Quando a cobertura cutânea apresenta lesão mais complexa, a fixação definitiva pode ser feita com fixador externo (linear ou circular). Entretanto, em situações especiais, podemos lançar mão do tratamento incruento preconizado por Sarmiento, chamado de tratamento funcional (imobilização gessada permitindo carga e mobilidade do joelho precoce). Atualmente, para o tratamento incruento, a fratura deve ter pouco desvio, não ter lesões cutâneas e reservada a pacientes com pouco demanda funcional. Sarmiento preconiza inicialmente gessada cruropodálica com possibilidade de carga de acordo com a tolerância do paciente, por um período inicial variando entre 3 a 6 semanas, dependendo da estabilidade da fratura. Após esse período, troca-se essa imobilização gessada por um gesso PTB (Patellar tendon bearing). Esse gesso circular, usa uma pequena quantidade de algodão ortopédico nas proeminências ósseas, moldagem no tendão patelar de tal forma que permita extensão e flexão total do joelho, tornozelo mantido plantígrado e carga estimulada desde o início. Same Same . We primarily do PGS in our clinic and its safe to say that most US IVF clinics do a large proportion of them too. Bc of this we hatch embryos on Day 3 so that by the morning of Day 5, they are alreading extruding out the zona. This makes the biopsy faster and more efficient for the lab bc when we do not hatch we get embryos like pic 1. In those cases we would need to lazer the hole in the morning and then wait for the extrusion. Or, go in and pull out the cells, which we find more harmful. I recently conducted a survey that i sent to labs around the US and other countries and the vast majority hatch prior on Day 3, not the morning of biopsy, so we are in line with the majority. The 2 embryos at 4Aa i saw this week were actually a nice treat bc we did not need to biopsy them as it was not requested and we froze them instead. I just wanted to show you what 2 very similar aged embryos look like as the same grade but one hatched, one not. This way if you have embryos in either category you can relax and note that they are still the same. Embryos will hatch by themselves when its time! So let them! We just need to hatch Day 3 to make our work more efficient, so if you do not have hatching embryos on Day 5 and even Day 6......Dont Panic! . Enjoy the day! . Splenic laceration It may not look like much, but this can be a deadly injury if not dealt with quickly and definitively. This injury is also one of the most frequent reasons for requiring emergency surgery after blunt force trauma. The spleen is part of your immune system that besides being a filter for old cells circulating in your blood, it also is the main source of the body’s Complement system, which enhances your body’s ability to fight off a specific type of bacteria. Therefore, while you can live without your spleen, one of the trade offs is that you will require ongoing vaccinations against the most common of these organisms. The ABDOMEN X-RAY: I alway found this exam to be tougher than the chest X-ray. So what do I look for? 1. Bowel - small bowel is central and large bowel is around the periphery. Look for dilated loops of bowel filled with gas and or air/fluid levels to suggest obstruction. 2. Check for free air under the diaphragm. That’s a sign of bowel perforation. Your next move should be to pick up the phone and call the referring physician. Do not miss this! 3. Look at the organs, particularly the liver and spleen and determine if normal in size or enlarged. 4. Scan the kidneys and ureters and check them for calcifications indicating stones. 5. Look for calcifications in the right upper quadrant which indicates gallstones. 6. Check the bones of the spine and pelvis. Look for fractures or destructive lesions. Honestly, there is a lot more information that can be gathered from an X-ray of the abdomen, but those are some of the basics and I hope this helps you guys refine your search pattern. #anatomyoverlays - @theradiologyguy on Instagram

