Its #SlimeMoldSunday again! Todays photos are extreme closeup views of a very tiny slime mold called Arcyria pomiformis, less than 1 millimeter tall. I am absolutely captivated by what can be seen with a microscope lens mounted to my camera! The first photo shows the capillitium, the thread-like structures that contain the spores in the sporotheca (head) of the fruiting body. The second photo shows the whole fruiting body. The next two photos are the same fruiting body with its spores. After I took those photos I very gently blew out the spores so I could see just the capillitial threads. To give you a sense of scale, the last photos shows some Arcyria pomiformis fruiting bodies next to the tip of a very small pin. These photos were taken with a 10x microscope objective mounted to my camera. With such high magnification, focus stacking is a must to be able to see all of the detail. Each of these photos is a focus stack of about 250 individual images. Its a lot of work to create these images, but its also a lot of fun, and the results are always fascinating to me. Generally speaking in 𝐡𝐢𝐠𝐡 𝐫𝐢𝐬𝐤 𝐩𝐚𝐭𝐢𝐞𝐧𝐭𝐬 𝐎𝐍𝐋𝐘 An antibiotic for prophylaxis should be administered in a single dose before the procedure. ✳️ A𝐦𝐨𝐱𝐢𝐜𝐢𝐥𝐥𝐢𝐧 is the preferred choice for oral therapy because it is well absorbed in the GI tract and provides high and sustained serum concentrations. ▶️2 grams of amoxicillin given orally as a single dose 30-60 minutes before the procedure as the drug of choice. ✳️ For individuals who are allergic to penicillins or amoxicillin, the use of 𝐜𝐞𝐩𝐡𝐚𝐥𝐞𝐱𝐢𝐧 or another first-generation oral cephalosporin, 𝐜𝐥𝐢𝐧𝐝𝐚𝐦𝐲𝐜𝐢𝐧, 𝐚𝐳𝐢𝐭𝐡𝐫𝐨𝐦𝐲𝐜𝐢𝐧, 𝐨𝐫 𝐜𝐥𝐚𝐫𝐢𝐭𝐡𝐫𝐨𝐦𝐲𝐜𝐢𝐧 is recommended. ✳️Patients who are unable to tolerate an oral antibiotic may be treated with 𝐚𝐦𝐩𝐢𝐜𝐢𝐥𝐥𝐢𝐧, 𝐜𝐞𝐟𝐭𝐫𝐢𝐚𝐱𝐨𝐧𝐞, 𝐨𝐫 𝐜𝐞𝐟𝐚𝐳𝐨𝐥𝐢𝐧 administered intramuscularly or intravenously. ✳️ For Pen-allergic patients who are unable to tolerate an oral agent, therapy is recommended with parenteral c𝐞𝐟𝐚𝐳𝐨𝐥𝐢𝐧, 𝐜𝐞𝐟𝐭𝐫𝐢𝐚𝐱𝐨𝐧𝐞, 𝐨𝐫 𝐜𝐥𝐢𝐧𝐝𝐚𝐦𝐲𝐜𝐢𝐧. Low back pain is among the leading cause of disability globally, and nearly everyone will experience this at some point in their lifetime. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. This situation is called cauda equina, and is a neurosurgical emergency. #RetinaRocks Optic disc drusen (ODD) are refractile calcific concretions that form anterior to the lamina cribrosa. They are felt to be extruded extracellular calcified mitochondria, and may be inherited as an autosomal dominant trait. It is often difficult to differentiate ODD from true optic nerve edema. This is often the dilemma when seeing a child with a possible swollen nerve, although there are several tests that can help. ODD will often hyperfluoresce on fundus autofluorescence. We have found ultrasonography to be unhelpful, since ODD don’t calcify until later in life, by which time they usually become much more visible on examination. B-scan OCT imaging is often not helpful, although newer swept-source machines can directly visual the drusen. The retinal nerve fiber layer will be variably thinned with ODD and thickened with true disc edema. Fluorescein angiography is probably the best way to differentiate the two, since a swollen nerve will show late leakage, whereas ODD will not leak, often showing staining of the drusen. Our patient is a 15YO boy with quite pronounced bilateral optic nerve drusen visible clinically and on OCT. Although central vision was good, he was complaining of increasing peripheral vision loss due to the drusen compressing the surrounding nerve fibers. Unfortunately, there is no proven treatment although some have tried pressure lowering medications for neuroprotection. Whats the diagnosis? A 58-year old African-American woman presented with a 3-year history of scaly plaques on her legs. Prior treatment consisted of triamcinolone cream and compression stockings. Examination found thin, red-brown rhomboidal plaques with adherent scale on anterior legs bilaterally and no other skin involvement. Review of systems was positive for chronic dyspnea on exertion. Tuberculin test was negative. A punch biopsy found numerous deep dermal noncaseating granulomas composed of epithelioid cells and giant cells surrounded by a pauci-cellular cuff of lymphocytes. VITAL PULP THERAPY because It’s not all about anteriors in real life dentistry !!! Big decay on tooth 17, no pain, sensitivity test is normal. Partial removal failed. I went for full pulpotomy and direct pulp capping with bioceramics (total fill cement and total fill bc putty fast set) and direct composite