Na verdade, o uso de xaropes para tosse vem de muito longe, desde a tradição dos precursores da medicina, os árabes, de cuja língua (sharab) derivou etimologicamente o termo em português. Mas aí vem a pergunta: existe ciência, evidência e uma explicação mais cartesiana para o seu efeito? Sim, como você já leu em nas outras partes desse decálogo, durante uma inflamação aguda das vias aéreas, as terminações nervosas do tapete por onde o ar passa ficam hipersensíveis. Os xaropes, pela alta concentração de açúcares, formam uma espécie de filme protetor ao longo das vias aéreas, diminuindo esse efeito de reatividade das vias aéreas. Seria como a capa que protege o fio desencapado de dar choques. Além do uso do açúcar, pode ser usado o mel. Importante lembrar que o mel não deve ser utilizado na alimentação, como adoçante ou na confecção desses xaropes para os menores de 1 ano, pelo risco de botulismo. Mesmo havendo a restrição do uso do mel entre 1 e 2 anos de idade para adoçar alimentos, não há nenhum motivo para restringí-lo para esse uso medicamentoso. Uma revisão da Cochrane, publicada em 2018, concluiu: o mel provavelmente alivia os sintomas em maior extensão que nenhum tratamento, difenidramina (um anti-alérgico) e placebo. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ O mel reduz a duração da tosse melhor do que o placebo e o salbutamol (um broncodilatador). Não há evidência forte à favor ou contra o seu uso. Não houve diferença nos eventos adversos entre o mel e os grupos controle. Enfim, parece fazer bem e definitivamente não faz mal e nesse caso, opino que a tradição consagra seu uso. Depois do mel ou do açúcar, entra o tempero do lambedor. As variações são inúmeras e envolvem o uso combinado de ervas ou de hortaliças e frutas, como o hortelã, o abacaxi, a cebola, o alho, o gengibre, enfim, uma lista infindável.
