Your meniscus has two distinct regions that affect their ability to heal. . 🧠The red zone (outer edges) has blood supply, which means they can heal naturally. These meniscal tears may heal without the need for surgery. The white zone (inner edge) doesn’t have a blood supply and won’t heal naturally. Low back pain is among the leading cause of disability globally, and nearly everyone will experience this at some point in their lifetime. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. This situation is called cauda equina, and is a neurosurgical emergency. Suatu hari.. 4 bulan setelah hari pernikahan kami.. saya memulai #langkahkecil, dengan mengundurkan diri dari satu perusahaan tempat saya bekerja selama hampir 6 tahun. Keputusan itu diambil untuk mengupayakan ikhtiar untuk memperoleh keturunan setelah sebelumnya mengalami “blighted ovum”. Upaya saya dan suami ternyata ga sampai disitu saja.. ada proses panjang selama 2 tahun yang harus dilalui sampai akhirnya mengalami 3x “BO” dan mencoba “inseminasi” yang hasilnya belum sesuai harapan. Kecewa, marah, sedih, putus asa.. itu sedikit rasa yang bisa digambarkan saat itu dan pada akhirnya mencoba ikhlas. Tidak lama dari situ, kurang lebih 3 bulan Allah menitipkan anak pertama kami “Alula Mufia Lashira” dalam rahimku, dengan kehamilan yang lancar, sehat, tanpa kendala yang berarti. Saat itu kami paham bahwa memang ini waktu yang tepat dari Allah, hadiah dari Allah atas ikhtiar kami selama 2 tahun. Sangat bersyukur memiliki Anak yang tumbuh ceria, sehat, cerdas, membuat kami sangat jatuh cinta setiap saat.. dan pada akhirnya sekarang kami insyaAllah dititipkan kembali adik nya Alula dalam rahim. Throwback 😂💕🌸 Two years ago, I was in the final days of nesting and prepping my home-birth space! I was at the point where I didn’t think I’d ever go into labor, cranky and exhausted. SURPRISE I did indeed go into labor 3 days later. Marcus and I were taking walks every night with Mattie before her bedtime. We went team green and had no idea the sex of the baby. Fun fact I had the names picked out pretty early on but hadn’t shared them with Marcus yet in case he didn’t like them because I loved them 😂🤣 , So Marcus spent the final days of my pregnancy on baby sites searching for a boy/girl name. I vetoed all his ideas, and then the day before I went into labor, I texted him while he was at work “ Boy= Johnny Marvel + my girl option (which I’m not spilling 😂). He responded with YES DONE. That was easy!! Also keeping the name choices a secret from my mom was probably the HARDEST thing ever. I legit tell her everything 😂🤣 - @wearingmomjeans on Instagram
COVID restrictions emergency case recall. Patient presented 5 months ago - during the restrictions with an acute apical abscess under tooth 47. Pus was drained from the mesial canals. Felt good to get the patient out of pain and keep them away from the medical docs, who were busy! After dressing canals and resolution of symptoms the canals were filled and SDR base placed comp in access. Preps were Trunatomy prime Mesial canals and Protaper next X2 in D. It’s been quite around here because I recently endured a miscarriage. I have thought long and hard about if I wanted to share this part of my story. I needed a few weeks to attempt to grieve. October is pregnancy and infant loss awareness month. We are still nagivagating through. However, I wanted to honor our baby and bring awareness for all the women and families who suffer from this. After treatmeant for AML-APL leukemia I was told I most likely would not have more children. We have pursued some other options and every other option has been a, “no.” I don’t share every detail but I have shared a lot because AWARENESS is SO important! I now know the agony of losing a baby. I’m so so sorry for anyone who has endured the phyiscal and emotional pain and trauma caused by this. Or any type of infertility! Or any type of longing for something or missing someone. It has affected me in ways I could have never imagined and as you know we have been through a lot. This is a different pain. Leukemia is traumatizing, any life altering illness is. This is a different traumatizing! It’s not one and done after a few days or weeks. Cancer isn’t ether but it’s another hard I’m trying to learn to live with. I’ve needed time to physically