ACNE e Terapia Manual O título pode parecer estranho, mas não é sobre espinhas ou cravos que vamos falar. ACNE é um acrônimo para “Abdominal cutaneous nerve entrapment”, ou seja, aprisionamento dos nervos cutâneos abdominais, que pode ser uma causa negligenciada entre as quase 1000 causas de dor abdominal. Para entender a síndrome basta recorrer a anatomia. Os nervos tóracoabdominais oriundos de T7 a T12 que terminam como nervos cutâneos são ancorados em 6 pontos no seu trajeto: 1) a medula espinhal; 2) o ponto em que o ramo posterior se origina; 3) o ponto de origem do ramo lateral; 4) o ponto em que o ramo anterior faz uma curva de quase 90° para entrar no canal do reto; 5) o ponto a partir do qual os ramos acessórios entram no canal do reto, e 6) pele. A causa mais comum de dor na parede abdominal é o entrapment neural na borda lateral do músculo reto abdominal. No canal do reto, o nervo e seus vasos são cercados por gordura e tecido conjuntivo que ligam o nervo, artéria e veia em um feixe discreto capaz de funcionar como uma unidade independentemente do tecido circundante. Em um ponto localizado cerca de três quartos do caminho através do músculo reto (de trás para frente), há um anel fibroso que fornece uma superfície lisa através da qual o feixe pode deslizar. Apesar do feixe neuro vascular ser protegido por gordura ele é suscetível a compressão e estiramentos devido a estreita passagem através dos túneis fibroso por causa da angulação mais aguda. Os critérios para um diagnóstico clinico incluem: 1 - Dor unilateral loco regional na parede abdominal que dura mais de 1 mês 2 - “Spot” unilateral menor que 2cm na borda lateral do reto abdominal 3 - Sinal de Carnett positivo 4 - Teste do pinçar e rolar da parede abdominal positivo 5 - Exames de laboratório e imagem negativos 6 - Alívio temporário após injeção de anestésico local no foco da dor Muitas técnicas de terapia manual incluindo liberação miofascial, inibição dos trigger points, dermoneuromodulação, terapia manual integrada e trigger points podem ser utilizadas até porque as medicações orais não funcionam bem. Read on for tips I use when I look at an abdominal CT... 👨🏽💻Coming into radiology as someone who was mainly concerned with what was going on above the diaphragm I found the world of abdominal CT fairly daunting. But by getting to grips with the anatomy of the abdomen things became a whole lot clearer 👨🏽💻At this level of the abdomen we get a nice demonstration of the superior mesenteric artery – we can’t see the superior mesenteric vein here but normally the vein should lie to the right of the artery at the level of the pancreas, if this is reversed then consider intestinal malrotation which can predispose to midgut volvulus 👨🏽💻I always check to see there is a clean layer of fat surrounding the SMA – if this is infiltrated consider a malignant pathology such as a pancreatic tumour 👨🏽💻Always follow the vessels no matter which CT it is you’re looking at, be it head, neck, chest or abdomen. I always follow the portal veins within the liver formed at the confluence by the splenic vein (which runs posterior to the pancreas) and the superior mesenteric vein which runs vertically. In patients with liver cirrhosis and pancreatitis in particular you are looking for a filling defect or occluded vessel representing thrombosis 👨🏽💻There aren’t many retroperitoneal structures that cross the midline but the left renal vein is one of them – characteristically this passes anterior to the aorta from the left kidney to the IVC but in some people this can have a retroaortic course where it passes behind the aorta. I have seen the occasional incidental left renal vein thrombosis so it is worth checking for filling defects! This is a must in cases of renal cell carcinoma which likes to spread into the IVC via the renal veins 🔻@theradiologistpage - @theradiologistpage on Instagram

❗️A 10/10 fashion statement on your foot may be causing you 10/10 long term damage to your bodies functionality. ❗️ ⠀⠀⠀⠀⠀⠀⠀⠀⠀ If you’ve been following me since September you’ll know I’ve transitioned to being completely barefoot and I won’t ever go back. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ These photos are 173 days apart and I could show my current progress in May, but I want to show just what 5 months of being completely barefoot and using @correcttoes has done for me and potentially what it can do for you. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Prior to this change I was wearing traditional shoes such as: Adidas Rainbow Sandals Nike Flyknit Racers Aldo Dress Shoes Toms ⠀⠀⠀⠀⠀⠀⠀⠀⠀ All of these shoes are VERY popular and are very abundant but lack a lot of what our feet need. