Skin prick testing is a tried and true method for evaluating IgE mediated allergies in the office yet there are SO. MANY. MISCONCEPTIONS. IgE is the allergy antibody involved in rapid onset allergic reactions, ie food allergy, cat/dog/pollen allergies. If someone has IgE mediated allergies (reproducible symptoms upon exposure), we can apply a drop of liquid allergen to the skin, gently scratch the surface, introduce it to the allergy cells (mast cells) in the skin and wait 15 minutes to see if a small local reaction occurs. If someone has IgE, they will release histamine, which causes a raised bump, redness and itching. The size of the bump indicates the LIKELIHOOD allergy is present. There are a lot of false positives, so these are not good screening tests and we can’t use any raised bump to definitively say someone is allergic. A positive and negative control must always be used for comparison. Areas of misunderstanding: ➡️Skin test size does NOT indicate severity of reaction ➡️There is no age cutoff before skin testing can be done - if you’re old enough to be allergic, you’re old enough to have a skin test to show you’re allergic ➡️Antihistamines block skin testing, which measure histamine release, and need to be held for 4-5 days before testing ➡️Negative results are pretty reliable. Someone may absolutely have symptoms but it’s just not due to IgE allergy (vomiting, diarrhea, chronic abdominal pain, nasal congestion) Hope this primer helps understand a confusing area.
