A few weeks ago I had surgery to repair a wound I’ve been dealing with for a few years. My regular Doctor was allowed to observe the procedure and I gave her my phone to take pictures. This is a cropped pic of the surgeon using a hammer and chisel to take a piece of bone to biopsy for infection. Healing is going great but I still have roughly 3 weeks to go before the tissue is healed enough for me to start physical therapy. 5ish weeks until I can hopefully go home. - @imtoooldforig on Instagram

Imaging signs of #cirrhosis 1.Enlarged hilar periportal space: An enlarged hilar periportal space (defined as a distance between the right portal vein and the posterior edge of segment IV greater than 10 mm) may be the only morphological sign at earlier stages of cirrhosis. 2&3.Caudate-right lobe ratio: 2.Harbin reported that a C/RL greater than 0.65 was suggested a diagnosis of cirrhosis using the main portal vein bifurcation as a landmark between the two lobes. 3:More recently, Awaya noted that the hypertrophied caudate lobe extended beyond the main portal vein bifurcation, and proposed a modified C/RL, which uses the bifurcation of the right portal vein as a landmark. A ratio greater than 0.55 suggested a diagnosis of cirrhosis. 4.Right posterior hepatic notch sign: Hypertrophy of caudate lobe is also responsible for the right posterior hepatic notch sign.In normal liver, the right postero-inferior liver surface is concave. In cirrhotic liver, a sharp indentation (notch) in the right medial posterior liver surface can be observed. The notch corresponds to the boundary between the atrophied right posterior segments and the hypertrophied caudate lobe. 5.Enlarged gallbladder fossa 6.Nodular margins: Nodule formation and distortion of hepatic architecture have variable effect on liver surface.Smooth margins suggest micronodular cirrhosis (RNs 3 mm). As a kid, I was scared of rats and needles. As a teen, I conquered my fear of needles by having tattoos and piercings, but still afraid of rats. As a grown man, Im scared of seeing my wife angry/crying, rats and that Im still scared of unwanted needles. Getting blood samples, dextrose, and rapid tests; Because its different from having tattoos or nipple rings. But for my family and for my co-workers safety, I dont mind frequent rapid tests. Its my fifth time this month, and Ill be having it again in three days for another project. This 26 year old male had a motor vehicle accident and unfortunately battered his frontal, orbital and nasal bones. The frontal bone had both anterior and posterior table fractures with persistent cerebrospinal fluid discharge. The whole frontal bone had dished in and also was pushing the right eye out of globe. Fortunately his vision was intact and there was minimal frontal lobe injury. We had to intervene early so took him to the OR. Resident had a root canal recently that looked incomplete. Turns out it was likely similar to this case I will present. This young boy presented with a “hot” tooth and was in a lot of pain. I’m not even sure how my assistant managed to get this good of a PA. As you can see, gross decay and unformed apices. I removed decay and tooth bled forever. Still, eventually I got it to stop so I sealed with MTA. Over it I placed composite. First Pa is preop. Second is postop. Third is 18 month check. Somewhere I have a 6 year check too. Probably 8 years now. Here are the take home points. 1. Years later, tooth is still vital. Takes longer for patient to feel cold. But tooth is positive not lingering to Endo Ice. 2. Usually I place little MTA. This tooth required more inflamed tissue to be removed. Yet tooth is still vital. Shows how biocompatible mta really is. 3. Looks like an incomplete #rootcanal It sorta is. But not really. 4. Amazing how kids can recover. 5. My previous post examined the relationship between the percent of hemoglobin saturation with oxygen and the corresponding PaO2. What if you’re adding more oxygen to a patient? Theoretically, the percent of hemoglobin saturation should go up with the FiO2 going up, therefore increasing the PaO2. 💉 Every single number in the short hand form of an ABG is important. Let’s switch gears to the THIRD NUMBER in the ABG: The PaO2 and it’s relationship to the FiO2 and oxygen saturation. 