as long term restorative treatment. To me, premixed putty bioceramics take the huge advantage that it can be drilled with no setting time. So much faster than MTA or Biodentine. More expensive also !!! Bats are Back! Since April, the Maine Department of Inland Fisheries & Wildlife (IF&W) in collaboration with US Fish and Wildlife and the International Union with the Conservation of Nature has continued to track emerging research regarding human-wildlife interactions and the novel coronavirus (SARS-CoV-2), particularly with regard to the care and release of bats. As research was conducted, wildlife rehabilitators in the state were prohibited from admitting bats in the case that they became exposed to COVID which could potentially decimate a critically low population. Why such a focus on bats? Unfortunately several species of bats in our region are imperiled, and as bats often come in contact with human dwellings, the novel virus poses a great risk to bats as well as humans. IF&W visited several facilities, including the Center for Wildlife, that care for distressed bats discussing disease transmission prevention protocols, new testing options, and some potential research projects specific to bats, which are underway and available for participation by Maine rehabilitators. Injured or orphaned bats are now authorized to go to one of four approved bat rehabilitation centers in Maine including the Center for Wildlife. An added bonus is that approved bat rehabilitation centers will participate in a Tufts University study that monitors and better understands potential human-to-animal transmission of SARS-CoV-2/COVID-19. We are so pleased to be working with IF&W to continue safely treating and admitting our wild bat patients! There are more than 1,100 species of bats throughout the world, with 40 species in the United States alone and additional species being discovered almost monthly. Though small in physical size, bats have a large footprint, and make-up one-quarter of the worlds mammals. The FDA estimates that bats provide over $3.5 billion of pest control to farmers each year - for FREE! Just one big brown bat is capable of eating over 1,000 insects an hour. This is probably many dermatopathologist’s favorite lesion. ⠀ ⠀ When you first examine skin biopsies under the microscope, identifying a Porokeratosis is all about the column of retained nuclei within the stratum corneum…(buzzword to remember: cornoid lamella).⠀ ⠀ Except its not.⠀ ⠀ Its actually about the changes seen here at the bottom, within the epidermis. Its a clonal disorder of abnormal keratinization, where keratinocytes don’t mature properly. There’s some reaction to these keratinocytes, with vacuolated cells and inflammation. ⠀ ⠀ But the bigger picture is even bigger than that! Porokeratosis is one of those perfect clinical-pathologic cases, where the changes under the microscope produce a specific change on the skin. For porokeratosis, the lesion is a ring, with an atrophic center lined by a ridge of these abnormal keratinocytes. This picture is just a cross section of that ring.⠀ ⠀ ⠀ I’m drawn to pathology that highlights the act of looking itself. Porokeratosis does just that. So... Lungs and RA can be mild or complicated... I guess I sit in the complicated department. I have learned to just go with the flow! As most of you see, I appear to be THRIVING (especially in my @thrive_construction shirts) on most days!😉) Like a lot of us, there are so many aspects to this disease that people outside of our homes would never know. I don’t show you guys every minute of my day, but I do talk a lot about my daily routine and what can be expected! People who develop RA-related scarring and interstitial lung disease often have a poor prognosis. That sounds terrible right “poor prognosis!”🙄 Thanks for that... If that didn’t sound awesome to you, then this next line might shock you! According to research that looked at a number of studies, the median survival rate for people with interstitial lung disease due to RA was 3.2 years to 8.1 years from the time of diagnosis.🤔This use to freak me out, but honestly I stopped worrying about the years, what doctors say, and just concentrated on ME and what I want from this one chance we get! I try my imperfect best every day! For those of you who might think I am crazy for my push up the peak or for attempting to compete wearing high heels, this might give you a bit more insight to my why! My lungs have presented more problems in the last couple of years, but I am committed to being a warrior, so let’s just end this post with that! My journey with cancer isn’t easy for me to talk about, but its something I want to share with you ⠀⠀⠀⠀⠀⠀⠀⠀⠀ I had a 12 tumor removed that was attached to one of my ovaries. It was filled with fluid and when it was removed, it ruptured and spread throughout my entire abdomen. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ In my Oncologists words, the cancer needed to be hit hard and fast with an intense chemotherapy regime. I had a port put into my chest (for direct access to my heart) and received three rounds of chemo. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Ill never be able to put into words how I felt. I begged my family to let me not go to my last treatment. I had nothing left in me. The underlying words being, please let me go 🥀 ⠀⠀⠀⠀⠀⠀⠀⠀⠀ This is what I want to share with you. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ Three things that cancer taught me ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 1) 𝙍𝙚𝙫𝙚𝙧𝙚𝙣𝙘𝙚 𝙛𝙤𝙧 𝙢𝙮 𝙡𝙞𝙛𝙚. You have one life to live. Stop waiting to have a reason to start feeling good in your skin and enjoying your time here. ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 2) 𝙇𝙞𝙛𝙚 𝙞𝙨 𝙣𝙤𝙩 𝙝𝙖𝙥𝙥𝙚𝙣𝙞𝙣𝙜 𝙩𝙤 𝙮𝙤𝙪, 𝙞𝙩𝙨 𝙝𝙖𝙥𝙥𝙚𝙣𝙞𝙣𝙜 𝙛𝙤𝙧 𝙮𝙤𝙪. Throughout my journey, I constantly asked why. Why me? Why NOT me? The day that disappointment ended for me was the day I stopped asking why is this happening to me, and started asking - what is this here to teach me? ⠀⠀⠀⠀⠀⠀⠀⠀⠀ 3) 𝙎𝙩𝙤𝙥 𝙡𝙞𝙫𝙞𝙣𝙜 𝙮𝙤𝙪𝙧 𝙡𝙞𝙛𝙚 𝙡𝙞𝙠𝙚 𝙚𝙫𝙚𝙧𝙮𝙤𝙣𝙚 𝙞𝙨 𝙬𝙖𝙩𝙘𝙝𝙞𝙣𝙜 𝙮𝙤𝙪. I had a very hard time losing my hair. I was young and my image was everything to me. Then I had a stinging thought: to miss any of you that are equally passionate about neurosurgical anatomy and could not be with us today. That feeling is what fuels my posts... because when I was a day-dreaming medical student in Barcelona I would have done anything to be in an audience like this today... I should be in San Francisco right now. I was at the airport, I had everything I needed, but no amount of preparation will ever be able to control the actions of others. Before I could even finish a sentence, the TSA officer dumped out my medical bag onto a dirty table and went through it, without as much as changing her gloves. My TPN bags were thrown around, while my requests to keep them in the bag were ignored and explanations met with back talk and rolled eyes. After talking to my TPN team and the infusion pharmacist, I decided that the safest option would be to cancel my trip and go back home where they could get me new TPN bags. For me, it was not worth the risk, especially when traveling to the other side of the country. I’ve been sick for nearly ten years, but today was the first time I actually put my health first. In the past I would’ve gotten on that plane anyway and tried to survive four days without nutrition... I would’ve completely ignored the rational voice in my head for the sake of “living my life.” Instead, I did the thing I would’ve told someone else in my position to do: be safe and go home. In light of all this, I’m choosing to be thankful—thankful for my TPN NP who tried everything she could to salvage my trip, before she even started her day. For the kindness of the TSA supervisor who saw me crying on my way out and stopped to see if he could help. And, for my neighbor who drove all the way down to the airport to pick my sobbing self up, then talked to the airline to get my checked bag back and got me coffee before taking me home. The holidays are stressful for everyone, and I’m sure that woman didn’t mean to hurt me, so I’m choosing not to be angry. I’m upset I’m not in San Francisco with the rest of my family spending time with my grandparents, but I’m also proud of myself for finally putting my health first. Sometimes, my friends, it’s in the dark moments that we can see just how much we’ve grown💕 NOTE: I addressed the situation with the TSA supervisor and he handled it appropriately, therefore I do not feel a formal complaint is necessary at this time. - @fromthepatientperspective on Instagram

Hey everyone, my name is Mai @maity87 I’m a 32 year old Project Manager, Egyptian living and working in the UK. In 2018 I was diagnosed with stage 4 Hodgkin’s Lymphoma, following losing both my parents earlier that year. To say it was a tough year would be an understatement, a lot of pain, loss and the emotional turmoil of Cancer. After 12 rounds of ABVD chemotherapy I was in remission, but only lasted 3 months before I relapsed. I’ve had to go through more chemo, then now having a stem cell transplant.
