The health care heroes of the current crisis are cracking under the strain. Doctors, nurses and other professionals are not only fighting an intractable virus day after day, theyre also coping with isolation, shifting official guidelines and limited ways to recharge.⁣ ⁣ A study published in June by researchers from UCSF and other universities showed emergency medicine physicians reporting a median 60% increase in emotional exhaustion and burnout over prepandemic levels.⁣ ⁣ Before the pandemic hit, 42% of more than 15,000 physicians responding to an online survey by the medical news website Medscape reported being burned out. High rates of depression and suicide in the medical profession have long been a problem.⁣ ⁣ Experts fear the pandemic crisis will only make those numbers go up.⁣ ⁣ I agree with other healthcare workers who have reported that our colleagues are starting to withdraw emotionally and show increasing levels of cynicism. COVID-19 patients are sicker than the patients we have been accustomed to taking care of. When the stress of hospitals gets unbelievably high, it ceases to be a positive nor productive place.⁣ ⁣ But heartache and frustration doesn’t end at the hospital. When we go home and unlock the doors, we have children who run towards us because they haven’t seen us all day or night. But we head in the opposite direction. We can’t embrace our families until after we finished scrubbing ourselves raw in a scalding shower. Even after our shifts are over and we’re trying to relax at home, our minds wont relax.⁣ ⁣ Prepandemic, we used to disconnect from work by working out or going to dinner with friends. But now, gyms and dining out are risky. Also, many friends dont want to hang out, knowing we’ve been around sick people all day.⁣ ⁣ So please check-in on your healthcare friends. Doctors, in particular, arent known for seeking help. We need to change that mentality.⁣ ⁣ If you’re a healthcare worker, please seek help. It’s ok to not be ok. Talk to someone and/or a professional. No one should suffer alone or in silence. I see people starting to take it easy since the cases have been low. Please remember that the weather is changing and its much cooler now and flu season is beginning. ⁣ ⁣ Please also remember to wear your masks and wash your hands as much as you can. The mask you wear protects others around you. ⁣ ⁣ As annoying and time consuming as it is, I still wash all my fruits and veggies and wipe down my groceries and packages. Shower after work or going out. Overkill? Maybe, but it makes me feel safe so Ill continue to do it.⁣ ⁣ Additionally my family and I are getting the flu vaccine in the next few weeks. #whyophthalmology My friend and colleague @vickichanmd invited me to share why I chose to specialize in eyes after medical school. The top two reasons were - I liked the outpatient setting (versus inpatient which means primarily hospital based work) and also the ability to do work and surgery abroad in underserved communities. After I finished training I did 3 service trips in Mexico and India. Those trips were extremely formative for me in my outlook as a physician and as a human being in general. I am very grateful to have had them as a young physician. Whenever I need a reality check, I think about the people that had literally traveled hundreds of miles for a surgery that is readily available for people in first world countries. Now, I enjoy ophthalmology because I have long term relationships with my patients. Once a patient of mine starts seeing me, unless they move, I am usually their eye doctor for many years to come. I am a people person and I really enjoy having that long term relationship with the patients I have the privilege to care for. I am not particularly in love with the anatomy of the eyeball or anything but I do think it’s pretty cool that such a small part of your body is so extremely important ! Unfortunately due to a massive shortage of personal protection equipment, this is what doctors on the front line are wearing. #cdcguidelines recommend covering your hair, wearing protective glasses, and wearing a mask. However since we dont have enough equipment, this shower cap, swim goggles, and bandana is CDC approved and what many health care workers will have to wear for protection. Again, this is what we health care providers have to wear NOT because it is the best way to prevent #covid_19 spread or because its the best way to protect ourselves as health professionals, but because there is NOT enough medical equipment for nurses, surgical techs, EMTs, PAs, MAs, medical trainees, doctors, etc. . I am scared to practice medicine. My husband and I are both doctors. We are doing everything we can to protect ourselves, each other, our children, our parents - but it may not be enough. If your doctors are afraid, worried, or scared then you should be too. If you dont know of anyone who has been affected by Covid-19, you will. If you dont know of anyone who will die from Covid-19, you will. This is not some sort of scare tactic- this is our new reality. This is meant to warn you to take this seriously, like really seriously! . We are working hard to protect you and your family. I personally was a Healthcare Studies major at UTD because they offered different tracks for pre healthcare professions and I was able to take the pre dental track. This allowed me to take most of the recommended prerequisites prior to dental school while also taking electives that pertained to the healthcare field. Aside from that, Biology or science is typically ideal because it can really help you take the recommended classes for dental school without taking additional courses that arent needed for dental school, but if there is something that really sparks your interest such as psychology or business! would definitely say to go for it! Yes it will require more work and more classes to take but itll help you stand out as an applicant and youll have the opportunity to major in what interests you outside of science! Attempting to breath throughout the day with these N95/KN95 masks on with a surgical mask on top has been difficult, exhausting & gets a bit hot in there 🥵 ➖ Most of all, I feel blessed & very grateful to be back at work to be in service to my patients 🙏🏽. Let’s talk about Vital Signs! ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 🏩 Vital signs provide us with critical information needed to make treatment decisions. Above are the normal ranges for ADULT vital signs. Have these values memorized, you will need them for the rest of your nursing career! 🩺 We typically monitor our patient’s vital signs q4h, q8h or q12h (1-3 times) every shift but it varies for every patient! Know your patient’s baseline vitals - it will be easier for you to determine any abnormal values. I like to take a look at the patient’s chart prior to the start of the shift to make note of their previous vitals. Remember each patient will have different baseline values depending on their current health and health history. ❗️ Remember: patients with COPD often have lower oxygen saturation and the range between 88-92% is normal and safe. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 👩🏻‍⚕️ Nursing students: report any abnormal values to your clinical instructor or your nurse. Good friends go with friends to get Covid tests. Obviously we’re wearing masks all the time. But as we continue to have to venture out into the world, it’s important to get tested. Especially if you’ve been out and about. Ashley had to get tested as protocol for some upcoming excitement, but we’ve both been getting tested every couple of weeks. We’re living with my parents who are in the high risk category, and I will not put them in danger. I know there is a test shortage in some areas, so it’s probably not possible to get tested ALL the time, but it’s good to just check and make sure if you’re able and if the resources are available. When the results for my COVID test came two days later, doctors were doubtful about my reaction. I told them not to worry since Im a doctor myself. I knew that I could handle whatever the situation might be. Thats when they revealed that I was COVID-19 positive. As soon as the news spread, many of our neighbours panicked. The city administration council put up posters outside our door. People clicked photos and shot videos of my house. They shared it with my name, address, and personal details on different WhatsApp groups. Since people love to gossip on WhatsApp, ugly stories about me started circulating in social media. Even the news media put out the wrong kinds of stories. Despite going straight to the hospital from the airport, I had to be the victim of social media hatred. I didnt even go home to collect my personal belongings to make sure that I didnt transmit the virus to anyone. Thankfully, I started recovering and, after a few days, tested negative. But the hounding didnt stop after I was back home. I knew that going back home will be an unpleasant experience. Therefore, to avoid attention from neighbours and media, I insisted on going home late at night. Not just me but my entire family had to experience the harassment. My younger brother didnt step out of the house for two months. Neither did I. I would only step out during the night to avoid people. When I step out, I can still sense the eerie stares and gossip. It is like a bad dream - Im all alone. People refuse to hear when I plead to them that I didnt do anything wrong. The only way we can fight this pandemic is with the right balance of precaution and empathy. Yes, we need to be careful. But we also shouldnt let our fears overpower humanity within us”. Why I chose ophthalmology? 1️⃣It’s Very Much an Art Form:  When I rotated in ophthalmology as a med student, the intricacy of the human eye blew my mind. After seeing my first cataract surgery, I was hooked.  The detail & the precision required, was beyond fascinating. Growing up I was always very artistically inclined so for me surgery seemed a natural calling. And particularly with ophthalmology, where it’s all about the finer nuances of microsurgeries, I realized this was my place. 2️⃣It’s Incredibly Rewarding Of course, most importantly, my job is about giving someone back their ability to see. This is what I love best. Patients who have cataract surgery come in the next day & are amazingly already seeing 20/20 or those who I treat for various conditions & are able to regain the ability to carry on their normal life…their gratitude & their smiles are everything to me. 3️⃣ Continuity of Care In ophthalmology I get to see my patients over the course of several years; we inevitably develop a long term relationship. So being able to help them means that much more. The other good part is that my patients are usually for the most part healthy & happy. 