Bueno seguimos con vida que es lo importante. Thank you to everyone for the kind messages, I was able to get the Neurostimulator yesterday after 2 tries (cause nothing is ever easy with me 🥴). The goal is for the two leads connected in my spinal cord to intercept the pain signals and minimize the amount and intensity of my lower back & leg pain. I’ll have to wear this for a full week and then my team and I will decide if it would be best to install a permanent device. Injury Update! So I’ve been dealing with some serious neck pain for a long while now, years. It became acute and debilitating during flare ups. To the point where I had chronic numbness in my arms and hands. I saw many chiropractors (5) and my regular physician- got xrays through my insurance but they results came with no visible cause for my symptoms. Feeling frustrated- I heard about a cervical spine specialist in Auburn CA- about 45 mins away. I’m so glad I found this person! My first appointment lasted almost 2 hours, got xrays that my doctor didn’t order of my Atlas and axis. Fully educating me on my issue, neurology and how to solve it. The top two vertebrae are where he specializes. My Atlas was twisted nearly 60 degrees. After my first treatment with him 70 percent of my symptoms went away within days. I’m still under doctors orders of no biking, no running, no overhead lifts, no fast lifts and nothing that would jostle my spine. Which is remarkably hard for me. Hopefully in 4-6 weeks I can start doing the things I love and miss again. So grateful that I can live my life and feel excited to do my job again. I’ve been laying really low in recovery. I’m doing better and better every day. X: X is for X-Ray: A lot of people believe you cant breathe in a corset, that they are inherently bad for the body, or that you need to have ribs removed to achieve a small waist (see our last post). Its true that poorly fitted corsets can be quite uncomfortable, but modern corsets that have been made by someone experienced and knowledgeable are incredibly supportive and even comforting. As we discussed in our orthopedic corsetry post, well-made corsets provide great abdominal and back support. Corsets can assist in the correction of scoliosis and posture complications, and are in our opinion more supportive and comfortable than your average medical back brace. Those with full busts, often find corsets more comfortable and supportive than bras because the weight of the breasts is supported by the skeleton and distributed around the waist & hips, rather than drawing painfully on the spine, muscles of neck, shoulders and upper back. Dark Garden Lead Designer and Proprietress Autumn Adamme wore her first corset around age 10; she is pictured here in x-ray form: corseted and without. These images show healthy ribs with plenty of room for lungs and comfortable breathing, while also revealing evidence of her natural pelvis and rib asymmetry. Her vertebrae look normal, if slightly turned (per her MD) and in the corseted image it is clear to see that rather than being compressed, her spine is lengthened. Well-healed fractures to right wrist hamate and capitate bones. Noted history of roller skating injury at age 8. But in her teenage years, did suffer more lasting trauma. Right knee, medial meniscectomy, A sad end to a figure skater’s identity at age 16. Three times fractured right radius, once at age 8, as previously noted. Unknown history of adult right radius arm injuries, well-healed twisting spiral fractures noted. Suture anchors numbering 2 in left proximal humeral head, indicative of surgical repair of rotator cuff tendons. Bony prominence to right calcaneus at Achilles’ tendon insertion, attributable to history of surgical repair of Right Achilles’ tendon rupture. Bony prominence to left calcaneus, notably smaller than the right. Noted long term history of tendinitis. This skull has seen a trauma or three. We know a lot of how to hurt the brain, but not of how to heal it. These bones do stories tell of life lived, yet little of the softer organs. They do not speak of muscles torn, of swollen tonsils, chicken pox, of fallopian tubes tied. Prophylactic Antibiotics for Flare-ups High level evidence indicate no need for an antibiotic unless there is a clear presence of swelling. But what the studies do not generally review is the pre-operative condition of the tooth which is to say; was it a vital, non-vital, symptomatic or asymptomatic. Literature also fails to objectively define “flare-ups”. Let’s assume as Endodontists, we can differentiate between post-op pain and “flare-ups” (Swelling, Fever or acute exacerbation of pain levels in comparison to pre-op). In the past during my residency and academic career, I followed the textbook rules of