go through it because the phyiscal effects are painful. However the emotional part is worse. I have shared my thyroid has had issues since I finished chemo. I didn’t realize how important normal range thyroid levels are crucial to how our the human bodies function. Especially how even more critical/life threatening it is for a healthy pregnancy. So once I learned I was pregnant my hematologist also ordered remission labs and thyroid labs and learned my thyroid had plummeted. I had emergency thyroid care and medications, adjustments to medications dosages, plus the fluctuation of hormonal levels due to being pregnant and then then going through a miscarriage. We were so HAPPY the past few months. We almost made it to 11 weeks. My name is Morgan and on June 10th, 2018 my husband Jeff & I welcomed our first child into this world. Thomas Jeffrey was born at 25 weeks and 1 day weighing 1lb, 11oz and measuring 12.5in long. I had no idea babies could be born this early in pregnancy, and was shocked when I visited my OB with pain and learned I was 5cm dilated! Tommy was born a few short hours later, his odds of survival slim. After 115 days in the NICU, we brought our little fighter home and have been blessed by his miraculous growth & hilarious personality ever since. In March of this year, in the midst of a global pandemic, I got some unexpected news - I’m expecting! This wonderful blessing felt terrifying... I wasn’t ready, I worried about having another micropreemie, I feared that Tommy’s progress would regress if I was taking time away from him to be in the NICU... the list goes on. Doctors still don’t know why I delivered early, so I’ve been seen each week throughout my pregnancy, and receive makena shots to help keep baby cookin. I’ve had to take time away from this account and my volunteer work in our local NICU (a passion of mine) because of anxiety and PTSD, but now that I’m 28 WEEKS, I’m back and here to stay! I know my micropreemie parent community will understand - some days the NICU memories seem manageable, while on others they’re crippling - and might even say a prayer or send a good vibe my way that the little boy I’m carrying now will make it to term. Know that even though my feed may have been quiet, I never stopped rooting for you, praying for you, and sending you all my love. The spleen is part of your immune system that besides being a filter for old cells circulating in your blood, it also is the main source of the body’s Complement system, which enhances your body’s ability to fight off a specific type of bacteria. Therefore, while you can live without your spleen, one of the trade offs is that you will require ongoing vaccinations against the most common of these organisms. Er produziert Progesteron, das wiederum die Körpertemperatur ansteigen lässt und dafür sorgt, dass sie für die nächsten ca. 10-16 Tage erhöht bleibt. Deshalb ist der Temperaturanstieg eine sehr sichere Möglichkeit, um festzustellen, dass ein Eisprung stattgefunden hat. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ 💡Nach einer Befruchtung benötigt die Eizelle knapp eine Woche, um in die Gebärmutter zu wandern und weitere 1-2 Tage, um sich in der hoch aufgebauten Schleimhaut einzunisten. Bildet sich der Gelbkörper vorzeitig zurück, sinkt die Hormonkonzentration ab und die Periode tritt schon ein, bevor sich die Eizelle einnisten konnte. Zyklen mit zu kurzen Gelbkörperphasen sind deshalb unfruchtbar. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ 👉Wichtig: Eine Gelbkörperschwäche lässt sich nicht am Temperaturniveau der Hochlage erkennen und auch nicht daran, wie deutlich oder schnell der Anstieg ausfällt. Das einzige verlässliche Zeichen ist die Länge der Hochlage, die mindestens 10 Tage dauern sollte - gezählt wird von der ersten höheren Messung bis zum Tag vor der nächsten Periode. Wische einmal nach rechts, um in der #kurvedesmonats zu sehen, wie sich eine Gelbkörperschwäche in der Zykluskurve darstellt. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ 👉Nochmal wichtig: Auch wenn die Gelbkörperphase grundsätzlich stabiler ist als die Eireifungsphase, können auch besondere Lebensumstände wie zum Beispiel großer Stress oder das Absetzen der Pille kurzzeitig zu verkürzten Gelbkörperphasen führen. Das ist im Einzelfall unproblematisch und bedarf keiner weiteren Abklärung. Treten häufiger verkürzte Hochlagen auf, würde ich empfehlen, den Frauenarzt darauf anzusprechen. ⠀⠀⠀⠀⠀⠀⠀⠀ Insgesamt weisen knapp 10% aller Zyklen eine verkürzte Gelbkörperphase auf. Grundvoraussetzung, um sie an der eigenen Zykluskurve zu erkennen, ist natürlich die richtige Auswertung der Temperatur - wenn du dir damit unsicher bist, können wir deine Kurve gern zusammen unter die Lupe nehmen! - @undtschuesshormone on Instagram

