Idiopathic Ventricular Tachycardia (VT) I was inspired to do this #meded post today based on an interesting case I had while on call the other night. What is it? Most VT we see is caused by structural heart disease, however, 10% of VT cases have no structural pathology. This is called Idiopathic Ventricular Tachycardia. Subtypes defined by clinical presentation or mechanism. They have ECG characteristics that can pinpoint where it’s anatomically located and usually have a great prognosis Outflow tract VT 🔑 localized around left or right ventricular outflow tract (RVOT vs LVOT) 🔑Earlier age onset 🔑Nonsustained, repetitive, monomorphic VT or exercise-induced sustained VT (I.e. post exercise stress test) 🔑Symptoms: palpitations and dizziness 🔑Baseline ECG and echocardiogram are usually normal, but MRI may show abnormalities (I.e. RV changes) 🔑RVOT VT ➡️ LBBB with inferior axis LVOT VT ➡️ LBBB with inferior axis + small R-waves in V1 and early precordial transition or RBBB with inferior axis 🔑Treatment: Adenosine, beta blockers, verapamil 🔑Alternative therapy: class IA, IC and III agents 🔑Ablation: treatment of choice for patients with symptomatic, drug refractory VT, drug intolerance or for those who do not desire long-term drug therapy Fascicular VT 🔑present younger - 15 -40 years. More common in males 🔑Symptoms: palpitations, fatigue, dyspnea, dizziness and presyncope. Syncope and sudden death are rare 🔑Most symptoms at rest however can be triggered by exercise and emotional stress 🔑ECG is normal in most patients or can show transient T-wave inversions related to T-wave memory after tachycardia episode terminates 🔑ECG during tachycardia - RBBB with a left superior axis 🔑Treatment: mild to moderate symptoms ➡️ verapamil severe symptoms ➡️ ablation - @drambreenmohamed on Instagram

Exercise has a positive influence on the gut microbiome and the human gut microbiome can exert wide-spread effects on mental and physical health. ⠀ Importantly, brain-gut signaling has been shown to be bi-directional, where not only can gut bacteria influence health and behavior, but psychological states can alter gut health. Bacteria can influence neural, hormonal and immune responses as well as the permeability of both the gut and the blood brain barrier. ⠀ Several bacterial metabolites have been identified as possible mechanisms through which exercise can influence how bacteria communicate to the gut and brain. For example, short chain fatty acids (SCFA, e.g., butyrate, acetate) produced by fermenting bacteria can suppress pro-inflammatory cytokines and interact with regulatory T cells to attenuate colitis. Exercise also alters the ratio of Firmicutes to Bacteroidetes. One study even showed that athletes had higher alpha diversity and a higher relative abundance of 40 different bacterial taxa. ⠀ The combined and independent mechanisms through which exercise influences brain health, the gut microbiome, and their interface have yet to fully understood. Read on for tips I use when I look at an abdominal CT... 👨🏽💻Coming into radiology as someone who was mainly concerned with what was going on above the diaphragm I found the world of abdominal CT fairly daunting. But by getting to grips with the anatomy of the abdomen things became a whole lot clearer 👨🏽💻At this level of the abdomen we get a nice demonstration of the superior mesenteric artery – we can’t see the superior mesenteric vein here but normally the vein should lie to the right of the artery at the level of the pancreas, if this is reversed then consider intestinal malrotation which can predispose to midgut volvulus 👨🏽💻I always check to see there is a clean layer of fat surrounding the SMA – if this is infiltrated consider a malignant pathology such as a pancreatic tumour 👨🏽💻Always follow the vessels no matter which CT it is you’re looking at, be it head, neck, chest or abdomen. I always follow the portal veins within the liver formed at the confluence by the splenic vein (which runs posterior to the pancreas) and the superior mesenteric vein which runs vertically. In patients with liver cirrhosis and pancreatitis in particular you are looking for a filling defect or occluded vessel representing thrombosis 👨🏽💻There aren’t many retroperitoneal structures that cross the midline but the left renal vein is one of them – characteristically this passes anterior to the aorta from the left kidney to the IVC but in some people this can have a retroaortic course where it passes behind the aorta. I have seen the occasional incidental left renal vein thrombosis so it is worth checking for filling defects! This is a must in cases of renal cell carcinoma which likes to spread into the IVC via the renal veins 🔻@theradiologistpage - @theradiologistpage on Instagram

