Let me tell you about failure. This is a patient who had a bad firework injury (on the left) that I treated with a free flap (second from the left) which ultimately was not successful (second from the right) and needed a different type of flap (on the right) to cover the nerves and bones which were injured. I literally broke down in tears when the first flap was not successful. This is the first flap I have ever lost, and it was incredibly heartbreaking. I do a lot of outreach and speaking for students in high school and college. A question students sometimes ask is if I ever make mistakes. The answer is yes, of course, we are all humans and therefore will all make mistakes. The most important thing is to have grace for yourself and to remember to keep giving your best. Medicine is a place where mistakes and growth are often met with humiliation. Mi amiga @ana.herruzo me visitó el otro día en el taller y me grabó pintando el fondo de esta pieza. Fue una suerte poder vernos en persona. Llevo más de un año trabajando la pintura con spray y esmalte sobre madera. Desarrollo personajes y composiciones en formatos grandes. A través de la pintura estoy explorando otra manera de crear personajes y mediante los formatos grandes intento componer obras a través de las cuales poder sumergirte en mi mundo creativo. Para eso es necesario mostrarlas porque la pintura hay que verla en persona. A partir del 11 de septiembre tengo la suerte de participar en una exposición colectiva en Madrid con motivo del 6°aniversario de @swintongallery Allí mostraré una de estas pinturas que he creado en el último año junto con las obras de otrxs artistas a quienes admiro. Restoration times vary depending on the level of damage. Our crews are trained to restore power through a three-phase process to get your lights back on as quickly and safely as possible. Here’s an explanation of the process from page 33 of our 2020 Osceola Hurricane Handbook. PHASES OF POWER RESTORATION: Phase 1: Assess and Protect We send highly trained workers into the field to locate and monitor safety hazards. These hazards can include downed wires and poles. Our crews then make sure electricity is off for your safety. They also ensure that power is flowing to critical facilities like hospitals, care facilities, police/fire stations and other essential services. Phase 2: Repair Damage Once damage has been assessed and safety measures enacted, we dispatch crews to make repairs. Substations and main electric lines and wires must be repaired to restore power to you. Even if you do not see our field crew teams, rest assured we are working nearby to get your power restored. Phase 3: Restore Power Once damage is repaired, we begin restoring power to homes and buildings. If you notice that a neighbor’s power is back before yours, don’t worry. This tumbler survived #tropicalstormarthur back in May in North Carolina. It was completely submerged when the work boat was flipped and resting on the marsh for 5 days before rescue. This is what happens when the system fails people over and over. This morning when I woke up, I told myself because of Corona Virus I couldnt risk going to the protest, that it wasnt worth it. All day something inside of me was screaming and itching, a little voice that said this cause is bigger than myself, the virus and its so important to go and stand up for what you believe in. As a filmmaker I couldnt help but document my experience today. Lately it feels like the end of the world, like we are stuck in Grand Theft Auto game. Using your voice is one of the greatest gifts we have as an American citizen and life is the greatest gift known to man. George Floyd has none of those things. He will never have another chance to fall in love, to see a sunset, to laugh with his friends and family, to listen to music, eat delicious food or watch a beautiful film. His life was ripped from him like so many others and that is why we must protest and use our voice and bodies. To show up in solidarity for our brothers and sisters. The mistreatment towards people of color, especially blacks from society and the police has gone on for way too long. People are tired of hurting and watching their loved ones getting murdered. I dont agree with violence or stealing but what else is there to do? Extreme times call for extreme measures. No police officer has ever been sentenced so quickly and if it takes burning this entire country down to make real change then I support that 100%. Maxillofavial Trauma Management - Airway ———————————————————————— After we handle the immediate life threats (massive hemorrhage), we have to consider the airway. Look, listen, and feel: take a look at the injuries and look inside the mouth for blood, vomit, broken teeth, dentures, chewing tobacco, etc. If any of these are present, grab your suction and clear it out. If dismounted or without mechanical suction, grab a 60cc syringe with a cathedral tip and a put an NPA on it to create a field expedient manual suction device. Give the lungs a listen and see if you’re getting adequate air movement. If the environment is keeping you from listening, place you’re