Unless you are a nurse or you’ve sat at a bedside watching a devoted nurse work for 12 and a half continuous hours to care for your loved one, it’s hard to grasp what this noble profession entails and how it breathes life into others. Contrary to medicinal and societal advancements, we’ve got a long way to go when it comes to understanding the unique and pivotal role nurses play in patient care. I come from a culture like many others where the taboo stands that nursing is a woman’s profession. There are others that reduce the profession to someone who only renders hygienic duties. It’s important to educate and show the world the behind the scenes. I don’t mean sharing filtered pictures. I mean sharing narratives. Such as the nurses that perform 3-4 hour dressing changes on complex burn patients, the nurses titrating medications drop by drop to keep a dying person’s heart barely alive, the midwife who stands all night to assure the birth of a new born, and yes, the nurse that cleans your loved one multiple times a day and night to restore their dignity while you return home in confidence that they will be okay and taken care off. I’m talking about the unsung heroes. So yes, we clean our patient’s bowel movements, clean up their vomit, bathe them when they are incapacitated, but we also love them like our own families, we also wipe away tears, we also pray with them, we also console fearful families, we also speak up and give them a voice and empower them. 01/07/2017 Há 2 anos atrás, eu estava indo pra um hospital bem longe, onde eu não tinha ideia do que aconteceria quando chegasse lá, a única coisa que sabia era que ia ser difícil, eu teria que ser o mais forte possível e que era realmente uma cirurgia complicada com alguns riscos. A cirurgia acabou, os dias de internação foram passando, a dor era horrível, a sensação de que eu não iria mais ser a mesma era grande, e a vontade de desistir era constante, mas Deus esteve cada segundo ao meu lado, me deu forças, e finalmente tudo passou. Eu realmente mudei, tenho cuidados que não tinha, dou bem mais valor à vida, e sei que se não fosse essa cirurgia, hoje provavelmente estaria em uma cama, ou pior, e com problemas bem maiores. Sou grata a cada dia por isso, e por mais que ainda exista dificuldades, não teria uma alternativa melhor! Sou grata também a toda a equipe médica que esteve comigo me dando todos os cuidados possíveis, a minha família e amigos que me deram forças, e principalmente a Deus, pois sem Ele nada seria possível, e eu nada seria! Nós achamos que não vamos superar e que nada vai passar, mas isso é só mais uma etapa desse processo chamado vida. On this day a year ago, I was sitting in the ER with the suspicion something might be wrong. I had lost all feeling in my right leg, could barely lift my left leg off the ground and my right hand wouldn’t follow directions I was telling it. Confusion marred my every thought, my speech was slurred and words seem to escape like blank memories. I felt like a stranger in my own body; I seemed to have lost myself in the matter of a week. Fear filled every corner of my mind that still seemed operational. I had to figure out what was going on, because nothing made sense and the pain was taking over. After ruling out several possibilities, my doctor ordered an MRI and with my family by my side he confirmed my dark suspicions. He shared with us that they discovered several golf ball sized lesions in my brain and over 15 on my spinal cord. On March 13, 2018, I was diagnosed with Multiple Sclerosis. It felt as though the room had collapsed around me and I might fall of the bed. The roar of intense fear rung in my ears the pounding of my heart beat drowned out what the doctor was saying to my dad. In what couldn’t have been more than seconds I scaled all stages of grief, imagining every terrible scenario that could lie ahead: dread turned into relief, relief morphed into anger, and that anger settled into sadness. I was someplace I never thought I would be. The days that followed were dark. After giving blood for the thirteenth time in a week, I remember throwing up in the doctor’s office alone and exhausted. The needles were starting to give me anxiety and with an already collapsed a vein the thought of one more stick made me ill. My thoughts were not clear, likely due to the intense medication I was taking to return my ability to walk, and I was lost. In those moments, I could not imagine living without the use of my hands or continuing to stumble and drag my feet. I felt it was better off to not be a burden on everyone around me – I thought death would be a better option. Terceiros molares inclusos. Vendo as postagens parece que bucos só fazem cirurgias hospitalares e de grande porte. Mas não. Os dentes do siso são rotina no consultório. Recebo muito casos assim de pacientes que ja sofreram com perda de sensibilidade (parestesia) na cirurgia do outro lado. Importante o planejamento, material adequado (2a foto) medicamentos necessários, acompanhamento após a cirurgia e experiência. Cirurgia feita em ambulatório, com anestesia local somente. Tempo? Relativo. Cirurgião opera o paciente e não relógio. Tem que ficar bom e nesse caso foi removido em tempo satisfatório e sem parestesia. Jadi nostalgia, salah satu kerjaan yg aku seneng BANGET adalah tindakan bedah-bedahan, menantang euy, dan kebawa senengnya sampai udah praktek mandiri. Dulu, dengan senang hati kalau ada temen yg minta bantuin tindakan bedah, kadang gemes pengen ikutan nge-handle, sampe praktek juga masih sering tiba2 di telpon sama temen minta tolong dibantuin, yah minimal bantu support atau jadi tim hore 🤣. Bukan berarti aku jago, tapi suka dengan pekerjaannya. Nah... Tertarik ambil spesialis bedah mulut? Belum tentu, hampir semua bidang kedokteran gigi aku suka. Jadi masa koass dulu tersiksa ga? Engga, malah seneng, bisa belajar banyak, bisa tanya-tanya, bisa tau “berjuang” sampe di luar batas nalar, yg aku pikir aku ga akan sanggup, eh ternyata sanggup. Jadi koass memang menyenangkan, tapi cukup jadi kenangan, tanpa perlu di ulang 🤪. . So... Lungs and RA can be mild or complicated... I guess I sit in the complicated department. I have learned to just go with the flow! As most of you see, I appear to be THRIVING (especially in my @thrive_construction shirts) on most days!😉) Like a lot of us, there are so many aspects to this disease that people outside of our homes would never know. I don’t show you guys every minute of my day, but I do talk a lot about my daily routine and what can be expected! People who develop RA-related scarring and interstitial lung disease often have a poor prognosis. That sounds terrible right “poor prognosis!”🙄 Thanks for that... If that didn’t sound awesome to you, then this next line might shock you! According to research that looked at a number of studies, the median survival rate for people with interstitial lung disease due to RA was 3.2 years to 8.1 years from the time of diagnosis.🤔This use to freak me out, but honestly I stopped worrying about the years, what doctors say, and just concentrated on ME and what I want from this one chance we get! I try my imperfect best every day! For those of you who might think I am crazy for my push up the peak or for attempting to compete wearing high heels, this might give you a bit more insight to my why! My lungs have presented more problems in the last couple of years, but I am committed to being a warrior, so let’s just end this post with that! *Warning- some supporters may find this post distressing* Sometimes we have good days and sometimes we have bad days in our line of work, but one thing we can say for sure is that our amazing team will always give 100% to ensure our animals have the best possible outcomes. Recently our vet team in the SCAR Community Animal Hospital had to perform an emergency caesarian surgery on a mum cat after she came in to our care having major labour difficulties. Within minutes of starting the surgery the kittens were out and our waiting team immediately started working on them, rubbing them down rigorously to help stimulate their breathing and trying to remove fluid from their mouths and nasal passages. We worked hard on her kittens for over an hour but unfortunately, despite our best efforts, we were unable to save them. We really tried our hardest. Mum is doing well and is now resting in a loving foster home where she will stay while she recovers from her ordeal. We want this to be a reminder to all cat owners about the importance of desexing your pets. Female cats can give birth up to five times a year, with litters averaging 4-8 kittens at a time.. that’s 20-40 kittens born EVERY YEAR from just 1 cat!! Desexing your cats (male and female) is the kindest and most responsible thing you can do as a pet owner, not only to help reduce the number of cats dying in pounds every year but also for the