Traumatic injury of the left foot! A 58-year-old male person was admitted to the Emergency Clinical Hospital Bucharest, approximately 7 hours after a traumatic injury of the left leg amputated by the blade of an electric saw. Replantation surgery of the left leg was performed under general anesthesia with ischemic time lasting 9 hours up to the arterial anastomosis. Metallic osteosynthesis and suture of the blood vessels, nerves and tendons were performed. On the initial physical examination this person presented no inflammatory signs of the skin cicatrisation process after replantation surgery, no edema and no pain syndrome. Some of you may or may not know In 2012 I was involved in a major motocycle accident that almost ended my life a red P plater drunk and high on drugs ran a red light coming off a freeway and T boned me my left leg and other bones were completely crushed and broken Some of the injuries were toes broken shin bone broken in 3 places dislocated hip pelvis out of alignment femoral shaft (thai bone) broken in 3 places broken wrist and fingers radius (forarm bone) broken elbow dislocated and many others this is just of the top of my head 5 operations later I now have many screws, and pins and rods holding my leg and bones together For almost 2yrs I was unable to walk properly I moved around my house in a office chair and did the baby bum shuffle on the floor lost over 40+ kg went down to 60/70kg looking like skin and bone I got even more depressed looking at myself because of how small I became so I started eating pizza daily KFC and what ever shit food I can find to get my weight back up. Today’s case is this crazy infection! This young 22 year old male developed necrotizing fasciitis, which caused this massive debridement to try to control the infection. . Nec fasc is a SCARY infection, due to the rapid spread through the tissue. It is normally caused by a polymicrobial organism (multiple bacterial organisms) and it’s important to start broad spectrum antibiotics ASAP. If the infection is unable to be controlled, a debridement, like pictured above, is anther control measure. However, if the rapidly spreading infection cannot be controlled, the lose of limbs could be eminent. An even worse outcome would be the infection spreading systemically causing multiorgan failure due to sepsis resulting in death. This is why if you’re concerned for nec fasc, you promptly want to order the proper tests/imaging and if possible, get a surgeon to lay hands on the patient to get their input! . How it starts is can be something as minor as a small break in the skin. The horror stories you hear is “they had a cut on their foot and they got a pedicure, causing the entry of bacteria and development of nec fasc.” You also may be seeing the spike in stories in the news where people are swimming in areas that have high concentrations of certain bacterias that cause nec fasc. Those people enter the water with a small cut on their extremity which is an entry point for the bacteria. In this case, it seems like he had a laproscopic surgery and the incision site (below the belly button) dehisced (opened up) and got infection. This may have been an entry point for the bacteria to spread to his right leg and scrotum. . With all this being said, THIS IS VERY RARE! So don’t stop swimming or getting your pedicures because this image may scare you! Chances are you won’t be the 1/1000000 and you’ll make it through life without even coming close to being diagnosed with nec fasc! - @trauma_time on Instagram

Compartment syndrome is a medical emergency and it needs to be recognized early. A similar situation may occur when an IO cannula is deployed in the wrong position or dislodged during the treatment. That is why you would want to make sure your IO device is in place. There are several ways to verify IO placement: 1. Check for aspiration of bone marrow 2. Flush and watch for swelling 3. Are the medications injected via the IO taking effect on the patient? 4. If available, bedside doppler may be effective in correct position confirmation. Early signs of compartment syndrome - Severe pain, swelling and redness of the affected limb Compartment syndrome is a rare complication associated with IO access. It may occur when the cannula is dislodged from the bone or over penetrated it. Therefore, correct location must be verified by aspiration of bone marrow, saline flush and observation of extravasation signs. In the image above is the result of a fasciotomy performed on a patient who had arrived to a field hospital with compartment syndrome, after being trapped under wreckage of an earthquake and rescued.