antibiotic prescription. Until this asymptomatic case (2 years ago) ended up with post tx Buccal space infection 🤢🥴🤒. Most of my previous flare-ups have happened in similar cases (didn’t matter 1 or 2-visits) IMHO in long standing asymptomatic non-vital or retreatment cases, the host-immune/bacterial harmony is abruptly disrupted, and hence after this case I started recommending shortened post-op antibiotic course (3 days only) for asymptomatic PARL cases. Never had a true flare-up since... (I desperately want someone to disprove my anecdotal science) What’s is your post-Endo Antibiotic Prescription philosophy in general ? Especially for long standing necrotic/previously treated cases with Asymptomatic lesions. How much pressure does post-op patient experience, feedback to RD and practice reputation play a role in your decision ? 01/07/2017 Há 2 anos atrás, eu estava indo pra um hospital bem longe, onde eu não tinha ideia do que aconteceria quando chegasse lá, a única coisa que sabia era que ia ser difícil, eu teria que ser o mais forte possível e que era realmente uma cirurgia complicada com alguns riscos. A cirurgia acabou, os dias de internação foram passando, a dor era horrível, a sensação de que eu não iria mais ser a mesma era grande, e a vontade de desistir era constante, mas Deus esteve cada segundo ao meu lado, me deu forças, e finalmente tudo passou. Eu realmente mudei, tenho cuidados que não tinha, dou bem mais valor à vida, e sei que se não fosse essa cirurgia, hoje provavelmente estaria em uma cama, ou pior, e com problemas bem maiores. Sou grata a cada dia por isso, e por mais que ainda exista dificuldades, não teria uma alternativa melhor! Sou grata também a toda a equipe médica que esteve comigo me dando todos os cuidados possíveis, a minha família e amigos que me deram forças, e principalmente a Deus, pois sem Ele nada seria possível, e eu nada seria! Basically, we have nerves and blood vessels that travel into the arms, and along the way there are a few roadblocks that can give us some issues overall. Compression on these structures will cause altered sensation and sometimes muscle weakness as well. . The nerves and vessels then branch into the arm and divide into the median, ulnar, and radial nerves which supply different areas of skin sensation and innervate different muscle groups. . Quite often we receive phone calls from potential new patients who are confused because they have been referred to the DHC for headache/migraine from their current physician, but are not experiencing a “normal headache”. These patients describe their condition as “head pain” rather than a “headache”. Don’t fear, you’re in the right place! • Back of the head pain is a symptom of migraine, but it could also be a symptom common to other headache diagnoses. When these symptoms are not paired with the other frequent indications of migraine like nausea/vomiting or sensitivity to sound, light and odors it could alternatively be Occipital Neuralgia. • Occipital Neuralgia is a distinct type of headache characterized by piercing, throbbing, or electric-shock-like pain in the upper neck, back of the head, and behind the ears, usually on one side of the head. Typically, the pain of occipital neuralgia begins in the neck and then spreads upwards. • This diagnosis can be treated with other common migraine treatments such as a change in lifestyle: a healthier diet and exercise regimen. Neck pain can be relieved by heat, massage and physical therapy. If this treatment plan is unsuccessful, occipital nerve blocks or radiofrequency ablations may be an alternative action if physician deems appropriate. A 58-year-old male person was admitted to the Emergency Clinical Hospital Bucharest, approximately 7 hours after a traumatic injury of the left leg amputated by the blade of an electric saw. Replantation surgery of the left leg was performed under general anesthesia with ischemic time lasting 9 hours up to the arterial anastomosis. Metallic osteosynthesis and suture of the blood vessels, nerves and tendons were performed. On the initial physical examination this person presented no inflammatory signs of the skin cicatrisation process after replantation surgery, no edema and no pain syndrome. Had this been irregular without discrete p waves you would have picked atrial fibrillation. It is also important to know that the ST segments can be distorted by flutter waves mimicking ST depression, elevation or prolonged QT. Always march out the flutter waves before making these diagnoses. 🧒🏻 Intermediate readers should know the mechanism of common arrhythmias. Atrial flutters are macroreentrant circuits where there is a slow zone of conduction and unidirectional block that allows the genesis and propagation of a large wavefront of electricity. The classic way to figure this out is continuous activity of the chamber involved, in this case the atrial where you see constant undulation of the baseline. 