Our REPARE team is always hard at work making sure that the biomedical equipment at SBH is in good working order. Recently, the autoclave in our surgery department stopped functioning. Our Chief Biomedical Equipment Engineer, Alix Ilophene, told us: “We know that to do surgery, sterilizing equipment is necessary. Without that, there is no surgery. It was not so difficult to diagnose the problem. The [autoclave’s] boiler displayed a low water alarm—we found the machine’s water piping was blocked. We cleaned it, and then cleaned the whole machine. We took about two hours to repair it. Acute (flash) pulmonary edema is a type of congestive heart failure (HF) exacerbation, essentially meaning too much fluid in the lungs. This occurs when pressures build and force fluid out of the capillaries ➡️ alveoli where air exchange normally occurs. This effects the bodys ability to exchange CO2 for O2 and therefore decreases the saturation of oxygen in the blood. There are many precipitants but often it’s due to medication noncompliance, diet, ischemia or infection. It often comes on rapidly and can be very life-threatening. Patients are typically very hypertensive (elevated blood pressure). Usually patients will present the with crackles or rales in their lung bases, decreased breath sounds (indicative of fluid in lungs or effusions), distension of the neck veins (JVD), possibly see cheyne stokes respirations but often just breathing very fast, tripoding with retractions of muscles, along with pitting edema (swelling) in legs. Wheezing can be seen as well (cardiac wheeze) which tricks many providers. Once in the ED, can be diagnosed clinically and treatment may be started prior to labs or imaging. Workup includes blood work including heart enzyme levels (troponin), bNP level (how dilated the ventricles are due to fluid overload). Chest X-ray to look for kerley-b lines, effusions, vascular congestion, or interstitial infiltrates all indicating pulmonary edema. Ultrasound is extremely useful to look at heart function (ejection fraction) and for effusions or b-lines indicating fluid in the lungs; this helps rapid differentiation from COPD which can present similarly. Treatment focuses on decreasing blood pressure, off loading the fluid and to improve work of breathing. Nitroglycerin will be used aggressively (decreases preload) and positive pressure ventilation will be started immediately depending on severity. A 58-year-old male person was admitted to the Emergency Clinical Hospital Bucharest, approximately 7 hours after a traumatic injury of the left leg amputated by the blade of an electric saw. Replantation surgery of the left leg was performed under general anesthesia with ischemic time lasting 9 hours up to the arterial anastomosis. Metallic osteosynthesis and suture of the blood vessels, nerves and tendons were performed. On the initial physical examination this person presented no inflammatory signs of the skin cicatrisation process after replantation surgery, no edema and no pain syndrome. Some of you may or may not know In 2012 I was involved in a major motocycle accident that almost ended my life a red P plater drunk and high on drugs ran a red light coming off a freeway and T boned me my left leg  and other bones were completely crushed and broken Some of the injuries were toes broken shin bone broken in 3 places dislocated hip  pelvis out of alignment femoral shaft (thai bone) broken in 3 places broken wrist and fingers radius (forarm bone) broken elbow dislocated and many others this is just of the top of my head 5 operations later I now have many screws, and pins and rods holding my leg and bones together For almost 2yrs I was unable to walk properly I moved around my house in a office chair and did the baby bum shuffle on the floor lost over 40+ kg went down to 60/70kg looking like skin and bone I got even more depressed looking at myself because of how small I became so I started eating pizza daily KFC and what ever shit food I can find to get my weight back up. 🔹Tres mujeres que tenían Covid-19 fallecieron en las últimas horas en la región| El día de hoy, Magallanes tuvo la segunda cifra más alta de casos nuevos de la enfermedad desde que comenzó la pandemia con 271 nuevos confirmados y a nivel nacional sólo nos superó la Región Metropolitana. Los indicadores de la pandemia en Magallanes siguen siendo trágicos. En contagios solo la comuna de Río Verde se mantiene sin casos hasta el momento. A little over a month down as an attending. I have truly been a “black cloud” in every sense of the word during this first month and I have been getting my share of very sick patients. Believe it that the learning curve continues beyond residency training and that experience continues to be the greatest teacher of all. It is important to continue to have an inquisitive mind and to always seek to get better. Luckily for me, about half of my shifts are with residents who continue to ask important questions, who seek guidance, and who challenge me each and every single day in my practice. I learn from them and hope that I am teaching them something too. It has been an exciting, rewarding month and I’m looking forward to the months ahead! I hope that you guys are striving to be better each and every single day and are doing all that you can to provide the best care for your patients as we continue you to push through this pandemic. Selain memperingati HUT Republik Indonesia, bulan ini adalah bulan dimana anak pertama kami Agus Harimurti Yudhoyono (AHY) dan cucu pertama Almira Tunggadewi Yudhoyono berulangtahun. AHY lahir pada 10 Agustus 1978 di sebuah klinik di