Let’s talk about Vital Signs! ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 🏩 Vital signs provide us with critical information needed to make treatment decisions. Above are the normal ranges for ADULT vital signs. Have these values memorized, you will need them for the rest of your nursing career! 🩺 We typically monitor our patient’s vital signs q4h, q8h or q12h (1-3 times) every shift but it varies for every patient! Know your patient’s baseline vitals - it will be easier for you to determine any abnormal values. I like to take a look at the patient’s chart prior to the start of the shift to make note of their previous vitals. Remember each patient will have different baseline values depending on their current health and health history. ❗️ Remember: patients with COPD often have lower oxygen saturation and the range between 88-92% is normal and safe. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 👩🏻⚕️ Nursing students: report any abnormal values to your clinical instructor or your nurse. When the results for my COVID test came two days later, doctors were doubtful about my reaction. I told them not to worry since Im a doctor myself. I knew that I could handle whatever the situation might be. Thats when they revealed that I was COVID-19 positive. As soon as the news spread, many of our neighbours panicked. The city administration council put up posters outside our door. People clicked photos and shot videos of my house. They shared it with my name, address, and personal details on different WhatsApp groups. Since people love to gossip on WhatsApp, ugly stories about me started circulating in social media. Even the news media put out the wrong kinds of stories. Despite going straight to the hospital from the airport, I had to be the victim of social media hatred. I didnt even go home to collect my personal belongings to make sure that I didnt transmit the virus to anyone. Thankfully, I started recovering and, after a few days, tested negative. But the hounding didnt stop after I was back home. I knew that going back home will be an unpleasant experience. Therefore, to avoid attention from neighbours and media, I insisted on going home late at night. Not just me but my entire family had to experience the harassment. My younger brother didnt step out of the house for two months. Neither did I. I would only step out during the night to avoid people. When I step out, I can still sense the eerie stares and gossip. It is like a bad dream - Im all alone. People refuse to hear when I plead to them that I didnt do anything wrong. The only way we can fight this pandemic is with the right balance of precaution and empathy. Yes, we need to be careful. But we also shouldnt let our fears overpower humanity within us”. Celebrated my last session of removing toxins from my mouth. 1 gold crown, 2 amalgam fillings, 5 mercury fillings. Took me close to two months and a ton of pain, but worth it. My CT scan showed no jawbone canvitations-whew! However, it did show my nasal tissue and sinuses are about 2-3x the normal size, which means i 99% do have MARCONS . Now I will treat my sinuses with Silver or BEG nasal spray. Please let this explain my 3 year chronic headache and pressure, among other things. Seeing a holistic- bioidentical dentist, my local CIRS doc, and the brilliant @dr.jess.md has been the best decision I have made in this journey. I’m not feeling better just yet, it may take a very long time. At least for once i feel like i am the right track. I most likely have a co-infection from being exposed to toxic mold most of my life. I’ve had positive tests for an old infection to lyme, still probably present and who knows what else. Stealth infections like to party together unfortunately. Does this look silly? I wish this were a joke!! Unfortunately due to a massive shortage of personal protection equipment, this is what doctors on the front line are wearing. #cdcguidelines recommend covering your hair, wearing protective glasses, and wearing a mask. However since we dont have enough equipment, this shower cap, swim goggles, and bandana is CDC approved and what many health care workers will have to wear for protection. Again, this is what we health care providers have to wear NOT because it is the best way to prevent #covid_19 spread or because its the best way to protect ourselves as health professionals, but because there is NOT enough medical equipment for nurses, surgical techs, EMTs, PAs, MAs, medical trainees, doctors, etc. . I am scared to practice medicine. My husband and I are both doctors. We are doing everything we can to protect ourselves, each other, our children, our parents - but it may not be enough. If your doctors are afraid, worried, or scared then you should be too. If you dont know of anyone who has been affected by Covid-19, you will. If you dont know of anyone who will die from Covid-19, you will. This is not some sort of scare tactic- this is our new reality. This is meant to warn you to take this seriously, like really seriously! . We are working hard to protect you and your family. Do your part and STAY HOME to protect us- the health care providers caring for you all. This really should not