💉 On room air, the normal PaO2 is between 80-100. On an FiO2 of 100%,the PaO2 is around 500. As a rough rule of thumb, multiply the FiO2 by 5 to get the corresponding PaO2. #cheatsheet for #residents and #fellows If these numbers don’t match up, examine reasons for #hypoxia 🤓 💉 Oxygenation is just as important on ABGs as determining an acidosis or alkalosis. The PaO2 reflects the amount of oxygen that has dissolved in the blood. Simplistically, it tells you how well the lungs are transferring oxygen to the blood with the amount of FiO2 you are giving. It can also help determine etiologies of hypoxia with utilization of tools such as the A-a gradient, and it can offer information for the oxygen delivery equation. La lima manual me marca si puedo emplear lima rotatoria para remover más gutapercha. En este caso empleo #zr2 de @zarc4endo (25/04 de 25 mm). 500 rpm/torque 4.0 10. Intentar permeabilizar. Siempre acompañado de hipoclorito de sodio. 11. No suelo usar solventes. En caso de necesitar empleo cloroformo únicamente. 12. Una vez localizado los conductos omitidos y permeabilizado los demás continúo con el tratamiento convencional. A pesar de tener un protocolo siempre recordad que hay que adaptarlo al caso. COVID restrictions emergency case recall. Patient presented 5 months ago - during the restrictions with an acute apical abscess under tooth 47. Pus was drained from the mesial canals. Felt good to get the patient out of pain and keep them away from the medical docs, who were busy! After dressing canals and resolution of symptoms the canals were filled and SDR base placed comp in access. Preps were Trunatomy prime Mesial canals and Protaper next X2 in D. We all know the risks when we go out, you just never imagine it will be you. Last Thursday started like most peahi days grabbing a couple rides - it was looking to be an epic low wind day. Then on a not extraordinary large one, I tried to tuck into the barrel and got clipped by the lip, something I’ve done before. But this time as my board came back over the falls, it struck my leg rail first. I instantly knew something was wrong as my leg was squirreling around by itself in the white water. Luckily my cousin @trevor.808 who runs rescue was right there with the sled and was able to drag me into safety of the channel. From there the quick actions of water safety @kolomona1_ and @dklegend jumped into action. I am extremely blessed these guys were there and quickly assessed and got me to Maliko gulch to an awaiting Ambulance. Later that night surgery and a new 40cm rod in my leg. Truly blessed with how everything happened. I don’t think it could’ve gone smoother. Bummed this season is over. I now start the road back to again surf the wave I truly love. Again my Thank you to everyone who got me home safe and to the surgeon who put me back together. Ended up snapping my femur, clean break. Blessed 🙏🏼 - @ryan.funk on Instagram

Have a look at the X-Ray and swipe left for the explanation⁣ ⁣ ⁣ ⁣ ⁣ NEUROGENIC TUMOUR⁣ 👨🏽‍💻The key to this X-Ray is to remember to use a system! It would be easy to say there is no fracture or dislocation and move on but by reviewing systematically we find an incidental lesion that has implications for the patient ⁣ ⁣ 👨🏽‍💻It’s important to assess the lung in all shoulder and clavicle X-Rays - in cases of trauma it is especially important to look for pneumothorax looking for lucency, lack of lung markings and a ‘pleural line’ - a thin white line paralleling the pleural edge ⁣ ⁣ 👨🏽‍💻In this case we see an opacity at the apex - in older patients the main differential would be a primary lung cancer however a) this lesion is well defined and b) this is fairly uncommon in this age group so we need to think of other causes⁣ ⁣ 👨🏽‍💻A pleural fibroma would be possible, however this turned out to be a neurogenic tumour (schwannoma). Had the lesion entered the neural foramen then a neurogenic tumour would have been easier to diagnose however we can see the neural foramen is clear on MRI. Lack of foramen involvement doesn’t exclude a neurogenic tumour as this can form anywhere along the intercostal nerve as it has here ⁣ ⁣ 👨🏽‍💻Schwannomas are the most common nerve sheath tumour and show high T2 signal on MRI, with heterogeneity due to cystic degeneration/haemorrhage in contrast to fibromas which are typically low in T2 signal however can be heterogenous due to necrosis. Both enhance post contrast⁣ ⁣ 👨🏽‍💻Schwannomas are typically benign but can slowly grow over time ⁣ ⁣ Hopefully this case will remind you to assess shoulder and clavicle X-Rays systematically!⁣ ⁣ 🔻@theradiologistpage - @theradiologistpage on Instagram