Consistency is the key to minimizing mistakes. So I use the ABCS method every time I look at a plain film. X-ray of the hand: Alignment - look at every bone and joint space to make sure they are properly lined up. Bones - trace the cortex of every bone to make sure there isn’t a subtle fracture. Displaced fractures are easy to see. By tracing the outline of every bone you’ll pick up more subtle fractures and cortical buckles. Also a good opportunity to look for destructive lesions. Cartilage - check for joint space narrowing and subchondral fractures. Soft tissues - look for gas, swelling, foreign bodies and calcifications. Hopefully this is a helpful review for you and helps you out next time you’re looking at a hand X-ray. #anatomyoverlays @theradiologyguy - @theradiologyguy on Instagram

Read on to find out more about my review areas on a hand X-Ray... 👨🏽💻Hand x-rays are performed in cases of patients with suspected arthritis to try and evaluate the cause but more commonly are performed in cases of trauma 👨🏽💻When it comes to trauma there are a few areas I look at in particular every time - these review areas have helped me become more confident when looking at these X-Rays and hopefully they can do the same for you: ▫️Always start any trauma related X-Ray by looking at the soft tissues - the site of swelling can lead you to the fracture ▫️Check the metacarpal heads - an indented 4th or 5th metacarpal is common after a punching injury - termed a Boxer’s fracture. Don’t stop there in these patients - draw a line across the base of each metacarpal and this should join up. Sometimes the 4th/5th metacarpals can dislocate and travel downwards so they overlap the carpal bones - here your line won’t join up. Overlapping bones on a hand X-Ray should always make you think! ▫️Outline the base of the thumb metacarpal - forced abduction can cause an intra-articular fracture known as Bennett’s fracture (or Rolando fracture if comminuted) ▫️Do all of the fingers look well aligned? Check for subluxation and dislocation - you may need further views to be sure ▫️When it comes to carpal bones, scaphoid injuries are most common but you will need a whole scaphoid series. A normal X-Ray can’t exclude a scaphoid fracture. Practice varies with what to do next, some bring back people with snuffbox tenderness in 7-10 days to do an X-Ray, some to do a repeat clinical assessment whilst in some institutions they come back for an MRI ▫️The spaces between the carpal bones should be equal, if you see a widened distance between the scaphoid and lunate consider scapholunate dissociation (Terry Thomas sign) 🔻As always take your time, zoom up and outline the cortex of each bone, use a system and check two views! @theradiologistpage - @theradiologistpage on Instagram

VITAL PULP THERAPY because It’s not all about anteriors in real life dentistry !!! Big decay on tooth 17, no pain, sensitivity test is normal. Partial removal failed. I went for full pulpotomy and direct pulp capping with bioceramics (total fill cement and total fill bc putty fast set) and direct composite as long term restorative treatment. To me, premixed putty bioceramics take the huge advantage that it can be drilled with no setting time. So much faster than MTA or Biodentine. More expensive also !!! This patient was hit by a vehicle and suffered significant fractures as seen in the CT scan in the second image. The midface (LeFort segment) was completely pushed in by the force of the impact. Using Rowe forceps, they are clamped on to the palate and inside the maxilla. Using a gentle rocking motion, they are used to complete the fracture and pull out the LeFort segment, reducing it back into anatomical position. If you listen closely, you can hear the crack when the maxilla is finally mobilized. As seen on CT, this patient also had a mandible fracture, bilateral zygoma