🌴☀️The vagus nerve originates in the brainstem, is an important element of the nervous system which detects, processes and regulates the vast majority of the automatic functions of our organism !! The vagus nerve is the direct link between the brain and the intestinal microbiota, it manages digestion, assimilation, but also the constantly changing flora of our digestive tract. Any imbalance of the vagus nerve, that is to say any over-activation of one of the two branches, sympathetic and parasympathetic, can lead to diseases. The vagus nerve regulates: -beating of the heart - blinking of the eyelids - the rhythm and depth of breathing - constriction and dilation of blood vessels - detoxification of the body by the liver and kidneys - the DIGESTION - the opening and closing of the sweat glands - the production of saliva and tears - If the vagus nerve is damaged and does not send these important signals to the stomach cells: this causes digestive problems, too low a level of stomach acid (necessary to denature proteins). It controls the release of digestive enzymes from the pancreas. It regulates the motor activity of the digestive tract. Thus, chronic constipation like diarrhea are the signs of poor vagal tone and insufficient peristalsis !!! Do some research of the importance of this nerve in our digestive process !! Tomorrow is world suicide prevention day. Suicide can still be a taboo subject but nearly everyone had been affected or knows someone who has been affected by suicide. It very important that we talk about this and have as much knowledge about this as we can, through evidence based information based on good quality research and through the stories of people with experience. I’m proud to have contributed this drawing for @afspnational last year for their real convo campaign which aimed to open up conversation about mental health so that people feel supported and to reduce isolation and hopelessness. We can often be scared to ask about mental health in case it makes things worse, feels awkward or we just don’t know what to say. And we can often feel scared to talk about our own mental health as it seems shameful, we are worried it may affect the other Person or what they think of us (amongst other reasons). This drawing was part of the campaign and aimed to help give some tips and ideas to help people feel more confident in being able to open these conversations up and offer a safe space when people need it. If you are affected by these issues then there is good information about what you can do and the help available on @afsp (USA) @papyrus_uk (UK) and @samaritanscharity which is available in most countries. Permite avaliação diagnóstica do estado metabólico acidobásico e da troca gasosa pulmonar com medidas diretas de pH, PCO2, PaO2, SpO2, HCO3 e BE. Sistema Tampão: neutraliza o excesso e em seguida tenta recompor a relação normal (pH baixo: acidose ou excesso de H+ no sangue e pH alto: alcalose ou consumo de H+ no sangue). Sistema Respiratório: também responde rapidamente para o ajuste do pH por meio da retenção ou eliminação de CO2 no sangue. Hipoventilação (diminuição da FR): aumenta CO2 que se liga a molécula de água e vira o hidrogênio (aumentando os níveis de H), diminui o pH. Hiperventilação (aumento da FR): diminui os níveis de CO2 no sangue, aumentando o pH. Sistema Renal: quando o pH sanguíneo se altera, os rins eliminam urina ácida ou alcalina para regular a concentração de H+. Elevação lateral, você sabe executar de uma forma segura? Você sabe o que é Síndrome do impacto Subacromial (SIS)? Por incrível que pareça, apesar do exercício se chamar elevação lateral, ele não deve ser executado exatamente nas laterais do corpo, deve seguir um movimento em diagonal, respeitando o plano escapular (movimento feito pelas escapulas). Muito se fala sobre fazer com cotovelo estendido ou flexionado, mas o mais importante a se analisar nesse exercício, são os ossos Úmero e Acrômio, que o atrito dos mesmos podem desenvolver a SIS. Dessa forma, quando fazemos o movimento em diagonal (no plano escapular) evitamos esse atrito entre os ossos. 