Our Amrita Hospital performed a surgery on a rare case saving a mother and her newborn, as the first in India. We are extremely happy to share the birth of female baby born to a mother with juvenile pompe disease on #enzyme #replacement #therapy since 6 years at our #Amrita Institute of Medical Sciences, #Kochi campus. This is the first case of a woman with pompe disease giving birth to a baby from india. #Pompe #disease is a rare, multisystemic, #hereditary disease, which is caused by pathogenic abnormalities / mutations in the ‘GAA gene’. Patients with this condition have muscle weakness to spectrum of severe complications. As this was a very high risk pregnancy with the patient having severe proximal muscle weakness she was planned for elective Lower Segment Caesarian Surgery under regional aesthesia. The surgical team consisted of Dr. Radhamoni, Dr. Vinitha, Dr. Girija, Dr. Jean and Dr. Neethu as anaesthetist. Prenatal work up was done to check whether her husband was a carrier and as he was not a carrier for pompe disease invasive test was not offered as the baby will not be affected as the condition is an autosomal recessive disorder. Mother was continued on #enzyme #replacement #therapy throughout pregnancy and baby can be breast fed too. The genetic team includes Dr Sheela Namboothiri, HOD, Department of Pediatric Genetics, along with Dr Dhanya and Dr Shwetha. #PrescriptionGratitude For sensitivity and confidentiality reasons I’ve decided to delete this story. So I’m simply taking this opportunity to ask again, strongly, that everyone take this very seriously and please stay home and adhere to physical distancing and hygiene recommendations. The incubation period (the period between the infection of an individual by a pathogen and the manifestation of the illness or disease it causes via sign/symptoms) has been reported to be 4-14 days (but in some case reports up to 21 days) for COVID-19 with rapid (hours-days) decline thereafter; think about the number of potential individuals and surfaces an infected and potentially asymptomatic individual could have come in contact with in that time - including one’s family. As healthcare workers we are proud to risk our lives to help our patients and we will continue to do so. Already looking at the epidemiology we know this is going to take many months to control, and we quite literally cannot do it without you. I advise all my fellow colleagues to take full precautions for any respiratory patient and even a simple mask for the others. Although there are trends we have been finding (normal/low WBC + lymphopenia, elevated transaminases/Dimer/Cr/CRP/Fer, cxr findings, etc.), these may be falsely reassuring if negative (as it was in this tragic case) and since swabs take days to come back we can’t be too careful. Let’s protect ourselves, protect our colleagues, and protect our loved ones to the best of our ability - we know how. Educate yourself, minimize aerosolization (anything over 6L O2 is a no go, otherwise consider allowing lower O2 sats or early intubation), run resus/transfer scenarios and get your N95s fit properly if you haven’t yet done so. Whether we like it or not we are now making history, let’s do it right. Final Exams Fast Approaching! Just mapped out my Final Written Exam/ Performance Exam STUDY schedules! And it is a DOOZY! This is the final step before the DMDAS Class of 22’ Heads into the clinic to serve the great patients of the University of Illinois! During this time, the anxiety and pressure start to creep it’s way in, you begin to question yourself,”did I study enough? Did i practice enough? Stitches are out! Not to wake you all up with a sudden & highly exposed image of my leg but, here’s my leg 😆 ⁣ ⁣ I really debated showing this photo because it feels so vulnerable but I have had so many questions about this surgery that I decided if I was going to share this journey so others can learn and hopefully be empowered by it than I need to show the beginning and so here it is. ⁣ This photo was taken a few minutes before my stitches came out.⁣ ⁣ The reason we did this surgery was that I ran into a secondary issue that resulted from the injury I had last year. The padding at the bottom of my leg atrophied which made walking in a prosthetic really uncomfortable. ⁣ So I decided to see one of the top plastic reconstructive surgeons in the country Dr. Carty, who is doing a new clinical trial on amputation revisions and I was the first to do this surgery. ⁣ ⁣ Randomly the procedure is called “The Amy” but spelled “AMI” so maybe it was meant to be?! We will see!⁣ ⁣ During the 5 hour procedure he took off a couple centimeters of bone and then he reconnected my muscles so they can work for the first time in 20 years. The muscles are paired together as they would be in an ankle joint so they can actually rebuild strength vs atrophy over time.⁣ ⁣ I don’t want to make this post too long but I am so grateful for my amazing team of Drs & also the bodies ability to heal. Just another reason to take care of your health & body! ⁣ ⁣ I still have more steps in my journey to walking but this was a big one, I’m happy to check it off my list! ⁣ ⁣ Thank you to everyone for your continued support! ⁣❤️💪🏼❤️ - @amypurdygurl on Instagram