Poor Mav decided he wanted to be more like his old man, so he ran into the side of a car today and broke his leg in 3 places requiring surgical implantation of some hardware to stabilize everything. Big thanks to all of the concerned citizens and fellow dog walkers who witnessed Mav’s traumatic experience, and helped care for him and get us to the pet hospital together. The man in the 3rd photo (Robert) even phoned in afterwards to donate $400 towards Mav’s vet bill, without leaving me his contact info - talk about a Good Samaritan! In the landmark International Study of Unruptured Intracranial Aneurysms (ISUIA) study, aneurysms were stratified by risk of rupture based upon size and location. Giant aneurysms, defined as at least 25 mm have the greatest risk of rupture, with anterior circulation aneurysms conferring 5-year rupture risks of 40%. Here we see a large aneurysm in the anterior circulation, with an estimate rupture risk of 14.5%. Treatment options include endovascular techniques such as coil embolization, or open surgical techniques including clipping. Today the Pentagon officially released three videos of UFOs that were originally released to the public by To The Stars Academy of Arts & Sciences” (yes, Tom DeLonges research group) in 2017 & 2018. Two of the videos were recorded in 2015 and the other back in 2004. Today is exactly one year since my surgery! It has been the hardest year of my life but also the best year of my life. I am in pain everyday and mentally that can wear on a person but I stay positive and think about all the room my heart has to beat and all the air my lungs get now. I can walk up the stairs and not be short of breath. I can go to the grocery store and not have to sit down because I feel like I’m going to pass out. Today’s case we have this scalp laceration! This man had a little too much to drink which resulted in a fall. Unfortunately, he lost the battle with the cement ground and sustained this good sized laceration. Luckily, the CT head was normal and showed no intracranial hemorrhages (aka subdural, epidural, subarachnoid or intraparanchymal hemorrhages). Therefore, this should be a fairly easy closure! . . . Did you know that SCALP is actually an acronym for the layers of tissue on your head?! I know right, mind blown! It stands for skin, connective tissue, aponeurosis, loose connective tissue and pericranium. You have a thick skin and connective tissue layer which allows for easy repair of any laceration using staples. We do not worry too much about cosmetics on the skull because any scar is hidden within the hair! When you get to the aponeurosis layer, you actually have to take a little more care and throw dissolvable sutures to close aponeurosis before stapling the superficial layers. This is actually the aponeurosis or galea which connects the occipital and frontal bellies of the occipitofrontalis muscle together. This muscle is important because it assists in raising the eyebrows and wrinkling the forehead. This is actually the muscle botox targets to paralyze and get rid of those pesky wrinkles! When going deeper to the loose connective tissue layer, we have to worry more about hematoma formation as this layer is very vascular. Lastly, the pericranium is important because it is the outer periosteal (bone) layer of the skull. You have to ensure there are not major breaks in this bone as right under it is your cranial cavity, where your brain is found! Thought I’d illustrate another cool #regenerative case. Patient had caries in the tooth before it even erupted. Not sure if @rickschwartz78 or @endogenie or anyone knows the term for that? I’ve always been curious. So tooth tested vital. But you can see I pulped out a little when I excavated. As expected. I placed MTA and later composite. Final PA is from her GP 3 years later. Tooth is still vital and fine. Been 8 years now. Will hopefully get a new Pa soon. But I wanted to show this because the pics are good and I placed way less MTA than the case I depicted before this one. The main takeaway is that a big apex leads to great blood supply and likely success. Dr Hargreaves says he and the literature recommends bleach and #chlorhexidine to rinse but I’ve always just used bleach. What a day yesterday. I got to spend some time with @teagueholmes and were both injured. We came upon a parking attendant who told us that we could park over there and run over to the other side of the building. We both cant run, so I drove him over there and he hopped out of the car to do his errand. After that I went to the doctor for my 6 week checkup and he said that I could now sleep without my back brace on We both immediately started reminiscing about the time we got to ski the Grand Envers with Cédric Aubert. - A clamshell thoracotomy performed due to a high velocity round to the chest.

