Do we think I’m smiling under here? Lol first week of general surgery in the books. Now that I think about it these four weeks are going to fly. My preceptor is a general surgeon who deals with a lot of pancreatic cancers and GI issues. Standing all day definitely has the bottom of my feet aching. But it doesn’t beat the experience. So far I’ve seen patients in clinic, performed H&Ps on inpatient patients to determine surgery eligibility, scrubbed in, worked the camera in laparoscopic surgeries, sutured, and retracted. It’s been a great experience this far! Each day brings room for opportunity and growth. Normalize it. There was a reason Blacks weren’t taught to read, write & given the chance to vote years ago, b/c there’s nothing more dangerous & powerful than an educated POC w/ a purpose. You remember that time it was cool/popular to post a black square on your Instagram in the name of racial injustice & black lives mattering? Oo me too, except that time wasnt just a moment (it’s my EVERYDAY) & that fight isn’t over (it’s still raging, as it has for centuries). If only the image of a POC in a white coat, in a role of intellect & strength were appropriated in healthcare, as much as that of the impoverished & underserved. The significance of that imagery, that someone that looks like me could actually treat & care for me...a new form of restricting power or a tale as old as time? Revered by some & admonished by others, when we normalize the image of possibility & diversity, we strike fear in those that rebuke change. For a changing tide yields a new landscape, one of betterment, advocacy & new growth. My face when I see a Pediatric patient going into my room - 🥰 but inside - 😬 ⁣ ⁣ I love kids and making them laugh + talking to them, but once they become my patients I stiffen up like a board. In nursing school, I could hardly breathe during my pediatric clinicals. I was so scared I’d hurt the pediatric patients, make them cry, or worse...make mom or dad angry🥴⁣ ⁣ I want to give the littles the best care esp since if they’re with me in ED they’re usually terrified of what’s going on and having the worst day imaginable. What I’ve done to ease my anxiety and make it a smoother experience for everyone... ⁣ ⁣ 🧸 Involve little ones in their care - you can have them count with you or sing a song while you’re pushing an IV medication, baby shark is and always will be the jam. They’re less scared of what’s going on if you let them feel involved and make it enjoyable⁣ ⁣ 🧸 Involve parents in the care - mom n dad are probably just as scared as their kid but they’re better at hiding it. If you kick parents to the side while you work both they and patient will be anxious to the max - keep parents involved like having them hold baby, prompting them to soothe while you’re giving a med or need to start an IV⁣ ⁣ 🧸 Communicate - Everyone is nervous, maybe even you. Be sensitive to that and keep everyone in the loop. Stay vocal about what you’re doing, why, and how long it’ll take. This moment of communication will usually calm kids if they’re old enough to understand and parents who now know the plan and can even help you get things done ⁣ 🧸 Lastly, Breathe! - baby’s and kids are resilient, strong little beings that can smell fear! Stay calm, be kind to them and kids will reciprocate the behavior. You won’t break them & crying is not something to be scared of. Stay confident in yourself and focus, parents will appreciate you being calm for their child as well as for them. Nobody responds well to panic ⁣ Do any of you work with Peds patients or aspiring to?? Pleaaaase drop any and all your advice + knowledge on how you do your amazing jobs, I’m sure everyone (including me) will appreciate it. Though Im certainly a little zoom fatigued, and in many ways still adjusting to the changes, Im very blessed to be starting this journey. And so excited to be starting it here at (dare I say) MSU! The face I *used* to make when thinking about other careers 😉 my zigzag journey to Naturopathic Medical School, Part 3! Ok so we left off where I had just finished shadowing and ND and I hated it and wrote off the entire profession (I know, a little dramatic). So now what? I started exploring other careers and learned about Physician Assistants (PAs). This was it. Everything about this job checked all my boxes. Only 2 years of school, great salary, job growth, flexibility to switch between fields- its great! So I found a PA to shadow and it was truly the best experience. The patients loved him, the staff loved him, he treated me with respect and even taught me how to interpret MRIs. He loved his job so much that spending 4 hours watching him do injections and pre/post-op care somehow flew by. After 8 weeks of shadowing, I was convinced. PA school it is! Now all I have to do is register for the GRE... *Spoiler alert, I did not register for the GRE... 