👨🏻🦳 Advanced readers should know that typical flutters have discordance between V1 and inferior lead p wave axis. In this case both are positive. Plus the rate is slower that typical flutter (220 vs 300). Atypical flutters are usually related to prior scar in the atrial (surgery or ablation). Here the circuit is posterior to anterior in V1 and top to bottom in the inferior leads. It can be challenging to know what the exact circuit is, but you should be thinking of a left atrial circuit with this morphology. We terminated this flutter in the mitral isthmus. Big thanks to all of the concerned citizens and fellow dog walkers who witnessed Mav’s traumatic experience, and helped care for him and get us to the pet hospital together. TUBERCULOSIS 👨🏽💻This was a tricky case - there was a clear abnormality at the right hilum with consolidation in the right mid zone but the more subtle findings were: ▫️Consolidation extending behind the right heart shadow - remember to look behind the heart, the density should be roughly equal on both sides ▫️Well defined apical opacity - compare both apices and look between the ribs, here there was an opacity on the right ▫️Left lower rib erosion - follow all of the ribs! Sometimes a subtle rib erosion can be the only abnormality on the film 👨🏽💻These findings could represent either a primary lung cancer or TB - given the age of the patient TB is the most likely diagnosis here. Ok @mangos_endo_house before I wish you and everyone a great weekend I wanted to share this case because it reminded me of the issue we discussed earlier today. Periodontist refereed this woman to me for #rootcanaltreatment on her 1st molar. Not sure you could pop more lateral canals than I did. Mesial for sure. Palatal everywhere. Apical for sure. And mid root. Could be strip perf’s but I doubt it. Didn’t open it much and used typical.04 taper. Also, as you will see, palatal healed great. Everything healed great except for MB. Is there and MB2. Amazingly no. The canal is centered the entire length of the scan. But the last PA is a 6 year check which shows the big palatal RL healed but the mesial did not fully heal. So what to do? So far nothing. It’s been 9!years now and she is fine. Some day I will have to cut the MB to perfectly fix. But it’s been 9!years already and so far, we cool. Not sure if I will ever cut this? Or maybe next year? Not everything is so straightforward. Patient will hopefully come next March for the 10year check. Devido a estas propriedades anabólicas, estes hormônios vêm sendo utilizados na prática recreativa com fins competitivos e estéticos. 🧠Entretanto, este uso está concomitantemente associado a efeitos deletérios, principalmente a nível de sistema nervoso central (SNC). Este fato se torna ainda mais preocupante em conjunto com evidências que mostram que estes hormônios podem causar dependência. 🧠Tem sido sugerido que os mecanismos com que os EAA podem causar prejuízos ao SNC podem envolver a alteração na transcrição gênica através dos ARs do SNC, interação com proteínas e canais iônicos independentemente da transcrição gênica e interação com sítios modulatório de neuroesteroides no SNC. Nesse sentido, é evidente que estas substâncias causam prejuízos ao SNC, principalmente levando-se em conta a estes mecanismos já elucidados na literatura os quais em conjunto, ou somente um, podem ser responsáveis por seus efeitos no sistema de recompensa, e consequentemente, possível dependência. 🧠No sistema de recompensa, os efeitos dos EAAs são mais obscuros e parece exercer suas funções reforçadoras independente da dopamina. Today is exactly one year since my surgery! It has been the hardest year of my life but also the best year of my life. I am in pain everyday and mentally that can wear on a person but I stay positive and think about all the room my heart has to beat and all the air my lungs get now. ⚠️70-year-old female pedestrian was struck by a motor vehicle at high-speed, sustained multiple injuries including injuries to her lungs and head. She also sustained a mangled injury to her right lower extremity with what’s considered a full degloving of the extremity and multiple fractures and neurovascular injuries. Patient was taken to the operating room immediately and managed by multiple trauma surgeons for her injuries. A high above knee amputation was done. Above-the-knee amputations may be necessary for many reasons. These can include trauma to the lower leg, which results in a non-viable leg at or near the level of the knee. Below-the-knee amputation may adequately address a more distal injury. Many