Después de 5 meses y al ver que crece y que produce dolor local e irradiado al oído. Al examen clínico podemos ver una masa tumoral ulcerada que produce desviación de la lengua hacia el lado de la lesión. Esto es un signo clínico de infiltración de los músculos de la lengua. VITAL PULP THERAPY because It’s not all about anteriors in real life dentistry !!! Big decay on tooth 17, no pain, sensitivity test is normal. Partial removal failed. I went for full pulpotomy and direct pulp capping with bioceramics (total fill cement and total fill bc putty fast set) and direct composite as long term restorative treatment. To me, premixed putty bioceramics take the huge advantage that it can be drilled with no setting time. So much faster than MTA or Biodentine. More expensive also !!! The ABDOMEN X-RAY: I alway found this exam to be tougher than the chest X-ray. So what do I look for? 1. Bowel - small bowel is central and large bowel is around the periphery. Look for dilated loops of bowel filled with gas and or air/fluid levels to suggest obstruction. 2. Check for free air under the diaphragm. That’s a sign of bowel perforation. Your next move should be to pick up the phone and call the referring physician. Do not miss this! 3. Look at the organs, particularly the liver and spleen and determine if normal in size or enlarged. 4. Scan the kidneys and ureters and check them for calcifications indicating stones. 5. Look for calcifications in the right upper quadrant which indicates gallstones. 6. Check the bones of the spine and pelvis. Look for fractures or destructive lesions. Honestly, there is a lot more information that can be gathered from an X-ray of the abdomen, but those are some of the basics and I hope this helps you guys refine your search pattern. #anatomyoverlays - @theradiologyguy on Instagram

Elevated homocysteine is associated with significant increases in dementia incidence. . 👉🏼In the Rotterdam Scan Study [2003;126(1):170-175], which used MRI images of brains to prospectively examine the relationship between homocysteine and brain health, high homocysteine was associated with significant brain atrophy. . 👉🏼Smith et al. conducted a 2yr RCT investigating the effect of combination B6/B9/B12 on rate of brain atrophy. In the images above, on the left is a brain MRI with lots of blue/orange/red, indicative of brain atrophy in the placebo group. . 👉🏼However, on the right is an image from the intervention, in which the B-vitamin intervention resulted in significant protection against brain atrophy. [ PODEMOS TRATAR CONSERVADORAMENTE FRATURAS DOS OSSOS DA PERNA?] As fraturas da diáfise da tíbia estão entre as fraturas mais frequentes dos ossos longos. Uma porção considerável delas são expostas e geralmente consequente a trauma de alta energia acometendo um grupo de pessoas jovens e ativas. Para essas situações, o tratamento cirúrgico está bem estabelecido através da osteossíntese interna, quer seja com placa quer seja com haste intramedular. Quando a cobertura cutânea apresenta lesão mais complexa, a fixação definitiva pode ser feita com fixador externo (linear ou circular). Entretanto, em situações especiais, podemos lançar mão do tratamento incruento preconizado por Sarmiento, chamado de tratamento funcional (imobilização gessada permitindo carga e mobilidade do joelho precoce). Atualmente, para o tratamento incruento, a fratura deve ter pouco desvio, não ter lesões cutâneas e reservada a pacientes com pouco demanda funcional. Sarmiento preconiza inicialmente gessada cruropodálica com possibilidade de carga de acordo com a tolerância do paciente, por um período inicial variando entre 3 a 6 semanas, dependendo da estabilidade da fratura. Após esse período, troca-se essa imobilização gessada por um gesso PTB (Patellar tendon bearing). Esse gesso circular, usa uma pequena quantidade de algodão ortopédico nas proeminências ósseas, moldagem no tendão patelar de tal forma que permita extensão e flexão total do joelho, tornozelo mantido plantígrado e carga estimulada desde o início. Splenic laceration It may not look like much, but this can be a deadly injury if not dealt with quickly and definitively. This injury is also one of the most frequent reasons for requiring emergency surgery after blunt force trauma. The spleen is part of your immune system that besides being a filter for old cells circulating in your blood, it also is the main source of the body’s Complement system, which enhances your body’s ability to fight off a specific type of bacteria. Therefore, while you can live without your spleen, one of the trade offs is that you will require ongoing vaccinations against the most common of these organisms. O antebraço da criança As fraturas do antebraço na crianca são o 3o lugar entre as fraturas mais comuns na criança. Geralmente ocorrem por queda em cama elástica e queda de brinquedos como monkey bar retratado na foto. (fig.2) Usualmente se apresenta de forma pouco grave, necessitando muitas vezes apenas de imobilização gessada por 4 a 6 semanas.(fig. 3) Em crianças menores, devido ao potencial de remodelamento ósseo aceitam-se maiores desvios e espera-se um ótimo resultado funcional com desaparecimento da deformidade na maioria dos casos tratados com gesso. Embora em crianças mais velhas o tratamento não-operatório também seja possível, no caso fraturas irredutíveis ou instáveis preconiza-se o tratamento cirurgico pelo menor potencial de remodelamento. Dentre as opções terapêuticas citamos por exemplo tutores intramedulares como fios de Kirschner (fig. 4) ou hastes flexíveis, esses de uso mais frequente. Não podemos esquecer também que o tratamento com tutores extra-medulares como placa e parafusos não é exclusivo aos pacientes adultos. Essas fraturas, devido a anatomia (fig.5) podem desencadear consequências ainda mais graves do que a própria fratura como lesões nervosas, vasculares ou sindrome compartimental Mesmo após o tratamento correto a criança deve ficar afastada de atividades de alto impacto por pelo menos 6 meses ou até a recanalização do osso visualizada nas radiografias devido o alto índice de refraturas. A spatial