The word coloboma comes from the Greek koloboma, meaning mutilated, curtailed, or with defect. We use the term to describe defects to nearly every part of the eye: eyelids, iris, lens, ciliary body, zonules, choroid, retina or optic nerve. Most of these are birth defects, where normal tissue in or around the eye is missing at birth. The eye develops quickly during a fetus first three months of growth. A gap, known as the choroidal fissure, appears at the bottom of the stalks that eventually forms the eye. This fissure generally closes by the seventh week of pregnancy. If it does not close, a coloboma or space forms. This patient has what’s called a lens coloboma, but that’s a bit of a misnomer because the defect is in the zonules rather than the lens. Lens coloboma is more accurately referred to as coloboma of the zonule and/or ciliary body. If the defect is severe, an ophthalmologist will perform surgery to remove the lens and replace it with an artificial lens, also known as an IOL. Aberration-free or aspheric IOLs may be preferable in these eyes to decrease the effect of lens decentration. A silicone IOL is not recommended. The patient may later develop a complex retinal detachment that requires silicone oil use.