health of your pet. Well-healed fractures to right wrist hamate and capitate bones. Noted history of roller skating injury at age 8. But in her teenage years, did suffer more lasting trauma. Right knee, medial meniscectomy, A sad end to a figure skater’s identity at age 16. Three times fractured right radius, once at age 8, as previously noted. Unknown history of adult right radius arm injuries, well-healed twisting spiral fractures noted. Suture anchors numbering 2 in left proximal humeral head, indicative of surgical repair of rotator cuff tendons. Bony prominence to right calcaneus at Achilles’ tendon insertion, attributable to history of surgical repair of Right Achilles’ tendon rupture. Bony prominence to left calcaneus, notably smaller than the right. Noted long term history of tendinitis. This skull has seen a trauma or three. We know a lot of how to hurt the brain, but not of how to heal it. These bones do stories tell of life lived, yet little of the softer organs. They do not speak of muscles torn, of swollen tonsils, chicken pox, of fallopian tubes tied. She was admitted to a hospital in Delhi and her condition was critical. Unfortunately, she lost this war between life and death. The incident is from Hathras, UP. Victims family alleges that police didnt help initially and only helped after the outrage. The incident happened on 14th September. As per a report on DNA (27th September), all four named accused in the case have been arrested. On social media, not many people are talking about this case, no prominent calls for justice were made. No prominent calls for the arrest of these criminals were heard. Transformers: Revenge of the Fallen was directed by Michael Bay with cinematography by Ben Seresin, ASC. Following through on its promise to be a sequel, the story is told through explosions, gunfire, and loud vrooming noises set to voice-over in a nod to its core audience. Today we’re taking a look at lighting the first fight scene of the film, which takes place at night and sets up the premise of the plot: Fallen Transformers are out for Revenge. To acquire this crucial bit of dialogue, lots of army men and sentient cars drive around dodging explosions and shooting at one another. All of this took place on location at the Bethlehem Steel Campus in Bethlehem, PA. The factory had shut down years before in 1995, so there was plenty of room to drive fast cars and blow stuff up. The entire campus is 10 acres, but the set was contained to 16,000 square feet. Lighting this scene involved four nights of pre-lighting, three Bebee trucks, and 11 generators. One of the biggest challenges to this location was lighting the five, quarter-mile high, furnace stacks so they stood out in the night and rigging lights at their tops. Because there was so much movement and so many explosions, cameras were everywhere, which meant that all lighting on the ground needed to look purposeful. The stacks were lit from their bases with 12 Light Maxi Brutes and mini-nine lights to up-light them, while 12K LRX heads were rigged from the top to act as spotlights. Additionally, a Blackhawk helicopter was rigged with a xenon spotlight manned by @iatselocal728 members Erica Kim @itsericakim and Michael Schwartz to operate the focus as the explosions, guns, and cars whirled about below. I loved the design of the TX-225 GAVw the moment I first saw it on screen. This is my modified version of @lucasinc14 s model as sold on @brick.vault which is inspired by @luca_s_projects and Maelven on Flickr. I redesigned the front and top armor and lowered the structure of the underbelly to create space for a somewhat detailed interior. The tank comfortably holds three Tank Troopers with hatches either open or closed. Additionally, I smoothed out as much of the surface as possible and added some details like long range comms, rusty spots and spare tracks. Here is a new story Ive written about a railroad crossing at Water Street in Fairport. It has been out of commission for over sixty years, for good reason. Treacherous Railroad Crossing Closed in 1959 By Bill Poray, Perinton Town Historian Accidents at Fairport railroad crossings were far more common in the days prior to automatic signals and gates, activated when a train approaches the crossing. Over sixty years ago, a dangerous