notion, combined with a deep knowledge of head and neck anatomy allows the development of techniques that brilliantly restore a large part of the aesthetic-functional complaint that a patient already mutilated by cancer usually suffers. Today we bring you a case of a 65-year-old patient with a dysphonia history of approximately 1 year evolving with a growing cervical mass. Cervical dissection was then performed using Crile (sacrifice of the accessory nerve, jugular vein and sternocleidomastoid muscle). A flap of the pectoralis major muscle was planned to cover the defect and protect the carotid artery. Note that the design and planning of the flap that involves the entire vascularization considering the thoracoacromial artery and its pedicles that irrigate this entire region. Today’s case is this crazy infection! This young 22 year old male developed necrotizing fasciitis, which caused this massive debridement to try to control the infection. . Nec fasc is a SCARY infection, due to the rapid spread through the tissue. It is normally caused by a polymicrobial organism (multiple bacterial organisms) and it’s important to start broad spectrum antibiotics ASAP. If the infection is unable to be controlled, a debridement, like pictured above, is anther control measure. However, if the rapidly spreading infection cannot be controlled, the lose of limbs could be eminent. An even worse outcome would be the infection spreading systemically causing multiorgan failure due to sepsis resulting in death. This is why if you’re concerned for nec fasc, you promptly want to order the proper tests/imaging and if possible, get a surgeon to lay hands on the patient to get their input! . How it starts is can be something as minor as a small break in the skin. The horror stories you hear is “they had a cut on their foot and they got a pedicure, causing the entry of bacteria and development of nec fasc.” You also may be seeing the spike in stories in the news where people are swimming in areas that have high concentrations of certain bacterias that cause nec fasc. Those people enter the water with a small cut on their extremity which is an entry point for the bacteria. In this case, it seems like he had a laproscopic surgery and the incision site (below the belly button) dehisced (opened up) and got infection. This may have been an entry point for the bacteria to spread to his right leg and scrotum. . With all this being said, THIS IS VERY RARE! So don’t stop swimming or getting your pedicures because this image may scare you! Chances are you won’t be the 1/1000000 and you’ll make it through life without even coming close to being diagnosed with nec fasc! - @trauma_time on Instagram

What a day yesterday. I got to spend some time with @teagueholmes and were both injured. We came upon a parking attendant who told us that we could park over there and run over to the other side of the building. We both cant run, so I drove him over there and he hopped out of the car to do his errand. After that I went to the doctor for my 6 week checkup and he said that I could now sleep without my back brace on We both immediately started reminiscing about the time we got to ski the Grand Envers with Cédric Aubert. A espessura da fáscia pode ser aumentada para mais de 4,5 mm. . Ultrassom é, freqüentemente, a modalidade de imagem inicial de escolha. A ultrassonografia geralmente mostra aumento da espessura da fáscia ( 4,5 mm) e uma fáscia hipoecóica . Os achados de ressonância magnética incluem: - espessamento fascial plantar: frequentemente fusiforme e tipicamente envolve a porção proximal e se estende até a inserção do calcâneo - aumento da intensidade do sinal T2 / STIR da fáscia plantar proximal - edema da almofada de gordura adjacente e tecidos moles subjacentes - edema medular limitado dentro da tuberosidade medial do calcâneo também pode ser visto. A 55-year-old man with diabetes mellitus type 2 presented with persisted right homonymous hemianopia of 72 hours evolution. At admission, the laboratory exam revealed blood glucose of 497 mg/dL and hyperosmolality without ketosis; and the EEG did not show epileptic discharges. EEG done during the following days did not show any abnormalities. Brain MRI showed a subcortical lesion on the left occipital lobe (figure). Patients symptoms and MRI abnormalities resolved favorably after glucose-lowering treatment. Nonketotic hyperglycemic hyperosmolar state is a condition associated with neurological deficits that could mimic an acute ischemic stroke. The underlying mechanism for MRI findings in this condition have not been fully elucidated, however, it has been postulated that increased plasma osmolality with the subsequent development hyperviscosity may lead to a hypoxic-ischemic insult resulting in the release of free radicals in the brain. 01/07/2017 Há 2 anos atrás, eu estava indo pra um hospital bem longe, onde eu não tinha ideia do que aconteceria quando chegasse lá, a única coisa que sabia era que ia ser difícil, eu teria que ser o mais forte possível e que era realmente uma cirurgia complicada com alguns riscos. A cirurgia acabou, os dias de internação foram passando, a dor era horrível, a sensação de que eu não iria mais ser a mesma era grande, e a vontade de desistir era constante, mas Deus esteve cada segundo ao meu lado, me deu forças, e finalmente tudo passou. Eu realmente mudei, tenho cuidados que não tinha, dou bem mais valor à vida, e sei que se não fosse essa cirurgia, hoje provavelmente estaria em uma cama, ou pior, e com problemas bem maiores. Sou grata a cada dia por isso, e por mais que ainda exista dificuldades, não teria uma alternativa melhor! Sou grata também a toda a equipe médica que esteve comigo me dando todos os cuidados possíveis, a minha família e amigos que me deram forças, e principalmente a Deus, pois sem Ele nada seria possível, e eu nada seria! Low back pain is among the leading cause of disability globally, and nearly everyone will experience this at some point in their lifetime. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. This situation is called cauda equina, and is a neurosurgical emergency.