💪🏻Had my First ANATOMY LAB this morning! I was SO excited!! I’ve literally been dreaming for about being able to do this for years. A lot of you have been asking how this is going down with COVID at my school and it’s a pretty efficient system. We each get a partner and have 45 minutes to go through 11 stations with having a worksheet and dissection videos ahead of time (the anatomy faculty perform prosections prior to us attending the lab so the cadavers are already cut when we get to lab.) I actually really loved it! I learned a ton with my partner and felt like I got a lot out of lab today! We learned the Extrinsic Back muscles and the spinal vertebrae. I started my first week of hospital service/call on Monday. By the nature of our procedures, starting early in the morning is important. This means getting things prepared for the day ahead of time—even if that means a 4 am alarm. I wanted to share the simple things I do to set myself up for success—even during the busiest times👇🏻 🥦 Prepare meals and healthy snacks for the entire week ahead of time. It doesn’t have to be complex or time consuming. You’ll be glad you spent the time later. 💤 Prioritize sleep. Set a responsible bedtime based on when your alarm is going off to get 7-8 hours if possible. 🏃🏼‍♀️ Move your body. Squeeze in a workout, run, or walk even on the busiest days. It helps your physical well-being as well as your mental health. 🙏🏻 Set aside time to be still. Quiet time spent in prayer and meditation is worth every minute. This may be your only time of focus and peace the entire day. 💄 Dress for success. When you look your best, you’ll feel your best, and you’ll do your best. 𝗧𝗜𝗠𝗘 𝗙𝗢𝗥 𝗦𝗢𝗠𝗘 𝗛𝗢𝗡𝗘𝗦𝗧𝗬⠀ ⠀ To be honest, I’ve been having a challenging time lately in my current contract. I always want to give you guys #realgram and the transparency you deserve whether you are a nursing student, or an aspiring travel nurse.⠀ ⠀ I have worked countless jobs to date. From tutoring, to a residence hall manager, to even MCDONALDS. I worked 3 jobs in college to make ends meet and graduated with a near perfect, impeccable GPA. ⠀ ⠀ Throughout this pandemic and after a full two years of acute care, hospital nursing I have never been held to such unrealistic, drastic expectations. I have never been so disrespected as an individual the way that I am on a daily basis going into work as a Registered Nurse. The past few weeks I have endured physical abuse, been hit by hands covered in fecal contents, and then have the disrespect and audacity of management asking you why you didn’t chart irrelevant components of X, Y, and Z.⠀ ⠀ I still have my moments of grace. When I give a COVID patient their first bath and days, the satisfaction I get from making someone feel human again while they live in the boundaries of a box. I sweat so bad my mascara leaks into my eyes, it burns but I do it anyway. I leave with bruises on my face from my same sweat soaked N95 mask.⠀ ⠀ These are the moments I hold onto in a thankless profession. I post this picture because although I do enjoy bits and pieces of my job, the micromanaged and unrealistic components take every bit of smiling energy out of my soul. ⠀ ⠀ Nurses, we are “yes” people. And a lot of us are women. A lot of us are walked all over and I KNOW that a significant portion of that is comprised of inequalities that are deeply rooted in the patriarchy of our country. We need change, and so do the patients we serve.⠀ ⠀ So here is a picture of me smiling, with the person who holds me together. In undergrad I *THOUGHT* I wanted to be a nurse, but wasn’t entirely convinced (because who knows exactly what they want to do for the rest of their life at 18, right?). After 1 semester with a professor who told me she was “concerned about my progression in the program”, I changed my major and graduated with a degree in biology and health science instead. Listening to that professor was my first mistake. I questioned my own competence and let myself down. A few years later I decided to give nursing a second shot when Rush University took a chance on me. Now here I am today, living my dream as a masters-prepared OR nurse, spending every day doing what I love the most... getting my hands dirty and learning everything there is to know about surgery. Maybe not your typical day at the office, but I wouldn’t have it any other way 😉 Moral of the story, don’t EVER let someone else tell you what your capabilities are. There will always be people out there who test your limits and try to push you past your breaking point, but it’s most important to stay true to yourself and always keep faith in your skills and knowledge. The path to success isn’t linear and there’s no blueprint to follow. Everyone’s journey looks a little different and that’s okay. Lend a helping hand to your colleagues and those following in your footsteps because they’re the future of this beautiful profession. Although uncommon, I definitely knew I wanted to practice primary care from the start, which is why I applied for