🔺Progression Principles🔺 ⠀⠀ Consistently applying a principle based approach to training & rehab is at the core of what makes a great coach or therapist. Understanding these fundamental principles will allow you to scale & adjust appropriately for any client that comes into your gym or clinic. ⠀⠀ 🔹Load: Begin by teaching clients unloaded variations & progressively add load as they demonstrate mastery. Don’t forget that the clients own bodyweight may be adequate for progressive overload in the early stages. ⠀⠀ 🔹Speed: Progress the client along the velocity spectrum as they develop. Begin with Isometric/Eccentric movements to develop stability prior to progressing into dynamic variations. An example of this would be developing strength in the hinge pattern with a deadlift before progressing to a kettlebell swing. Similarly, we should develop traditional lower body strength with squats/split squats to support higher velocity movements like running, hopping and bounding. ⠀ 🔹Support: Help beginners with assistance devices like a TRX or a Dowel. Progress from the ground up moving from positions that are more stable (Supine/Prone) to positions that are less stable (1/2 Kneeling/Standing). ⠀⠀⠀ 🔹Volume: De-conditioned clients will have less tolerance to volume than more advanced trainees. Start with less volume to ensure mastery/limit soreness & progressively introduce more volume as they get stronger. ⠀⠀ 🔹Frequency: Reduced frequency to a stressor allows the client to adequately recover in between bouts. Practically, this might mean dosing knee dominant exercise or running only once per week following a knee injury & gradually introducing more as the clients capacity increases. ⠀⠀ 🔹Recovery: Progressively challenge the clients work capacity by moving from complete recovery to incomplete recovery. Novices may require full rest between sets to maintain adequate form & strength/power output. I wrote this years ago and I feel like many of my patients use my Facebook and Instagram posts to do just this... When we moved to Irving and I decided to join a practice in Lewisville, I realized I wanted to know more about the schools in the area and get a feel for the culture of the teachers that would be teaching my patients and kids. I searched online and found great and encouraging information about Carrollton-Farmers Branch, Irving and Lewisville school districts. I also looked up several teachers and administrators While I expected to get good information, I never expected to learn a concept that I hope to incorporate into my medical practice, but that is exactly what happened. I followed an educator on Twitter (@LaurenBibby) who is the Technology Integration Specialist for Lewisville ISD (@LewisvilleISD) and she has since “introduced” me to a concept known as the flipped classroom (even though she doesn’t know it yet). Wikipedia describes flipped learning as “a form of blended learning in which students learn new content online by watching video lectures, usually at home, and what used to be homework (assigned problems) is now done in class with teachers offering more personalized guidance and interaction with students, instead of lecturing.” So, students watch lectures at home in the afternoon then come to school the next day to do homework and other projects during class time. This gives the teacher time and opportunity to address what the student doesn’t understand from the lectures rather than have students turn to their parents for their Calculus homework (when dad had trouble passing Algebra). So, how does this apply to me and my practice? I’d like to have more patients to do “flipped check-ups.” In the flipped classroom, student listen to lectures at home and come prepared to do work and application related to the material. In a flipped check-up, parents would read and learn about the upcoming checkup (check out this page on my blog: Check-Ups for an example), develop specific questions and then allow me to get into deeper, higher-level education when they come in for their check-up... #celltember2020 Bone cells Bones are in charge of giving support to the body, being light and solid at the same time. Despite an apparent stability, bones are quite dynamic, being capable of regenerating upon injury with a pretty impressive velocity. On the other hand, and in order to repair damage and maintain quality, aged bones are resorbed, allowing the release of minerals stored in their structure, such as calcium, which is needed for many cellular functions. When this formation/resorption equilibrium is broken, bone structure is disturbed giving rise to disorders such as osteoporosis or osteopetrosis (bones harden becoming denser). Bone formation is performed by Osteoblasts. Isolated, osteoblasts cannot make bones. They need to act in groups, as they depend on the formation of cell-cell junctions. These cellular groups are localized at the surface of forming bones, where they can be distinguished by their cuboidal shape. On the other hand, bone resorption is performed by Osteoclasts. These cells are specialized in eating and degrading bones, which requires many degradative enzymes. This function is necessary because it allows bone remodelling and calcium recovery, which is then distributed through the blood. Osteoclast precursor cells accumulate in places where they perform absorption, but to do their job, they must fuse together, generating large multinucleated cells –the osteoclasts. Fresh IBCC chapter & cast on parenteral anticoagulants in critical care 🤓 🩸Lots of pharmacology here but I think the most interesting bits are about heparin resistance (when it’s impossible or difficult to achieve anticoagulation with a heparin infusion). We are rarely taught about this, but it’s reasonably common in ICU (especially in COVID pts). 