Assess for bruit and may need a doppler. 2. OPEN FRACTURES: needs early reduction/surgical management. Updated Tdap and give antibiotics (ex. Ancef 2g) quantify energy pattern--wound size, fracture pattern, soft tissue injury size, contamination, neurovasc injury present. GUSTILO & ANDERSON CLASSIFICATION TYPE 1 - wound1cm, minimal damage TYPE 2 - wound 1cm, moderate soft tissue damage, minimal periosteal stripping TYPE 3a - severe soft tissue injury, contamination with adequate coverage TYPE 3b - severe soft tissue injury, contamination, inadequate coverage TYPE 3c - arterial injury needing repair 3. NEURO COMPROMISE 4. COMPARTMENT SYNDROME: can lead to vascular compromise, neuro injury or necrotic tissue. Risk of cellular destruction and hyperkalemia. 5. HIP DISLOCATIONS (not if prostetic) 6. OSTEO/SEPTIC JOINTS 7. PELVIC UNSTABLE FRACTURE: risk of serious vascular/neuro injury. May be life threatening bleeding. Place a pelvic binder. Will need examination of vaginal vault, male genitals, and/or rectal exam. May need massive transfusions and interventional radiology intervention. NOTE: Yes, you CAN place TQ on open fractures is bleeding is difficult to control with pressure dressings. Frequent reassessments of neurovasc status are needed.
