My ring light is a new pal. Like a friend who always scores a killer shot in a busy restaurant with bad lighting and still delivers (remember restaurants and wetness and booths?) I have #type1 diabetes which has been super challenging since being diagnosis 3.5 years ago. I don’t post everything all diabetes, all day, but when I receive a nice little token of sweetness from a family dealing with a child with type 1, I post. I was lucky enough to receive some great little sticker art from @lukasparttimepancreas for my robot piece. This little sensor has made my life easier (when it works) and having some art to represent families dealing with this invisible illness brings me a smile. So does pasta, but that also brings me an epic high blood sugar. We are all dealing with a lot right now. Mentally, physically, emotionally. She was originally drawn to a career in women’s reproductive rights and completed her Obstetrics and Gynecology Residency at Brown University/Women & Infants Hospital in Rhode Island. Early in her training she realized her passion for caring for women with cancer and was fortunate to have excellent breast surgery mentors who supported her transition into breast surgical oncology. WORLD BREASTFEEDING WEEK 2020 threw at us our biggest challenge as a family yet. After two full term pregnancies and given my baby in recovery for us to establish breastfeeding, this was completely different. In February our 3rd baby arrived very unexpectedly within an hr of being taken to hospital. With absolutely no control over what was happening our littlest one was delivered at 27 weeks in a hospital I had never stepped foot in. I had never seen NICU and I had no idea where our baby was or where he had been taken too or how he was. As I was in recovery in absolute shock (mind & body) of what had happened it didnt even occur to me how my baby would be fed. I never saw him until 24hrs later and all I remember is the nurses saying will you be able to Express as they handed me some tiny syringes. They discussed PHM milk which we needed in the beginning as I began the expressing. This was the start of almost 14 weeks of expressing every 3 hrs, setting alarms, understanding what was now my new normal. It was hard, I would sit at home and Express around my older two kids but there was no baby in sight, the emotional journey that came with how feeding looked for me now. Establishing attachment and suck feeds took us a lot longer. It was all new for both of us. We both had to learn with establishing breast feeding. It was hard and there were so many times I wanted to give up. I was tired, exhausted, ups and downs. I persevered I cried we worked hard and we slowly got there. After having our little boy home from NICU I wanted to donate my expressed milk back to the NICU. I was able to help another mum who was on her own breastfeeding journey and who had reached out for breast milk over this platform. Breastfeeding is different for everyone. What us Mamas need the most is feel supported no matter how feeding looks like for us and our baby. Our little guy had donar milk, he had my expressed milk for 14 weeks until we could breast feed. He was tube fed, milk in a bottle. It was a journey that will always be apart of what breastfeeding looked like for our 3rd little baby. Last year was one of the hardest years in my life. Actually NO. It was THEE absolute hardest year of my life. June 16th 2019 I woke up at 2am with excruciating pain in my back. Nothing that I took or did helped with the pain. I finally was able to get comfortable and fell asleep for about 30mins. I woke up again IN PAIN and not being able to walk. I loss all sensation in my legs. I called the doctor and she had me go in to see her that next morning. From there we scheduled an MRI on the 18th at 6am. At this point I was just really hoping that I just had a pinched nerve or slipped disc in my back from working out. Nothing too serious was going through my mind. The morning of the 18th came and I went in for my mri. I drove to the hospital and went in for my MRI. I’ve never had this done before. Going into a machine that’s a tunnel when you’re basically claustrophobic was pretty nerve wrecking. Not gunna lie the anxiety was real at that point. After 1 hour I was finally done. The lady came out, looked at me and said we need you to drive to the ER right now. Now I’m panicking. Thankfully I had my mom with me. I get to the ER, and waited a bit. They finally took me back and said that they were going to do another MRI on my back. I still have no clue why they are doing it again. They weren’t telling me anything at this point. This time they wanted to do the MRI with contrast(something that gets Injected into the body and enhances the quality of the mri so the doctors have a better idea of what they are looking at) At first I declined the contrast dye. I didn’t know enough about it to just let the doctor inject something into my body. Yes, the doctors were annoyed that I declined. But you need to know and research things before they are put into your body. They took me in for the MRI without contrast. When I was done a little while later the doctors came in and said that I have a tumor in my spinal cord that was pretty big and pressing into my spinal cord and bleeding(which was causing all the back pain) CONTINUE READING IN COMMENTS - @funnel_mom on Instagram

