Read on for tips I use when I look at an abdominal CT... 👨🏽💻Coming into radiology as someone who was mainly concerned with what was going on above the diaphragm I found the world of abdominal CT fairly daunting. But by getting to grips with the anatomy of the abdomen things became a whole lot clearer 👨🏽💻At this level of the abdomen we get a nice demonstration of the superior mesenteric artery – we can’t see the superior mesenteric vein here but normally the vein should lie to the right of the artery at the level of the pancreas, if this is reversed then consider intestinal malrotation which can predispose to midgut volvulus 👨🏽💻I always check to see there is a clean layer of fat surrounding the SMA – if this is infiltrated consider a malignant pathology such as a pancreatic tumour 👨🏽💻Always follow the vessels no matter which CT it is you’re looking at, be it head, neck, chest or abdomen. I always follow the portal veins within the liver formed at the confluence by the splenic vein (which runs posterior to the pancreas) and the superior mesenteric vein which runs vertically. In patients with liver cirrhosis and pancreatitis in particular you are looking for a filling defect or occluded vessel representing thrombosis 👨🏽💻There aren’t many retroperitoneal structures that cross the midline but the left renal vein is one of them – characteristically this passes anterior to the aorta from the left kidney to the IVC but in some people this can have a retroaortic course where it passes behind the aorta. I have seen the occasional incidental left renal vein thrombosis so it is worth checking for filling defects! This topic is something I discussed in my Detox Starter Series last month. I see it so often....people doing everything right... they are sweating, eating healthy, trying to take care of the gut, and hydrating, but things still arent working. Maybe theyre even taking binders, which helps, but maybe is not enough. ⠀ ⠀ There is a process called enterohepatic circulation that is not discussed enough. I think I got maybe 5 minutes of this topic in graduate school. This process is where all that toxicity that your lovely liver has bound up into a beautiful package is being released again in the intestines and sent back out into circulation and back through the liver yet again! So, basically, your liver is doing its job fairly well and packaging up those lovely toxins, sending them through the bile to the small intestine and then BAM, the GI system opens the package like a greedy kid on Christmas, recycles the bile and sends it back into circulation where the liver has to do it all over again (and the kidneys and lymphatics take a hit too!!). This IS a normal process b/c it is your bodys way of re-using the bile it created...the problem lies in the fact that our BILE is now TOXIC. So, we dont want to re-use that bile even though it is the most efficient way. We want to dump the bile with the toxins bound to it. ⠀ ⠀ So, why does this happen? Well, toxicity in general causes this issue b/c the toxicity is bound to the bile, but other things contribute too. It could be issues such as SIBO, parasites, chronic GI viral infections, or mold/candida colonized in the gut.⠀ ⠀ What can you do about it? calcium d glucarate 3x/day dosing, soluble fiber (think beans!) 4-6x/day dosing (1/4 cup beans each dose), some insoluble fiber (chia, flax, psyllium husk), intestinal binders like charcoal, fruit pectin or zeolite, and then of course a whole gut protocol to get rid of the bacteria that is releasing the beta-glucuronidase enzyme that could be part of what is causing the issue. ⠀ ⠀ Dont let your liver do the same work time and time again...look into your gut in order to help your liver! - @twopurplecarrots on Instagram