the NHSC scholarship my M1 year!! (the primary care scholarship) BUT KELLY, how did you commit to primary care when you hadn’t even start rotations yet? Simply put, I saw enough as a premed LOL PLUS there’s a reason the NHSC scholarship allows you to apply early in your med school career !! Here are some things that helped me deicide: ⁣ ⓵ Exposure! I spent some time at a Level 1 Trauma Center and mingled with some EM docs and surgeons. I didn’t like it much (but much respect for these folks!!) I spent majority of my time in family practice and geriatrics and LOVED it. Simple! Try to shadow as much as possible in as many different specialties as possible and do it EARLY ON so you have time to explore. ⁣ ⓶ Personality! Whether or not you agree with this, the truth is that there are certain specialties where the people are just…nicer…LOL. If you find that this isn’t true, it’s probably because you’ve found an exception, not a majority 😂 Examples: family physicians are known for being the “nicest”, EM docs are known for being more chill and lax etc. ⁣ ⓷ Are you a manager, fixer, or diagnostician? The managers would be your internists and family physicians, the fixers would be your surgeons, the diagnosticians would be your radiologists (not an exhaustive list obvi just examples) ⁣ ⓸ Do you like procedures and surgeries? Do you like high-acuity settings? My answer to both of these questions was no. I’m not too great with my hands and I’m also spatially challenged, which rules out surgery lol. I’m a fan of preventive medicine and I enjoy the outpatient setting with some inpatient mixed in! ⁣ ⓹ You can’t imagine yourself in any other field!! We do more than just put people to sleep 💤 and put in a breathing tube.💨 As an anesthesia resident nearing the end of my #residency, Ive provided #anesthesia and cared for heart/lung/liver/kidney transplants, trauma, neurosurgery 🧠, vascular, orthopedic 🦴, pediatric 🍼, and obstetric patients. 👩🏻‍⚕️ Anesthesiologists also manage pain and work in the ICU! 💉💊 Given our unique skillset, anesthesiologists are one of the first specialty #physicians to be deployed in the #ICU to help out during these times. 🩺💉 #frontlineworkers #healthcareworkers 📸: This picture was taken after I finished my last scheduled ICU rotation in August. That of which includes the loss of family members, jobs, and resources. I have personally lost family members to COVID-19 and I had the virus myself. I can say without a doubt I have never experienced anything like it in my life. I have never experienced mental, emotional, and physical pain at the same time. Happy Monday from clinical class (with covid precautions of course)! I planned on posting more but boy oh boy is graduate nursing school, mostly online, during a pandemic quite a feat! Apologies for the time in between posts 😂 So far our program has been predominantly online with Zoom synchronous lectures, meaning we all login and get a lecture in real time, and then we have rotating shifts of on campus clinical activities. Some days we do work on our own and check in but today was my on campus day! So here we are practicing intramuscular and subcutaneous injections on mannequins and giving oral medications. I find online learning difficult so I love being able to be hands on part of the week and physically practice what we’re learning. Today was really fun and I can’t wait to be back on campus Thursday. Hi friends! Remember when I got that big hip surgery last year and was on crutches for three months? Doing it all over again on the other side in a week! Executive summary: Hip dysplasia—not just for dogs. Cutting my hip into pieces and screwing back together again involves an epic surgery and recovery, but it prevents me from becoming the Town Cripple for the rest of my life. I’m dazzled at how painful the advanced activity of “sitting in a chair” has become. Right side started failing quickly after the left side got done. She got FOMO or something. The surgeries at @hspecialsurgery on Wednesday (the 19th): right periacetabular osteotomy, right arthroscopy, and left screw removal. Not gonna bomb this account out with too much surgery stuff. We experience a degree of pain when we see fear in our patient’s eyes when one desperately holds onto hope despite their grim medical prognosis. Our mental-fortitude weakened when we’re overwhelmed with emotions due to devastating patient outcomes. Being overcome with joy when patients heal + recover miraculously that crying becomes a sensible method of coping. Most importantly, remembering to decompress + switch our ‘work’ emotions off before we head home to care for our own families + loved ones. I just saw a post by a Montrealer with a lot of followers frustrated with the governments new measures (I get it, it sucks) write something like and schools get to stay open, with kids the biggest super spreaders And then someone I know (a relative) shared it 🤦‍♀️ 😟Im not trying to call anyone out. This stems from disappointment, confusion, frustration. We are getting mixed messages and sometimes it feels like nothing makes sense. But we gave to be careful about adding to the noise and confusion! 