Story of a NON COVID Physician rather a Warrior as they say.. Does it feel like one? A warrior? Definitely not. Working in a non covid hospital is keeping your guard on for all the pediatric, geriatric, surgical and immunodeficient patients, especially when were based in a rural set up where we were supplied PPEs super late. Weve been highly alert during the past six months. Or seven. Sorry, Ive lost count of time now. Even after being highly vigilant, we got patients feeding us with false history and hiding their symptoms at the stake of healths of skilled labor. Thats what we physicians are. Skilled laborers. Why do I say so? Well, the wave started with patients testing positive, unfurling the physicians, the staff and my colleagues as well. I had my roommate turn positive and I got symptoms even after being in isolation. I was ill for 20 days straight but thankfully tested negative twice. I resumed duty right after but I still see my colleagues turning positive every day, hell, even while youre reading this piece, one of our fellow physician wouldve turned positive. Still we work for the greater good, keeping our emotions, exams, anxieties, fear and families aside to the point of being indifferent. The world is moving on but our battle still continues, we just put on a happy face because such is life. Weve lived in so much uncertainty that were now prepared to face anything and everything. Bring the crash cart!! ‼️😨⁠ ⁠ Roles nurses can play during a code to make sure the event runs smoothly:⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁠ 🚨Recorder: The recorder should be writing the time the code began (or if you’re in ED, ask the EMT’s bringing the patient in what time compressions began) then record any medication dose given and the time. Be aware of the time because you will advise the physician how long it’s been since the last dose and when it’s time to check for a pulse ⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Airway: typically once a person is coding the physician will intubate. Once the patient has been intubated, the Respiratory Therapist present at the code will be maintaining the airway and using the AmbuBag to ventilate. The physician then focuses solely on running the code⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Compressions: there should be at least 2-3 people cycling through compressions. This is a great way for the new nurses to be involved in the code. Remember 2.5in deep on the compressions at a rate of 100-120bpm ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨IV’s/Medications: In ED, these full arrests may not have IV access. This becomes top priority since without it you cannot give medications. Two nurses should be obtaining access and if all else fails: IO access. Once access is available, one nurse who put in the IV should stay in place and administer the medications, being sure to call out loud and clear the dosages and when the medication goes in ⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Runner: For the code/full arrest to be effective there should always be 1-2 people at the ready to run and grab more supplies as needed. To see many pronounce this pandemic as overblown and hear increasing calls to reopen our country. And then this weekend to see NYC’s parks crowded with people without masks. It leaves you feeling discouraged.⁣ ⁣ I promise you. It is not overblown. It has had a very real impact. Especially on our family.⁣ ⁣ Over the last 2 months, I’ve lived in constant fear about the safety of my sister, brother, and father who are all NYC frontline physicians. I’ve watched my father and sister fall sick to the virus while treating patients with insufficient PPE. My mom was forced to move out once my dad became ill. And my sister and I both moved home to do everything in our power to prevent him from being hospitalized. ⁣ ⁣ Simultaneously, I was redeployed to work on the front lines in a Brooklyn COVID unit. Which has entailed long hours, caring for critical patients, watching coresidents fall sick, dealing with unexpected death, and communicating to families that their loved ones have passed.⁣ ⁣ And then this past weekend, I lost my grandfather to a 6 week battle with COVID. And it has left me feeling guilty. That I couldn’t completely be there for him. That the last time he saw me I was covered head to toe in PPE. And that the last time I held his hand it was through a glove.⁣ ⁣ This pandemic has hit our family hard. It has been physically and emotionally exhausting. And for those who still dont believe it. Yes, the devastation of COVID has been very real. ⁣ ⁣ I get it. I too want more than ever for everything to return to normal. But I am concerned with summer around the corner and a hasty reopening of cities, that we are on the verge of a second wave.⁣ ⁣ Yes, we cant all isolate forever. This also coincides with the prospect of a potential uptick in cases. Nurse Unit Manager Janelle Taylor from The Prince Charles Hospital is one of our amazing members on the #COVID19 frontline. She talks about how proud she is of her nursing and midwifery colleagues through these uncommon times... There is no doubt these are uncommon times and frontline staff across our hospitals, including all my nursing and midwifery colleagues have experienced elevated workloads and associated levels of stress and anxiety. It’s a good thing nurses and midwives are natural born problem solvers, it’s what we do. We have a strong desire to keep the community safe. Today and every day I have been focussed on being there for my team of hardworking nurses and our patients. The priority was ensuring we kept our heads above water and offered support to each other during demanding times for our workforce. As messaging and procedures changed rapidly, guided by advice from experts across our health service and around the country, we have adapted along with it, talking clearly to one another about issues like PPE. It’s important to remember that throughout