⚠️70-year-old female pedestrian was struck by a motor vehicle at high-speed, sustained multiple injuries including injuries to her lungs and head. She also sustained a mangled injury to her right lower extremity with what’s considered a full degloving of the extremity and multiple fractures and neurovascular injuries. Patient was taken to the operating room immediately and managed by multiple trauma surgeons for her injuries. A high above knee amputation was done. Above-the-knee amputations may be necessary for many reasons. These can include trauma to the lower leg, which results in a non-viable leg at or near the level of the knee. Below-the-knee amputation may adequately address a more distal injury. Many studies have attempted to create algorithms to help physicians decide when to reconstruct versus amputate. One of these is the Mangled Extremity Severity Score (MESS), which takes into account skeletal/soft tissue injury, limb ischemia, shock, and patient age. Stupid hurts. Sunday, I banged my shin on a bike rack in the garage. Started bleeding like crazy. I didn’t want to pay for co-pay at urgent care so I came up with my own wrap. Was still bleeding a day later so then I had to go to urgent care anyway...and after paying the co-pay they sent me to the darned ER! And then the ER doc said too much time had passed so they couldn’t do stitches. So now I paid for urgent care and ER visit and yet got nothing but some over priced band aids. While inside the mother, a fetus does not use the lungs to breathe — all oxygen comes from the blood vessels of the placenta. (Which is why nuchal cords arent as concerning, in most cases, as most think. Thats another post 😉) As the due date nears, the babys lungs begin to clear the fluid in response to hormonal changes. During a physiological vaginal delivery, 30% of this fluid is expelled by thoracic (chest) compression during transit through the vagina. After the birth, as a newborn takes those first breaths, the lungs fill with air and more fluid is pushed out of the lungs. Any remaining fluid is then coughed out or gradually absorbed into the body through the bloodstream and lymphatic system. When the thoracic compression is lacking or inefficient, such as in delivery via cesarean section, there is retention and subsequent delay in the resorption of normal fetal lung liquid. Scaling going WAY down. My bath days are coming and going right now. Typically my skin demands the bath every 48 hrs (previously 12 hrs, then 24 hrs, the increment between baths has been slowly increasing = a measure of progress. . Now, I dont HAVE to get into the tub to exist. I do love my baths and will always take them even after the psoriasis is gone. . Update on my bite on my arm situation!!! SWIPE LEFT!!! I’m out the woods and clear from the worst that could have happened! doc said all of the poison etc was pretty much flushed out and stopped from spreading. So being hooked to iv and putting doses of antibiotics straight into my vein was a success! Plus I’ve been heavy on meds, antibiotics etc for this whole time which helped tremendously as well! My arm over all is still sore and tender in a lot of spots. Also I don’t have my full feeling back in my arm yet but all in due time! 🙏🏽 the wound is still draining a little, but not half as much as it was before! So now I’m just allowing the hole/wound to heal up and close up all the way! As you can see the hole had gotten a good size smaller then before. I clean it a few times a day using a special soap called hibiclens to keep the bacteria out. But all in all it’s looking way better than it was THANK THE LORD🙏🏽 - @trillbarber on Instagram

Last fall, I had surgery to remove carcinoma from the side of my nose. A lesion that appeared to be the size of a small pimple was actually pretty invasive. It made my life pretty complicated for about eight weeks. The following are pictures documenting the surgery and the healing process. Am posting it because many of you might find the pictures useful as reference. And others might be fascinated by the specifics of the surgery (which Ive learned is actually quite common). Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued. 🚨🚨🚨🚨🚨GRAPHIC SWIPE LEFT!!!! LATEST UPDATE ON MY SPIDER BITE INJURY! I BEEN PRAYING ITS GETTING BETTER. I STILL HAVE A HOLE IN MY ARM. THE FIRST PIC WAS WHEN I PULLED THE BANDAGE OFF, and the LAST VIDEO WAS AFTER I WASHED IT WITH THIS SPECIAL SOAP THEY PRESCRIBED!!!! I have to clean it twice a day. BUT ANYWAY, I WENT BACK TO THE EMERGENCY ROOM TO HAVE THEM HOOK ME TO THE IV, SINCE THEY COULDNT GET ALL OF THE POISON OUT... THEY GAVE ME A LOT OF ANTIBIOTICS STRAIGHT INTO MY BLOODSTREAM! Which I believe was a big help to try to flush all of the rest of the poison/venom out. Also morphine etc for the pain! I definitely needed something for the pain! It’s non stop! Still keep sending them prayers up! Remember my main concern is that this poison doesn’t spread to my joint, lord knows I can’t have my arm amputated!!! So Im prayin that this will do the trick this time and everything will heal up normal and in a timely manner! My recovery went amazing! I was off the crutches 