🧒There is currently NO evidence that kids are COVID19 superspreaders. In fact there is some data to suggest the opposite (see my story for more on this - recent @jamapeds article). The truth is we have some data, but not enough to know for sure. So how can anyone be posting this as if it is a fact? We are talking about informing public health policy that has important implications. If we spread false information, we undermine efforts to curb the spread of the virus. 🚸 GIven the many benefits of keeping schools open (for children and their families and society at large), I dont see how we can equate bar closures to education for our kids. Thank you @stapaw for this. I get a lot of questions about my tattoos and if it affects my schooling or career. The short answer is no. I have to cover them for school and clinical but my job at the hospital allows me to show them as long as it is not offensive. . Now, there was one patient who asked about them. He asked “what if your husband doesn’t like tattoos?” I laughed on the inside because I would never marry someone who judges someone for their physical appearance and the choices they make but I answered him by saying “I want to marry someone who loves me for me... and that includes my choices and my tattoos.” . What I also didn’t tell him is my sleeve is completely for my grandma and my grandma is the reason I pursued nursing. My sleeve consists of a tree of life and angel wings as the roots (She loved angel wings. I have them engraved on her urn as well), a rose (her favorite flower), and a clock (showing the time she was born). . I choose to get the ink on my arm because I wanted people to see the dedication I have for my grandma. This dedication also applies to nursing. My ink doesn’t affect the way I will care for a patient. It doesn’t affect the compassion I have for them, even if they point out they do not like tattoos. That is their prerogative and I’m not one to judge them for that opinion. ✨The (masked) face of a med student who got to perform her first set of injections today!✨ Did you know that Botulinum toxin-A injections are used to treat Cerebral Palsy? In fact, Botox injections are first-line for treating spasticity and dystonia to reduce muscle overactivity and stiffness. They are used to help improve movement and reduce pain. The injections can be done wherever the patient is currently feeling pain or stiffness. The provider checks for spasticity and clonus, and palpates the muscles to find the best spot to inject. In this case, Botox prevents the release of acetylcholine, which stops the muscle cells from contracting. So, it’s best if you inject the toxin at the neuromuscular junction, where the Ach is usually released. Hence, anatomy is really important! Yay learning! Here’s how I look at it— that’s cool that you’re an honor student and all but how is your bedside manner? Do you work well with others? . While hand skills are crucial as a dentist, people skills are equally as important because at the end of the day you will not be able to practice dentistry unless patients trust you and say yes to your treatment plans. . So don’t forget to invest in yourself outside of the classroom. 🔹Be a student leader or join a club sports team so you can practice working in teams and communicating with others. 🔹Take time to shadow docs in their offices so you can observe different chair side personalities. 🔹Consider a side-gig so you can learn how to work and interact with people outside of your dental school bubble. . P C O S and P R E G N A C Y Three days left in PCOS Awareness Month and I think it’s time we talk about effects of PCOS on pregnancy. (And how we can mitigate some of those risks!) PCOS in pregnancy is associated with a higher rate of gestational diabetes - probably no surprise here given the inherent changes in #insulin sensitivity tied to PCOS. However, for nondiabetic women with PCOS who are pregnant, #metformin use reduces the risk of developing #diabetes by 10-fold! Gestational #diabetes is tied to other adverse events in pregnancy, such as an increased risk of having a c-section, #preterm #birth, and high blood pressure / #preeclampsia. Metformin might reduce these risks too. When I talk to patients about getting pregnant, I always preface the conversation with this: My goal is to help you achieve the healthiest pregnancy possible. And that starts with YOU. So especially when we have PCOS it’s important to optimize mom’s #health prior to #pregnancy. Assess and address insulin resistance. #Empower with information on the best types of #nutrition and #exercise. And add medication if needed. Good morning! 