hands on the chest and feel for bilaterally equal rise and fall of the chest. ———————————————————————— I would personally prefer to skip the intubation attempt and go for a surgical cricothyroidotomy. Orotracheal and nasotracheal intubation may be attempted but be aware that musculoskeletal tissue disruption will likely increase the difficulty of intubation. LMAs and I-Gels can be used to buy you some time to prep for a cric, but they don’t prevent regurgitation and aspiration of vomitus and may increase vomiting unless a paralytic has been administered (which is unlikely due to the fact that we likely haven’t gained vascular access at this point in our assessment). Um acidente envolvendo uma moto e um caminhão deixou três pessoas feridas na rodovia Anchieta na altura de Cubatão (SP), na manhã desta sexta-feira (18). O caminhão colidiu com a estrutura de uma ponte e ficou destruído. Segundo apurado pelo G1 junto à Ecovias, concessionária que administra o trecho, o acidente aconteceu por volta das 10h30 na rodovia Anchieta, na altura do km 53. Um caminhão, que estava na pista sentido litoral, perdeu o freio no trecho de serra e bateu de frente com a estrutura de uma ponte, atingindo o motociclista que trafegava na mesma direção. O motociclista saiu da colisão com ferimentos leves, enquanto o motorista do caminhão e um passageiro foram socorridos pelo Serviço de Atendimento Móvel de Urgência (Samu) em estado grave e moderado, sendo encaminhados a uma unidade de saúde de Cubatão. Segundo a Ecovias, por volta das 11h30, a faixa da direita ainda estava bloqueada porque os veículos envolvidos no acidente permaneciam no local. Por volta das 12h, o trecho foi liberado. Há congestionamento apenas no planalto, do km 39 ao km 42. O SAI está em Operação Normal (5x5). Para a descida, os motoristas utilizam a pista sul da via Anchieta e pista sul da Imigrantes. to go off-screen, our current online exhibition, a solo show by Petra Szemán (@petra.szeman), curated by Bob Bicknell-Knight (@bob.bk1), is available to view until the 15th August. The exhibition features a number of Szemán’s videos, alongside an extended interview between Bicknell-Knight and Szemán, speaking about the series, her practice and artistic influences. Below is a short excerpt. BBK: Going back to the form that your work takes, I’m interested by what drew you to animation, and why you originally started placing animations into footage captured within the physical world? PSz: Despite having been a fan of anime etc for most of my youth, I haven’t really made any connections between my own artistic practice and animation until I happened to sign up for a moving image workshop with Kelly Richardson in the 2nd year of my Fine Art BA at Newcastle University. I think that’s where I was first confronted with the breadth of artists’ moving image, and I remember being immediately mesmerised by Michael Bell-Smith’s Up and Away and David Blandy’s Background in particular. Both of these works build heavily on backgrounds from 16-bit video games, and I was taken in by the new worlds they made by using parts of already existing ones. Sort-of building on this I started making collage-like videos from game and anime stills, often using bits from Makoto Shinkai’s animated films, sometimes overlaid with a character that would eventually become Yourself. Shinkai’s films reference RL parts of Japan quite extensively. I proposed a project where I would visit these places myself, to some extent re-exploring the memories that my digitised self would’ve had of the virtual versions of these real life locations. Making the same memories but in parallel realms and parallel bodies, only separated by time. I got to fly to Japan and do this initial ‘pilgrimage’ thanks to the Bartlett Travel Award offered by the university, and the whole thing just spiralled from there, merging the two realms of experience together. Last year, while our aftercare team was following up with a survivor in one of our partnering aftercare homes, they found out about a need that another young girl had in the home. We believe in supporting survivors, children of survivors, and aftercare homes as a whole. Little 2-year-old “Mariana” was born with club feet, a seizure disorder, and a heart condition that made doctors in her country hesitant to perform surgery. We sent out a message looking for doctors in the states who would be willing to help, and a team leader in Utah asked his roommate if his father, Dr. Nelson of Cache Valley Hospital, would be willing. He and Dr. Daniel Huff gladly obliged to help. This incredible act of service would not have happened without our amazing network of volunteers! Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued. Amig@s a partir de ahora queremos compartir con ustedes este proyecto. Es la transformación de este bus escolar dado de baja en nuestra casa 🏠 . Poco a poco subiremos fotografías de las modificaciones que lo llevo a convertirse en lo que ahora llamamos nuestro hogar, aquí es donde pondremos todo nuestro compromiso por el medio ambiente reutilizando materiales y adaptando el bus para ser una casa auto sustentable.