3 weeks after surgery and out the brace after about 6 to 7 weeks after surgery. Maxillofavial Trauma Management - Airway ———————————————————————— After we handle the immediate life threats (massive hemorrhage), we have to consider the airway. Look, listen, and feel: take a look at the injuries and look inside the mouth for blood, vomit, broken teeth, dentures, chewing tobacco, etc. If any of these are present, grab your suction and clear it out. If dismounted or without mechanical suction, grab a 60cc syringe with a cathedral tip and a put an NPA on it to create a field expedient manual suction device. Give the lungs a listen and see if you’re getting adequate air movement. If the environment is keeping you from listening, place you’re hands on the chest and feel for bilaterally equal rise and fall of the chest. ———————————————————————— I would personally prefer to skip the intubation attempt and go for a surgical cricothyroidotomy. Orotracheal and nasotracheal intubation may be attempted but be aware that musculoskeletal tissue disruption will likely increase the difficulty of intubation. LMAs and I-Gels can be used to buy you some time to prep for a cric, but they don’t prevent regurgitation and aspiration of vomitus and may increase vomiting unless a paralytic has been administered (which is unlikely due to the fact that we likely haven’t gained vascular access at this point in our assessment). Let me tell you about failure. This is a patient who had a bad firework injury (on the left) that I treated with a free flap (second from the left) which ultimately was not successful (second from the right) and needed a different type of flap (on the right) to cover the nerves and bones which were injured. I literally broke down in tears when the first flap was not successful. This is the first flap I have ever lost, and it was incredibly heartbreaking. I do a lot of outreach and speaking for students in high school and college. A question students sometimes ask is if I ever make mistakes. The answer is yes, of course, we are all humans and therefore will all make mistakes. The most important thing is to have grace for yourself and to remember to keep giving your best. Medicine is a place where mistakes and growth are often met with humiliation. ”I’ve been in two minds whether to post this but my husband convinced me as he said if I hadn’t been here he wouldn’t have realised. I am sure there are other parents who wouldn’t either and the only reason I knew is because it had happened to a friend’s son 2 years ago and she had shared. A week or so ago the littlest fell over at the zoo. He took quite a bashing but once we got home I cleaned him up. I rang school on Farm school day to make sure he washed his hands after digging and I tried hard to ensure it was kept clean (hand and elbow). He’s an 8 yr old boy, however. The wounds didn’t look infected- they’d got bigger so I was concerned but they weren’t gunky etc. Yesterday on our way to the beach he showed me his hand. I wasn’t happy as I noticed red tracking down his vein. I then checked his elbow - the same. I took him down to the out of hours feeling a bit silly but when the doctor saw it he commended me on recognising it and getting down ASAP. This is blood poisoning/ sepsis. It isn’t something you can “leave” until Monday when the doctors are back in the office. Thankfully the antibiotics are working and he is well in himself! If you spot this red line running from a wound get your child is seen straight away. Hopefully, my post might help someone the way my friend’s post from 2 years ago helped me 😍” This red line is called Lymphangitis. This means the infection is in the lymphatic system and it causes red lines from the wound that travel in the direction of the nearest lymph gland. If treated quickly, the condition can be mild but if left untreated, complications can occur, and the condition can become very serious. When the infection crosses over into the bloodstream, like it did for this boy, it’s known as sepsis. Well done to this quick-thinking Mama for noticing that something wasnt right and seeking urgent medical care for her little one. Lawnmower injury 🔪 shown here is a partial amputation of the 1st, 2 and 3rd phalanges (toes) of the right foot. These types of injuries are quite common in trauma and typically result in amputation due to the extensive damage caused by the lawnmower blades. Initial treatment includes Betadine rinse, Tdap vaccine if 10 years, and pain management. The patient was then treated with IV antibiotics and taken to the OR where an attempt to salvage was performed with pinning each toe. Ultimately, amputation of the 1st toe was preformed. SKOLIOSIS 2016 vs 2020 4 tahun lalu ketika saya medical check up, secara gak sengaja ketauan bahwa saya punya skoliosis (tulang punggung melengkung ke samping). Penyebabnya? Biasanya sih karena kebiasaan yang gak baik, misal posisi duduk yang ngasal selama berjam2 bertahun2, kebiasaaan berdiri yang gak seimbang (misal bertumpu di satu kaki), bawa tas berat yang selalu di salah satu pundak aja, dll Setelah saya tau saya skoliosis, saya jadi paham kenapa saya kadang2 merasa back