4️⃣Variety This job never gets boring! My patients are of all ages & from all walks of life which is keeps it interesting. Also, my schedule is variable; I get time in clinic seeing patients as well as doing in office procedures, & I also get to spend days in the main operating room. This specialty is a perfect blend of surgeries & office-based care. 5️⃣The Potential To Give Back Taking it even a step beyond…cataracts are one of the leading causes of blindness worldwide. This is something I have the ability to fix in under 15 minutes. Being able to extend my services globally to those in need is a dream of mine. 6️⃣A Great Work/ life balance  Finally, ophthalmology is one of those specialties that really does offer a somewhat manageable work/life balance. The times I am called in for an emergency is minimal. Buttttt most toothpastes contains water that will naturally foam the paste, lubricating the brush which makes the brushing experience that bit more comfortable There is no right ✅ or wrong ❌ but again the more comfortable it is, the longer you are likely to brush for and that is whats important 💞 What is key, however, is not rinsing the mouth immediately after brushing Instead, simply spit out excess toothpaste. Today is #OrangeShirtDay, an opportunity for us to reflect on Indigenous peoples experiences, forced, year after year, generation after generation, to surrender their children to Canadas Indian Residential School System (IRRS). In this system, school graveyards were as common as school playgrounds, with 24 to 69 percent of children dead. Our Canadian healthcare system, often revered for its universality and accessibility, played a sinister and prominent role in this genocide. We better understand today that the historical relationship between Canadian healthcare systems and Indigenous peoples has been one of colonial violence, otherwise known as medical colonialism. This is why Dr. Samir Shaheen-Hussain, Dr. Suzanne Shoush, @SemirBulle & I teamed up to share our thoughts in a new opinion piece published today in @TheTorontoStar. It outlines why Orange Shirt Day is an important national day to honour the resilience, strength, dignity, and perseverance of Indigenous Peoples in Canada despite the genocide of the IRSS. But it also asks us to reflect on this painful part of Canada’s history with medical colonialism as we strive to begin healing these wounds by committing to building a relationship of understanding, trust and respect between our healthcare systems and Indigenous peoples. Olá, me chamo Gabriela Barbosa. ☺️ Cresci sabendo que a Medicina Veterinária estava no meu propósito de vida, assim, ingressei na Universidade Federal Fluminense (UFF) em 2009. Durante a graduação tive a oportunidade de participar do programa de intercâmbio na Universidade do Porto (Portugal), onde pude vivenciar ainda mais minhas áreas de interesse (Cirurgia e Clínica Médica). - Ok. Were all stuck inside feeling stressed. So I decided to revisit some of the hot TV/film medical professionals who made me feel like everything was going to be ok. This one is a throwback. George Clooney as Dr Doug Ross.

Why did I choose ophthalmology as a career? My journey to ophthalmology has been unconventional. It pretty much passed me by as a med student (well it was right before finals!), and so I stumbled into general medicine first instead. I had always had a passion for global health so I thought I could do tropical medicine and that would work well (and sounded like a cool job). Unfortunately during medicine I realised it was definitely not for me. I took 6 months off and did the diploma in tropical medicine to get me ready for what I thought would be some time overseas next. However during the course there was one inspiring lecture by the incredible Allen Foster on international ophthalmology and in that one instant I got what could only be called a lightbulb moment! I went to speak to him straight after and after a few more conversations, and observations I dived straight into my first ophthalmology job! It was a big risk - I had no idea if I could operate- but I loved it :) When my first ophthalmology on calls were from home, I knew I had found a speciality to call home!! I now combine paediatric ophthalmology and being a global child eye health academic. So, for me the ability to combine global health work with ophthalmology is what got me into the speciality and is what keeps me here to this day- and I can’t imagine anything better! Let’s talk about vaginismus! I’ve gotten so many messages recently on this. Unfortunately I cannot give advice on social media. However I would like to share some education! What is primary vaginismus? Primary vaginismus is when vaginal penetration has never been achieved ever. So this means you have never been able to put in a tampon, have penetrative intercourse or digital penetration. What is secondary vaginismus? Secondary vaginismus is when vaginal penetration has happened before, but is no longer possible, potentially due to factors such as gynecologic surgery, abuse, trauma, menopause or radiation. When people have pain with intercourse this is called Dyspareunia- just the medical term for it. It is often confused with vaginismus. People can have painful intercourse for many reasons that are not vaginismus (like endometriosis). Vaginismus is an involuntary tightening of the vaginal muscles which makes penetration difficult or impossible. Many people who have vaginismus often have a fear of vaginal penetration and decreased sexual desire related to penetration. Which is not surprising! My vaginismus patients are a motivated bunch seeking help! I am incredibly proud of my patients when they make progress with their treatment! Its daunting to work with a therapist, use dilators and suppositories and have injections to an area that is sensitive. To all the women struggling with vaginismus do not give up!! I see so much success so often! It’s truly amazing to see when women are able to have a sexually satisfying experience with or without penetration! (Which is key to remember that penetrative intercourse is not the only way to have sex.) You are not broken!
