Some of you may or may not know In 2012 I was involved in a major motocycle accident that almost ended my life a red P plater drunk and high on drugs ran a red light coming off a freeway and T boned me my left leg and other bones were completely crushed and broken Some of the injuries were toes broken shin bone broken in 3 places dislocated hip pelvis out of alignment femoral shaft (thai bone) broken in 3 places broken wrist and fingers radius (forarm bone) broken elbow dislocated and many others this is just of the top of my head 5 operations later I now have many screws, and pins and rods holding my leg and bones together For almost 2yrs I was unable to walk properly I moved around my house in a office chair and did the baby bum shuffle on the floor lost over 40+ kg went down to 60/70kg looking like skin and bone I got even more depressed looking at myself because of how small I became so I started eating pizza daily KFC and what ever shit food I can find to get my weight back up. Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued. Today’s case we have this 20 year old male who was involved in a motor cycle accident! The poor guy was just riding along when he was tboned by a car that was not paying attention. The patient was NOT wearing any safety riding equipment (including helmet) at the time of the accident. As a result, his foot was crushed between the car and the cycle. To add salt to the wound (no pun intended), the car that struck him fled the scene! . Upon arrival to the ED, trauma protocol was followed as this mangled foot could just be a distracting injury. Luckily, no other injuries were found, including on the helmet-less head and neck! They found his foot to have a double lisfranc fracture (displacement of the metatarsophalangeal joints due to fracture or injury), dislocated navicular bone and dislocated and fracture talus. In this case, they were able to save his foot! Due to the extensive damage to both bone and muscle, however, it will take many monthly to years to be able to walk normally again! . I propose a question to everyone...would you rather having this outcome whereas your foot is saved, but, even after possibly years of rehab, you may not be able to reestablish full motor control and sensory back to the foot OR have the foot amputated and just learn how to walk with a prosthetic? The spleen is part of your immune system that besides being a filter for old cells circulating in your blood, it also is the main source of the body’s Complement system, which enhances your body’s ability to fight off a specific type of bacteria. Therefore, while you can live without your spleen, one of the trade offs is that you will require ongoing vaccinations against the most common of these organisms. She also sustained a mangled injury to her right lower extremity with what’s considered a full degloving of the extremity and multiple fractures and neurovascular injuries. Patient was taken to the operating room immediately and managed by multiple trauma surgeons for her injuries. A high above knee amputation was done. Above-the-knee amputations may be necessary for many reasons. These can include trauma to the lower leg, which results in a non-viable leg at or near the level of the knee. Below-the-knee amputation may adequately address a more distal injury. Many studies have attempted to create algorithms to help physicians decide when to reconstruct versus amputate. One of these is the Mangled Extremity Severity Score (MESS), which takes into account skeletal/soft tissue injury, limb ischemia, shock, and patient age. A spatial notion, combined with a deep knowledge of head and neck anatomy allows the development of techniques that brilliantly restore a large part of the aesthetic-functional complaint that a patient already mutilated by cancer usually suffers. Today we bring you a case of a 65-year-old patient with a dysphonia history of approximately 1 year evolving with a growing cervical mass. Cervical dissection was then performed using Crile (sacrifice of the accessory nerve, jugular vein and sternocleidomastoid muscle). A flap of the pectoralis major muscle was planned to cover the defect and protect the carotid artery. Note that the design and planning of the flap that involves the entire vascularization considering the thoracoacromial artery and its pedicles that irrigate this entire region. The first boy who shackled me with sweet words and thoughtful but measured gestures knew exactly why not to. 2:30 am working on a makeup... for an upcoming project. Made me think of these pictures I just found on an old memory card. This was from the garage #trap for #saw 7, I was standing only a few feet away from her body and watched the back wheel of the car spinning on the head. The black marks on her face and body were from the burning rubber as the wheel was spinning. I can honestly say that this was the only thing I had ever shot that made me feel uncomfortable. Chris Thorne is the proud owner of the 1st BIG cut I ever worked. I was working his corner alongside @parrumpaatt at #FightTime promotions vs Shaha Bobonis. It was a gritty fight, lots of dirty boxing & inside fighting. Late in the 1st round Chris caught a knee opening him up bad. We knew from the moment it started leaking the fight was over once the action stopped. To Chriss credit he fought his ass off doing the best he could to push the pace. When the round ended I got right to work cleaning him up. I stuck a huge self wrapped swab in his head (Thick as a pen) the cut took the whole swab & half thumb. Dr. Fields came over & pulled it open like a bag of chips. You could see Chriss skull, but Chris never conceeded. Told the ref & Dr. He was fine. That he delt with worse and was ready to go on. As the ref waived the fight off you could hear Chris plead with the ref to let him keep fighting. IM A MARINE! I CAN FIGHT! but the cut was too bad. He was sitting in the drivers seat without his seat belt and crushed his face on the steering wheel during an auto accident. After the initial examination, it was observed that the patients bilateral orbits, the maxillary, zygomatic and nasal bone structures were no longer present on his face. Incredibly, he was conscious and breathing spontaneously. After stabilization, reconstructive surgery was planned by a multidisciplinary team. The right eyeball was avulsed (not found) and the left eyeball was displaced anteroposteriorly. The last image shows the final aspect of the reconstruction. It is at this time that the question remains: what knowledge does a surgeon have to master to deal with such a case? Are you prepared? Is that what you want for your life? - Massive hemothorax due to the rupture of the intercostal artery after placement of the drainage tube. Hemothorax is an accumulation of blood in the pleural cavity [the cavity between the lung and chest wall] due to bleeding from the vessels of the lungs, heart, or chest wall.
This 26 year old male had a motor vehicle accident and unfortunately battered his frontal, orbital and nasal bones. The frontal bone had both anterior and posterior table fractures with persistent cerebrospinal fluid discharge. The whole frontal bone had dished in and also was pushing the right eye out of globe. Fortunately his vision was intact and there was minimal frontal lobe injury. We had to intervene early so took him to the OR.