As a physician, I have to trust my instincts, as well as the years of medical training and practice that have brought me to where I am today. When a patient comes in concerned about something, sometimes it is my job to reassure them that whatever they have is absolutely fine. For example, lots of patients come for a new “mole” that is growing, new, or bleeding. And many times, all it is a benign seborrheic keratosis, affectionately called “wisdom spots”, or as I like to call them, “maturity spots.” 😉Sometimes it is a completely benign appearing mole, in which case my job is also to reassure them that it is fine and does NOT need to be removed. A good dermatologist will know when to remove something, and when NOT to remove something. That’s where the wisdom, knowledge, and training play a key role in medical management. On the other hand, my job is also to know when to be concerned, even when the patient is not sure they need to be. They come in saying: ⚠️It’s just a non-healing burn from my curling iron.” ➡️ Basal cell skin cancer ⚠️”It’s just a wart, but it’s a little painful.” ➡️ Squamous cell skin cancer ⚠️”It’s a spider bite.” ➡️ Pyoderma gangrenosum This patient came in saying that he had accidentally caught his finger in a tight metal clip a few months ago. It immediately turned into a dark bruise, but it had stayed dark in color ever since. Prior to that accident, “my skin was clear.” He had a previous history of melanoma that was treated several years back with a partial amputation. Dermoscopy revealed a pink-brown macule that was hazy and structureless throughout with associated fine white streaks, along with brown reticulation at the lateral edge. I convinced him that a biopsy was needed, though that is not what he wanted. •Diagnosis: deep 1 mm invasive melanoma• The melanoma was not caused by the clip accident. But it likely drew the patient’s attention to it. So despite his thinking that it was just a benign bruise, my job was to gently inform him otherwise. Trust your doctor. CATARACTS According to the @who, cataracts are responsible for more than 50% of the worlds’ blindness. Although cataract surgery is one of the most cost-effective sight restoring interventions, people remain blind mainly (and sadly) due to a lack of access to eye care or inability to afford surgery ☹️ In developed countries, vision impairment due to cataract is 5% but in low-income, developing countries and remote areas, this figure skyrockets up to over 50% 💔 Here on the island, cataract surgery is widely available at no cost to local patients but fear of surgery remains one of the huge barriers to patients seeking care earlier. My colleagues and I frequently encounter mature white, brunescent and black cataracts, and our lists tend to be a mixed bag of ECCE/SICS (where the lens has to be manually expressed out through a larger corneal wound) and phacoemulsication. Unfortunately, with the current pandemic, patients’ sight restoring surgeries have been delayed and we’re still working through the backlog. We’re getting there 💪🏻 Please bring your loved ones to your community eye clinic or optometrists for a check up! In my experience, the elderly population here tends to suffer in silence far longer than they have to and when I see them in clinic, their cataracts have reached a stage where they can barely see. This procedure is reserved primarily for mitral stenosis (most commonly rheumatic MS). . . . 🎈The balloon seen here is called the Inoue balloon. It is shaped like a figure 8 and inflates proximal to distal one order to seat itself well within the mitral annulus. The primary goal is to “crack” the fused commissures of the rheumatic mitral valve. Access is obtained via the femoral vein and a transseptal puncture is used to gain access to the left atrium. . . . Severe MS is defined as: . ⭐️MVA 1.5 mm2 . ⭐️Mitral mean gradient of 5-10 mmHg . ⭐️keep in mind Afib and PH . . 🧮Mitral valve area calculation from P1/2t (220/PHT) is the most useful way to assess severity as MS is very load dependent and can change with varying heart rates. . . . 