Kevin Young is the author of a previous book of nonfiction, The Grey Album, a New York Times Notable Book, and of eleven books of poetry. He is also the director of the famous Schomburg Center for Research in Black Culture and the poetry editor for The New Yorker. Here we’ll discuss the basics of cardiac arrest. It’s a hotly debated topic so stay tuned for more detailed posts later on! Cardiac arrest is when the heart’s stopped and can’t perfuse any organs. There are many causes but treatment is based on only a few rhythms. In emergency medicine, we are trained to treat the ABCs (airway, breathing, circulation) but for cardiac arrest, we prioritize the circulation first. The two most important parts of a cardiac arrest is good CPR and early defibrillation (unsynchronized jolt of energy across the heart)! Markers of good CPR include allowing full recoil of the chest, going a full 2 with each compression at a rate of 100-120 beats per minute. Defibrillation should occur as soon as possible if the rhythm is either [pulseless] ventricular tachycardia (VT) or ventricular fibrillation (VF). After all this, we then prioritize maintaining the airway and giving rescue breaths. An endotracheal tube (ETT) or a supraglottic device (LMA, King) should be placed when possible, with multiple studies supporting the latter being just as effective as the former. The ER doc might switch out the SGD with an ETT if return of spontaneous circulation (ROSC). Best outcomes are with VF or VT as these are commonly assoc with cardiac causes (ie heart attacks). These are treated with defibrillation and anti-arrhythmics (ie amiodarone or lidocaine). Less likely to survive include rhythms of PEA or Asystole, which are not amenable to defibrillation or anti-arrhythmics. Considerations of the Hs and Ts should be considered and treated. All rhythms receive epinephrine unless many doses have been given without ROSC (data for epi is actually very poor for survival to discharge). Medications such as calcium, magnesium, bicarbonate used to be standard of care for all arrests but data does not support the use unless for a specific reason as they can cause more harm than good! Cardiac arrest patients are very sick and many do not survive, however knowing a bit about it and knowing to start CPR can save lives! Antibiotic resistance is now considered a global public health crisis. Some researchers are predicting that by 2050 our resistance to antibiotics will cause up to 10 million deaths every year, outdoing cancer as the leading cause of death worldwide. Antibiotics are (over) used in animal farming due to the intense farming processes that exist within the meat industry. The practice of animals being forced to live in confined, crowded spaces; not being given enough time to recover in-between births; and baby animals being taken away from their mothers too early places a strain on the animals’ health, compromises their immune systems, and increases the risk of disease transmission. Due to this risk some countries use antibiotics as a preventive measure, meaning these medicines are given to animals as soon as they are born, whether they are ill or not. Other countries use antibiotics to make livestock animals grow faster, so that profit margins are increased. Every time we use antibiotics (whether in humans or animals) we risk the development of drug-resistant bacteria (a superbug). By using antibiotics in animals when they are not sick, we are adding to the risk of drug-resistant bacteria developing. Remember that saying ‘you are what you eat’? Because our health is closely linked with the health of our ecosystems, and the animals in it, the use of antibiotics in animals can affect humans as diseases can be shared between humans and animals through direct contact. This is not to say we should stop using antibiotics altogether. Antibiotics are needed to protect the health of humans and animals, but should only be used when the animals are already sick. While the vast majority of antibiotic resistance is still caused by overuse in humans, researchers continue to acknowledge that the use of antibiotics in animal agriculture does pose a risk to human health. This is an array of mostly ramen, dry aged steak and pasta (all scratch or mostly scratch). . Im constantly inspired by @adamliaw @outdoorcheflife @massimobottura and of course @gordongram. If you can follow directions, there are more free resources to learn to cook than ever before. Find one or two things to do well and slowly expand from there! . Just like my last post, the hardest part is starting. Ill tell you though, my grocery and dining out budget is roughly a combined $350 per month and I eat like a king. .