railroad crossing was eliminated in Fairport. Since the 1880s, the crossing allowed pedestrians, horse-drawn wagons and carriages, and later, cars and trucks, to pass over the railroad tracks at Water Street. The crossing, just north of Donnelley’s Public House, the old Green Tavern, was removed in August of 1959, and for a good reason. It was a menace to safety. At the time, there was much debate regarding the benefits of replacing railroad employees functioning as crossing guards with automatic gates. A 1957 traffic study recorded 86 trains - 47 passenger and 39 freight trains, passed through the village of Fairport in a 24-hour period. According to testimony at a hearing before the Public Service Commission in Albany in 1957, the maximum speed of trains passing through Fairport was recorded at “50 miles per hour for freight, and 80 miles per hour for main line passenger trains.” In the case of the Water Street crossing, 750 feet east of North Main Street, pedestrians and vehicle traffic were on their own. There was no crossing guard or gates of any kind. On a snowy day in February of 1941, a fast-moving eastbound train originating from Chicago approached the village of Fairport. Just seconds after crossing Main Street, it slammed into an automobile operated by Adrian Zonneville, traveling south on Water Street, killing the driver. A page-one report of the accident in the Fairport Herald-Mail stated, “This crossing is guarded in no way, either by a watchman, flasher, or gates.” The crossing at Water Street included five sets of tracks, and a siding, meaning vehicles and pedestrians were required to travel a distance of 80 feet or more to cross all the tracks. Dari momen itu, maka 28 September ditetapkan sebagai Hari Kereta Api Nasional di Indonesia. Hal ini sekaligus menandai berdirinya Djawatan Kereta Api Indonesia Republik Indonesia (DKARI). . Sejarah kereta api Indonesia berawal ketika pencangkulan pertama jalur kereta api Semarang-Vorstenlanden (Solo-Yogyakarta) di Desa Kemijen oleh Gubernur Jenderal Hindia Belanda Mr LAJ Baron Sloet van de Beele pada 17 Juli 1864.Kemudian pada 1876, jalur pertama kereta api Indonesia di Desa Kamijen, Semarang, yang saat ini menjadi Stasiun Gudang Semarang dioperasikan. Pada masa itu, kereta api yang digunakan masih menggunakan mesin uap pada lokomotif kereta, di mana petugasnya bekerja dengan membakar kayu dan menjaga api tetap menyala. Ini nih pergantian nama-nama Perkeretaapian Indonesia sampai Menjadi KAI Pergantian nama mengacu pada perjanjian damai Konfrensi Meja Bundar (KMB) Desember 1949, bahwa harus dilakukan pengambilalihan aset-aset milik pemerintah Hindia Belanda. Pengalihan dalam bentuk penggabungan antara DKARI dan SS/VS menjadi Djawatan Kereta Api (DKA) tahun 1950. Lalu pada tanggal 25 Mei DKA berganti menjadi Perusahaan Negara Kereta Api (PNKA). Pada tahun tersebut mulai diperkenalkan juga lambang Wahana Daya Pertiwi yang mencerminkan transformasi Perkeretaapian Indonesia sebagai sarana transportasi andalan guna mewujudkan kesejahteraan bangsa tanah air. Selanjutnya pemerintah mengubah struktur PNKA menjadi Perusahaan Jawatan Kereta Api (PJKA) tahun 1971. Dalam rangka meningkatkan pelayanan jasa angkutan, PJKA berubah bentuk menjadi Perusahaan Umum Kereta Api (Perumka) tahun 1991. Tahap terakhir, perumka berubah menjadi Perseroan Terbatas, PT. Kereta Api (Persero) tahun 1998. Pada tahun 2011 nama perusahaan PT. Kereta Api (Persero) berubah menjadi PT. It’s Throwback Thursday! In 1912, the Richmond Locomotive Works built the first group of Z1 engines. Once delivered to the Norfolk & Western, the 427,000-pound engines worked on the Pocahontas, Radford, and Norfolk divisions. The 2-6-6-2 mallet locomotive was compact and powerful, ideally suited to hauling heavy tonnage trains. Efficient and economical, these engines became the standard for heavy freight power, adding efficiency and economy to the railway’s operations. Today’s image was captured by a railroad photographer in 1917. He took several photographs of Engine 1302 and included an unidentified man in this shot—peeking around the engine—for scale. Then and now, #innovation is an important part of the Norfolk Southern story. Railroads and railroaders deliver the goods and work hard every day to keep the steel wheels rolling.