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Our society has changed our footwear standards by having our feet fit the shoe and not our shoe fit our feet. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ In just 5 months of being barefoot and focusing on the functional health of my feet I have: 1. Developed an arch with ZERO arch support. 2. My toes have spread allowing for natural arch formation. 3. Anterior pelvic tilt (butt sticking out) has immensely gone down by removing elevation of the shoe. 4. Knee cave in due to foot pronation (flat feet) has gone away and my bodies ability to use my hips and foot for functional movement has increased. 5. Deep squat hip pain has been largely reduced. 6. Propioception has largely increased allowing for conscious heel striking and awareness of walking habits. 7. A Síndrome cruzada superior (Upper Cross Syndrome) foi descrita pelo Dr. Wladimir Janda em 1979, por definição, uma alteração postural levando a hiperativação de alguns músculos e inibindo outros em padrão cruzado❌ Alguns anos depois, Janda descreveu a Síndrome Cruzada Inferior em 1987, como uma forma de se observar desequilibrios musculares na região abdomino- pélvica. Essa abordagem simplifica a forma de se analisar essas sinergias musculares que estabilizam a pelve no plano sagital. No caso da superior, devido á utilização cada vez mais acentuada de dispositivos móveis📱, o índice de pacientes apresentando tal alteração tem crescido de forma exponencial com anteriorização da cabeça, aumento da lordose cervical, e da cifose dorsal, além da elevação e anteriorização do ombro e escápula alada, ocasionando um “desbalanço” muscular acentuado. Já no caso da Síndrome Inferior, músculos do abdômen e glúteos inibidos , enquanto os extensores lombares apresentam encurtamento e hiperatividade juntamente com os flexores de quadril. Os tratamentos para ambas as síndromes cruzadas (superior e inferior) que trazem bons resultados são aqueles em que se associam as correções posturais globais após trabalhos específicos de reequilíbrio dos músculos afetados, ou seja, não adianta apenas alongar os músculos encurtados e fortalecer os inibidos sem que haja um trabalho postural holístico ♾concomitante e a manutenção desta postura adquirida com exercícios sensório-motores pós-tratamento. Neste cabo-de-guerra muscular, a cadeia mais forte irá ganhar. Nosso trabalho como profissionais é equilibra-los e devolver às pessoas um corpo mais adaptável , para que não haja compensações e lesões futuras. Vanity kwam naar me toe dat ze haar been niet meer zelf kan optillen en staan ging al helemaal niet. Ze had niks aparts gedaan . Ze was thuis en wilde van de bank opstaan. En kon daarna opeens niet meer op haar linker been staan . Als ze er op stond deed het heel zeer. En ze kon haar been niet optillen. Toen ze bij mij kwam heb wat functie onderzoeken verricht en had al gauw gezien dat het probleem in de heup en lies zat, namelijk de iliopsoas en sartorius (zie plaatje).De zaten klem. Na nog meer tests heb ik de spieren gedryneedled en de quads en heup gemobiliseerd. Ze had daarna meer bewegingsvrijheid iets minder pijn en kon er nog niet volledig op staan . Maar het was al een hele verbetering. Vandaag voor de tweede keer behandeld weer gedryneedled en fasciemassage gedaan. En als laatst spieren los gemasseerd (zie filmpje). Blessure was volledig weg.Vanity kan zich nu weer voorbereiden voor wedstrijden. En kan nu weer trainen. Want great groins of fire? The adductors are the fab five muscles that make up what we know as groin musculature. They are another often undervalued and overlooked in terms of the role in performance and human movement. . Here’s a breakdown of the muscle itself and the biggest issues commonly seen: . Anatomical facts: 1️⃣They attach to the inner portions of the thigh and also to the pelvis at different locations and make up around 23% of the total mass of the lower extremities. The longus, brevis and magnus attach more anteriorly. The pectineus and gracilis attach more posteriorly and are a part of the anterior oblique subsystem and lateral subsystem. 2️⃣They play an important role in hip stabilization especially during walking and injury prevention. 3️⃣They are also direct antagonists to glute max, TFL and gluteus medius. These sons-of- a-bitches can either be facilitated or inhibited depending on what might be causing the issue(s). . . The biggest problems: 1️⃣If the glutes are on vacation and not functioning properly, the hamstrings often take over the primary work of the glutes to extend the hip, which creates strain on these muscles to adduct the hip because the glutes are inhibited. 2️⃣The most common groin pulls are pectineus and adductor longus/brevis. 