About 7 years ago, few months after I started climbing, I saw an old video ad of @kjorgeson doing this climb. I climbed about V3, and I didn’t really understand outdoor climbing, I just knew I was going to send this boulder. I’ll save you the stories and the sessions where I wasn’t strong enough, bled too much, or just gave up. Hundreds and hundreds of attempts of questioning my ability in climbing, and contemplating whether or not to keep trying. Instead, I’ll highlight the amazing group of friends that always came to support me. People who genuinely believed in my capabilities and shared my stoke. I’ll highlight the pure beauty of bouldering that is shown through this piece of rock. I’ll highlight the battle I had today, sending on my last attempt, 4 hours into the session, skin was low, it was freezing, beta was perfecting, grammar is failing.. Climb because it inspires you! Idiopathic Ventricular Tachycardia (VT) I was inspired to do this #meded post today based on an interesting case I had while on call the other night. What is it? Most VT we see is caused by structural heart disease, however, 10% of VT cases have no structural pathology. This is called Idiopathic Ventricular Tachycardia. Subtypes defined by clinical presentation or mechanism. They have ECG characteristics that can pinpoint where it’s anatomically located and usually have a great prognosis Outflow tract VT 🔑 localized around left or right ventricular outflow tract (RVOT vs LVOT) 🔑Earlier age onset 🔑Nonsustained, repetitive, monomorphic VT or exercise-induced sustained VT (I.e. post exercise stress test) 🔑Symptoms: palpitations and dizziness 🔑Baseline ECG and echocardiogram are usually normal, but MRI may show abnormalities (I.e. RV changes) 🔑RVOT VT ➡️ LBBB with inferior axis LVOT VT ➡️ LBBB with inferior axis + small R-waves in V1 and early precordial transition or RBBB with inferior axis 🔑Treatment: Adenosine, beta blockers, verapamil 🔑Alternative therapy: class IA, IC and III agents 🔑Ablation: treatment of choice for patients with symptomatic, drug refractory VT, drug intolerance or for those who do not desire long-term drug therapy Fascicular VT 🔑present younger - 15 -40 years. More common in males 🔑Symptoms: palpitations, fatigue, dyspnea, dizziness and presyncope. Artículo: Relación entre el Rendimiento Incremental Máximo y el Ejercicio de Intervalos de Alta Intensidad en Atletas de Élite⁣ ⁣⁣ Este estudio descriptivo tuvo como objetivo explorar los factores fisiológicos que determinan la tolerancia al esfuerzo durante el ejercicio de intervalos de alta intensidad. Cuarenta y siete mujeres jóvenes (15–28 años) se inscribieron: 23 atletas de equipos de reserva nacional de Taiwán y 24 mujeres moderadamente activas. Cada participante se sometió a una prueba de ejercicio cardiopulmonar incremental máxima INC (protocolo de Bruce modificado) en el primer día y una prueba de intervalos de alta intensidad (HIIT) en el segundo día, ambas pruebas realizadas en una cinta de correr. El protocolo de HIIT implicó la alternancia entre esfuerzos de 1 minuto al 120% de la velocidad máxima, en la misma pendiente alcanzada al final del test INC, y un descanso de 1 minuto hasta el agotamiento voluntario. El intercambio de gases, la frecuencia cardíaca (FC) y la oxigenación muscular en el vasto lateral derecho, fueron medidos por espectroscopía de infrarrojo cercano. El número de repeticiones completadas (Rlim) por cada participante se consideró como el índice de tolerancia HIIT. Los resultados mostraron una gran diferencia en el Rlim (rango, 2.6–12.0 repeticiones) entre las participantes. La regresión lineal paso a paso reveló que la variación en el Rlim dentro de la cohorte estaba relacionada con las tasas de recuperación del consumo de oxígeno (VO2), la FC en el segundo minuto después del INC y el índice de saturación del tejido muscular al agotamiento (R = 0.644).
