You are 4 cm dilated and you are going to have this baby Frozen in fear. Crippled in disbelief. One week before this event I sat in the same office explaining to my Obstetrician that things didnt seem right but was told that everything I was experiencing was normal. My concerns dismissed and a clearance to fly was written to travel to work. It will ALWAYS sting, no matter how much time passes. I did not challenge or question her advice. In my eyes at the time, the specialist knows best so everything is fine. I was a first time mum, what did I know?! I then went on to dismiss any other signs presenting themsleves as normal and did not back myself to stand up, speak up + demand more answers. I dont tell you this to feel anger or to feel sorry for me. I tell you this share my lessons in hope it will resonate with someone and potentially change the course of their life. The day my heart was broken. Was the day I found my voice. Trust was broken so I learnt to trust myself. It can be difficult to march to your own drum beat + listen to your instincts when the world around you is noisy. You learn to live life never being the same again. You learn to love harder and appreciate the simple things. You learn deep sorrow but also learn unspeakable joy. You learn that happy people have often experienced as much adversity as those who are unhappy. What sets them apart is that they have the good sense to manage their memories in a way that enriches their lives. No matter the tragedy look for the lesson. There will always be one! Today 8 years ago, a little baby boy was born. He was simply perfect 💙 but he was just born too early for this world. Maybe things could have been different but they werent. This is our story, This is our journey and will forever be the reason we bring meaning to our lives each + every day! *Who will be present during a C section?* . All of us at The Birth Collective are used to working on labour wards, where looking after women during and after C sections is pretty much our ‘bread and butter’. It is the first operation obstetricians learn to perform, and we would expect around 3 to take place every labour ward shift. 👀Though for us, this set up is very normal, we are aware that as parents-to-be, being wheeled into an operating theatre, awake, often with little notice, may be an alien and terrifying experience! We wanted to share a little insight to hopefully make you feel calmer if the situation arises for you. . 👥Many women and their partners, are often overwhelmed by how many people are present in the theatre. Remember that everyone there has a role to play in looking after you and your baby: . 👤Obstetricians - Surgeons who perform the C section. We need 2 (at least). The third, the consultant, may be elsewhere available by phone if there are complications. . 👤Scrub nurse - specially trained nurses assist in operations by scrubbing in, handle instruments, ensuring the surgeons are presented with the tools they need at the right time. . 👤Anaesthetist - A doctor specially trained at providing pain relief, and putting you to sleep if needed. They sit near your head, closely monitoring vital signs and can reassure you and talk to you throughout. . 👤Operating department Practitioners (ODP) - Assistants to the anaesthetist who prepare the environment and equipment they need . 👤Midwife - Even if birth is surgical, a midwife is there to support you, check over baby and help with feeding and postnatal recovery . 👤Paediatrician - Usually baby doctors will be present for any emergency C sections, and some elective ones to check the baby at birth and assist if needed. . 👤Healthcare assistants - vital team members for moving patients, cleaning and assisting other staff . Other staff members may be called on who aren’t listed here . Ok your baby isnt actually born covered in cream cheese! That thick white greasy substance is called Vernix and its amazing stuff! It helps protect your babys skin inside the uterus, kind of like a wet suit. The earlier your baby is born, the more vernix they will have. Babies born on or after their due dates may only have vernix in places like the armpits or groin, or none at all as it slowly wears away as the due date approaches. Vernix continues to be amazing after birth! It helps keep your babys skin moisturized and protects against bacterial infections. Until youve lived it, its hard for someone to understand the pain and trauma that comes with a pregnancy loss at any stage whether its the first trimester or the last. You create a bond with that baby the minute you know its in you. The excitement builds, you buy things, you pick names. And then, out of nowhere, for no reason, its just gone yet your body still hasnt caught up to the change. . To all the people in our community whove suffered loss, we see you and were so sorry. We need to normalize pregnancy loss in the sense that we need it to be OK to talk about. We need women to not feel shame. About 15% of pregnancies end in miscarriage in the first trimester. That number is large, but it still feels isolating. . Rather than telling you that miscarriage happens, or to just keep trying, were here to tell you that its OK to sit in your pain for a while and grieve. 