Incredibly, he was conscious and breathing spontaneously. After stabilization, reconstructive surgery was planned by a multidisciplinary team. The right eyeball was avulsed (not found) and the left eyeball was displaced anteroposteriorly. The last image shows the final aspect of the reconstruction. I looked him dead in the eye, reached between my legs, grabbed the blood, and marked it across my face like war paint. Then I started laughing. Without words, two other women reached between my legs and put my blood on their faces. It felt very primal and raw, and for a moment I became an animal, my ego took the back seat, and I felt primordial energy surging thru my body. I felt present, light yet intense, and connected to everything. Menstruation is such an interesting experience, I often felt connected to a deep part of myself when I am bleeding. Maxillofavial Trauma Management - Airway ———————————————————————— After we handle the immediate life threats (massive hemorrhage), we have to consider the airway. Look, listen, and feel: take a look at the injuries and look inside the mouth for blood, vomit, broken teeth, dentures, chewing tobacco, etc. If any of these are present, grab your suction and clear it out. If dismounted or without mechanical suction, grab a 60cc syringe with a cathedral tip and a put an NPA on it to create a field expedient manual suction device. Give the lungs a listen and see if you’re getting adequate air movement. If the environment is keeping you from listening, place you’re hands on the chest and feel for bilaterally equal rise and fall of the chest. ———————————————————————— I would personally prefer to skip the intubation attempt and go for a surgical cricothyroidotomy. Orotracheal and nasotracheal intubation may be attempted but be aware that musculoskeletal tissue disruption will likely increase the difficulty of intubation. LMAs and I-Gels can be used to buy you some time to prep for a cric, but they don’t prevent regurgitation and aspiration of vomitus and may increase vomiting unless a paralytic has been administered (which is unlikely due to the fact that we likely haven’t gained vascular access at this point in our assessment). Stupid hurts. Sunday, I banged my shin on a bike rack in the garage. Started bleeding like crazy. I didn’t want to pay for co-pay at urgent care so I came up with my own wrap. Was still bleeding a day later so then I had to go to urgent care anyway...and after paying the co-pay they sent me to the darned ER! And then the ER doc said too much time had passed so they couldn’t do stitches. So now I paid for urgent care and ER visit and yet got nothing but some over priced band aids. O tratamento inicial do indivíduo traumatizado prioriza a manutenção ou estabelecimento das vias aéreas, controle da hemorragia e estabilização das lesões a outros sistemas principais antes da avaliação das feridas de tecidos moles faciais. . . Após uma anamnese detalhada, o exame físico se faz de forma sistematizada (ordenada) e minuciosa. Nesta avaliação lançamos mão de manobras semiotécnicas como inspeção e palpação. Inicialmente investiga-se todo tecido mole da região de cabeça, face e pescoço, podendo o paciente apresentar lesões como abrasões, contusões e lacerações. . . Sobre as lacerações, devemos nos atentar à extensão, profundidade, presença de sangramentos ativos (hemorragias) ou corpos estranhos e sujidade, região anatômica e relação com estruturas vitais, tempo do trauma, formato da laceração, mecanismo do ferimento, sinais de infecção e vitalidade do tecido. . . A reparação dos tecidos moles pode ser feita sob anestesia local ou geral, dependendo das circunstâncias, e o tratamento consiste basicamente no debridamento da ferida (lavagem da pele com soro fisiológico 0,9% e remoção de corpos estranhos) e sutura por camadas do tecido lesado. . . Atenção: a laceração pode indicar o acometimento de estruturas mais profundas, como fraturas ósseas. O caso clínico elucida justamente isso: paciente vítima de acidente motociclístico evoluindo com extensa laceração em hemiface esquerda e fraturas nasal e maxilar, tendo avulsão de um dos segmentos fraturados. . . O paciente foi abordado imediatamente após admissão no hospital, sob anestesia geral, pois apresentava quadro de sangramento contínuo. This is probably my second foam latex paint job and application. The facial prosthetic itself is from Rubie’s costumes and was given to me by a friend a few years back (@gorybutlovely I hope it did you proud girl 😊) It was a little daunting to me, painting foam latex but I think I pulled it alright. Definitely learned some new things during all this ———————— Products used: Stitched foam latex facial prosthetic from Rubie’s costumes. Prosaide for application and edge blending Bald cap two different prosthetic transfers from @tinsleytransfers (stitched, and Frankenstein bolts on the neck) Homemade pax paint (acrylic paint and prosaide) I tinted for skin tones and initial base paint job. @skinillustratorofficial alcohol palettes to add depth to the cuts and wrinkles and break up skin tones (bloody five, Joel Harlow on set, and my zombie palettes) I added a touch of blood to the cuts later using fake blood from redrum (The eye was photoshopped. I’m basically blind without glasses or contacts, and with having to cover one of my eyes underneath the prosthetic and using the mesh contact lens I wanted in the other eye would’ve resulted in me being just about totally blind. Another fun one from @forlifeabc this was a busy day on set, shooting more of the prison yard riot/ fight. I applied this makeup on Peter Greene We didn’t know what the director wanted, until the day, so we brought a lot of appliances to come up with something cool. In the show he gets hit in the head with a 45 lb plate weight. This