Keratoprosthesis is a surgical procedure where a diseased cornea is replaced with an artificial cornea. Traditionally, keratoprosthesis is recommended after a person has had a failure of one or more donor corneal transplants. More recently, a less invasive, non-penetrating artificial cornea has been developed which can be used in more routine cases of corneal blindness. While conventional cornea transplant uses donor tissue for transplant, an artificial cornea is used in the Keratoprosthesis procedure. The surgery is performed to restore vision in patients suffering from severely damaged cornea due to congenital birth defects, infections, injuries and burns. Keratoprotheses are made of clear plastic with excellent tissue tolerance and optical properties. They vary in design, size and even the implantation techniques may differ across different treatment centers. The procedure is done by ophthalmologists, often on an outpatient basis. Although many keratoprostheses have been developed, only four models are currently in commercial use: the Boston keratoprosthesis, Osteo-Odonto-Keratoprosthesis (OOKP), AlphaCor and the KeraKlear Artificial Cornea. Symptoms usually include pain, swelling, joint stiffness, limited range of motion and difficulty with many functional tasks. Severe pain or functional loss associated with OA of the knee or hip often leads to joint replacement surgery. . 🔎In a study by Deyle GD et al (citation below), investigators aimed to discover the difference between subjects with OA performing a home exercise program and another group of subjects performing a home exercise program with the addition of several visits of manual therapy. Outcomes were measured at 4 weeks, 8 weeks and one year. The results demonstrated that the addition of manual therapy provided greater symptom relief temporarily and that by 1 year, both groups had improved almost equally as compared to baseline. . 🧠Maybe the main takeaway from this research is that a simple home exercise program can have a dramatic impact of the life of an individual with OA of the knee. While manual therapy provided by in-person physical therapy helped in the short-term, it did not significantly alter long-term outcomes. . ✅Swipe left to see a few of the exercises that were included in this study. . 1️⃣Heel Slide with Strap - Slide as far as tolerable. 10 reps with a 3-second hold. 2️⃣Calf Stretch - Perform daily (3x30 secs). 3️⃣Hamstring Stretch - Raise as high as tolerable. Consistency is the key to minimizing mistakes. So I use the ABCS method every time I look at a plain film. X-ray of the hand: Alignment - look at every bone and joint space to make sure they are properly lined up. Bones - trace the cortex of every bone to make sure there isn’t a subtle fracture. Displaced fractures are easy to see. By tracing the outline of every bone you’ll pick up more subtle fractures and cortical buckles. Also a good opportunity to look for destructive lesions. Cartilage - check for joint space narrowing and subchondral fractures. Soft tissues - look for gas, swelling, foreign bodies and calcifications. 📚🔬 Taping for Patellofemoral Pain Syndrome ℹ️ℹ️ BACKGROUND: Patellofemoral pain syndrome (PFPS) is characterized by anterior knee pain and/or pain in the retro-patellar and/or peri-patellar region. ℹ️ Pain typically increases with activities such as: Squatting. Kneeling. Stair climbing. Prolonged sitting. ℹ️ The underlying causes of PFPS are multifactorial and may be associated with biomechanical and or neurophysiological changes at the pelvis, hip, knee or ankle regions. ℹ️ Taping is frequently used as part of the multi-modal management for PFPS, including: McConnell Patellofemoral Joint Taping (PFJT). Tibial Internal Rotation Limitation Taping (TIRLT). ℹ️ Clifford et al. (2020), investigated the effects of TIRLT, PFJT and no taping on perceived pain and lower limb kinematics during a lunge and single leg squat (SLS) in people with PFPS. 🔬🔬 METHODS: This cross-sectional study compared the effects of TIRLT, PFJT and no taping, on knee pain and lower limb kinematics during two pain-provoking movements in people with PFPS. 🔬 Participants with PFPS (n = 23) performed a lunge and SLS under three randomised conditions: TIRLT, PFJT and no taping. 🔬 The Codamotion system captured and analysed lower limb kinematic data in the sagittal, transverse and coronal planes. 🔬 Peak knee pain intensity during the movement was assessed using the Numerical Rating Scale (NRS). 📊📊 RESULTS: Participants reported significantly less pain with the TIRLT and PFJT techniques compared with no tape during the lunge and SLS. 📊 There was no evidence of altered lower limb kinematics accompanying pain reductions with either taping technique. ✅✅ CLINICAL SIGNIFICANCE: Both forms of taping may be useful adjuncts as the short-term benefit of pain relief may enable participation in more active forms of rehabilitation. 🧠🧠 Thoughts? Наши диагнозы почти никогда не верифицируются патологоанатомически. Отсюда столько заблуждений, основанных лишь на личном опыте докторов. «Так делать хорошо, а вот так плохо» - скажет вам любой доктор. А почему? ⚠️70-year-old female pedestrian was struck by a motor vehicle at high-speed, sustained multiple injuries including injuries to her lungs and head. She also sustained a mangled injury to her right lower extremity with what’s considered a full degloving of the extremity and multiple fractures and neurovascular injuries. Patient was taken to the operating room immediately and managed by multiple trauma surgeons for her injuries. A high above knee amputation was done. Above-the-knee amputations may be necessary for many reasons. These can include trauma to the lower leg, which results in a non-viable leg at or near the level of the knee. Below-the-knee amputation may adequately address a more distal injury. Many studies have attempted to create algorithms to help physicians decide when to reconstruct versus amputate. One of these is the Mangled Extremity Severity Score (MESS), which takes into account skeletal/soft tissue injury, limb ischemia, shock, and patient age. 01/07/2017 Há 2 anos atrás, eu estava indo pra um hospital bem longe, onde eu não tinha ideia do que aconteceria quando chegasse lá, a única coisa que sabia era que ia ser difícil, eu teria que ser o mais forte possível e que era realmente uma cirurgia complicada com alguns riscos. A cirurgia acabou, os dias de internação foram passando, a dor era horrível, a sensação de que eu não iria mais ser a mesma era grande, e a vontade de desistir era constante, mas Deus esteve cada segundo ao meu lado, me deu forças, e finalmente tudo passou. Eu realmente mudei, tenho cuidados que não tinha, dou bem mais valor à vida, e sei que se não fosse essa cirurgia, hoje provavelmente estaria em uma cama, ou pior, e com problemas bem maiores. Sou grata a cada dia por isso, e por mais que ainda exista dificuldades, não teria uma alternativa melhor! Sou grata também a toda a equipe médica que esteve comigo me dando todos os cuidados possíveis, a minha família e amigos que me deram forças, e principalmente a Deus, pois sem Ele nada seria possível, e eu nada seria! Nós achamos que não vamos superar e que nada vai passar, mas isso é só mais uma etapa desse processo chamado vida.
