Is immune testing necessary before #IVF? I frequently get this question. Sometimes more is not better and tests that are not proven to be useful may do more harm than good. Today is the #transferversary of this embryo that now is a healthy baby. After struggling at another fertility center, this couple had all kinds of expensive tests that are not necessary before IVF. My new patient was terrified that she had an immune problem that would prevent her from carrying a term pregnancy and that would affect her health. All kinds of interventions were recommended including blood thinners, steroids, intralipids. Take a step back. We knew why this couple had infertility and it had nothing to do with her immune system. She was otherwise normal and healthy. I suggested she see a medical immunologist who agreed these tests were not necessary or predictive of any underlying immune problem. I literally wrote in her chart that we are ignoring all the prior results. It would have been easy to accept them and let another doctor prescribe unnecessary medications. She might have even gotten pregnant in spite of them and thought they helped and were necessary for her future pregnancies too. Any way, on her first transfer at #columbiafertility she conceived with a natural cycle frozen embryo transfer and delivered a healthy baby. This was good lesson why we should only order tests or recommend treatments that have solid evidence to support them unless it’s part of a research trial. #ttc #ttccommunity #ivfjourney #miscarriagesucks - @ericformanmd on Instagram

Ever heard of #RedTide? Red tide is the common name for a type of harmful algae bloom (HAB). HAB’s are caused by a combination of factors relating to temperature, nutrient availability, salinity and other factors. Human activity and nutrient pollution has been proven to increase the chances of a HAB. Red tide HABs can be detrimental to all sorts of marine organisms, and can even cause human health problems. In recent years, Florida has been especially hard hit by red tide blooms, closing beaches in 2018 and causing local economies to suffer. Research is ongoing to study the effects of red tide on marine ecosystems. Using gene therapy to restore vision to people with an inherited retinal disorder is one of the most exciting achievements in eye research. The one-time treatment for Leber congenital amaurosis uses a harmless virus to deliver healthy copies of a missing gene to the retina. Some young patients have regained vision within weeks after treatment. Despite the miraculous outcomes, ophthalmologist continue to refine their technique for delivering gene therapy to children’s eyes. In the June issue of Ophthalmology Retina, Audina Berrocal, MD, and colleagues show a new technique for gene delivery in children. Here’s a partial description of their technique: After loading the AAV vector syringe onto a 38G/25G Subretinal PolyTip Cannula and purging all air bubbles, the injector was delivered into the subretinal space with a beveled tip. Beveling of the cannula tip was performed with scissors at a 45-degree angle, and the cannula was entered into the subretinal space in a bevel-up fashion. The EVA Phaco-Vitrectomy System with the viscous fluid control system was used for a single-surgeon, foot pedal-controlled linear infusion of the drug. When bleb tension and risks of macular hole formation are deemed too high, more than 1 bleb is recommended. With more than 1 extrafoveal bleb, subretinal delivery began nearer to the arcades but included the macula. In all cases, a total of 300 mL of vector was released (image shows multiple elevated subretinal blebs with a foveal detachment after injection). Cyanobacteria (blue-green algae) are found in a broad range of environments, from hot springs to marine and fresh waters, and soil crusts to polar icecaps. By understanding exactly how cyanobacteria harvest light and convert it to energy, scientists may be able to engineer highly efficient photosynthetic units to help increase crop yields. In a paper published recently in Nature Plants (doi.org/10.1038/s41477-020-0718-z) Vincenzo Mascoli (PhD student at Vrije Universiteit @vuamsterdam) and colleagues investigated cyanobacteria from shaded environments to learn more about the trade-offs between covering the largest portion of the solar spectrum for photosynthesis and achieving the highest yields of energy conversion. - Plants ‘see’ underground by channeling light to their roots. Plant stem acts like a fibre-optic cable, conducting light down to receptors in the roots known as phytochromes. These trigger the production of a protein called HY5, which promotes healthy root growth.
Limbal stem cells are the only source of corneal epithelial cells and they must undergo self-renewal,proliferation, and differentiation to maintain the ocular surface and achieve wound healing. The current failure of pharmacologic treatments for Neurotrophic Keratopathy is the result of the gap in our basic understanding of the paracrine crosstalk between the corneal nerve fibers and the epithelial cell populations. A beautiful picture of a cell we injected today in my lab with a fluorescent dye. This is a Broad thorny retinal ganglion cell (species: common marmoset/callithrix jacchus), a type of neuron located in the retina. These neurons help provide us with our sense of sight and are involved in a number of complex processes mediated through our visual neural circuitry. Cells like this one are involved in processes such as “visual pursuit” which guides our eyes as they focus on different or moving targets. For reference, this cell has a diameter of approximately 300 um, or about 3/10 of a mm. Yesterday was #WorldHeartDay. This image shows various RNA transcripts in a section of heart tissue. Its from @caltechedus Tissue Mapping Center, one of the components of the #NIHCommonFunds Human BioMolecular Atlas Project (#hubmap). HuBMAP has brought together researchers from 18 different institutes to create the framework necessary to build a cell atlas of the healthy adult human body.
