I november 2019 våldtog en man en kvinna. En kvinna han kände sedan tidigare och han följde med henne hem. Där blir hon våldtagen, biten och utsatt för andra kränkningar och våld. Hon tar sig dagen därpå till sjukhus och anmäler våldtäkten. Mannen hävdar frivilligt sex och blir friad av tingsrätten. Enligt domen så är det ”högst besynnerligt att hon inte ropade på hjälp under det påstådda övergreppet” då en kompis till kvinnan sov i ett annat rum. Rättens ordförande, alltså den enda med juridisk kompetens och erfarenhet, vill döma honom. Det här visar än en gång på hur mycket kunskap påverkar. Att ens ha en idé om att en kvinna som blir utsatt för våldtäkt ska agera på ett visst sätt är förödande. Och motsägs av både forskning och erfarenhet av professionella. Jag vet inte hur många unga tjejer jag pratat med som berättar om övergrepp - ”men det var inte en våldtäkt för jag försökte inte slå eller sparka mig fri”. När sju av tio inte kan. Inte ens har kroppslig förmåga att göra något. Och därtill rädsla, chock, tankar om att inte göra något värre och allt det andra som tillkommer. Åter igen har rättssystemet (nämndemännen i det här fallet) berättat för kvinnan att HON gjort fel. Inte att HAN gjort det. Nu kom hovrättens dom. Det är alltså samma bevisning som i tingsrätten. Men de värderar bevisen annorlunda. Hovrätten menar att det varken är konstigt i sig att hon anmälde först dagen efter eller hennes chockade agerande i samband med våldtäkten. Han dömdes nu till 2 år och tre månader för våldtäkten. Och det är bra. Det är en upprättelse och en tydlig markering från hovrätten. Men INGEN utsatt ska behöva läsa att HENNES beteende, före, under eller efter våldtäkten ska ses som tecken på att hon INTE blivit våldtagen. Det får inte ske. A new paper by @drvictoriasampson explores the link between poor oral health and developing COVID-19 complications. What do the risk factors associated with developing complications from COVID-19 such as heart disease, diabetes, obesity and high blood pressure all have in common? Altered bacteria in the mouth. These COVID-19 risk factors also share similar dental complications such as periodontal disease and increased oral bacteria. This paper highlights the strong link between patients suffering from severe COVID-19 complications, and bacterial superinfections. Furthermore, new research has shown that high levels of oral bacteria are present in many patients suffering from COVID-19 complications such as pneumonia. The effect of the timing of norepinephrine initiation on clinical outcomes in patients with septic shock is uncertain. Advanced respiratory monitoring in COVID-19 patients: use less PEEP! . . . Discussion: We demonstrate that mechanically ventilated patientswith COVID-19 have a relatively high lung compliance, high dead space ventilation, and gas exchange impairment. In almost all patients, lung compliance decreased and dead space ventilation increased with increasing PEEP levels. The decrease in lung compliance and increase in dead space ventilation in response to higher PEEP levels indicate that COVID-19 lesions were not recruited and that higher PEEP levels cause hyperinflation of the more compliant parts of the lung [1]. These results are in accordance with recent findings in COVID-19 patients [2]. When lung compliance increases in response to higher PEEP levels (patient 1), recruitment is likely and PEEP should be set accordingly [1, 2]. All patients responded extremely well to prone positioning, although the exact mechanism is unclear. Redistribution of blood flow seems to be an important mechanism. In conclusion, we show that higher PEEP levels de-crease lung compliance and in most cases increase dead space ventilation, indicating that high PEEP levels probably cause hyperinflation in patients with COVID-19.
