Application of a split thickness skin graft: This patient underwent extensive debridement of the forearm due to a necrotizing infection. Once the patient recovered from the infection, reconstruction began with application of Integra bilayer matrix, which is essentially a synthetic layer of dermis. Once this healed, a skin graft was applied, as seen here. This is a split thickness skin graft which was taken from the thigh using a powered dermatome. This harvests the skin at a thickness of 0.012. It is meshed (first video) to allow coverage of a greater surface area (stretches) and allow for fluid egress. If fluid builds up under the graft, it will not take. It is applied here as seen in the second video and can be secured with sutures or staples. Negative pressure wound therapy (NPWT) is applied (last video) and serves two purposes. First it constantly removes fluid, preventing accumulation. Second, it acts as a bolster, holding the graft firmly against the wound bed, giving it the best chance for take. Here you can see the sponge shrinking down as suction is applied.
