I saw something last week or the Q and A I did sparked this idea in my head. It’s interesting we put so much stock into special tests despite 96% of them having no utility in isolation and that there are very few validated test clusters. . . But most of the biomecdical model is built on an isolated understanding of how the body works. It focuses on just a few body systems and how form of something in the body must equal function. But special tests are often compared to arthroscopic findings mostly in people who already have pain. But I wonder what the statistics would be if you included just the general population due to the number of people with asymptomatic findings. . . So far the biomedical model has given us a model but there is more mounting evidence that we need something different and the body is more complex than the model sometimes conveys. Also our understanding of the body isn’t as vast as we’d like to think. For example there are still relationships in the body that aren’t fully fleshed out. Also not that long ago we didn’t really understand much about the human brain and surgeons used to think the meniscus in the knee didn’t do anything. . . As our understanding of the body as a system evolves the system we use to assess perceived problems needs to as well. The pendulum doesn’t need to swing to full anarchy like some perceive pain science purists to be but the needle does need to move. . . We keep living with this idea that we can objectively measure someone’s pain experience but everyone’s experience will be slightly different. The best we can do is adapt a model that recognizes strong points of the biomedical model but embrace other areas of uncertainty and allow us to adapt and evolve how we do things as new information comes out. - @strengthinevidence_physio on Instagram