🌞 Here’s a little update post since it’s been little over a year since I signed my lease and almost one year since I opened doors to @lv.dentistry. Owning a dental practice has been pretty crazy with having to wear all of the hats and constantly dealing with various issues on a daily basis. It truly has been worth it to have this amazing and unique opportunity to provide dentistry to my community and have an amazing team that genuinely cares. It was rough with having to shut down my business for 3 months in my first year of ownership. Now we’re picking back up fast and constantly working on ways to improve as an office for our patients. I have always appreciated to be from such a beautiful & diverse neighborhood, Linda Vista. the amount of support that I received during this journey is UNREAL.. I just want to take a second to recognize and honor inpatient nurses because I had no idea this is what you guys deal with lol! My transition from ER to Mother/Baby has not been completely smooth sailing. I’ve definitely had some very overwhelming shifts where i questioned myself and my decision to switch specialities. You guys know I’m always super transparent with y’all and want to share the best and worse sides of nursing because it isn’t always glamorous. Nonetheless, I am getting so much more confident every day and thankfully have an amazing preceptor to show me the ropes. This past Monday was my LAST first day of school EVER, and I just finished up my first week of transplant surgery as a fourth year. It’s a very different experience this time around, with a lot more responsibility. Fourth year rotations are centered on learning how to be a good resident. You already know that you want to do said specialty, so now you put in the work to show it. I went from carrying maybe 3-4 patients as an M3, to now being expected to lead morning rounds, placing admission orders and solving floor work issues all as priority over scrubbing cases. It’s a huge learning curve 😳 But this week has really confirmed that I’m picking the right specialty for me and I haven’t felt more belonging anywhere else like I do here. Bring the crash cart!! ‼️😨⁠ ⁠ Roles nurses can play during a code to make sure the event runs smoothly:⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁠ 🚨Recorder: The recorder should be writing the time the code began (or if you’re in ED, ask the EMT’s bringing the patient in what time compressions began) then record any medication dose given and the time. Be aware of the time because you will advise the physician how long it’s been since the last dose and when it’s time to check for a pulse ⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Airway: typically once a person is coding the physician will intubate. Once the patient has been intubated, the Respiratory Therapist present at the code will be maintaining the airway and using the AmbuBag to ventilate. The physician then focuses solely on running the code⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Compressions: there should be at least 2-3 people cycling through compressions. This is a great way for the new nurses to be involved in the code. Remember 2.5in deep on the compressions at a rate of 100-120bpm ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨IV’s/Medications: In ED, these full arrests may not have IV access. This becomes top priority since without it you cannot give medications. Two nurses should be obtaining access and if all else fails: IO access. Once access is available, one nurse who put in the IV should stay in place and administer the medications, being sure to call out loud and clear the dosages and when the medication goes in ⁣⁣⁣⁠ ⁣⁣⁣⁣⁣⁣⁣⁣⁣⁣⁠ 🚨Runner: For the code/full arrest to be effective there should always be 1-2 people at the ready to run and grab more supplies as needed. What are you wearing? 👀 Did you think about what you wore to work today? Or what your veterinarian was wearing the last time you saw them? Think it matters? Well according to a recent study in JAVMA, it does. (Coe, 2020) A study of 449 pet owners in Canada found that “veterinarians’ attire is a form of nonverbal communication that is likely to inform clients’ first impressions and may influence clients’ trust in, confidence in, and comfort with a veterinarian.” Participants were randomly assigned to complete a questionnaire containing photos of a male or female model veterinarian photographed in 8 attire types (formal attire, white dress shirt with black pants, white casual shirt with khaki pants, surgical scrubs, white casual shirt with jeans, surgical scrub top with jeans, surgical scrub top with khaki pants, and white laboratory coat with khaki pants) According to this, I am in the formal category, often with a lab coat. ➡️ Swipe to see some examples over the years. But recently in the COVID days as clients were not longer permitting in the hospital, I have been mixing it up, wearing jeans and scrubs. I recently fell in love with my new @wearfigs scrubs. (They are way nicer than the ones I had in my internship and residency - and they have pockets!) Apparently, scrubs get the highest scores! In the study, participants’ trust, comfort and confidence score were highest when the model veterinarians were pictured in surgical scrubs. Lab coats also increase trust and confidence. In contrast, comfort score was lowest when the model veterinarians were wearing formal attire, and trust and confidence scores were lowest when the model veterinarians were wearing a casual shirt and jeans. One. I received one medical school interview and one medical school acceptance. I took that one opportunity and put EVERYTHING I had into it. During my second year of medical school is when I connected with my now husband. I remember anytime I had an exam, I would disappear for days at a time. I think this used to make him nervous and question if I was truly interested. During a visit to Florida to see him I remember having a conversation with his mother. I told her how I tried to get into medical school for a few years and that I finally had an opportunity. I told her that I was relentlessly throwing myself into it. She heard me...and from that point on kept her son in check, and reminded him of my relentless pursuit. I put everything I had into medical school...literally. My priorities were Jesus, studying, exercise, sleep, and good food... all things that supported the goal. I never missed a Sunday service while in medical school, because I believed after studying as much as I humanly could, I would need the supernatural to see me through. Everyday after all day classes, I would sit at a desk in the study hall for hours, until I was at the threshold of not being able to drive home safely. I missed weddings, birthdays, anniversaries, and all kind of celebrations. But I did all these things with a grateful heart, because I understood that I had been blessed with ONE opportunity, and it was up to me what I did with it. I say this to say... all it takes is one. 👩🏻‍⚕️I’m a FACS!👩🏻‍⚕️ You might’ve seen these letters behind someone’s name and wondered what it meant - it stands for Fellow of the American College of Surgeons, and it means that the surgeon has gone through a rigorous evaluation of their surgical training, skills, and ethics. (See my friend @doctorjacci ‘s post for a great outline of the extensive requirements!) ⁣ ⁣ The process was lengthy- my entire practice was reviewed by a committee, I had to get endorsements by other surgeons, submit case logs, and then there was an interview which was pretty intimidating- but now more than ever I see the importance of staying active within our medical community to ensure the best possible care. Been back in clinic and it feels so good! Love that many of my clinics are anterior seg focused with specialty CL fits, myopia control and dry eye. Saw my first corneal transplant patient and refit their sclerals, used the sMap for the first time to map out their scleral topography for a new custom fit, had my first atropine patient and first ortho-k patient! Dear prehealth student, Congratulations on starting college! We’ll be honest with you...the premed/predental journey isn’t easy. There were sleepless nights, very harsh curves and competitive students. There were days when we felt like we would never make it through “weed out classes” like orgo or biochem. We both scored 50s on exams for premed classes. We both were in the lowest standard deviation on our first orgo exam. It took a lot of studying to boost grades back up. Work hard, the grind is worth it. But remember, ONE GRADE WILL NOT DEFINE YOU. It’s so important to remember that these premed classes will not dictate your future ability or projectory. Do not think that you’re not fit to be a doctor, dentist, pharmacist or whatever your dream profession is because of one poor grade. You got this!!🤍 ps. • 𝐈𝐬 𝐬𝐜𝐫𝐢𝐛𝐢𝐧𝐠 𝐛𝐞𝐟𝐨𝐫𝐞 𝐏𝐀 𝐬𝐜𝐡𝐨𝐨𝐥 𝐰𝐨𝐫𝐭𝐡 𝐢𝐭? 𝐃𝐨𝐞𝐬 𝐢𝐭 𝐜𝐨𝐮𝐧𝐭 𝐟𝐨𝐫 𝐏𝐂𝐄? 𝐃𝐨 𝐲𝐨𝐮 𝐧𝐞𝐞𝐝 𝐭𝐨 𝐠𝐞𝐭 𝐜𝐞𝐫𝐭𝐢𝐟𝐢𝐞𝐝? •⁣ ⁣ I wanted to answer a couple common questions I get about scribing. Swipe to see my daily tasks and what I learned as an emergency room scribe 👉🏼👉🏼👉🏼⁣ ⁣ I honestly LOVED scribing and would recommend it to anyone thinking of going to PA or med school. I got to see so many different cases and learned how each provider ruled out their list of differential diagnoses. ⁣ ⁣ A lot of providers also spent time teaching us how to read EKGs, interpret lab values, and understand the physiology behind specific diseases - which was always my favorite part of the job. You really get to form a close relationship with the providers and 2 of my LORs were actually a PA and an MD that I scribed for! ⁣ ⁣ Scribing also helped me get my foot in the door and eventually land a job as an ER tech at the same hospital, because I had no prior medical experience and did not need to get certified to be a scribe. How to present a SOAP note! One of the biggest skills you will hone as a 3rd year med student is how to effectively present a patient on rounds! Mastering this can definitely impress your attending! There’s a steep learning curve to presenting but once you get it down, it becomes second nature! SOAP: Subjective, Objective, Assessment and Plan. This is how you should present ADMITTED patients, which is different than the initial H&P! 