📚Other things to remember when assessing a patient for this procedure is the Wikin-Abascal score which looks at valvular calcification, leaflet thickness, leaflet mobility, and subvalvular thickening. A score of 8 is indicative a success for PBMC. Dermatology residency has been everything I could have imagined and more. Love that our field is so diverse and that I get to see a different mix of diseases + treat people both medically and surgically every day. It has also been one of the most challenging times in my life. Starting a subspecialty is like learning a whole new language. Most medical students receive a few days to a week of lectures in subspecialties such as Dermatology. The vast amount of knowledge we are required to know as Dermatologists is quite overwhelming. What we do for a living may look easy on the surface but there is SO much more to this field than popping pimples and injecting botox. And not everything is a “maculopapular” rash. There is a vast amount of information one must know to become a Dermatologist. We treat people of all ages and are trained in complex medical dermatology, dermatoscopy, dermatologic surgery, dermatopathology, pediatric dermatology, and cosmetics. Infact there are over 3000 dermatologic diseases that we are required to know everything about, down to the microscopic proteins and genetic mutations. We work 9-12hr days in busy clinics and are expected to cover several hours of dense reading every evening + on weekends. Have been taking some time to focus on study habits and creating a routine where I am able to balance performing well in the clinic and life outside of it! This patient came to see me for a rhinoplasty in order to make her nose a less prominent part of her face and allow her beautiful eyes to better shine through. When I perform rhinoplasty or nose job surgery, I adjust and reposition the position and the size of the cartilage and bone. The aim of these maneuvers in surgery is to create an external appearance of the nose that creates a more pleasing interplay between the light and shadows on the face. By doing so, we allow the eyes and lips to become more highly visible creating a more attractive appearance. Starting as a brand new doctor is always challenging but right now it is more than ever. Every year on the first Wed of August brand new UK doctors start for the first time and trainee doctors rotate to a new job. How strange and slightly exciting it was to introduce myself as doctor for the first time to patients. What would I loved to have said to myself back then? 👩🏽⚕️ You are not alone! Never hesitate to ask for help or ask a question (even the stupid ones!). I so wanted to prove I could it and knew what I was doing that I would hesitate to ask for help. Dont. Soon I learnt its the greatest strength in medicine to have humility and self awareness of what you dont know. Caring for patients is a team effort. 👩🏽⚕️ Help others! Look out for each other, do other colleagues jobs if you finish early or theyre struggling. Agree to oncall swops when you can and generally have each others back. It makes everyones life just soooo much better. 👩🏽⚕️ Take care of yourself. Slow down in your life, especially at first. You can still do the things you love (and make sure you do) but dont overdo it. Prioritise whats most important and make sure youre always top of that list. 👩🏽⚕️ Care for your patients. I mean really care, empathise. Its easy to get lost ticking off jobs in your to-do list. But dont forget its the little things that sometimes matter. Sometimes youll be able to do a lot for your patients (look at me saving lives!) other times you feel like you cant do much (but you really are). By caring when people are at their most vulnerable youre making a difference- and its a real privilege to have that opportunity. 👩🏽⚕️ Remember to enjoy your time. It wont last forever (now I feel old for saying that!!). If there are serious issues (bullying etc) get the right help. Otherwise expect some challenges but also know you can have an amazing time while doing an amazing job. Well here is my #medbikini response post because these pictures were taken less than 5 hrs apart. The first I was scrubbing in to a #covidpositive c-section and the other one a few hrs later, enjoying the beach wearing whatever I want and makes me feel beautiful after working a 24-hr shift ! Female physicians can wear whatever we want off the clock, have some wine/ drinks , have a normal social life like everyone else. We will still come to work, coffee in hand, and still be competent, professional, and give you the absolute very best care we can give. I absolutely love the response of my colleagues to this article and the fact that it was retracted. Keep going lady bosses! Jadi nostalgia, salah satu kerjaan yg aku seneng BANGET adalah tindakan bedah-bedahan, menantang euy, dan kebawa senengnya sampai udah praktek mandiri. Dulu, dengan senang hati kalau ada temen yg minta bantuin tindakan bedah, kadang gemes pengen ikutan nge-handle, sampe praktek juga masih sering tiba2 di telpon sama temen minta tolong dibantuin, yah minimal bantu support atau jadi tim hore 🤣. Bukan berarti aku jago, tapi suka dengan pekerjaannya. Nah... Tertarik ambil spesialis bedah mulut? Belum tentu, hampir semua