studies have attempted to create algorithms to help physicians decide when to reconstruct versus amputate. One of these is the Mangled Extremity Severity Score (MESS), which takes into account skeletal/soft tissue injury, limb ischemia, shock, and patient age. Day 14 in the Hospital after my injury at #SteamersLane in Santa Cruz. I’m in the immobilization & recovery period. I’m incredibly lucky to have had such a skilled surgical team and thankful for my Dr’s, Nurse’s, CNA’s and social workers here at #skagitvalleyhosptial. So far 3 surgeries and they were a success. ⠀ ⠀ This injury has been a really big challenge being paraplegic with with 1 limb to use. I have a new appreciation for being a para and having 1 arm. ⠀ ⠀ 14 days down with probably another week to go before I can go home. We’ve had some pretty major problems though with my health insurance. @kpthrive won’t pay for a certain lift I need called a “take along lift with a “seated sling” for my home. It’s the only lift PT & OT could find that will work with my injury based on the pectoral tear and the surgical area. Kaiser has declined it now 3x and was supposed to have an independent review and that party was still a part-time Kaiser employee which seems really wrong. I’m really bummed this has already happened and can’t surf for quite a while. To also be having to fight for my insurance to do the right thing is really tough. I’ve had good and bad past experiences with Kaiser but generally they always do the right thing. This time, it makes no sense... because of covid-19 and the extra challenges that adds. I can’t have friends come and help lift me safely at home, nor can I afford the lift on my own. They are not giving me any options but possibly having to stay in a skilled nursing facility which I’m scared to death of having to go into with Covid-19. Does anyone or anyone who follows @wagsofsci have any ideas or recommendations? That’s unfortunately the only seated lift that will work, as No Hoyer lift and or other sling combos will not cause damage to my repaired injury. I don’t understand how they will pay a crazy amount of money for 6 weeks at a skilled facility that really isn’t needed or necessary but won’t cover a $2600 manual lift. Read on to find out more about my review areas on a hand X-Ray... 👨🏽💻Hand x-rays are performed in cases of patients with suspected arthritis to try and evaluate the cause but more commonly are performed in cases of trauma 👨🏽💻When it comes to trauma there are a few areas I look at in particular every time - these review areas have helped me become more confident when looking at these X-Rays and hopefully they can do the same for you: ▫️Always start any trauma related X-Ray by looking at the soft tissues - the site of swelling can lead you to the fracture ▫️Check the metacarpal heads - an indented 4th or 5th metacarpal is common after a punching injury - termed a Boxer’s fracture. Don’t stop there in these patients - draw a line across the base of each metacarpal and this should join up. Sometimes the 4th/5th metacarpals can dislocate and travel downwards so they overlap the carpal bones - here your line won’t join up. Overlapping bones on a hand X-Ray should always make you think! ▫️Outline the base of the thumb metacarpal - forced abduction can cause an intra-articular fracture known as Bennett’s fracture (or Rolando fracture if comminuted) ▫️Do all of the fingers look well aligned? Check for subluxation and dislocation - you may need further views to be sure ▫️When it comes to carpal bones, scaphoid injuries are most common but you will need a whole scaphoid series. A normal X-Ray can’t exclude a scaphoid fracture. Practice varies with what to do next, some bring back people with snuffbox tenderness in 7-10 days to do an X-Ray, some to do a repeat clinical assessment whilst in some institutions they come back for an MRI ▫️The spaces between the carpal bones should be equal, if you see a widened distance between the scaphoid and lunate consider scapholunate dissociation (Terry Thomas sign) 🔻As always take your time, zoom up and outline the cortex of each bone, use a system and check two views! @theradiologistpage - @theradiologistpage on Instagram