Soo back in May I was trying to get a clip on this rail and ended up dislocating and tearing my plantar plate in my left pinky toe. Who knew a pinky toe could be so serious but I got surgery in July and I had to get a pin that stuck out of my pinky toe to stabilize it. The doctor told me after surgery that this injury is one of the rarest type of injuries and that my case was the 2nd recorded case in the country. Tenho dividido alguns casos do meu consultório com intuito de ajudar colegas pediatras e trazer para vcs situações e alertas importantes. Ontem foi um dia difícil, algo q gostaria q nenhum pediatra e nenhum paciente passasse. Em um momento como este de pandemia, em q algumas consultas são on-line, fico com mais certeza q ir ao pediatra e fazer consulta presencial é essencial, q bom q mãe quis vir ao consultório e não ao PS. Sim as consultas on-line ajudam em algumas situações, mas o exame físico é importantíssimo!Não deixem de marcar e de fazer as consultas de rotina com seu pediatra mesmo neste momento. Na sexta passada avaliei uma querida paciente de 3anos. Com queixa de dor de ouvido e febre, algo bem comum após quadros virais respiratórios. Normalmente em uma consulta de urgência avaliamos especificamente a queixa. E ela realmente tinha uma otite média aguda, q precisava de tratamento com antibiótico para uma resolução sem complicações. Mas resolvi examina-la de maneira completa, pesamos, medimos e fiz o exame físico habitual. Pois nesta paciente em uma consulta para avaliar uma otite achei algo diferente na palpação do seu abdome. Agradeço muito por ter feito todo o exame. Mostrando sempre q cada vez q estiver com um paciente devemos avaliá-lo por completo! Cada consulta uma oportunidade q não pode ser perdida para exame detalhado. Pelo meu rosto a mãe percebeu minha preocupação. Palpar uma tumoração não é esperado nunca. Conversamos, tentei acalma-lá e programamos um ultra som. Conseguimos fazer ontem e confirmou o difícil diagnóstico, um câncer renal. Câncer, tumor são palavras q assustam muito, e em criança muito mais! Faz família e pediatra sofrerem juntos! Super importante falar q o prognóstico em crianças é muito bom! 90-92% de cura. E ela Já está em acompanhamento com a equipe de oncologia do Einstein, fizemos o estadiamento e graças a Deus sem metástase. Começaremos uma longa jornada de tratamento, inicialmente com quimioterapia para diminuir o tamanho do tumor e depois cirurgia para retirar totalmente a lesão, e tenho certeza q vai dar tudo certo! Computed tomography (CT) angiography images of the face, cranium, and the neck were obtained and revealed multiple fractures, including: comminuted fracture of the mandible; a Le Fort 2 fracture; a midline fracture of the maxilla; extensive fractures of the walls of the right maxillary sinus and both right and left orbital floor; a right zygomatic arch fracture and fracture of the right temporal bone at the temporomandibular joint. Low back pain is among the leading cause of disability globally, and nearly everyone will experience this at some point in their lifetime. In this instance, a disk herniation is causing more havoc than just pain. A massive fragmentation of the intervertebral disk at the L4/L5 space compressed the normal fat signal (false colored purple) surrounding the L4 nerve root as well as impinging all the descending roots within the spinal canal. This situation is called cauda equina, and is a neurosurgical emergency. - My severe scoliosis pretreatment

When I was a chief resident, a 19 year old kid was brought into STC having been shot multiple times in his chest. He rapidly deteriorated and cardiac arrested right in front of us. My team immediately responded with our resuscitative efforts. I then opened his left chest, quickly set up the Finochietto retractor, placed a clamp on his aorta and opened his pericardium to find a large amount of blood and multiple bullet fragments. I quickly whip stitched the holes in his atria and ventricle closed and began manual cardiac massage as we poured blood back into him. We worked on him for over 40 minutes. But we couldn’t get him back. I stood there holding this kids lifeless flaccid heart in my hands surrounded by a huge highly specialized team that could often do the impossible, but we failed that night. I failed that night. These moments make you feel like you aren’t good enough. They makes you question every single moment and decision. And then you begin to rationalize it some how and tell yourself “well this was some gang kid that probably got shot selling drugs etc.” And while that many times WAS the case in Baltimore, I never let those thoughts hang around in my mind too long. I didn’t want to use that as my crutch. I never wanted to dehumanize people, regardless of the circumstances that brought them in. The reality is that moments like this one, where I was unable to save a life, haunt me long after the time has past. I wake up at night in a cold sweat and routinely have nightmares about many of these dramatic moments where my efforts weren’t good enough. A lot of surgeons don’t feel comfortable openly talking about this because we are supposed to have extremely thick skin and be bulletproof ourselves... They are afraid to be vulnerable, but the reality is it’s a burden we all feel in one way or another. It’s also the reason why I ride my residents asses so hard and expect so much out of them because I truly want them to become physically and emotionally tough and better; better than I am and to never ever become complacent and always keep getting after it! - @adamthespecimen on Instagram