Dayeuhkolot, Bandung. Sedangkan Almira lahir pada 17 Agustus 2008, di Jakarta. Saya ingat, dulu waktu istri tercinta Ani mau melahirkan AHY, air ketubannya pecah di kendaraan jip militer yang saya kemudikan. Sehingga begitu sampai di pelataran poliklinik persalinan, saya langsung menggendongnya menuju ke kamar persalinan. Bahkan, saya menunggui dan memberikan semangat saat bidan membantu proses kelahiran AHY di ruang persalinan. Sedangkan di tahun 2008, saya dan Almh Ibu Ani sungguh berbahagia karena dikaruniai cucu perempuan yang lahirnya tepat di Hari Ulang Tahun Kemerdekaan RI ke-63. Saya ingat hari itu adalah hari yang sangat panjang. Tengah malam memimpin Renungan Suci di TMP Kalibata; pagi hari memimpin Upacara Detik-Detik Proklamasi; sore hari Penurunan Bendera serta malam harinya Resepsi Kenegaraan. Namun, sebelum upacara bendera pagi hari saya dan almarhumah menyempatkan waktu untuk menunggui proses persalinan Annisa di RSPI. Bahkan almarhumah menunggui dan memberikan semangat di ruang persalinan kepada Annisa. Selamat ulang tahun yang ke-42 untuk anakku AHY dan ke-12 untuk cucuku Almira. Today marked the discharge of two COVID-19 patients with the longest stays at St. Marys General. Our patient, Graciela who resides in Clifton, was admitted on April 4th and spent a whopping 61 days as an inpatient, and treated between ICU and 6 East. Discharged on the same day by great coincidence, our patient Saina, a resident of Passaic, was admitted on April 13th and spent 52 days as an inpatient, was vented for several weeks, and received care from the ICU and later 6 East. Both patients, aged 59, tested negative for COVID-19 upon discharge and were brought to tears when expressing their gratitude for the excellent work of our ICU and 6 East who saw them off today. I found myself in a position rather opposite to what I’m used to: the other side of the stretcher. Before I go any further, I am ok and no longer in the ER so please don’t worry. I woke up with horrible neck pain. It radiated down my arm. My fingers numb. After a 2nd sleepless night I couldn’t take it any more and went to the ED. Walking through those doors not as a consultant to give insulin recs but instead to have my own vitals checked, felt awkward. My rational side had a sense it was likely a disc but my anxious medical mind wondered “what if it’s something more ominous?” The fear set in. And then, the curtain pulled back and my friend Kamini walked in. I felt my body relax. I knew I was going to be ok because I had her, a brilliant and caring ER physician. After Imaging and morphine, the curtain pulled back again this time revealing another familiar face -my buddy Sean the neurologist, complete with rose gold reflex hammer and all. When the MRI resulted, the read was worse than what I was hoping for, not one but two C spine herniations. I felt broken, two weeks shy of my 35th birthday and my spine looked like that of a man twice as old. The years and stress of residency, covid, and life seemed to pull the polposis from between my vertebral bodies and leave me stricken w searing pain I guess fitting for This year. Will I need surgery? I wondered. My fear mounted again and then like clockwork the curtain slid back and Yosef the neurosurgeon popped in with his big smile and perfectly quaffed hair “Natter! What the hell happened to your neck!?” I hugged him and immediately felt relieved. Yosef is like the Mayor of NYU, he’s known and loved by everyone and somehow manages to have a robust social life despite being a neurosurgery resident. By this time my Incredible girlfriend Alice had also arrived, who without I surely would have crumbled into pieces. I’m home now, still with some pain and on fistfulls of meds but also filled with immense gratitude for my friends colleagues and loved ones who I always knew as great doctors, but I can say now from experience, truly are. De los 3.352 fallecimientos comunicados, 1789 son hombres (1701 residentes en la provincia de Buenos Aires, uno en la ciudad de Buenos Aires, 25 en Chubut, cuatro en Chaco, dos en Córdoba, cinco en Jujuy, uno en Mendoza, tres en Neuquén, tres en Río Negro, 13 en Salta, 12 en San Juan, 10 en San Luis, cuatro en Santa Cruz, cuatro en Santa Fe y uno en La Rioja), y 1542 mujeres (1477 en la provincia de Buenos Aires, cinco en Chaco, 28 en Chubut, una en Córdoba, seis en Jujuy, cuatro en Salta, nueve en San Juan, una en Santa Fe, dos en La Rioja, tres en Neuquén y seis en San Luis). During a Prolonged Field Care situation, you just took an airway on a multi-system trauma patient. You have a BVM with a PEEP valve but no ventilator. 1.) What would you consider for initial bag rate, “volume”, and PEEP setting on an average male using BVM? 2.) If your EMMA capnography says ETCO2 is 65, what can you change or consider? 3.) So this is not how I imagined my spring. Right now I’m meant to be in Costa Rica... But my job reminds me every day that there are many people going through far, far worse than I am. We keep hearing about saving lives but there seems to be a wider purpose of intensive care - of all care - at this time: compassion. To provide compassionate care to all patients and families – and to each other – during Covid-19 is a measure of our humanity. It is how we will be judged and how we should be judged. There are no heroes in this but there is real, real courage everywhere. And of course patients and there families are showing the most courage of all. I can’t wait to hug my family again. I may never let go. P.S. I may not be in Costa Rica with my friends but to celebrate International Nurses’ Day today, I’m certainly drinking a lot of daiquiris.