Thought I’d illustrate another cool #regenerative case. Patient had caries in the tooth before it even erupted. Not sure if @rickschwartz78 or @endogenie or anyone knows the term for that? I’ve always been curious. So tooth tested vital. But you can see I pulped out a little when I excavated. As expected. I placed MTA and later composite. Final PA is from her GP 3 years later. Tooth is still vital and fine. Been 8 years now. Will hopefully get a new Pa soon. But I wanted to show this because the pics are good and I placed way less MTA than the case I depicted before this one. The main takeaway is that a big apex leads to great blood supply and likely success. Dr Hargreaves says he and the literature recommends bleach and #chlorhexidine to rinse but I’ve always just used bleach. *Pre-COVID-19 case* This is a patient who has had chronic pain in the midfoot, despite being treated by another physician with a CAM boot and RICE therapy. The patient presented to my office for a second opinion. Midfoot arthritis can be subtle on plain radiographs. You can see degenerative changes at the second tarsometatarsal joint in the lateral view mostly. The patient responded well to custom orthotics and steroid injections for only a few months. After failed long term conservative treatment including RICE, modification of shoe gear, custom orthotics and steroid injections (dorsal osteophyte was palpable), a MRI was ordered (images shown of the 2nd and 3rd tarsometatarsal joints). I did have to fight on behalf of the patient to have that MRI covered by insurance by speaking to the insurance company’s physician (with whom I had an interesting conversation I’ve added to my reservoir of “I can’t make this💩up”). Intraoperatively, there was significant degeneration of the joint and surrounding bone at the base of the 2nd metatarsal involving the LisFranc ligament complex. Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued. Have a look at the X-Ray and swipe left for the explanation NEUROGENIC TUMOUR 👨🏽💻The key to this X-Ray is to remember to use a system! It would be easy to say there is no fracture or dislocation and move on but by reviewing systematically we find an incidental lesion that has implications for the patient 👨🏽💻It’s important to assess the lung in all shoulder and clavicle X-Rays - in cases of trauma it is especially important to look for pneumothorax looking for lucency, lack of lung markings and a ‘pleural line’ - a thin white line paralleling the pleural edge 👨🏽💻In this case we see an opacity at the apex - in older patients the main differential would be a primary lung cancer however a) this lesion is well defined and b) this is fairly uncommon in this age group so we need to think of other causes 👨🏽💻A pleural fibroma would be possible, however this turned out to be a neurogenic tumour (schwannoma). Had the lesion entered the neural foramen then a neurogenic tumour would have been easier to diagnose however we can see the neural foramen is clear on MRI. Lack of foramen involvement doesn’t exclude a neurogenic tumour as this can form anywhere along the intercostal nerve as it has here 👨🏽💻Schwannomas are the most common nerve sheath tumour and show high T2 signal on MRI, with heterogeneity due to cystic degeneration/haemorrhage in contrast to fibromas which are typically low in T2 signal however can be heterogenous due to necrosis. Both enhance post contrast 👨🏽💻Schwannomas are typically benign but can slowly grow over time Hopefully this case will remind you to assess shoulder and clavicle X-Rays systematically! 🔻@theradiologistpage - @theradiologistpage on Instagram

Today’s case we have this 20 year old male who was involved in a motor cycle accident! The poor guy was just riding along when he was tboned by a car that was not paying attention. The patient was NOT wearing any safety riding equipment (including helmet) at the time of the accident. As a result, his foot was crushed between the car and the cycle. To add salt to the wound (no pun intended), the car that struck him fled the scene! . Upon arrival to the ED, trauma protocol was followed as this mangled foot could just be a distracting injury. Luckily, no other injuries were found, including on the helmet-less head and neck! They found his foot to have a double lisfranc fracture (displacement of the metatarsophalangeal joints due to fracture or injury), dislocated navicular bone and dislocated and fracture talus. In this case, they were able to save his foot! Due to the extensive damage to both bone and muscle, however, it will take many monthly to years to be able to walk normally again! . I propose a question to everyone...would you rather having this outcome whereas your foot is saved, but, even after possibly years of rehab, you may not be able to reestablish full motor control and sensory back to the foot OR have the foot amputated and just learn how to walk with a prosthetic? Maxillofavial Trauma Management - Airway ———————————————————————— After we handle the immediate life threats (massive hemorrhage), we have to consider the airway. Look, listen, and feel: take a look at the injuries and look inside the mouth for blood, vomit, broken teeth, dentures, chewing tobacco, etc. If any of these are present, grab your suction and clear it out. If dismounted or without mechanical suction, grab