ORTHOPEDIC EMERGENCIES These are must knows that can lead to great morbidity and/or mortality if missed. Think VON CHOP 1. VASCULAR COMPROMISE: from direct injury, extrensic compression, or internal swelling/obstruction. Please obatain pulses and capillary refill in all extremities. Assess for bruit and may need a doppler. 2. OPEN FRACTURES: needs early reduction/surgical management. Updated Tdap and give antibiotics (ex. Ancef 2g) quantify energy pattern--wound size, fracture pattern, soft tissue injury size, contamination, neurovasc injury present. GUSTILO & ANDERSON CLASSIFICATION TYPE 1 - wound1cm, minimal damage TYPE 2 - wound 1cm, moderate soft tissue damage, minimal periosteal stripping TYPE 3a - severe soft tissue injury, contamination with adequate coverage TYPE 3b - severe soft tissue injury, contamination, inadequate coverage TYPE 3c - arterial injury needing repair 3. NEURO COMPROMISE 4. COMPARTMENT SYNDROME: can lead to vascular compromise, neuro injury or necrotic tissue. Risk of cellular destruction and hyperkalemia. 5. HIP DISLOCATIONS (not if prostetic) 6. OSTEO/SEPTIC JOINTS 7. PELVIC UNSTABLE FRACTURE: risk of serious vascular/neuro injury. May be life threatening bleeding. Place a pelvic binder. Will need examination of vaginal vault, male genitals, and/or rectal exam. May need massive transfusions and interventional radiology intervention. NOTE: Yes, you CAN place TQ on open fractures is bleeding is difficult to control with pressure dressings. Frequent reassessments of neurovasc status are needed. Please always compare to other extremities and take shoes/socks off with lower extremity trauma. 📷gonesk8ing.com #ems #emt #dr #bone #leg #legs #fracture #skate #ambulance #ortho #medic #paramedic #ankle #foot #open #surgery #surgeon #or #md #do #doctor #physician #rn #nurse #medicine #medical #anatomy #medicalstudent #resident #residency - @the_resuscitationist on Instagram

📚🔬 Taping for Patellofemoral Pain Syndrome ℹ️ℹ️ BACKGROUND: Patellofemoral pain syndrome (PFPS) is characterized by anterior knee pain and/or pain in the retro-patellar and/or peri-patellar region. ℹ️ Pain typically increases with activities such as: Squatting. Kneeling. Stair climbing. Prolonged sitting. ℹ️ The underlying causes of PFPS are multifactorial and may be associated with biomechanical and or neurophysiological changes at the pelvis, hip, knee or ankle regions. ℹ️ Taping is frequently used as part of the multi-modal management for PFPS, including: McConnell Patellofemoral Joint Taping (PFJT). Tibial Internal Rotation Limitation Taping (TIRLT). ℹ️ Clifford et al. (2020), investigated the effects of TIRLT, PFJT and no taping on perceived pain and lower limb kinematics during a lunge and single leg squat (SLS) in people with PFPS. 🔬🔬 METHODS: This cross-sectional study compared the effects of TIRLT, PFJT and no taping, on knee pain and lower limb kinematics during two pain-provoking movements in people with PFPS. 🔬 Participants with PFPS (n = 23) performed a lunge and SLS under three randomised conditions: TIRLT, PFJT and no taping. 🔬 The Codamotion system captured and analysed lower limb kinematic data in the sagittal, transverse and coronal planes. 🔬 Peak knee pain intensity during the movement was assessed using the Numerical Rating Scale (NRS). 📊📊 RESULTS: Participants reported significantly less pain with the TIRLT and PFJT techniques compared with no tape during the lunge and SLS. 📊 There was no evidence of altered lower limb kinematics accompanying pain reductions with either taping technique. ✅✅ CLINICAL SIGNIFICANCE: Both forms of taping may be useful adjuncts as the short-term benefit of pain relief may enable participation in more active forms of rehabilitation. 🧠🧠 Thoughts? Questions? Comments? 👇🏽 Write them below. 📚📚📚 SOURCE: Clifford et al. 2020. 🦈 SHARK ATTACK 🦈 Would you swim in a beach that had this happen? Post and Let us know below ⬇️⬇️⬇️. A picture showing gruesome wounds after a shark attack! A 22-year-old woman was out tubing with her friends in Fort Lauderdale when a shark sunk its teeth into her right leg. The fish – thought to be a bull shark – also whacked her on the head with its fin before she had any idea what was even happening to her. She was rushed to shore where she was taken to the hospital and had a two-hour operation to repair the damage to her leg. Incredibly, she survived the terrifying incident and despite the nature of her injuries, is expected to make a full recovery. Credit: Andy Wells - @plasticsurgerypictures on Instagram

Traumatic injury of the left foot! A 58-year-old male person was admitted to the Emergency Clinical Hospital Bucharest, approximately 7 hours after a traumatic injury of the left leg amputated by the blade of an electric saw. Replantation surgery of the left leg was performed under general anesthesia with ischemic time lasting 9 hours up to the arterial anastomosis. Metallic osteosynthesis and suture of the blood vessels, nerves and tendons were performed. On the initial physical examination this person presented no inflammatory signs of the skin cicatrisation process after replantation surgery, no edema and no pain syndrome. Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued.