🩸Heparin pseudo-resistance: this is a lab artefact when the PTT is artificially low due to excessive levels of fibrinogen and factor VIII. This is *dangerous* if not recognized, because the heparin drip may be cranked way too high, causing hemorrhage. Diagnostic key is to check the anti-Xa level (will be therapeutic or supra-therapeutic). 🩸True heparin resistance may be due to pharmacologic changes in heparin metabolism or anti-thrombin deficiency. If impossible to achieve therapeutic anticoagulation with heparin, switching to an IV direct thrombin inhibitor may be the safest way to deal with this (e.g. IV argatroban). #celltember2020 Lymphocytes Lymphocytes are another specialized cell of the immune response. Their discovery is credited to William Hewson, in 1773, who described different morphologies among the “colourless cells” in the blood. However, he was unable to separate them clearly from other immune cells. By the end of the 19th century, Paul Ehrlich noticed that lymphocytes were not coloured with dyes, such as eosinophils (see Celltember #19). For this reason, he proposed that these cells had a different origin compared to other immune cells, which resulted to be true. Today, we know lymphocytes include three main cell types: B-lymphocytes, T-lymphocytes and NK-cells. B-lymphocytes produce antibodies against almost everything, being the actors of the soluble immunity observed almost 150 years ago. Regarding T-lymphocytes, depending on their subtype, they can kill other cells, or orchestrate the action of other leukocytes (CD8 or CD4 subtypes, respectively). Notably, T and B lymphocyte’s identification occurred only in the 1960s, by the groups of Jacques Miller and Max Cooper. NK-cells were discovered a bit later, in the early 1970s, when it was reported that some other (nor B nor T) lymphocytes possessed and intrinsic capacity to kill cancer cells, being later called “natural killers (NK)”. Overall, the multiple leukocyte functions we have discussed over the last days provide an idea of the exquisite mechanisms that protect us against almost any threat and at the same time allow tolerance to non-harmful elements. Maintenance of this balance is key to living in complex environments rich in varieties of compounds and microorganisms, being a beautiful example of what we know as evolution. #celltember2020 Dendritic Cell Dendritic cells were discovered recently, in 1973, by Ralph Steinman. These leukocytes are unique since they do not attack aggressors directly. In case of tissue infection, dendritic cells recognize pathogens and initiate a process called maturation. Along maturation, dendritic cells change their migration properties and do the opposite to most leukocytes: they escape from the injury site. Their mission is to travel to the lymph nodes, where they will encounter and activate “lymphocytes” (see next Celltember). In a way, they are the messengers or sentinels of the immune system, linking pathogen detection to the initiation of a systemic response involving different organs. Thus, Dendritic cells are considered to be the orchestrators of the immune response. Dendritic cells’ discovery filled a huge gap in the understanding of the immune system. Based on the seminal works from Ehrlich and Metchnikoff (see Celltember 19-21), it was clear that cellular and soluble factors were responsible for protecting our organism. Among the soluble factors are antibodies and cytokines, and cellular factors are phagocytes and lymphocytes. However, how the connection between these two responses was missing. Today we know that Dendritic cells eat noxious elements at the injury site and transfer them to lymph nodes. This ensures for lymphocytes to directly encounter the harmful elements that would otherwise not reach these organs. Thanks to this, only specific lymphocytes are activated in the lymph nodes, providing specificity to the so-called adaptive immune response. Due to the relevance of this discovery for the understanding of the immune response, Ralph Steinman received the Medicine Nobel Prize in 2011.