be that difficult - your life and mine are on the line. . . COVID19 is deadly. So were viruses that killed Indigenous children in residential schools. Today is #OrangeShirtDay, an opportunity for us to reflect on Indigenous peoples experiences, forced, year after year, generation after generation, to surrender their children to Canadas Indian Residential School System (IRRS). In this system, school graveyards were as common as school playgrounds, with 24 to 69 percent of children dead. Our Canadian healthcare system, often revered for its universality and accessibility, played a sinister and prominent role in this genocide. We better understand today that the historical relationship between Canadian healthcare systems and Indigenous peoples has been one of colonial violence, otherwise known as medical colonialism. This is why Dr. Samir Shaheen-Hussain, Dr. Suzanne Shoush, @SemirBulle & I teamed up to share our thoughts in a new opinion piece published today in @TheTorontoStar. It outlines why Orange Shirt Day is an important national day to honour the resilience, strength, dignity, and perseverance of Indigenous Peoples in Canada despite the genocide of the IRSS. But it also asks us to reflect on this painful part of Canada’s history with medical colonialism as we strive to begin healing these wounds by committing to building a relationship of understanding, trust and respect between our healthcare systems and Indigenous peoples. If we are ever to see any true, lasting solution to ending health inequities faced by Indigenous peoples in this country, including those exacerbated by the COVID-19 pandemic, this is a necessary first step. A few tools that I’ve found super helpful this year have been my iPad mini and Apple Pencil. At the beginning of the year, we traded in our old iPads for these new minis. During the day, I’m able to look up information using resources like UpToDate and Epocrates. I can also write notes using the Notability app which helps keep everything organized. My friends @vranpariya and @joshrosh recommended getting an @apple Pencil earlier this year and it’s been so useful in the clinic. Today is World Suicide Prevention Day. Did you know over 300 physicians die by suicide annually in the US (JAMA, 2019)? Suicide rates for male doctors is 1.41 times higher than the general male population, and rates for female doctors is 2.27 times greater than the general female population (American Journal of Psychiatry). Many precipitating factors of burnout include work-load, lack of autonomy & meaning at work, and work-home conflict. It takes more than an occasional wellness activity to fix the issues at hand, but checking in with yourself and loved ones is the first step. Yesterday was my first day as a home heath and hospice nurse and I truly think I’m going to love it! My schedule is 9-5:30 M-F with on-call hours if my patients aren’t doing well or pass away. I’ll be acting as an RN and a case manager and coordinating care, meds, DME, and other medical supply delivery with MDs, NPs, RNs, HAs, LVNs, chaplains, social workers and more! I’ll also be spending time getting to know my patients and their families while doing my assessments and making sure they have everything they need and all of their questions are answered. I feel so blessed to be able to work with patients and their families during difficult times and to support them through the grieving process. Being a patient advocate has always been a passion of mine and I love that it will be a large part of this job! To the new nursing student: welcome to the crazy world of nursing school. You’ll probably feel like you’ve got this for the first few days… and then you’ll quickly realize the amount of work involved for the semester. Try not to completely freak out. It’s normal to be overwhelmed once you’ve received all your syllabi, assignment due dates, clinical dates, tests, exams, lab dates, etc. OH and don’t forget the chapters and chapters of reading. I’ve been there - it’s A LOT. So, here are my suggestions: Print off a 4 month calendar sheet and write out your entire semester’s due dates - this is a great way to visualize your heavy and light weeks You will not be able to read through every single assigned reading page - you’ll have to be selective. Look to see what the professor emphasizes in lecture + in the powerpoint slides - focus on reading THIS information in the textbook For all my visual + auditory learners out there, check out Khan Academy on Youtube. They do a great job of breaking down common nursing school concepts Don’t completely get rid of your ‘normal routine’ in order to 100% focus on nursing school.. you will eventually burn out 😔 Even if it’s 1 or 2 walks a week, a dinner with friends every other week, date night on Fridays… try to maintain some sort of school/life balance Annndd the most important tip I can give (I’ve already shared this many times before): DO NOT JUST MEMORIZE EVERYTHING - genuinely learn & understand the concept. It won’t do you any good to simply memorize everything.. maybe you’ll get a better grade, but you’re studying to save lives.. not to ace every single test in school - @yournursingeducator on Instagram