3️⃣They are also analogous to tibialis posterior and peroneals roll and stabilizing the ankle during frontal plane movement. In addition, they are involved in eccentric deceleration and aspects of rotation. 4️⃣If adductors are chronically tight, they will always battle with the glutes or other musculature and wont allow the pelvis to get out of a large anterior tilt with internal rotation. That in turn can cause issues throughout the chain and even create knee pain. . 📍Scroll to see some movements to either strengthen or stretch the adductors. . 📍Take-home message: In most cases, the adductors are overworking (facilitated), and sometimes inhibited (underworking). Release adductors if you have hip, knee or groin pain, but first find out why they are overworking/inhibited in the first place. . - @drjmike on Instagram

💡Scoliosis Exercise Ideas💡 — ❗️There are many types of scoliosis, S curves, C curves, curves in the cervical spine, curves in the thoracic spine, curves in the lumbar spine or a combination of any of these! That means there is no one-size-fits-all exercise or rehab plan. — ❗️For instance, I have a minimal Right sided C curve in my thoracic spine. (The image on the cover is as close to mine as I could find). My main problems are my left shoulder is slightly lower than my right shoulder, my left Lat gets super tight & bound down & I have a lot of tightness in my left QL (Quadratus Lumborum) which causes the left side of my hip/pelvis to be a little more elevated. Oh & I’m also limited in left thoracic rotation. — — ❗️There is literally no way, without surgery, to “fix” my curvature, as it is structural. But even with it being structural, there are certain muscles that remain tighter (the shortened side) & certain muscles that remain weaker (the lengthened side), that can be perpetuating the problem & these are the things we can work on to help maintain the level of curvature so it doesn’t get worse over time. — ❗️So for me, it’s important to mobilize the things that are stiff (rib cage/ thoracic spine) & stretch the things that are tight (lats / QL) & then of course strengthen & stabilize the “core” & midline. — ❗️The following are some of the exercises I use to accomplish those goals. Hallux Valgus is a progressive foot deformity characterised by a valgus (lateral) ➡️ deviation of the big toe and varus (medial) ⬅️ deviation of the first metatarsal (area at the base of your big toe). Usually, this results in a bunion cyst developing in the outside of the big toe. . Bunions tend to occur due to a combination of intrinsic and extrinsic causes, such as: . - constricting footwear (e.g. high heels)👠 - excessive foot pronation (increased pressure on the inside aspect of the big toe) - increased length of the first metatarsal and hallux - trauma to the medial and plantar ligament complex and medial sesamoid bone . This post will be beneficial for those with bunions primarily caused by intrinsic factors (movement dysfunctions) or constricting footwear, and are in early-mid stage. It is important to NOTE, for those of you with bunions as a result of anatomical and/or structural changes that surgery may be the way to go 🔨as it is unlikely these exercises will make a difference. HOWEVER, it won’t hurt to have more intrinsic foot strength and control 😉 . The exercises seen in this post focus on developing abduction flexibility, control and strength of the big toe, big toe flexion & extension, as well as general arch strengthening with ideal big toe/ankle alignment, in order to prevent excessive pronation of the foot. . ⚡️H A N D•N U M B N E S S⚡️ Do you find your hand frequently falling asleep, feeling numb, like pins & needles, or a random tingling? • For many people this is a very common occurrence which comes every once in a while, but doesn’t necessarily mean there is something “wrong”. • For others, this issue persists and effects them every time they move their arm or neck a certain way and even worse - may be felt constantly. • Personally, even I frequently feel numbness in my hands when I’m laying awkwardly, on my phone scrolling IG for hours on end, and when I go for long bike rides through the city. • The following exercises explain some things you can do if you’re frequently effected by hand numbness. • What is important to note though, is that while many times our hand falling asleep may be nothing, it can also be a sign of another issue like: 1️⃣Cervical Disc Herniations 2️⃣Cervical Spine Stenosis 3️⃣Thoracic Outlet Syndrome 4️⃣Peripheral Neuropathy 5️⃣Carpal Tunnel Syndrome • While these exercises can still be helpful for each of these diagnoses, it is still best to always be assessed by a physical therapist (like myself) to find the real source of your symptoms and create the best plan of care.