bidang kedokteran gigi aku suka. Jadi masa koass dulu tersiksa ga? Engga, malah seneng, bisa belajar banyak, bisa tanya-tanya, bisa tau “berjuang” sampe di luar batas nalar, yg aku pikir aku ga akan sanggup, eh ternyata sanggup. Jadi koass memang menyenangkan, tapi cukup jadi kenangan, tanpa perlu di ulang 🤪. . Reposting this from a few months back- As a female physician, educator, solo practice owner, mother, wife, daughter, and sister, I am always giving myself to others. Patient care, being on call, refill and authorization requests, cleaning up around the house, laundry, meal planning, groceries, helping with homework, making sure household bills are taken care of, etc, etc. Putting the needs of others before my own has been a way of life. But it is draining- physically, mentally, emotionally, and spiritually. I realized that I need take care of myself, which means prioritizing my own needs. All that is needed are little things help me recharge and stay balanced. Building your grad school application list. Idk about yall but this was a challenging and time consuming step. There are so many universities and programs to choose from and on top of that going through all of the professors’ research bio. . After talking to my friend @_brysa_ about it she was like ‘Have you heard of NIH RePORTER?’ . Here is a tool I wish I had known about when I applied. NIH RePORTER ( National Institute of Health, Research Portfolio Online Reporting Tool) ‘is an electronic tool that allows users to search a database of NIH-funded research projects ...’ . How to use it 1. Search for ‘NIH Reporter’ and choose https://projectreporter.nih.gov/ 2. Select their Matchmaker Tab and insert your research field/s of interest into the space provided 3. A list of current NIH grants will pop up along with the advisor and university. Under the Act column try to stick to grants that’s are R01 and R21. Long gone are the days of reading long research bios and deciding if you find that research interesting. Hopefully this helps you put together your list of grad schools to apply to. . #myjourney Tips on Observeship the United States. Once you have secured the observership in the US- make sure you do everything possible to impress the people you are working with. Hard to do that when you don’t have anything hands on. Right? It is hard but here are my tips based on what worked for me and now what I like to see as an attending. 1. Show up on time for the rounds. Always dress well and clean. This includes clean white coat too. You don’t want to look odd in your first impression. 2. Try to shadow the residents and see if you can write history yourself in your words in a word document. Ask the permission of the resident if you can present the patient. Important to tell the attending you observed the resident but if he/she doesn’t mind, you’d like to present the patient. Don’t be pushy at any point( believe me I see this All the TIME) 3. Be a team player. Offer to help- small things like calling the pharmacy to get the medication or calling the doctors office and getting records. On a busy medicine service- the resident will appreciate if you can be helpful 3. Don’t Interrupt rounds. In medicine - we fight with time constantly and work needs to get done. In some hospitals- rounds are extremely educational and attendings or residents may encourage you to ask questions or may ask you questions on a subject. That’s where you shine. Read on the subject to prep yourself. But try not to show off or put the residents down by giving answers unless you are asked the question specifically. This is where The United States is very different. EQIQ Always be mindful of that 4. Be curious and proactive. My attending during my 3 months observership was impressed with my interest in cardiology and he asked me to prep a talk on PVCs. That was my first ever talk in the US for academic medicine. This worked out well since after this- the attending EP cardiologist at Jefferson decided to give me his research project and I ended up presenting 2 posters on the subject as a medical student. So the key my friends is be curious/ proactive/ enthusiastic/ teamplayer/ professional. Everything in Balance. You want to shine but not by dimming others 💡 - @drpurviparwani on Instagram