piece had a great look to it. We talked pulp fiction and being the villain in the mask with Jim Carey 😂 I had a few makeups to do that day so I stayed in the makeup room and wonderful artist @libbyrosemakeup handled his on set blood dressing! Hey everyone. As most of you know last month I crashed hard while practicing at Conondale. The result was fractures from T4-T7 and 10 broken ribs. I was lucky that medics were on site and all the correct procedures were taken. Initially I suffered with some numbness across my chest but that has resided after having a successful surgery 4 days after the accident. The first month of recovery has been tough but now I’m feeling really mobile and have completed my first day of physical rehab 😁 It was literally 10min of the lightest possible exercise you could imagine but a massive step for me. Anyway, with every event that happens in life their is equally drawback and benefits and I can see many positives that have come with this injury already. Thank you so much to all medical personnel that was involved in assisting. You guys are my true hero’s 👊🏻 I’m also very thankful that Australia has such a great medical system available to everyone 🇦🇺 I will post more often to share where I’m at along my journey. Today’s case we have this 20 year old male who was involved in a motor cycle accident! The poor guy was just riding along when he was tboned by a car that was not paying attention. The patient was NOT wearing any safety riding equipment (including helmet) at the time of the accident. As a result, his foot was crushed between the car and the cycle. To add salt to the wound (no pun intended), the car that struck him fled the scene! . Upon arrival to the ED, trauma protocol was followed as this mangled foot could just be a distracting injury. Luckily, no other injuries were found, including on the helmet-less head and neck! They found his foot to have a double lisfranc fracture (displacement of the metatarsophalangeal joints due to fracture or injury), dislocated navicular bone and dislocated and fracture talus. In this case, they were able to save his foot! Due to the extensive damage to both bone and muscle, however, it will take many monthly to years to be able to walk normally again! . I propose a question to everyone...would you rather having this outcome whereas your foot is saved, but, even after possibly years of rehab, you may not be able to reestablish full motor control and sensory back to the foot OR have the foot amputated and just learn how to walk with a prosthetic? OOPSY... Well... Meeting new mountains can be a good thing sometimes but also dangerous. I fell into a river with a glass bottle in my hand. (not even alcohol...). I severed two flexor tendons and crushed a nerve. I need at least three months of intensive / daily rehabilitation. Im afraid well have to be a little patient with drawings and tattoos because surprise it is my right hand ! Haha I dont know how long it will take to recover and be able to use it as before. I will keep you informed as soon as I see improvement. SFX design- Tooth pick and Fork injected forearm!🔪💉 •• To Recreate This look: •Prep and Clean the area •First roll out your cotton balls into thin strips •Set the cotton strips aside and place your fork onto your arm then take some scar wax and mold it on top of the fork •After you have molded the scar wax on top of the fork put a coat of liquid latex on top of the scar wax and around the base of the scar wax •Put a layer of cotton where you just placed the liquid latex then put more liquid latex on top of the cotton and let the cotton and latex dry •Once your cotton and latex has dried apply your water or alcohol based paint onto the area •When you have finished applying the paint put your tooth picks into the cotton and latex •After you have painted the cotton and latex and put your tooth picks in you can apply your fake blood and your done! Most nasal amputations result from animal bites, human bites, and lacerations from glass. Successful replantation yields the best aesthetic and functional outcomes and is preferred compared with multistage nasal reconstruction. However, nasal replantation is technically challenging; establishing venous outflow can be particularly difficult. Today we bring you a extremelly rare case: a 17-year-old male sustained a complete nose and upper lip amputation in a motor vehicle accident. The midface segment was emergently replanted. Two arteries (left dorsal nasal artery, left superior labial artery) and 1 vein (branch of the left supratrochlear artery) were anastomosed using microsurgical technique. Was so happy with how these scars I designed for Grace (Lili Reinhart’s character) for Chemical Hearts turned out. I referenced a lot of car accident scars and knee replacement surgeries. And then there were the bruises from Grace hitting her knee with her cane. @brianspearsfx made and created the transfers for us and my key (and huge asset) @rachelhevesi applied and colored. Some days I’d tag in too. Many have asked how Bridger is doing with his healing. We are so grateful for the concerns, thoughts, and prayers. We have, in a very real and tangible way, been blessed by your positivity. The bottom right was the night of his injury. The bottom middle was after half the stitches came out the following week. The bottom left was after all the stitches were removed. The top two photos are before (right) and after (left) his first laser treatment. The laser treatment was amazing at resurfacing the raised areas and helping with the general coloring. However, it does take some time for all the redness caused by the laser to dissipate. We are so grateful to the amazing medical professionals who have gotten him to this point in such a short amount of time. @Dr. Joseph F.