pain (sakit punggung). Kalau sedang parah, duduk/berdiri aja bisa kesakitan Kita semua pasti ada ketidakseimbangan di badan (tentu dengan derajat yang berbeda2), tapi gak semua aware dengan kondisi kita masing2. Kalau kita nendang bola, misalnya, pasti ada kaki yang lebih dominan. Atau ada tangan yang jauh lebih aktif daripada yang satunya. Kalau kita sedang stretching, pasti ada satu gerakan yang tangan kanannya bisa sampai tapi tangan kirinya gak bisa, atau sebaliknya Itu semua adalah gejala2 ketidakseimbangan Untungnya ada temen yang mengenalkan saya ke guru yoga Mas @ariwaw. Today’s case we have this 20 year old male who was involved in a motor cycle accident! The poor guy was just riding along when he was tboned by a car that was not paying attention. The patient was NOT wearing any safety riding equipment (including helmet) at the time of the accident. As a result, his foot was crushed between the car and the cycle. To add salt to the wound (no pun intended), the car that struck him fled the scene! . Upon arrival to the ED, trauma protocol was followed as this mangled foot could just be a distracting injury. Luckily, no other injuries were found, including on the helmet-less head and neck! They found his foot to have a double lisfranc fracture (displacement of the metatarsophalangeal joints due to fracture or injury), dislocated navicular bone and dislocated and fracture talus. In this case, they were able to save his foot! Due to the extensive damage to both bone and muscle, however, it will take many monthly to years to be able to walk normally again! . I propose a question to everyone...would you rather having this outcome whereas your foot is saved, but, even after possibly years of rehab, you may not be able to reestablish full motor control and sensory back to the foot OR have the foot amputated and just learn how to walk with a prosthetic? - I had a haemangioma removed from my head under local anaesthetic. My GP initially tried freezing it with CO2, but it didn’t work. A haemangioma is just a blood filled lump that is usually no cause for concern. Mine was removed because it kept burning in the sun and getting caught in my clippers.

After having both my parents die from this shitty disease, I truly thought it would never cross my path again! But here we are! Not one of my most prettiest photos, but I am sharing this side of me and my skin. I felt an uneven and nasty growth on my back. So 2 weeks ago I called on a wonderful dermatologist here Sian Hartshorn and she had to remove it straight away and test for skin cancer. Thankfully I am clear of cancer, but my mother was not so lucky. We lost her to melanoma when I was a teen. So I religiously check my skin to ensure I am not at risk. Last year we also lost a good friend to skin melanoma. Please, do regular checks. Just as you would for all other health issues. We all tend to focus on breast cancer, but skin cancer is far more dangerous and prevalent for men and for women. And our sunny weather and exposure places all of us at greater risk than other countries. I am grateful my stitches are out but you can be sure I remain vigilant. And please look after your kids. All my freckles came from years running around carefree as a child in sunny South Africa without any sunscreen. Routine puncture wounds from blood draws turn into big bruises when one is on blood thinning medication! Quick update: discharged from the hospital yesterday. Now on two blood thinners as the first one I was given did not work. I am feeling so much better compared to just a couple of days ago. So very grateful. Awaiting lots of test results to help figure out why I ended up with the extensive blood clots to the veins in my liver. This is an unusual situation as my blood clots are very different than one gets with a DVT. As such, the Hematology doctors are looking at some esoteric and rare diagnoses to see if anything shows up. No matter what, I’m very lucky and extraordinarily grateful and blessed by the tremendous outpouring of love and support from so so many people. I’ll keep updating here as there are so many lessons and blessings I want to share. My life has been turned completely upside down in one week. I never saw this coming, but I wouldn’t want it any other way! Wound and Bus Update: OOPS (thanks for everyone’s concern, I’m mending well, thanks to mom, some good doctors and some glue). I spent the morning cleaning up all the crap around the bus and the idiot weeds. The last storm shown me very clearly where I need to seal more (so of course I’ll now over-seal 🤣). I went to Home Depot for yet again, more silicone and a few more things. I got set up and began cleaning the outside sheet metal and seams for application and well... Our sweet Jordan burned the fool out of her arm last night. She was making cookies for her brother and accidentally bumped the pan. ⠀ We immediately put ice on it and then followed that with lavender. ⠀ When she showered the skin came off and this is what it looks like now. Poor kiddo. ⠀ We are now applying a serum to it to help it heal up and hopefully not scar. We are using Lavender, Frankincense and Copiaba in Jojoba Oil.