Let’s talk about vaginismus! I’ve gotten so many messages recently on this. Unfortunately I cannot give advice on social media. However I would like to share some education! What is primary vaginismus? Primary vaginismus is when vaginal penetration has never been achieved ever. So this means you have never been able to put in a tampon, have penetrative intercourse or digital penetration. What is secondary vaginismus? Secondary vaginismus is when vaginal penetration has happened before, but is no longer possible, potentially due to factors such as gynecologic surgery, abuse, trauma, menopause or radiation. When people have pain with intercourse this is called Dyspareunia- just the medical term for it. It is often confused with vaginismus. People can have painful intercourse for many reasons that are not vaginismus (like endometriosis). Vaginismus is an involuntary tightening of the vaginal muscles which makes penetration difficult or impossible. Many people who have vaginismus often have a fear of vaginal penetration and decreased sexual desire related to penetration. Which is not surprising! My vaginismus patients are a motivated bunch seeking help! I am incredibly proud of my patients when they make progress with their treatment! Its daunting to work with a therapist, use dilators and suppositories and have injections to an area that is sensitive. To all the women struggling with vaginismus do not give up!! I see so much success so often! It’s truly amazing to see when women are able to have a sexually satisfying experience with or without penetration! (Which is key to remember that penetrative intercourse is not the only way to have sex.) You are not broken! The question of working to live or living to work comes up often especially during a pandemic where a lot of the joys of life have been taken away. Are we just living to work because there is not much else to do? When I decided to become a doctor I didn’t realize most of my 20’s would be dedicated to my education. But to be honest I never really minded it. I often think back to it being some of the best times. I met my husband, made awesome friends, learned a lot and honed my skills, and even had first baby I training. At that time I was living and learning. When I went part time right after residency- I was certainly working to live. I worked minimally to maintain my skills and sanity but also enjoy my babies at home. Now as an attending physician working most days, whether it’s in the office or not, I catch myself “living to work”. I have to remind myself that burnout is real. No matter how much I love my job and my patients I need to remember- I am no good to anyone if I am burned out! “The COVID-19 pandemic is exacerbating the national public health problem of physician burnout and weakening patients’ tie to the essential physician care they need to manage chronic conditions.” An article from American Medical Association also goes on to say that as a result women are not getting mammograms, children are not getting immunizations and patients with diabetes or hypertension are not having their chronic conditions properly managed. During this pandemic I was diagnosed with hypothyroidism. Though I have a genetic predisposition to it, I was shocked. For someone who never took medications this is an even more important reminder to schedule bloodwork follow ups for medication management and cancer prevention screenings and regular vaccinations like the flu shot! No matter what field of work you are in ask yourself are you living to work or work to live? Im at home resting now. All is well. I had my entire week planned out. I was set to appear today on @NPR, on @Guy.Raz podcast, @HowIBuiltThisNpr. The universe had other plans. Yet another stark reminder that we are not in control. That life can change at a moments notice. One night you are having a lovely glass of wine with dinner, and the next you are in the ER with stabbing pain about to be opened up. Appendicitis. Nothing big. Nothing complicated. Two days later I am home and feeling just fine. But the lesson is not lost on me. The lesson this whole year has been pounding on us. Have your priorities straight. Figure out what really matters to you and put that up front. It has made me think about the people that I cherish most. The ones I truly yearn to see when all of this is over. To me, the big lesson is not to wait. See the people I love more often, do what makes me happy more often, and really appreciate those moments. Just dont wait. Finally finding the opportunity to weigh In on the #medbikini movement on Instagram because sexism in medicine is clearly alive and well! The double standards exist. In case you haven’t heard I’m referring to an article that has since been retracted (after this viral movement) where male screeners created fake social media accounts to spy on the accounts of vascular surgeon residents . The screeners deeemed some of the the following items as “unprofessional” - wearing bikinis - Halloween costumes - Holding/ consuming alcohol - Discussing “controversial” topics, such as religion, etc Clearly the paper exhibits underlying gender bias towards women. I do want to note that I do think professionalism is important but does that mean female doctors can’t have a life outside of medicine? And who defines what professionalism looks like? Are women expected to wear scrubs at the beach? Can a woman wear a bikini and still be a fantastic surgeon? Of course she can! . I stand in solidarity with my female colleagues who have had enough. . 