When understanding hemodynamics and shock, it is important to understand the pathophysiology. Knowing what makes up the mean arterial pressure (or MAP) is a great starting point because all types of shock affect this. ❤️ The mean arterial pressure is 2/3 of the diastolic blood pressure plus 1/3 of the systolic blood pressure. It’s the number that’s below the blood pressure on most telemetry monitors. ❤️ Cardiac Output (CO): this is determined by the stroke volume (SV) and Heart Rate (HR). Contractility, preload, and afterload determine the stroke volume. 💕 PRELOAD: the stretch of sarcomeres during ventricular filling. Think LVEDP (left ventricular end diastolic blood pressure). 💕AFTERLOAD: pressure that the heart works against to allow ventricular contraction. 💕 CONTRACTILITY: an elusive term, but it’s basically how strong the heart is when pumping. More on this when we discuss the “tropies”!! 🤓 ❤️ The systemic vascular resistance (SVR) is what creates the blood pressure. It is determined by the length of the vessels and it is inversely proportional to the radius to the fourth power. This means that the higher the radius, the less the SVR is...by A LOT. This relationship is important to identify when looking at shock because the slightest change in the radius of a vessel can change the SVR profoundly. Day 60 spent at Jackson Hole, it seems circular to have spent both day 50 and day 60 at #JacksonHole. Day 50 was spent celebrating my anticipated #winter time off and day 60 was spent enjoying the new found freedom of no work on the visual horizon....what a difference a month makes. I slept in the #subaru last night with a low of -2, skied all day, enjoyed a not-exactly-poached hot tub to warm up, then bedded back down in the Subaru for another cold night with as many strategic layers as I had at my disposal...thank you #Patagonia and #kammok. I did this trip 8 years ago and again, it seems circular to do it again now. Met a lifelong Jackson Hole employee/guide to show me around today, exactly as I did 8 years ago...same Subaru, soooo much more life under my belt.... Just wanted to share another dyno sheet. This is my personal shop car vs a stockish car. Both BK2 3.8s both dynoed at my shop on my dyno. After years of work, testing and tweaking, we finally got very close to our goal of breaking 350whp NA. Ive spent the last few weeks working with @alphaspeedmw to see what we could get out of this thing (even risking blowing it up trying radical settings) and we have pretty much hit the wall on all sides. If I were to dyno at sea level (or 50* temps), it would def make over 350whp which means were at the point where well make at the wheels, what hyundai advertises at the crank! Notice that im still making 320whp at 7500 rpm. Instead of the nasty drop off that ALL 3.8s see around 6900rpm, we continue to make power up top. I was hoping to be closer to 350 at 7500, but were limited by airflow i think. Built heads may fix that issue, but thats an expensive project to take on, so for now well leave it as is. If you notice the torque, were making 310 wtq which is crazy for an NA setup. You can feel the car pulling all the way and it continues to pull, even when its 120* IATs. We are now offering tuning adjustments for those who want it. 7500 rpm limiter with the performance updates weve been testing out. This will be limited to BK2 3.8s on E85. BK1s can still get performance updates on E85, but not the increased limiter.