fractures, frontal bone and skull fractures which were all repaired at the same time. When I was a chief resident, a 19 year old kid was brought into STC having been shot multiple times in his chest. He rapidly deteriorated and cardiac arrested right in front of us. My team immediately responded with our resuscitative efforts. I then opened his left chest, quickly set up the Finochietto retractor, placed a clamp on his aorta and opened his pericardium to find a large amount of blood and multiple bullet fragments. I quickly whip stitched the holes in his atria and ventricle closed and began manual cardiac massage as we poured blood back into him. We worked on him for over 40 minutes. But we couldn’t get him back. I stood there holding this kids lifeless flaccid heart in my hands surrounded by a huge highly specialized team that could often do the impossible, but we failed that night. I failed that night. These moments make you feel like you aren’t good enough. They makes you question every single moment and decision. And then you begin to rationalize it some how and tell yourself “well this was some gang kid that probably got shot selling drugs etc.” And while that many times WAS the case in Baltimore, I never let those thoughts hang around in my mind too long. I didn’t want to use that as my crutch. I never wanted to dehumanize people, regardless of the circumstances that brought them in. The reality is that moments like this one, where I was unable to save a life, haunt me long after the time has past. I wake up at night in a cold sweat and routinely have nightmares about many of these dramatic moments where my efforts weren’t good enough. A lot of surgeons don’t feel comfortable openly talking about this because we are supposed to have extremely thick skin and be bulletproof ourselves... They are afraid to be vulnerable, but the reality is it’s a burden we all feel in one way or another. It’s also the reason why I ride my residents asses so hard and expect so much out of them because I truly want them to become physically and emotionally tough and better; better than I am and to never ever become complacent and always keep getting after it! - @adamthespecimen on Instagram

Cardiac tamponade is a large enough pericardial effusion (fluid around the heart), causing compression of the chambers of the heart and impedance of the venous return leading to decreased cardiac output. Causes include trauma, ventricular rupture post MI, cancer, pericarditis, post radiation, HIV, SLE to name a few. Patients will present with chest pain or shortness of breath. They will have tachycardia (elev HR), hypotension (low BP) with exam findings of JVD (jugular venous distention), pulsus paradoxus, muffled heart sounds (Becks Triad). In this case, we are discussing traumatic etiology. In the setting of trauma patient with hypotension, bedside ultrasound is performed to evaluate different causes. Patient is too unstable for ct scan. Other findings include cardiomegaly on cxr, electrical alterans on EKG. Ultrasound will show the heart surrounded by anechoic (black) fluid, end diastolic collapse of the right ventricle and dilated IVC without collapse. Once diagnosed, treatment is performing a pericardiocentesis using ultrasound guidance. We will be posting later on the details of the procedure. Most of you may know that it’s been very difficult road for me, suffering from endometriosis since I was 16, being told from several doctors that it will be very difficult for me to get pregnant. REAL INDICATIVO PARA CESÁREA: PLACENTA PRÉVIA ⠀ A placenta prévia, é causada pelo posicionamento da placenta, que se implanta na parte inferior do útero, cobrindo parcial ou totalmente o colo do útero. A cada mil grávidas, de três a seis mulheres tem o problema. Se a placenta estiver obstruindo completamente o colo do útero, impossibilitando o parto normal. ⠀ Durante a gravidez, a placenta se move conforme o útero é esticado e cresce. É muito comum a placenta estar no baixo útero no começo da gravidez. Porém, conforme a gravidez continua, a placenta se move para a parte superior do útero. Por volta do terceiro trimestre, a placenta deve estar próxima do topo do útero, de forma que o cérvice esteja livre para o parto. ⠀ O seu médico pode diagnosticar esta condição através de um ultrassom gestacional.