4/ It’s possible to move on & have a different experience.. These past few days I’ve shared bits of my story. It’s been heavy but I have chosen to speak up about this for #birthtraumaawarenessweek in the hope that it speaks to any mum who is struggling right now. When you are in the pit of a challenging time - for mental health often is a silent and hidden struggle - you can’t see another way. You feel very alone. You feel there is no help. After a challenging birth, many women feel they can never have another baby again. They often avoid gynaecological appointments too (smears for example - ps. guilty!) and carry the pain for years. If this speaks to you, please seek help now. My only regret is that I wish I’d asked earlier. I never thought I’d ever say this, but thanks to the help I got from my maternity team, my birth this time - despite all the past trauma and pandemic nightmare - was magical, beautiful and I’ll go as far as to say mystical. Also for anyone telling you a c-section can’t be all those beautiful things, it can! My psychologist, midwife and consultant worked together and promised me a different experience and they helped me deliver my beautiful little girl. I learnt I could birth without “trauma”. I learnt that I could have a beautiful birth. I felt the magic that I often heard other mums speak of but couldn’t connect with. I experienced that love at first sight that I missed out on the first time. I learnt it wasn’t me. I learnt I wasn’t broken. I learnt I could truly forgive me & the past. I learnt that it was possible to move on and heal. So I’m here to tell you that there is help available and that there is lots of support. This pandemic has added layers of anxiety to pregnant women. Birthing experiences have been variable but full of uncertainties. From absent partners at scans and births to isolation and lack of support in the community, a new wave of warrior mums have been born but for many, it has been traumatic. If you’ve had a challenging pregnancy or birth experience, ask your midwife or obstetrician early on (ideally whilst in hospital) for a debrief to understand what happened and why. Just before I embarked to this Covid madness (one day before I left for Wisma Atlet), Daniel, the director of @msf_indonesia told me, this will be a very long, strenuous fight. One day, youll feel like you want to be a superhero. Dont be. Dont try to be a superhero, because youre only human. And a fallen hero will kill more than an average joe. The pandemic now hits 6 months. Too many people have died, too many friends have fallen. Yet, this agonizing number of deaths seems to not have any impact on us. If I had one word of advice to new grads on how to improve patient compliance - it is to EDUCATE! 📖 Even speaking in laymans term, patients often are overwhelmed by the information given to them and the eye is quite a complicated part of the body. ✨ Here are my top three things to help you get the message across: 1) Eye Models - everyone loves 3D models, and it helps a patient understand where exactly the problem is located! (Is a cataract a film over the eye or a clouding in the lens inside?) 2) Diagnostic Imaging- a picture is worth a thousand words. Describe a retinal tear in someones eye isnt the same as showing them the tear! Likewise, imaging can be expanded to anterior problems. 3) Brochures- you might think thats old school and will just go to waste, but people do glance at them, especially your elderly patients! My first textbook chapter is officially in print! 🎉🤓 . It was a privilege to co-author a chapter on Intraorbital Tumors and Management in “Endoscopic Surgery of the Orbit”, edited by Dr. Raj Sindwani. Oculofacial plastic surgery is a broad field, and operating in the orbit to treat cancer is among the most incredible things we do. Thank you to one of my dearest oculoplastic surgery mentors, Dr. Roxana Rivera, and ocular oncologist Dr. Arun Singh for inviting me to collaborate on this publication 🙏🏻 . On a more personal note, the path to becoming a doctor is long and arduous. Having strong mentors who you look up to and guide your career is priceless. I am so incredibly fortunate to have met Dr. Rivera back in residency...she is one of the strongest women and incredible surgeons I’ve met. I still use the pearls she taught me every day! I am proud to have this textbook as a reminder of our time together 💫 . Something I can proudly say after finishing fellowship is that I’m not scared of the vitrector anymore! I used to dread the “V” word, but now I actually even enjoy the noise the machine makes 😅 I think the most important part here is to remember that even with a PCR, patients are going to do just fine if we keep calm. Stop, assess the situation, breath, and don’t forget to inject viscoelastic before you come out with your instruments! I know I am lucky to have had amazing mentors who taught me well but there are lots of great resources available for free online! 