE. I didnt want to leave my patient unattended for such a long time. I told myself, ‘Ill bear the cramps’. After 15-20 minutes, the patient’s vitals improved; I went back to my chair, rested for a while and continued my duty. Let’s talk about Vital Signs! ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 🏩 Vital signs provide us with critical information needed to make treatment decisions. Above are the normal ranges for ADULT vital signs. Have these values memorized, you will need them for the rest of your nursing career! 🩺 We typically monitor our patient’s vital signs q4h, q8h or q12h (1-3 times) every shift but it varies for every patient! Know your patient’s baseline vitals - it will be easier for you to determine any abnormal values. I like to take a look at the patient’s chart prior to the start of the shift to make note of their previous vitals. Remember each patient will have different baseline values depending on their current health and health history. ❗️ Remember: patients with COPD often have lower oxygen saturation and the range between 88-92% is normal and safe. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 👩🏻‍⚕️ Nursing students: report any abnormal values to your clinical instructor or your nurse. To the new nursing student: welcome to the crazy world of nursing school. You’ll probably feel like you’ve got this for the first few days… and then you’ll quickly realize the amount of work involved for the semester.⁣ ⁣ Try not to completely freak out.⁣ ⁣ It’s normal to be overwhelmed once you’ve received all your syllabi, assignment due dates, clinical dates, tests, exams, lab dates, etc. OH and don’t forget the chapters and chapters of reading. I’ve been there - it’s A LOT. ⁣ ⁣ So, here are my suggestions:⁣ ⁣ Print off a 4 month calendar sheet and write out your entire semester’s due dates - this is a great way to visualize your heavy and light weeks⁣ ⁣ You will not be able to read through every single assigned reading page - you’ll have to be selective. Look to see what the professor emphasizes in lecture + in the powerpoint slides - focus on reading THIS information in the textbook⁣ ⁣ For all my visual + auditory learners out there, check out Khan Academy on Youtube. They do a great job of breaking down common nursing school concepts⁣ ⁣ Don’t completely get rid of your ‘normal routine’ in order to 100% focus on nursing school.. you will eventually burn out 😔 Even if it’s 1 or 2 walks a week, a dinner with friends every other week, date night on Fridays… try to maintain some sort of school/life balance⁣ ⁣ Annndd the most important tip I can give (I’ve already shared this many times before): DO NOT JUST MEMORIZE EVERYTHING - genuinely learn & understand the concept. It won’t do you any good to simply memorize everything.. maybe you’ll get a better grade, but you’re studying to save lives.. not to ace every single test in school - @yournursingeducator on Instagram