COVID-19, we have been treating not only pandemic related patients but expectant mothers, car accident victims, cancer patients, heart attacks and many others – all with a heightened sense of fear around their condition and COVID-19. We have worked hard to help keep the friends and families of patients calm and protected. I am so proud of the team at TPCH and nurses, midwives and health care workers everywhere. So this is not how I imagined my spring. Right now I’m meant to be in Costa Rica... But my job reminds me every day that there are many people going through far, far worse than I am. We keep hearing about saving lives but there seems to be a wider purpose of intensive care - of all care - at this time: compassion. To provide compassionate care to all patients and families – and to each other – during Covid-19 is a measure of our humanity. It is how we will be judged and how we should be judged. There are no heroes in this but there is real, real courage everywhere. And of course patients and there families are showing the most courage of all. I can’t wait to hug my family again. I may never let go. P.S. I may not be in Costa Rica with my friends but to celebrate International Nurses’ Day today, I’m certainly drinking a lot of daiquiris. One of my favourite diagnostic procedures in ophthalmology is the B-scan ultrasound 🕵️‍♀️ . We are able to differentiate periocular, orbital and intraocular pathology and provide appropriate treatment. While I was in @kelloggeyecenter I have seen how accurate ultrasound diagnostics can be when in hands of a true expert like Dr. Ayers is. Children and our Community⁣ ⁣ I’ve been in a pensive state lately. So much is happening in the world and also in my local community. I chose to go into pediatrics because I love the opportunity to help children have a bright, healthy future. Some children have chronic diseases, but this does not stop them from fulfilling their vast potential! ⁣ ⁣ The resiliency, honesty, and pain felt by children is real. Children watch us and learn by what we do, be it in our food choices, how we address adversity, or what we stand up for.⁣ ⁣ Some of us have children, others may have close friends with children—but all of us will eventually be led by the young people in our community one day. Jose Alvarez remembers his mom sitting on their maroon couch, tears streaming down her face. That was the day when his dad told Jose that he, just 8 years old, was now the man of the house, when he left the family behind, slamming the door on his way out. The years that followed weren’t easy for Alvarez’s mother or the five children. But strength can arise from adversity. For Alvarez, the hard years became his call to action. Alvarez attributes his success in the years since his father left — in honors courses in high school and now at Fresno State — to the inspiration from his mother as she led and took care of their family. COVID19 is deadly. So were viruses that killed Indigenous children in residential schools. Today is #OrangeShirtDay, an opportunity for us to reflect on Indigenous peoples experiences, forced, year after year, generation after generation, to surrender their children to Canadas Indian Residential School System (IRRS). In this system, school graveyards were as common as school playgrounds, with 24 to 69 percent of children dead. Our Canadian healthcare system, often revered for its universality and accessibility, played a sinister and prominent role in this genocide. We better understand today that the historical relationship between Canadian healthcare systems and Indigenous peoples has been one of colonial violence, otherwise known as medical colonialism. This is why Dr. Samir Shaheen-Hussain, Dr. Suzanne Shoush, @SemirBulle & I teamed up to share our thoughts in a new opinion piece published today in @TheTorontoStar. It outlines why Orange Shirt Day is an important national day to honour the resilience, strength, dignity, and perseverance of Indigenous Peoples in Canada despite the genocide of the IRSS. Before med school, I was a history and philosophy of sex and science researcher for nearly 4 years. I’m trying really hard to normalize sex (and pleasure informed) conversations in medicine and help other health workers learn about the importance of removing stigma and shame from the doctors office. I also am passionate about teaching the anatomy of the clitoral complex that is missing from most medical text books and that most doctors are NEVER taught in medical school (which is scary for the future OBGYNs, urologists, and plastic surgeons that operate on nearby tissues!) . Let’s talk about Vital Signs! ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 🏩 Vital signs provide us with critical information needed to make treatment decisions. Above are the normal ranges for ADULT vital signs. Have these values memorized, you will need them for the rest of your nursing career! 🩺 We typically monitor our patient’s vital signs q4h, q8h or q12h (1-3 times) every shift but it varies for every patient! Know your patient’s baseline vitals - it will be easier for you to determine any abnormal values. I like to take a look at the patient’s chart prior to the start of the shift to make note of their previous vitals. Remember each patient will have different baseline values depending on their current health and health history. ❗️ Remember: patients with COPD often have lower oxygen saturation and the range between 88-92% is normal and safe. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 👩🏻‍⚕️ Nursing students: report any abnormal values to your clinical instructor or your nurse.