💛Your life = the sum of parts that make you whole ▪️Being a physician and practicing medicine is a huge part of who I am and therefore a huge part of my life. ▪️We as physicians are bread to do our best and be our best because what we do isnt easy, its life-saving. We honor that responsibility. ▪️But what about the other parts of my identity, the other parts of my life? ▪️Putting in the time for my career and my work was always a given, right from the start, but putting in time for myself and creating boundaries were foreign concepts and initially hard to grasp. And honestly, these concepts continue to be an ongoing reflection for me as life changes. ▪️What I have learned and continue to learn is that in order to be my best and live my best, I have to integrate time and care for myself by creating boundaries, and doing the things that make me whole. I have to put in the appropriate time to all the parts of my life. ▪️I am passionate about my career as a physician and dedicated to caring for my patients. I also have limits on what I can do, and have realized its ok to say no. I now actively integrate my pastimes (eg: traveling) and extracurriculars (eg: swimming, yoga, dancing) into my daily and monthly routines. ▪️I am a doctor, friend, sister, daughter, and human. Brace yourself for this post. It’s one that many people who already work in the field will be familiar with, but often gets overlooked when it comes to selling skincare in the beauty industry. I use the images (scroll through) frequently in talks and seminars and would like to share the science with you. We often mistakenly think that skin ageing means the same thing as facial ageing. In fact, it seems obvious if you think about it, but the skin is only part of your facial anatomy - below the skin is fat, muscle and bone. These are also subject to change as we grow older and understanding these changes is key in recognising why your skincare can’t really stop your skin becoming lax or starting to sag with age. *** Let’s look at what happens underneath the skin as we get older. Scroll through to the second image and what this picture demonstrates is the multiple compartments of fat underneath our skin. When we are young, these fat pads are plump and sit together neatly to function as one continuous unit. As we age, the fat pads become thin or atrophy and separate, no longer slotting together as one. As these fat pads slip or are prone to the effects of gravity, we get deepening of the lines between our nose and mouth (nasolabial folds) as well as visibility of the underlying muscle under the eye. Lack of fat under the eye leads to hollowing (tear trough formation) and exposure of the muscle which has a rich blood supply leading to visible dark circles. *** Scroll again - not only do the fat pads change, but believe it or not, so does our facial skeleton. As we get older, our bone remodels over time and resorption (bony loss) is common. The pyriform aperture (the hollow where the nose sits) increases in size with age and the mandible bone shrinks (compare the size of the bone below the hollow where the nose sits in the young and old). This leads to sinking of the soft tissues, further deepening the nose to mouth lines and laxity of the skin as there is less bone to support it from underneath. I can’t tell you how many times I’ve had patients put off a minor annoyance, only to come in with a blossoming sight-threatening issue like a fungal corneal ulcer, neovascular glaucoma, or retinal detachment. ... Since reopening our clinic, I’ve seen it all—patients who lost vision because they delayed their care due to COVID. I’ve had to operate on a backlog of dense cataracts back-to-back these past weeks. One particular cataract was so ripe, its leaky proteins had clogged up the eyes’s drainage system, resulting in pressure so high, it caused permanent blindness (aka phacolytic glaucoma). More than restoring sight, I also take comfort in knowing that each patient I treat with an “elective procedure” like cataract surgery or corneal transplantation will be less likely to suffer from a life-threatening fall, the number one cause of injury-related fatalities in older adults. Work to live or live to work? Thanks @vickichanmd for asking to join the #worktolive🩺 campaign. . For me, the answer is not so simple. I feel so fortunate to have a job and career I love and feel passionate about. So do I live to work? In a way, yes. I can’t really imagine my life without working - without having a sense of purpose, reward, & stimulation. But I also can’t imagine my life without my outside personal life. So do I work to live? In a sense, yes. Being a mom is the single most important role I have in life, but taking a break from that role to go to work actually makes me a better, more patient mom. Outside the hospital, I also wear the hats of wife, sister, friend, fitness