👶NEONATE RESUS ____ This is a High Acuity Low Opportunity situation (HALO) for which the rate of case exposure doesnt give enough experience! (hint: practice this to death!) . At birth check for tone, colour, crying. . APGAR at 1 min and 5 mins (swipe right) . . Dry baby leaving vernix on and put baby to breast. . Dont clamp/cut cord until it stops pulsating some minutes later. . If not responsive or poor tone, clamp cord at 10, 15 & 20cm from umbilicus and cut between the 15 & 20cm mark to prepare for resus. . Check HR, give two inflation breaths and ventilate for 30s at a rate of 40-60/min before reassessing. . If HR 60-100 continue ventilating, reassessing every 5 minutes. . If HR below 60 commence chest compressions at 120/min 3:1 compressions:ventilation. Best evidence practice suggests the two thumb method: place fingers on scapulas and compress lower half of sternum with both thumbs. . If unresponsive to resus efforts after 2 cycles give adrenaline 35mcg 1:10,000 IV. (most guidelines state 10mcg/kg therefore 3.5kg neonate gets 35mcg). . Hypoxia is the cause of a neonate needing resus in 99% of cases, therefore if unresponsive to good ventilations and chest compressions for ten minutes outcomes are generally nil. . What do you wish for your children? . For me, I want to instill in them boundaries. Ones that I was missing with their dad, and ended so mentally traumatizing for me (financial abuse, emotional abuse, seeing sex workers while I had just given birth etc). So I never want my kids to ever have to experience that for years, and then come out of it fighting, wondering why we just didn’t say ‘no’, or listen to our own intuition. I want them to always know that they can leave, a party, a commitment, a job. Caitlin Graham found out she was pregnant with her second child on New Year’s Eve as wild bushfires tore through Victoria, turning the daytime skies dark and filling the air with stifling, black smoke. A magical tiny miraculous deep breath, a rare perfect gift for this moment of reflection. Here we are. 3 months. 3 months old. 3 months postpartum. Our 4th trimester coming to a close. I want to tear out my hair and wail. I want to leap for joy. I really just want to go to bed. We made it. This year long journey is coming to a close. And yet our life together is just beginning. What a year. What a fucking year. I went to the depths and came back with this beautiful soul in my arms. It took everything. And yes she was worth it and more. But god was it painful. I broke down. I crumbled. I turned to ash and dust. I screamed and sobbed and gagged. over and over. repeat and repeat. And then I roared. And then I held her. My Thea. My Moon. My Beloved. Our Ancient Song. Its #pcosawarenessmonth Its time to educate. Many patients with Polycystic Ovary Syndrome (PCOS) are given their diagnosis and told they may never be able to have children. This isnt true for everyone. The main issue that women may face with PCOS is lack of ovulation or irregular ovulation. There are various treatments that patients can seek out in order to have a baby of their own. Sometimes it involves taking medication. Other times it may involve more invasive measures such as intrauterine insemination (IUI) or in vitro fertilization (IVF). It may also benefit to be under the care of a Reproductive Endocrinologist. In addition, just because you have PCOS, it doesnt mean you cant get pregnant naturally on your own. Every patient experiences PCOS differently. Explore the options that work for you. =============== My experience: we needed help to get pregnant with my first two children. It took years of trying on our own, then with fertility treatments. It was emotionally difficult at times. The medications can be taxing. The procedures can be invasive. The process can be stressful. Theres a lot of bloodwork involved (for which my veins may never forgive me), internal sonograms, failed ovulation tests, and failed pregnancy tests.... but THEN theres the ONE POSITIVE test that changes your entire world that makes the entire process worth it. We needed a fertility expert to help us with our first two pregnancies. Our third - well, thats where PCOS threw us for a loop. I managed to get my PCOS under control for one year through diet and exercise and that resulted in our third pregnancy. Often times new parents are surprised at how little it takes to feed their newborn. When they see the small amount of colostrum their body is producing in the first days after birth they will sometimes worry if that is in fact enough. Most of the time, it is! Your newborn only needs about 1-1.5tsp (5-7ml) of your colostrum in the first two days of life. By day 3 they are eating more, about 3/4oz-1oz (22ml-30ml) per feeding. More times than not your body is making enough for your newborn in those first few days. If you’re pregnant and planning to breastfeed, get informed about how your body works, what helps this process, what can hinder the process and how much your baby needs. Talking to a lactation consultant or finding a breastfeeding support group like La Leche League can be a great place to start. - @misty_intuitivebirth on Instagram