a 60cc syringe with a cathedral tip and a put an NPA on it to create a field expedient manual suction device. Give the lungs a listen and see if you’re getting adequate air movement. If the environment is keeping you from listening, place you’re hands on the chest and feel for bilaterally equal rise and fall of the chest. ———————————————————————— I would personally prefer to skip the intubation attempt and go for a surgical cricothyroidotomy. Orotracheal and nasotracheal intubation may be attempted but be aware that musculoskeletal tissue disruption will likely increase the difficulty of intubation. LMAs and I-Gels can be used to buy you some time to prep for a cric, but they don’t prevent regurgitation and aspiration of vomitus and may increase vomiting unless a paralytic has been administered (which is unlikely due to the fact that we likely haven’t gained vascular access at this point in our assessment). Squats are essential for so many various reasons and are often referred to as the “king of lower body exercises” However, too many times I’ve heard trainers/coaches tell their clients to position their feet and legs is certain ways to squat, because “this is the way to squat”, Only to watch them struggle to maintain good mechanics and form. Knowing and understanding all the factors that contribute to how one moves is essential for their success in how well they WILL move and and how they progress. These are just some things to consider when teaching someone to do a good squat. Sometimes we have to think about factors we can’t control. Anatomy and physiology needs to at the top of your list. 🇺🇸 FACE EXPLOSION DUE TO FIREWORK! A 29-year-old man was hit directly in the face by a rocket in a New Years Eve. His airway was threatened due to serious bleeding from extensive soft-tissue injury and the patient was immediately intubated. Severe soft-tissue injuries to the right side of the face were observed. Computed tomography (CT) angiography images of the face, cranium, and the neck were obtained and revealed multiple fractures, including: comminuted fracture of the mandible; a Le Fort 2 fracture; a midline fracture of the maxilla; extensive fractures of the walls of the right maxillary sinus and both right and left orbital floor; a right zygomatic arch fracture and fracture of the right temporal bone at the temporomandibular joint. Soo back in May I was trying to get a clip on this rail and ended up dislocating and tearing my plantar plate in my left pinky toe. Who knew a pinky toe could be so serious but I got surgery in July and I had to get a pin that stuck out of my pinky toe to stabilize it. The doctor told me after surgery that this injury is one of the rarest type of injuries and that my case was the 2nd recorded case in the country. Now the doctor cleared me to skate but to take it slow and thankfully I was able to move out to California with some homies💯 Thank you God for all my blessings🙏🏾 - @keezo_ on Instagram

Low back pain is among the leading cause of disability globally, and nearly everyone will experience this at some point in their lifetime. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. This situation is called cauda equina, and is a neurosurgical emergency. But first, the basics. Most potassium is intracellular (98%) and a significant gradient infra- and extracellular plays a vital role in generation cellular action potentials. The renal system plays the most important role in regulation of potassium (90% of excretion). Normal levels are 3.5-4.5. Changes in these levels affect cell membrane potentials and result in cardiac and neuromuscular symptoms. The Na-K-ATPase pump plays a large role in cell membrane gradient. Causes include exogenous K+, excess endogenous K+ (hemolysis, rhabdo, burns, tumor lysis, trauma), redistribution, acidemia, meds (succinylcholine, beta blockers, insulin deficiency), decr in excretion (renal failure). Most commonly it is factitious (hemolysis, venipuncture issue). Symptoms are non specific and include general weakness, lethargy or confusion, depressed DTRs, GI effects like nausea vomiting diarrhea. ECG is one of the most diagnostic tests in hyperkalemia (see image above). Management is not dependent on cause but focusing on reversing to avoid dysrythmic complications. First, establish IV access, monitoring and o2. Treatment includes cardiac membrane stabilization which is the #1 priority first, transcellular shift and excretion (see image above). Keep giving these medications while watching for ECG improvements. Ultimately, dialysis is the most efficacious way to remove potassium. What about hyperkalemia in cardiac arrest? It alters the cardiomyocyte resting potential, resulting in inactivation of sodium channels and blocking conduction.