enthusiast, traveler, and foodie. While working gives me an overall purpose and sense of fulfillment, these outside passions, relationships, and hobbies keep me fulfilled on a different, personal level. Having this filled “cup” allows me to be a better doctor, because as they say, you can’t pour from an empty cup. My attempts at achieving “balance” of these roles and interests in and out of the workplace actually help keep me motivated and fulfilled on both sides, without feeling “burnt out.” . What is Academic medicine? Most physicians in the states don’t have any postgrad degrees other than an MD. This is not a requirement to go into academic medicine. In fact, there are basically no requirements at all. Academic medicine is simply being at a teaching hospital, with medical students, residents and/or fellows. Academic medicine simplified: - Research: pro is being able to do it, usually having dedicated time for it. Con is you have to be productive and have publications on a regular basis - Teaching: being around trainees and students keeps you young, sharp and up to date. There are also many opportunities for mentorship. This also means you will spend downtime doing just that - On call: because you have trainees, you are usually the supervising doctor. Your interaction with a patient is relatively small - Salary: generally, salary is lower compared to private practice - Leadership positions: many leadership positions are voluntary and are not compensated in academics. Basically, you’re doing it for free - Work-life: highly dependent on the job. Part time is rare in academics, at least in the MFM world. Most people who switched between the 2 world say work-life balance is better in private practice. There is a reason why most docs on IG are in private practice 😉 - Drama: the struggle of the institution to make money versus to be “academic” is real in many places. Money comes from seeing patients There are many hybrid practices these days, but this is the general layout. Every position is different, and the fit of the practice is what matters most! The days were long, but the years were short. 🏥🩺📟 . . . . I cant believe I finished anesthesia residency one month ago! 🤗 And Im going to start pain fellowship tomorrow! 🤭 . . . . Anesthesia actually wasnt my first choice of speciality during most of #medical school. At the beginning of my fourth year of medical school, I listened to those before me who showed me how amazing the field of #anesthesia is..and persuaded me not to do surgery. (Heh, sorry my #surgery friends! 😆Thank you for doing what you do! 🙇🏻♀️❤️❤️) Anesthesia is more than just putting people to sleep. 😴 It provides you with a special skill set important during critical situations, especially during this #pandemic. 🦠 Its a total of four years of #residency training after medical school. 🏥 The first year is #intern year so you actually dont do much anesthesia until youre in your second year of residency aka CA-1 year (Clinical Anesthesia year 1). 😷 #Fellowship is optional and most anesthesia fellowships are one year in duration. 👩🏻⚕️ - @endlessrounding on Instagram

Happy Friday! Wrapping up another work week and reflecting on the great campaign of a #weekinthemedlife initiated by @tiredsuperheroine & @doctorjacci. . I’m at a phase in life where I love what I do, and want to see my practice grow. I’m also at a phase in life where I have 3 young kids full of energy and joy, and want to see them grow. Managing both isn’t always easy or pretty, but creating a lifestyle that supports these goals is key. Here’s a sample of my typical #workweek, which is modeled to help me balance my goals: . 𝕄𝕠𝕟𝕕𝕒𝕪 - I start work late, so get to spend the morning exercising + spending time with my kids. I then start my #workweek by doing minor room procedures followed by clinic. (I love the integration of surgery + clinic in #oculoplastics🙌🏽) . 𝕋𝕦𝕖𝕤𝕕𝕒𝕪 - This is my traveling clinic day 🧳 where I do 1-2 surgery center cases in the morning, followed by clinic at a satellite office. Late afternoon is dedicated to the kids, and I then have bimonthly neuroradiology conference 👩🏻🏫 to review interesting scans. 𝕎𝕖𝕕𝕟𝕖𝕤𝕕𝕒𝕪 - This is my busiest clinic + minor room procedure day 🏃🏻♀️ , so I squeeze in an early morning run with my favorite dermatologist @emilymmintz to help jumpstart the day. . 𝕋𝕙𝕦𝕣𝕤𝕕𝕒𝕪 = OR 🏥 day so I start early and finish early afternoon. Rest of the day is spent on emails, charting and research, followed by peloton + kiddo time. Evenings I try to do date night or girls night. A little #selfcare can go a long way! 💫 . 𝔽𝕣𝕚𝕕𝕒𝕪 - I start the morning with grand rounds with the students and residents, followed by minor room cases + clinic! 🏁 . Creating a #workschedule that provides time for my children, husband and outside interests, while still fulfilling my passion for oculoplastics is what makes my medical #lifestyle (though not always perfect) work.