🔹Start with your one liner: “Mrs. M is a 55 yo F with a past medical history of HTN, COPD, who presented with SOB and is admitted for acute on chronic COPD exacerbation secondary to URI, on hospital day 3.” 🔹Subjective: Report how the patient is doing (w/in the last 24 hours). “Pt reports a mild headache, which was controlled with Tylenol. She reports that her SOB is improving. She denies any chest pain.” 🔹Objective: Vitals (in 24hr ranges) Temp: afebrile, 97.5-98.7 (If had a fever, say “Tmax= X” to indicate the highest temp.”) BP: 124-140/89-99 Pulse: 78-88 Resp rate: 11-12 O2 sat: 96-97% (on room air or if getting O2 by nasal cannula, specify amount) Ins/outs: In: 500 mL PO, Out: 1000 mL urine, Net: - 500 mL (important for pts on strict Is and Os!) 🔹Physical exam Go through the exam by system (General, Cardio, Pulm...). Highlight KEY findings. 🔹Labs/imaging/microbio Note any NEW (w/in last 24h) results. Pay attention to those pertinent to your patient. 🔹Assessment: Include your one liner with the patient’s current status, list any pertinent new complaints/labs/imaging/exam findings and ddx (if new findings). “Mrs. M is a 55 yo F with a PMHx of HTN & COPD who p/w SOB in the setting of a URI. Her symptoms are likely 2/2 to an acute exacerbation of chronic COPD. She is currently stable on 2L via NC and her SOB is improving.” 🔹Plan: Present by problem or by system (more common in the ICU). When you list by problem, start with the highest acuity. By problem: 1. Acute on chronic COPD exacerbation -continue albuterol/ipratropium tid -wean O2 as tolerated -etc. 🔹End with disposition: d/c home pending improvements in O2 require. The aspiration of becoming a nurse has been one of my biggest dreams practically since I was birthed. Upon “graduating” kindergarten, I recall telling my grandmother “ I ALREADY GRADUATED KINDERGARTEN!! CAN I BE A NURSE YET?” However, the road to becoming a nursing student was anything than what I imagined. Upon struggling with mental health issues and a lack of purpose, I began failing high school. I struggled to even graduate and felt as though I was wasting time not helping others. I shortly began taking college courses wanting to pursue psychology. While taking general courses, I had a medical issue arise that was extremely personal. Through the entirety of my care I was blessed with unbiased, selfless, and even loving care. I then knew I needed to recognize the dreams I’ve been pushing away since kindergarten; to be a nurse. I realized I wanted to make an impact on the well-being of other, even if just for an hour, a day, months, or years. Nursing has led me to goals of becoming a nurse-midwife where I can provide comprehensive reproductive care to all women. My ultimate goal in life is to own a birthing center in which all race, gender, snd sexual orientation are welcomed. E.R. nurses are a different kind of creature. We condition ourselves to roll with the punches..no matter how brutal..you roll. Somebodys life depends on it. Our eyes see the unfathomable...sights that would keep a normal person awake at night...but we roll. We condition ourselves to not feel it...not take life too seriously...we know how fragile and brief it is..we are reminded every time we swipe that time clock...every time...and we roll. I picked a young lady up off the floor this morning after she walked out of the room where her daddy died. She dropped to her knees...she wailed..she cried out for her dad...and without batting an eye, I knelt beside her and helped her the best way I could...death had won again. We become numb...our hearts dont feel things like the hearts who are protected from this type of consistent and repeated defeat...we cope..and maybe too well. Maybe we come off as cold and emotionally unavailable. Maybe we arent approachable. We come off a little bossy...because we have to be good patient advocates..to be the person our patient needs. Be patient with the ER nurse/EMS/PD/FD in your life. Your eyes have not seen what theirs have. You havent felt it...you havent battled death as much and as hard as they have. You could never see their world through their eyes...because if they love you..they wouldnt want you to..they will protect you from it. Your girl is finally a Patient Care Associate !! I’m so excited to begin this chapter of my life. After getting rejected countless times even getting certified as a phlebotomist didn’t work. “You don’t have any experience” is what everyone would tell me. Yes, I got discouraged and stopped searching for a while but i thought Hey someone will give me a chance eventually. Two years later I got an interview and a week later HR calls me saying i got the job.