#Repost @trauma_time・・・ Today’s case we have this severe hand injury! This 15 year old male was playing with some homemade fireworks when...kaboom! The explosion resulted in this severely mangled right hand, which is unfortunately his dominant side. To orient yourself a little better, the pinky is to the left, index to the right, and thumb is completely disarticulated and hanging toward the bottom of the picture. The team opted to amputate the hand at the wrist after extensive examination of the wound. You could argue that the index and pinky fingers had a chance to be salvaged, however, the extent of the surgery would be vast. From this picture, it seems that the damage extends into the wrist, effecting the flexor muscles, which would greatly increase the difficulty of the salvaging. I think they made a great decision to amputate when weighing the risks vs the benefits! This type of injury was one that my parents always warned against every 4th of July. This is a lesson for everyone...leave the fireworking to the professionals, who use mass produced explosives and take all necessary safety precautions! ⠀ •⠀ •⠀ •⠀ Don’t forget to tag your friends down in the comment section! Read on to find out more about my review areas on a hand X-Ray...⁣ ⁣ ⁣ ⁣ 👨🏽‍💻Hand x-rays are performed in cases of patients with suspected arthritis to try and evaluate the cause but more commonly are performed in cases of trauma⁣ ⁣ 👨🏽‍💻When it comes to trauma there are a few areas I look at in particular every time - these review areas have helped me become more confident when looking at these X-Rays and hopefully they can do the same for you:⁣ ⁣ ⁣ ▫️Always start any trauma related X-Ray by looking at the soft tissues - the site of swelling can lead you to the fracture ⁣ ▫️Check the metacarpal heads - an indented 4th or 5th metacarpal is common after a punching injury - termed a Boxer’s fracture. Don’t stop there in these patients - draw a line across the base of each metacarpal and this should join up. Sometimes the 4th/5th metacarpals can dislocate and travel downwards so they overlap the carpal bones - here your line won’t join up. Overlapping bones on a hand X-Ray should always make you think!⁣ ▫️Outline the base of the thumb metacarpal - forced abduction can cause an intra-articular fracture known as Bennett’s fracture (or Rolando fracture if comminuted)⁣ ▫️Do all of the fingers look well aligned? Check for subluxation and dislocation - you may need further views to be sure⁣ ▫️When it comes to carpal bones, scaphoid injuries are most common but you will need a whole scaphoid series. A normal X-Ray can’t exclude a scaphoid fracture. Practice varies with what to do next, some bring back people with snuffbox tenderness in 7-10 days to do an X-Ray, some to do a repeat clinical assessment whilst in some institutions they come back for an MRI ⁣ ▫️The spaces between the carpal bones should be equal, if you see a widened distance between the scaphoid and lunate consider scapholunate dissociation (Terry Thomas sign)⁣ ⁣ 🔻As always take your time, zoom up and outline the cortex of each bone, use a system and check two views! ⁣ ⁣ @theradiologistpage - @theradiologistpage on Instagram

Maxillofavial Trauma Management - Airway ———————————————————————— After we handle the immediate life threats (massive hemorrhage), we have to consider the airway. Look, listen, and feel: take a look at the injuries and look inside the mouth for blood, vomit, broken teeth, dentures, chewing tobacco, etc. If any of these are present, grab your suction and clear it out. If dismounted or without mechanical suction, grab a 60cc syringe with a cathedral tip and a put an NPA on it to create a field expedient manual suction device. Give the lungs a listen and see if you’re getting adequate air movement. If the environment is keeping you from listening, place you’re hands on the chest and feel for bilaterally equal rise and fall of the chest. ———————————————————————— I would personally prefer to skip the intubation attempt and go for a surgical cricothyroidotomy. Orotracheal and nasotracheal intubation may be attempted but be aware that musculoskeletal tissue disruption will likely increase the difficulty of intubation. LMAs and I-Gels can be used to buy you some time to prep for a cric, but they don’t prevent regurgitation and aspiration of vomitus and may increase vomiting unless a paralytic has been administered (which is unlikely due to the fact that we likely haven’t gained vascular access at this point in our assessment).
